"Rinosinusitis merupakan inflamasi simtomatik yang terjadi pada sinus paranasal dan rongga hidung. Prevalensi rinosinusitis di dunia, termasuk di Indonesia, cukup tinggi. Apotek merupakan suatu sarana pelayanan kefarmasian tempat dilakukannya praktik kefarmasian, salah satu contohnya adalah pengkajian resep obat, oleh Apoteker. Pengkajian resep penting dilakukan untuk menganalisa adanya masalah terkait obat, serta sebagai upaya Apoteker untuk mencegah terjadinya kesalahan pemberian obat pada pasien (medication error), sehingga pasien dapat menggunakan obat secara rasional. Oleh karena itu, penting untuk melakukan pengkajian resep di Apotek. Penulisan tugas khusus ini bertujuan untuk mengkaji resep obat rinosinusitis yang terdapat di Apotek Roxy Sawangan periode Desember 2022 berdasarkan aspek administratif, kesesuaian farmasetik, dan pertimbangan klinis. Penulisan ini dilakukan dengan melakukan pemilihan resep obat rinosinusitis, melakukan studi literatur, dan melakukan pengkajian resep. Berdasarkan pengkajian, didapat kesimpulan bahwa masih terdapat beberapa persyaratan aspek yang belum terpenuhi. Pada aspek administratif, diketahui alamat pasien, berat dan tinggi badan pasien, serta tanggal penulisan resep tidak dituliskan pada resep. Pada aspek kesesuaian farmasetik, bentuk sediaan untuk semua obat serta kekuatan sediaan untuk beberapa sediaan tidak tertulis pada resep. Selain itu berdasarkan aspek pertimbangan klinis, diketahui terdapat ketidaktepatan dosis dan cara pemakaian pada beberapa sediaan.

Rhinosinusitis is a symptomatic inflammation that occurs in the paranasal sinuses and nasal cavity. The prevalence of rhinosinusitis in the world, including Indonesia, is quite high. A pharmacy is a pharmaceutical service facility where pharmaceutical practice is carried out, one example of which is the review of drug prescriptions by pharmacists. Reviewing prescriptions is important to analyze any problems related to medicines, as well as as an effort by pharmacists to prevent errors in administering medicines to patients (medication errors), so that patients can use medicines rationally. Therefore, it is important to review the prescription at the pharmacy. The purpose of writing this special assignment report is to examine prescriptions for rhinosinusitis medication available at the Roxy Sawangan Pharmacy for the period December 2022 based on administrative aspects, pharmaceutical suitability, and clinical considerations. This writing was done by selecting a prescription for rhinosinusitis medication, conducting a literature study, and reviewing the prescription. Based on the study, it was concluded that there were still several aspects of the requirements that had not been met. In the administrative aspect, it is known that the patient's address, the patient's weight and height, and the date the prescription was written are not written on the prescription. In the aspect of pharmaceutical suitability, the dosage form for all drugs and the dosage strength for some preparations are not written on the prescription. Based on aspects of clinical consideration, it is known that there are inaccuracies in the dosage and method of use in some preparations."

"Seorang apoteker memiliki peran krusial dalam menjaga keselamatan pasien dengan memastikan pengelolaan maupun penggunaan obat yang aman. Adapun,  kesalahan pengobatan yang dapat membahayakan keselamatan pasien dapat terjadi  di berbagai tahap pelayanan kefarmasian, baik pada proses penulisan resep,  pembacaan resep, peracikan resep, maupun saat penggunaan obat. Sehingga, untuk menghindari terjadinya kesalahan pengobatan,  maka peran seorang apoteker sangat penting dalam melakukan pengkajian resep yang merupakan bagian dari pelayanan kefarmasian. Kegiatan pengkajian ini setidaknya mencakup tiga aspek yakni aspek  administrasi,  farmasetik serta klinis. Pada tugas khusus Praktek Kerja Profesi Apoteker (PKPA) di apotek Atrika, dilakukan pengkajian dua resep obat jantung. Adapun, dari telaah dan pengkajian resep yang telah dilakukan, diketahui bahwa dua resep obat jantung tersebut tergolong resep yang baik dari segi administrasi, farmasetis serta klinis.

A pharmacist contributes to patient safety by ensuring the safe management and use of medications. Potential medication errors that pose risks to patient safety may occur at various stages of pharmaceutical care, including prescribing, transcribing, dispensing, and during medication administration. Therefore, the role of a pharmacist is essential in conducting prescription reviews to prevent medication errors. As part of Praktek Kerja Profesi Apoteker (PKPA) at Atrika Pharmacy, two prescriptions for cardiovascular drug were reviewed. Based on the analysis and review of the prescriptions, it can be concluded that both prescriptions are considered good in terms of administrative, pharmaceutical, and clinical aspects."

Apotek adalah sarana pelayanan kefarmasian tempat dilakukannya praktek kefarmasian oleh Apoteker. Standar Pelayanan Kefarmasian di Apotek meliputi standar pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai, dan pelayanan farmasi klinik. Salah satu pelayanan farmasi klinik di apotek adalah pengkajian resep. Pengkajian resep meliputi kajian administratif, farmasetik, dan klinis. Hal-hal yang termasuk dalam kajian klinis diantaranya yaitu ketepatan indikasi dan dosis obat, aturan, cara dan lama penggunaan obat, duplikasi dan atau polifarmasi, reaksi obat yang tidak diinginkan, kontra indikasi, dan interaksi. kajian klinis ini perlu dilakukan agar pengobatan untuk pasien tepat sehingga target terapi pasien dapat tercapai. Oleh karena itu, penelitian ini dilakukan untuk melakukan pengkajian klinis resep di Apotek Kimia Farma Siliwangi Cirebon bulan September 2022, diantaranya yaitu resep geriatri, pediatri, narkotika, psikotropika, dan polifarmasi. Melalui tugas khusus ini diketahui pada beberapa resep geriatri, pediatri, narkotika, dan psikotropika secara klinis untuk indikasi, dosis, waktu dan cara pemberian sudah sesuai, namun terdapat beberapa interaksi obat sehingga perlu dilakukan monitoring dan penyesuaian dosis, dan jika terdapat antibiotik pada resep disarankan tidak diracik bersama dengan obat lainnya. Untuk resep polifarmasi sebaiknya diperhatikan kembali karena polifarmasi dapat meningkatkan risiko terjadinya interaksi obat dan mungkin juga terdapat duplikasi obat.

One of the clinical pharmacy services in a pharmacy is prescription review. Prescription reviews include administrative, pharmaceutical, and clinical reviews. Things included in clinical studies such as the accuracy of drug indications and dosages, rules, method and duration of drug use, duplication and/or polypharmacy, unwanted drug reactions, contraindications, and interactions. This clinical study needs to be carried out so that the treatment for the patient is accurate so that the patient's therapeutic target can be achieved. Therefore, this research was conducted to obtain a clinical review of prescriptions at the Kimia Farma Siliwangi Pharmacy Cirebon in September 2022, including geriatric, pediatric, narcotics, psychotropic, and polypharmacy prescriptions. Through this special assignment it is known that several geriatric, pediatric, narcotics and psychotropic prescriptions are clinically appropriate for indications, dosage, time and method of administration, but there are several drug interactions so it is necessary to do monitoring and adjust doses, and if there is an antibiotic in the prescription it is recommended not mixed with other drugs. For polypharmacy prescriptions, must be reconsidered because polypharmacy can increase the risk of drug interactions and there may also be drug duplication.

"Kegiatan pelayanan farmasi klinik yang dilaksanakan di apotek salah satunya mencakup pengkajian dan pelayanan resep. Resep yang dilayani di apotek cukup beragam, mulai dari resep untuk penyakit akut maupun kronis seperti diabetes.Tujuan dari skrining dan analisis resep obat antidiabetes adalah untuk mengetahui obat antidiabetes yang diresepkan oleh dokter, mengetahui jumlah resep yang mengandung obat antidiabetes, serta menilai kerasionalan penggunaan obat antidiabetes pada resep yang dilayani di Apotek Roxy Biak selama periode Juli 2022. Dari seluruh resep yang masuk selama periode tersebut, resep yang mengandung obat antidiabetes dicatat dan dikumpulkan data, kemudian dipilih dua resep untuk dikaji kelengkapan resep berdasarkan aspek administratif, farmasetik, serta klinis. Pengkajian 2 (dua) resep antidiabetes di Apotek Roxy Biak dilihat dari aspek administratif, farmasetika dan klinis sudah sesuai. Namun pada aspek administratif penulisan umur dan berat badan pasien tidak dituliskan. Aspek administratif terkait umur dan berat badan perlu dikonfirmasi kembali kepada pasien atau keluarga pasien yang mengambil obat di apotek, hal ini penting untuk mengkaji ketepatan pengobatan yang akan diterima oleh pasien.

The purpose of screening and analysis of anti-diabetic drug prescriptions is to find out the anti-diabetic drugs prescribed by doctors, to find out the number of prescriptions containing anti-diabetic drugs, and to assess the rationale for drug use. antidiabetic prescriptions served at the Roxy Biak Pharmacy during the period July 2022. Of all prescriptions received during this period, prescriptions containing antidiabetic drugs were recorded and data collected, then two recipes were selected to review the completeness of the prescription based on administrative, pharmaceutical, and clinical aspects. The review of 2 (two) antidiabetic prescriptions at the Roxy Biak Pharmacy from an administrative, pharmaceutical and clinical perspective was appropriate. However, in the administrative aspect, the patient's age and weight were not written down. Administrative aspects related to age and weight need to be confirmed again with the patient or the patient's family who picks up the drug at the pharmacy, this is important to assess the accuracy of the treatment that the patient will receive."

"Pelayanan farmasi klinik merupakan bagian penting dari komitmen apotek untuk memberikan pelayanan prima. Pengkajian resep merupakan bagian penting dalam pelayanan farmasi klinik untuk menentukan masalah terkait obat dalam resep seperti obat antihipertensi. Tujuan dari pengkajian ini adalah menilai kerasionalan resep dalam aspek administratif, farmasetik, dan  klinis pada resep obat antihipertensi yang dilayani oleh Apotek Roxy Biak selama bulan April 2023. Dari seluruh resep yang masuk, dipilih dua resep untuk dikaji aspek administratif, farmasetik, dan klinisnya. Berdasarkan pengkajian dari aspek farmasetik, kedua resep tidak ada penyimpangan, namun dari aspek administratif  tidak terdapat berat badan pasien dan paraf pembuat resep dan dari aspek klinis terdapat adanya duplikasi dan interaksi antar obat yang perlu dimonitor. Hal tersebut perlu dikonfirmasi kembali untuk mencegah terjadinya kesalahan pemberian obat.

Of all the incoming prescriptions, two prescriptions were selected to be studied for administrative, pharmaceutical, and clinical aspects. Based on the assessment from the pharmaceutical aspect, there are no irregularities in the two prescriptions, but from the administrative aspect there is no patient weight and prescription-making paraphrasing and from the clinical aspect there is duplication and interaction between drugs that need to be monitored. This needs to be reconfirmed to prevent the occurrence of medication errors."

"Apotek adalah sarana pelayanan kefarmasian tempat dilakukan praktik kefarmasian oleh Apoteker. Pengkajian resep merupakan salah satu standar pelayanan kefarmasian yang dilakukan di Apotek. Pengkajian resep dilakukan untuk melakukan analisa adanya permasalahan terkait obat dan apabila ditemukan masalah terkait obat harus segera dikonsultasikan kepada dokter penulis resep. Tujuan dilakukannya pengkajian resep untuk mencegah permasalahan terkait obat sebelum obat diserahkan kepada pasien sehingga tidak terjadi medication error yang berdampak pada kegagalan terapi dan efek obat yang tidak diharapkan yang merugikan pasien. Penelitian ini bertujuan untuk mengetahui kelengkapan administrasi, kesesuaian farmasetik dan kesesuaian klinis pada peresepan di Apotek Kimia Farma Menteng Huis. Dalam penelitian ini, dilakukan pengkajian resep Pasien Rujuk Balik (PRB) pasien asma dengan mengkaji secara administratif, farmasetik dan klinis. Pada persyaratan administratif, didapatkan ketidaklengkapan berat badan pasien dan paraf dokter. Pada pengkajian secara farmasetik, resep yang tertulis sudah sesuai bentuk dan kekuatan sediaannya. Pada pengkajian secara klinis, terdapat duplikasi dan interaksi pada beberapa obat. Dapat disimpulkan bahwa masih banyak ketidaksesuaian dalam penulisan resep yang diamati sehingga harus dikonsultasikan kepada dokter penulis resep.

In this study, the prescription review of Back-Referral Program (PRB) was carried out in asthma patient by examining administrative, pharmaceutical and clinical aspects. In administrative requirements, incomplete patient weight and doctor's initials were obtained. In the pharmaceutical study, the written prescription have appropriate dosage forms and dosage strengths. In clinical studies, there are duplications and interactions with several drugs. It can be concluded that there are still many observed discrepancies in prescription writing, so a doctor who prescribes it must be consulted."

"Dalam praktiknya, pengkajian resep dilakukan pada semua jenis obat yang diresepkan oleh dokter. Termasuk di antaranya yaitu obat-obatan untuk penyakit jamur pada kulit dan kepala. Infeksi jamur yang terjadi pada kulit dan kepala ini sering ditemukan di kehidupan sehari-hari dari keparahan yang ringan hingga berat. Tujuan dari tugas khusus ini yaitu untuk mengkaji aspek administrasi, farmasetika, dan klinis yang terdapat pada resep obat jamur kulit dan kepala yang ada di Apotek Atrika. Metode yang digunakan dalam pengerjaan laporan ini yaitu studi literatur dari berbagai sumber seperti buku ISO, MIMS, dan referensi mengenai obat lainnya. Dari hasil pengkajian dua resep untuk obat jamur kulit dan kepala, ditemukan bahwa pada aspek administrasi masih ada informasi yang tidak lengkap, namun pada aspek farmasetik dan klinis informasi yang dicantumkan sudah cukup lengkap dan sesuai.

The method used in preparing this report is the study of literature from various sources such as ISO books, MIMS, and other drug references. From the results of the study of two prescription for the medicine for skin and head fungus, it was found that in the administrative aspect there was still incomplete information, but in the pharmaceutical and clinical aspects the information included was quite complete and appropriate."

"Tanggung jawab seorang apoteker dalam pelayanan klinis salah satunya adalah memastikan bahwa terapi yang diperoleh pasien sudah optimal. Apoteker harus dapat mengidentifikasi adanya permasalahan terkait obat serta memberikan solusi dan mencegah terjadinya hal yang tidak diharapkan tersebut. Masalah terkait obat yang mungkin terjadi di antaranya interaksi obat dan efek samping obat. Interaksi obat dapat disebabkan karena pengobatan polifarmasi atau penggunaan beberapa macam obat (lima atau lebih) secara bersamaan setiap hari. Untuk mencegah masalah terkait obat yang mungkin terjadi, dilakukan pengkajian analisis efek samping dan interaksi obat terhadap resep polifarmasi. Metode yang digunakan dalam tugas khusus ini adalah deskriptif retrospektif menggunakan resep yang dilayani oleh Apotek Roxy Depok pada bulan April 2023. Sampel resep yang memenuhi kriteria selanjutnya dianalisis efek samping obat dan interaksinya menggunakan pangkalan data seperti MedScape dan drugs.com interaction checker. Hasil dari pengkajian tiga resep polifarmasi yang diperoleh, ditemukan potensi efek samping yang didominasi dengan efek samping umum pada gangguan gastrointestinal termasuk nyeri perut, diare, konstipasi dan muntah, serta gangguan lain seperti sakit kepala, kelelahan berlebihan, edema, dan mialgia. Berdasakan analisis interaksi obat, ditemukan potensi interaksi major sebesar 5,55%, interaksi moderate 77,78% dan interaksi minor 16.67%.

One of the pharmacist's responsibilities in clinical services is to ensure that therapy received by the patient is optimal. Pharmacists must be able to identify drug-related problems and provide solutions to prevent adverse events. Potential drug-related problems include drug interactions and drug adverse effects. Drug interactions can occur due to polypharmacy or the use of multiple medications (five or more) simultaneously on a daily basis. To prevent potential drug-related problems, an analysis of adverse effects and drug interactions for polypharmacy prescriptions was conducted. This study employed a descriptive retrospective methodology using prescriptions filled at Roxy Depok Pharmacy in April 2023. Eligible prescriptions were further analyzed for drug side effects and interactions using databases such as MedScape and drugs.com interaction checkers. The analysis of three polypharmacy prescriptions revealed potential side effects predominantly involving common gastrointestinal adverse events, including abdominal pain, diarrhea, constipation, and vomiting, as well as other effects such as headache, excessive fatigue, edema, and myalgia. Drug interaction analysis identified major interactions at 5.55%, moderate interactions at 77.78%, and minor interactions at 16.67%."

"Layanan resep elektronik adalah salah satu usaha yang dapat dilakukan untuk mencegah dan menurunkan kesalahan peresepan obat. Pada pelaksanaannya, pelayanan resep elektronik juga memiliki beberapa kelemahan, seperti kesalahan input obat oleh pemberi resep, biaya sistem terkait, serta kurangnya keamanan informasi medis pasien. Berdasarkan hal tersebut, perlu dilakukan evaluasi pelaksanaan pelayanan resep elektronik untuk mengembangkan dan menyempurnakan sistem yang telah ada demi meningkatkan keamanan pasien. Penelitian dilakukan secara kualitatif dengan studi literatur dan identifikasi masalah di lapangan sebagai sumber untuk mengevaluasi keabsahan atau legalitas dan keselamatan pasien dalam sistem peresepan elektronik. Hasil yang didapat menunjukkan bahwa penggunaan resep elektronik memberikan manfaat berupa penurunan kesalahan membaca obat, ketidaklengkapan penulisan informasi, serta kecepatan waktu pelayanan sehingga dapat meningkatkan keselamatan pasien. Permasalahan peresepan elektronik terkait keselamatan pasien berupa kesalahan input obat dapat diantisipasi dengan menambahkan fitur alarm atau peringatan terkait peresepan pada aplikasi peresepan elektronik meliputi indikasi, interaksi, dosis, serta efek samping yang dapat terjadi pada pasien sehingga angka kejadian medication error semakin dapat berkurang.

Electronic prescription services are one of the efforts that can be made to prevent and reduce drug prescribing errors. In practice, electronic prescription services also have several weaknesses, such as drug input errors by prescribers, related system costs, and the lack of security of patient medical information. Based on this, it is necessary to evaluate the implementation of electronic prescription services to develop and perfect the existing system to improve patient safety. The research was conducted qualitatively by studying the literature and identifying problems in the field as a source for evaluating the legitimacy or legality and patient safety of the electronic prescribing system. The results obtained show that the use of electronic prescriptions provides benefits in the form of reducing drug reading errors, incomplete information writing, and speed of service time so as to improve patient safety. Electronic prescribing problems related to patient safety in the form of drug input errors can be anticipated by adding prescribing-related alarm or warning features to electronic prescribing applications including indications, interactions, doses, and side effects that can occur in patients so that the incidence of medication errors can be reduced."
