"SIMONA (Sistem Informasi Monitoring dan Pembinaan Fasilitas Pelayanan Kefarmasian) merupakan sistem monitoring yang digunakan untuk mengawasi dan membina fasilitas pelayanan kefarmasian, termasuk apotek. SIMONA membantu penyedia layanan kefarmasian untuk menilai kesesuaian dirinya terhadap standar pelayanan kefarmasian yang berlaku. Pada laporan ini dilakukan pembuatan laporan pelayanan farmasi klinis Apotek Roxy Poltangan periode Desember 2022. Laporan ini dibuat dengan menginput data laporan pelayanan kefarmasian bulan Desember 2022 Apotek Roxy Poltangan ke dalam sistem SIMONA. Hasil laporan pelayanan kefarmasian menunjukan bahwa telah dilakukan sebanyak 29 kali pengkajian dan pelayanan resep, 29 kali pemberian pelayanan informasi obat, 13 kali konseling, dan belum dilakukan telefarmasi. Hasil ini menunjukan bahwa Apotek Roxy Poltangan telah melakukan pelayanan kefarmasian yang sesuai dengan standar yang berlaku.

SIMONA (Sistem Informasi Monitoring dan Pembinaan Fasilitas Pelayanan Kefarmasian) is a monitoring system used to supervise and develop pharmaceutical service facilities, including pharmacies. "Apotek Roxy Poltangan merupakan perusahaan ritel farmasi yang bertujuan untuk memberikan pelayanan obat yang berkualitas kepada pasien atau masyarakat. Pelayanan farmasi klinis merupakan pelayanan langsung yang diberikan Apoteker kepada pasien untuk meningkatkan outcome terapi dan meminimalkan risiko efek samping karena obat. Pelayanan farmasi klinis dapat dilakukan dengan kegiatan konseling, PIO, Home Pharmacy Care, atau PTO. Sistem Informasi Monitoring dan Pembinaan Fasilitas Pelayanan Kefarmasian (SIMONA) merupakan sistem yang telah dikembangkan oleh Kementerian Kesehatan Republik Indonesia yang dilakukan untuk monitoring dan pembinaan fasilitas pelayanan kefarmasian di fasilitas kesehatan. Tujuan pembuatan laporan tugas khusus ini adalah untuk mengetahui manfaat dari laporan bulanan pelayanan kefarmasian yang dilakukan oleh Apotek Roxy Poltangan melalui SIMONA. Pembuatan laporan tugas khusus ini dilakukan dengan cara mengisi formulir Pelayanan Informasi Obat (PIO) dan konseling, sesuai dengan resep yang diterima pada bulan November. Resep dikaji dan dilakukan penelusuran literatur untuk pengisian formulir PIO dan konseling. Formulir PIO dan konseling pada bulan November kemudian dilakukan pelaporan melalui laman SIMONA. Pada bulan November, jumlah pelayanan informasi obat (PIO) yang didokumentasikan Apotek Roxy Poltangan adalah sebanyak 42 resep. Sedangkan, untuk total konseling yang berhasil didokumentasikan oleh Apotek Roxy Poltangan adalah sebanyak 15 konseling. Berdasarkan hasil tersebut, Apotek Roxy Poltangan telah melakukan pelaporan pelayanan farmasi klinik melalui laman SIMONA, dimana hal tersebut dilakukan untuk penjaminan mutu pelayanan kefarmasian sesuai dengan peraturan yang berlaku.

The Monitoring and Development Information System for Pharmaceutical Service Facilities (SIMONA) is a system that has been developed by the Ministry of Health of the Republic of Indonesia and is carried out for monitoring and developing pharmaceutical service facilities in health facilities. "Corona Virus 2019 (COVID-19) ialah penyakit menular yang berkembang sejak bulan Desember 2019 di Wuhan, ibu kota Provinsi Hubei China, sejak itu virus ini menyebar keseluruh dunia dan menjadi global pandemi. Di Indonesia, pada Juli 2021, dikeluarkanlah surat edaran tentang Pelaksanaan Distribusi Obat dengan Persetujuan Darurat untuk penanganan terapi COVID-19 yaitu; Remdesivir, Favipiravir, Oseltamivir, Immunoglobulin, Ivermectin, Tocilizumab, Azithromycin dan Dexamethasone. Pasien yang ingin membeli obat yang mengindikasikan COVID-19 diwajibkan membawa resep dokter. Analisis resep dilakukan sesuai dengan Surat Edaran (SE), meninjau aspek administratif, aspek farmasetik dan aspek klinis. Hasil penelitian ini dapat disimpulkan bahwa terdapat 11 resep pengobatan COVID-19 pada bulan Juli 2021. Terjadi ketidaklengkapan kajian resep baik secara administrasi, farmasetik, serta aspek klinis. Namun seluruh resep telah memiliki ketepatan indikasi dan dosis.

Corona Virus 2019 (COVID-19) is an infectious disease that developed in December 2019 in Wuhan, the capital of China's Hubei Province, since then this virus has spread throughout the world and has become a global pandemic. In Indonesia, in July 2021, a circular letter regarding the Implementation of Drug Distribution with Emergency Approval was issued for the handling of COVID-19 therapy, namely; Remdesivir, Favipiravir, Oseltamivir, Immunoglobulin, Ivermectin, Tocilizumab, Azithromycin, and Dexamethasone. Patients who want to buy drugs that indicate COVID-19 are required to bring a doctor's prescription. Prescription analysis was carried out by the Circular Letter, reviewing administrative aspects, pharmaceutical aspects, and clinical aspects. The results of this study can be concluded that there were 11 prescriptions for COVID-19 treatment in July 2021. There were incomplete prescription studies both administratively, pharmaceutically, and clinically. However, all prescriptions have accurate indications and dosages.

Vitacimin is a lozenge product containing vitamin C, produced by PT Takeda Indonesia. The high demand for Vitamin C products has continued to surge dramatically since the global entry of the COVID-19 pandemic in Indonesia in early 2020. With the increasing market demand for Vitacimin, it is also necessary to analyze and optimize the duties and work of employees (packers) in packaging Vitacimin products. This observation is focused on line 3 secondary packaging using machine vision tools that have optical functions, namely object inspection and inspection and pattern recognition. The machine vision tool used is Camera Vision Baumer, which is currently running on a trial period by placing employees who serve as selectors and back up the Camera Vision function. In this study, it can be concluded that the Camera Vision Baumer tool works well and the selector in charge of performing the back up function of the Camera Vision Baumer task can be switched."

"Apotek adalah tempat dimana pasien mencari obat. Pelayanan kefarmasian yang terdapat di apotek tidak hanya mencakup penyiapan dan penyerahan obat, hal lain yang perlu diperhatikan yaitu seperti fasilitas apotek dan ketersediaan obat yang ada di apotek. Agar pelayanan kefarmasian dapat mencukupi kebutuhan pasien, maka diperlukan survei pada pasien apotek terkait fasilitas yang perlu ditambahkan, layanan yang perlu ditingkatkan, dan obat yang harus selalu tersedia di apotek. Survei dilakukan pada pasien Apotek Kimia Farma 628 Sukahati yang membeli produk di apotek dan bersedia menjawab kuesioner. Adapun kuesioner berisikan pertanyaan mengenai data demografis pasien (nama, jenis kelamin, usia, jumlah berkunjung, penyakit, dan obat yang digunakan/dibeli) serta data kebutuhan pasien (fasilitas yang perlu ditambahkan, layanan yang perlu ditingkatkan, dan obat yang harus selalu tersedia di Apotek Kimia Farma 628 Sukahati). Kebutuhan pasien yang paling diinginkan ada di apotek dari hasil survei pada 65 pasien yaitu adanya praktek dokter umum dan terjaganya ketersediaan obat di apotek, terutama untuk obat swamedikasi yang menjadi obat yang paling sering dicari di Apotek Kimia Farma 628 Sukahati. Apotek Kimia Farma 628 Sukahati dapat meninjau kembali pengadaan obat di apotek untuk menghindari kekosongan obat dan membuka layanan praktek dokter umum di apotek.

Pharmaceutical services in the pharmacy do not only cover the preparation and delivery of drugs, there are other things that need to be considered such as pharmacy facilities and the availability of drugs in the pharmacy. In order for pharmacutical services to be sufficient for patient needs, a survey is needed on pharmacy patients regarding facilities that need to be added, services that need to be improved, and medicines that must always be available in the pharmacy. The survey was conducted on Kimia Farma 628 Sukahati Pharmacy patients who bought products at the pharmacy and were willing to answer the questionnaire. The questionnaire contains questions regarding patient demographic data (name, gender, age, number of visits, disease, and medicines used/purchased) as well as patient needs data (facilities that need to be added, services that need to be improved, and medicines that must always be available at Kimia Farma 628 Sukahati Pharmacy). According to the results of a survey of 65 patients, the most desired patient needs were the opening of general practitioner practice in the pharmacy and the sure availability of medicines in the pharmacy, especially for self-medication which is the most frequently sought after drug at Kimia Farma 628 Sukahati Pharmacy. Kimia Farma 628 Sukahati Pharmacy can review medicines procurement at pharmacies to avoid medicine shortages and open general practitioner practice services at the pharmacy."

"Apotek merupakan salah satu sarana pelayanan kefarmasian bagi apoteker dan tenaga kefarmasian lain. Pelayanan yang dimaksud berupa pelayanan langsung dan bertanggung jawab kepada pasien terkait sediaan farmasi yang bertujuan untuk mencapai hasil yang pasti dalam meningkatkan mutu kehidupan pasien (Kemenkes RI, 2017). The purpose of writing this special assignment is so that the writer can understand the implementation of the prescription review carried out at the Roxy Pitara Pharmacy in accordance with the standards set by the government, namely the Regulation of the Minister of Health of the Republic of Indonesia Number 73 of 2016 concerning Pharmaceutical Service Standards at Pharmacies. Based on the analysis above, it can be concluded that the first and second prescriptions were incomplete in the administrative aspect. Drug interactions were also found in the first prescription, namely an increased risk of hypoglycemia when using Diamicron DR and Trajenta and in the second prescription, an increased risk of peptic ulcer and bleeding when using Pirofel and Prednisone together."

Pembuatan laporan tugas khusus Praktik Kerja Profesi Apoteker (PKPA) di Apotek Roxy Biak diawali dengan mengumpulkan beberapa data berupa sampel resep bulan Februari 2023 dan dilakukan analisis resep berdasarkan telaah aspek administratif, farmasetik, dan klinis. Berdasarkan hasil kegiatan yang telah dilakukan, pelayanan farmasi klinis yang dilakukan di Apotek Roxy termasuk pelayanan dan pengkajian resep serta dispensing obat yang dilakukan sudah tepat dan sesuai dengan Standar Pelayanan Kefarmasian di Apotek, serta Resep obat yang diterima dan dilayani di Apotek Roxy biak sudah sesuai dengan kelengkapan data yang tepat berdasarkan pengkajian administrative, farmasetika, dan klinis.
