"edication error (interaksi obat dan duplikasi terapi), ketidakpatuhan dan efek samping obat sehingga diperlukan pengkajian resep, baik adminitrasi, farmasetik, dan klinis untuk meminimalkan risiko medication error akibat pemberian obat polifarmasi. Laporan ini bertujuan untuk melakukan pengkajian resep yang meliputi kajian administrasi, kajian farmasetik, dan kajian klinis terhadap resep polifarmasi yang masuk ke Apotek Roxy Depok. Metode yang digunakan adalah studi literatur. Hasil yang diperoleh menunjukkan, bahwa aspek administrasi berupa data berat badan dan nomor telepon dokter perlu dilengkapi pada kedua resep dan dari aspek farmasetik perlu ditanyakan kembali kepada dokter yang meresepkan terkait bentuk sediaan (resep 1) dan data stabilitas obat (resep 1 dan 2). Dari aspek klinis pada resep 1 perlu dilakukan penambahan aturan pakai medixon dan alopurinol sementara resep 2 terdapat potensi interaksi obat.

The use of drugs in large quantities can lead to an increased risk of medication errors (drug interactions and duplication of therapy), non-compliance and side effects of drugs, so it is necessary to study prescriptions, both administrative, pharmaceutical, and clinical to minimize the risk of medication errors due to the administration of polypharmacy drugs. This report aims to conduct a prescription review which includes administrative review, pharmaceutical review, and clinical review of polypharmacy prescriptions that enter the Roxy Depok Pharmacy. The method used is literature study. The results obtained show that the administrative aspects in the form of weight data and the doctor's telephone number need to be completed on both prescriptions and from the pharmaceutical aspect it is necessary to ask the prescribing doctor regarding the dosage form (prescription 1) and drug stability data (recipes 1 and 2). From a clinical aspect, prescription 1 needs to be added to the rules for using medixon and allopurinol, while prescription 2 has the potential for drug interactions."

Pengkajian dan pelayanan resep merupakan kegiatan pertama dalam memberikan pelayanan farmasi klinik. Tujuan penulisan ini adalah untuk memperoleh gambaran terkait kelengkapan resep di Apotek Roxy Depok berdasarkan pengkajian administratif, farmasetika, dan klinik periode Januari 2023. Metode pelaksanaan dilakukan secara retrospektif dengan melakukan pengkajian pada beberapa resep. Berdasarkan pengkajian, diperoleh bahwa resep telah memenuhi aspek administratif, farmasetik, dan klinik. Namun, masih ditemukan ketidaklengkapan terkait aspek administratif pada identitas dokter penulis resep.

A pharmacy is a pharmaceutical service facility where pharmaceutical practice is carried out by pharmacists. Reviewing and providing prescription services is the first activity in providing clinical pharmacy services. The purpose of this writing is to obtain an overview regarding the completeness of prescriptions at Apotek Roxy Depok based on administrative, pharmaceutical and clinical studies for the period January 2023. The implementation method is carried out retrospectively by reviewing several prescriptions. Based on the assessment, it was found that the prescription met administrative, pharmaceutical and clinical aspects. However,incompleteness was still found regarding the administrative aspects of the identity of the prescribing doctor."

"Apotek merupakan salah satu sarana pelayanan kefarmasian bagi apoteker dan tenaga kefarmasian lain. Pelayanan yang dimaksud berupa pelayanan langsung dan bertanggung jawab kepada pasien terkait sediaan farmasi yang bertujuan untuk mencapai hasil yang pasti dalam meningkatkan mutu kehidupan pasien (Kemenkes RI, 2017). Tujuan penulisan tugas khusus ini adalah agar penluis dapat memahami pelaksanaan pengkajian resep yang dilaksanakan di Apotek Roxy Pitara sesuai dengan standar yang telah ditetapkan oleh pemerintah yaitu Peraturan Menteri Kesehatan Republik Indonesia Nomor 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek Berdasarkan analisis di atas dapat disimpulkan bahwa resep pertama dan kedua tidak lengkap dalam aspek administratif. Ditemukan juga interaksi obat pada resep pertama yaitu peningkatan risiko hipoglikemia pada penggunaan Diamicron DR dan Trajenta dan pada resep kedua yaitu peningkatan risiko ulkus peptik dan pendarahan pada penggunaan Pirofel dan Prednisone secara bersamaan.

Pharmacy is a pharmaceutical service facility for pharmacists and other pharmaceutical personnel. The service in question is in the form of direct and responsible service to patients related to pharmaceutical preparations which aim to achieve definite results in improving the quality of life of patients (RI Ministry of Health, 2017). The purpose of writing this special assignment is so that the writer can understand the implementation of the prescription review carried out at the Roxy Pitara Pharmacy in accordance with the standards set by the government, namely the Regulation of the Minister of Health of the Republic of Indonesia Number 73 of 2016 concerning Pharmaceutical Service Standards at Pharmacies. Based on the analysis above, it can be concluded that the first and second prescriptions were incomplete in the administrative aspect. Drug interactions were also found in the first prescription, namely an increased risk of hypoglycemia when using Diamicron DR and Trajenta and in the second prescription, an increased risk of peptic ulcer and bleeding when using Pirofel and Prednisone together."

Tujuan pengkajian resep adalah untuk menganalisa adanya masalah terkait obat dan dilakukan untuk semua resep yang masuk tanpa ada kriteria khusus pasien. Pengkajian resep dapat dilakukan di fasilitas kesehatan seperti rumah sakit, apotek, dan puskesmas. Pada tugas khusus ini, pengkajian resep dilakukan untuk pasien dengan ulkus kornea di Apotek Roxy Sawangan. Ulkus kornea merupakan defek epitel kornea sampai stroma yang disertai dengan inflamasi. Ulkus kornea secara garis besar dibagi berdasarkan penyebabnya yaitu infeksi dan non-infeksi. Beberapa gejala dari ulkus kornea bakteri yang dapat dialami pasien adalah mata nyeri, kemerahan, penurunan visus, fotofobia, lakrimasi dan sensasi benda asing. Terapi farmakologi untuk pasien ulkus kornea bakteri adalah antibiotik golongan fluorokuinolon atau kombinasi fortified antibiotic topical. Berdasarkan pengkajian resep Nyonya D di Apotek Roxy Sawangan, dapat disimpulkan bahwa tidak ada medication error pada resep Nyonya D. Akan tetapi, terdapat ketidaksesuaian aspek administratif pada resep Nyonya D. Kemudian, berdasarkan aspek farmaseutik dan klinis yang telah dikaji dapat disimpulkan bahwa Nyonya D mengalami ulkus kornea yang disebabkan infeksi bakteri.

Prescription review is a series of activities consists of receiving, stock assessment, prescription review, preparing the medicine including compounding, dispensing, and providing information about the medicine to the patient. The purpose of prescription review is to analyse medication error and to be conducted for all prescription without any special criteria. Prescription review can be done in health care facilities such as hospital, drugstore, and public health center. On this report, prescription review was done for patient with corneal ulcer in Apotek Roxy Sawangan. Corneal ulcer is a defect in cornea epithelium until stroma with inflammation. In general, corneal ulcer classified based on its cause, with infection and non-infection. Some of the symptoms of bacterial corneal ulcers are eye pain, redness. Pharmacological therapy for patient with bacterial cornea ulcer is fluoroquinolone antibiotic or combination fortified antibiotic topical. Based on prescription review of Miss D in Apotek Roxy Sawangan, it can be concluded that there is no medication error in Miss D’s prescripition. However, there was discrepancy of administrative aspects in Miss D’s prescripition. Furthermore, based on pharmaceutical and clinical aspects of Miss D’s prescription, it can be concluded that Miss D has corneal ulcer caused by bacterial infection."

"Efek samping obat merupakan salah satu masalah terkait obat (drug related problems) terbesar yang sering ditemui pada pasien dan kemungkinan terjadinya lebih tinggi pada resep-resep polifarmasi yang penggunaan obatnya dalam jumlah banyak. Meski begitu, admisi rumah sakit karena efek samping obat masih dapat dicegah. Oleh karena itu, efek samping sebaiknya dicatat, didefinisikan, dan dikaji untuk meminimalkan risiko kemunculan ulang dan mengoptimalkan hasil pengobatan, terutama pada pasien-pasien lanjut usia dengan resep polifarmasi. Pemantauan efek samping obat ini harus dilakukan secara berkesinambungan untuk mengevaluasi konsistensi profil keamanannya, dan membutuhkan banyak kontribusi dari tenaga kesehatan, terutama apoteker, bahkan pasien. Tugas khusus ini dilakukan untuk melihat kemungkinan adanya efek samping dan/atau interaksi pada resep-resep polifarmasi periode Desember 2022 yang masuk ke Apotek Roxy Depok dengan skrining sampel resep polifarmasi di Apotek Roxy Depok periode Desember 2022 dan studi literatur mengenai efek samping dan interaksi obat pada resep tersebut. Untuk tugas akhir ini, diambil 2 sampel resep polifarmasi untuk dilihat kemungkinan efek samping dan/atau interaksi obatnya. Dari kedua sampel resep polifarmasi tersebut, terdapat kemungkinan efek samping maupun interaksi obat, tetapi masih dapat dicegah dan diatasi dengan berbagai cara.

Drug side effects are one of the biggest drug-related problems that are often encountered in patients. The possibility of drug side effects occurrence is also higher in polypharmacy prescriptions that involve large amounts of medication. Even so, hospital admissions due to drug side effects can still be prevented. Therefore, side effects should be recorded, defined, and assessed to minimize the risk of recurrence and optimize treatment outcomes, especially in elderly patients prescribed with polypharmacy. Monitoring the drug's side effects should be carried out continuously to evaluate the consistency of its safety profile, and requires a lot of contribution from health workers, especially pharmacists, and even patients. This project was carried out to see the possibility of drug side effects and/or interactions in polypharmacy prescriptions received by Apotek Roxy Depok throughout December 2022. This project was done by screening samples of polypharmacy prescriptions at the Roxy Depok Pharmacy for the December 2022 period and studying the literature regarding side effects and drug interactions in the prescriptions. 2 samples of polypharmacy prescriptions were taken and from those two samples, there is the possibility of side effects and drugs interactions, but they can still be prevented and overcome in various ways."

"Interaksi obat atau Drugs-Drugs Interaction (DDIs) merupakan kondisi terjadinya interaksi antara beberapa obat yang dikonsumsi bersamaan dan mengakibatkan adanya perubahan efek samping obat. Salah satu penyebab interaksi obat yaitu penggunaan obat dalam jumlah banyak dan tidak sesuai dengan kondisi pasien atau biasa disebut dengan polifarmasi. Pasien yang menerima lebih dari satu obat yang berpotensi mengalami DDIs menjadi perhatian tenaga kesehatan terutama bagi penulis resep untuk meningkatkan efektivitas terapi dan menghindari terjadinya efek obat yang tidak diharapkan. Penelitian ini bertujuan untuk mengetahui risiko efek samping dan interaksi obat pada resep polifarmasi di Apotek Roxy Depok. Metode penelitian yang dilakukan yaitu observasional deskriptif dengan rancangan cross-sectional yang dilakukan dalam sekali waktu. Sampel pada penelitian menggunakan data resep polifarmasi rawat jalan di Apotek Roxy Depok pada bulan November 2022. Pengambilan sampel menggunakan Teknik convenience sampling dan diperoleh sampel sebanyak 2 resep obat yang kemudian diidentifikasi terkait potensi efek samping dan interaksi obat pada masing-masing obat dalam resep. Hasil penelitian menunjukkan bahwa terdapat 4 jenis potensi interaksi obat dalam 2 resep polifarmasi pasen rawat jalan di apotek Roxy Depok yang dilihat berdasarkan mekanisme dan tingkatannya. Berdasarkan mekansime, mayoritas potensi interaksi obat yang ditemukan pada resep polifarmasi di apotek Roxy Depok adalah interaksi farmakodinamik (50%), sementara berdasarkan tingkatannya mayoritas adalah interaksi moderate (75%). Kesimpulan yang diperoleh yaitu terdapat potensi efek samping pada masing-masing obat dalam 2 resep polifarmasi pasien rawat jalan di Apotek Roxy Depok dengan level yang variatif.

Drug interactions or Drugs-Drugs Interactions (DDIs) are conditions where interactions occur between several drugs taken simultaneously and result in changes in drug side effects. One of the causes of drug interactions is the use of drugs in large quantities that are not appropriate to the patient's condition or what is usually called polypharmacy. Patients who receive more than one drug that has the potential to experience DDIs are a concern for health workers, especially for prescribers, to increase the effectiveness of therapy and avoid unexpected drug effects. This study aims to determine the risk of side effects and drug interactions in polypharmacy prescriptions at the Roxy Depok Pharmacy. The research method used was descriptive observational with a cross-sectional design carried out at one time. The sample in the study used outpatient polypharmacy prescription data at the Roxy Depok Pharmacy in November 2022. Sampling used a convenience sampling technique and a sample of 2 drug prescriptions was obtained which was then identified regarding potential side effects and drug interactions for each drug in the prescription. The results of the study showed that there were 4 types of potential drug interactions in 2 outpatient polypharmacy prescriptions at the Roxy Depok pharmacy, which were seen based on the mechanism and level. Based on the mechanism, the majority of potential drug interactions found in polypharmacy prescriptions at the Roxy Depok pharmacy are pharmacodynamic interactions (50%), while based on the level the majority are moderate interactions (75%). The conclusion obtained was that there were potential side effects for each drug in the 2 outpatient polypharmacy prescriptions at the Roxy Depok Pharmacy with varying levels."
