"Penelitian ini bertujuan menganalisis efek samping dan interaksi obat pada resep polifarmasi di Apotek Roxy Depok selama Maret 2023. Metode penelitian menggunakan convenience sampling dengan total 3 resep. Hasilnya menunjukkan adanya interaksi obat signifikan. Pada resep pertama, terdapat 1 interaksi obat mayor (Captopril dan Spironolakton) dan 10 interaksi obat moderate, seperti Asam Asetilsalisilat dengan Captopril, Clopidogrel, dan Furosemide. Pada resep kedua, terdapat 1 interaksi obat mayor (Candesartan dan Spironolakton) dan 1 interaksi obat moderate (Bisoprolol Fumarate dan Ivabradin). Pada resep ketiga, terdapat 3 interaksi obat moderate(Amlodipin dengan Clopidogrel dan Fenitoin). Implikasi penelitian ini adalah kesadaran penting terhadap potensi efek samping dan interaksi obat pada resep polifarmasi. Interaksi obat dapat meningkatkan risiko efek samping serius, seperti hipotensi, nefrotoksik, hiperkalemia, dan gangguan gastrointestinal. Pengetahuan mengenai interaksi obat pada resep polifarmasi dapat membantu apoteker dan tenaga medis dalam mengevaluasi risiko manfaat obat yang diresepkan kepada pasien. Kesimpulan penelitian ini adalah terdapat 19 interaksi obat pada resep polifarmasi yang dianalisis, terdiri dari 1 interaksi mayor dan 18 interaksi moderate. Potensi efek samping obat meningkat akibat interaksi obat atau akumulasi efek samping dari penggunaan obat polifarmasi. Penting bagi tenaga medis mempertimbangkan efek samping dan interaksi obat saat meresepkan obat polifarmasi kepada pasien, serta melakukan pemantauan yang tepat terhadap pasien yang mengonsumsi obat tersebut.

"Interaksi obat atau Drugs-Drugs Interaction (DDIs) merupakan kondisi terjadinya interaksi antara beberapa obat yang dikonsumsi bersamaan dan mengakibatkan adanya perubahan efek samping obat. Salah satu penyebab interaksi obat yaitu penggunaan obat dalam jumlah banyak dan tidak sesuai dengan kondisi pasien atau biasa disebut dengan polifarmasi. Pasien yang menerima lebih dari satu obat yang berpotensi mengalami DDIs menjadi perhatian tenaga kesehatan terutama bagi penulis resep untuk meningkatkan efektivitas terapi dan menghindari terjadinya efek obat yang tidak diharapkan. Penelitian ini bertujuan untuk mengetahui risiko efek samping dan interaksi obat pada resep polifarmasi di Apotek Roxy Depok. Metode penelitian yang dilakukan yaitu observasional deskriptif dengan rancangan cross-sectional yang dilakukan dalam sekali waktu. Sampel pada penelitian menggunakan data resep polifarmasi rawat jalan di Apotek Roxy Depok pada bulan November 2022. Pengambilan sampel menggunakan Teknik convenience sampling dan diperoleh sampel sebanyak 2 resep obat yang kemudian diidentifikasi terkait potensi efek samping dan interaksi obat pada masing-masing obat dalam resep. Hasil penelitian menunjukkan bahwa terdapat 4 jenis potensi interaksi obat dalam 2 resep polifarmasi pasen rawat jalan di apotek Roxy Depok yang dilihat berdasarkan mekanisme dan tingkatannya. Berdasarkan mekansime, mayoritas potensi interaksi obat yang ditemukan pada resep polifarmasi di apotek Roxy Depok adalah interaksi farmakodinamik (50%), sementara berdasarkan tingkatannya mayoritas adalah interaksi moderate (75%). Kesimpulan yang diperoleh yaitu terdapat potensi efek samping pada masing-masing obat dalam 2 resep polifarmasi pasien rawat jalan di Apotek Roxy Depok dengan level yang variatif.

Drug interactions or Drugs-Drugs Interactions (DDIs) are conditions where interactions occur between several drugs taken simultaneously and result in changes in drug side effects. One of the causes of drug interactions is the use of drugs in large quantities that are not appropriate to the patient's condition or what is usually called polypharmacy. Patients who receive more than one drug that has the potential to experience DDIs are a concern for health workers, especially for prescribers, to increase the effectiveness of therapy and avoid unexpected drug effects. This study aims to determine the risk of side effects and drug interactions in polypharmacy prescriptions at the Roxy Depok Pharmacy. The research method used was descriptive observational with a cross-sectional design carried out at one time. The sample in the study used outpatient polypharmacy prescription data at the Roxy Depok Pharmacy in November 2022. Sampling used a convenience sampling technique and a sample of 2 drug prescriptions was obtained which was then identified regarding potential side effects and drug interactions for each drug in the prescription. The results of the study showed that there were 4 types of potential drug interactions in 2 outpatient polypharmacy prescriptions at the Roxy Depok pharmacy, which were seen based on the mechanism and level. Based on the mechanism, the majority of potential drug interactions found in polypharmacy prescriptions at the Roxy Depok pharmacy are pharmacodynamic interactions (50%), while based on the level the majority are moderate interactions (75%). The conclusion obtained was that there were potential side effects for each drug in the 2 outpatient polypharmacy prescriptions at the Roxy Depok Pharmacy with varying levels."

"Tanggung jawab seorang apoteker dalam pelayanan klinis salah satunya adalah memastikan bahwa terapi yang diperoleh pasien sudah optimal. Apoteker harus dapat mengidentifikasi adanya permasalahan terkait obat serta memberikan solusi dan mencegah terjadinya hal yang tidak diharapkan tersebut. Masalah terkait obat yang mungkin terjadi di antaranya interaksi obat dan efek samping obat. Interaksi obat dapat disebabkan karena pengobatan polifarmasi atau penggunaan beberapa macam obat (lima atau lebih) secara bersamaan setiap hari. Untuk mencegah masalah terkait obat yang mungkin terjadi, dilakukan pengkajian analisis efek samping dan interaksi obat terhadap resep polifarmasi. Metode yang digunakan dalam tugas khusus ini adalah deskriptif retrospektif menggunakan resep yang dilayani oleh Apotek Roxy Depok pada bulan April 2023. Sampel resep yang memenuhi kriteria selanjutnya dianalisis efek samping obat dan interaksinya menggunakan pangkalan data seperti MedScape dan drugs.com interaction checker. Hasil dari pengkajian tiga resep polifarmasi yang diperoleh, ditemukan potensi efek samping yang didominasi dengan efek samping umum pada gangguan gastrointestinal termasuk nyeri perut, diare, konstipasi dan muntah, serta gangguan lain seperti sakit kepala, kelelahan berlebihan, edema, dan mialgia. Berdasakan analisis interaksi obat, ditemukan potensi interaksi major sebesar 5,55%, interaksi moderate 77,78% dan interaksi minor 16.67%.

One of the pharmacist's responsibilities in clinical services is to ensure that therapy received by the patient is optimal. Pharmacists must be able to identify drug-related problems and provide solutions to prevent adverse events. Potential drug-related problems include drug interactions and drug adverse effects. Drug interactions can occur due to polypharmacy or the use of multiple medications (five or more) simultaneously on a daily basis. To prevent potential drug-related problems, an analysis of adverse effects and drug interactions for polypharmacy prescriptions was conducted. This study employed a descriptive retrospective methodology using prescriptions filled at Roxy Depok Pharmacy in April 2023. Eligible prescriptions were further analyzed for drug side effects and interactions using databases such as MedScape and drugs.com interaction checkers. The analysis of three polypharmacy prescriptions revealed potential side effects predominantly involving common gastrointestinal adverse events, including abdominal pain, diarrhea, constipation, and vomiting, as well as other effects such as headache, excessive fatigue, edema, and myalgia. Drug interaction analysis identified major interactions at 5.55%, moderate interactions at 77.78%, and minor interactions at 16.67%."

"Dalam meningkatkan mutu maupun cakupan pelayanan kefarmasian yang baik di apotek, standar pelayanan kefarmasian perlu diperhatikan agar sesuai dengan protokol untuk pelayanan pelanggan di apotek tersebut. Bentuk pelayanan resep, mulai dari penerimaan, pemeriksaan ketersediaan, penyiapan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai termasuk peracikan obat, pemeriksaan, hingga penyerahan obat pada pasien perlu disertai pemberian informasi yang tepat pada pasien. Hal tersebut agar pengobatan yang diberikan pada pasien tepat dan sesuai harapan terapi yang diinginkan. Pada setiap tahap alur pelayanan resep juga perlu dipastikan sebagai upaya pencegahan terjadinya kesalahan pemberian obat (medication error), salah satunya terjadinya interaksi obat maupun efek samping yang tidak diinginkan. Pengamatan terkait efek samping dan interaksi obat pada obat-obatan yang diresepkan pasien dilakukan pada lima resep polifarmasi racikan yang diberikan per Januari 2023. Obat-obatan yang diresepkan tersebut biasanya terdiri atas lima atau lebih kandungan zat aktif obat yang dilakukan peracikan untuk dijadikan bentuk sediaan lain yang lebih praktis digunakan pasien. Beberapa resep obat racikan polifarmasi yang telah diamati dapat berpotensi menimbulkan interaksi obat, baik interaksi dalam level menengah (intermediate) maupun level mayor serta beberapa efek samping tidak diinginkan dapat timbul dari penggunaan obat, baik sebagai akibat dari penggunaan bersamaan dengan obat lain maupun akibat penggunaan jangka panjang.

In improving the quality and scope of good pharmaceutical services in pharmacies, it is necessary to pay attention to pharmaceutical service standards in accordance with the protocols for customer service in these pharmacies. The form of prescription services, starting from reception, checking availability, preparation of pharmaceutical preparations, medical devices and consumable medical materials including dispensing drugs, examinations, to dispensing drugs to patients needs to be accompanied by providing appropriate information to patients. This is so that the treatment given to the patient is appropriate and according to the expectations of the desired therapy. At each stage of the prescription service flows it is also necessary to ensure as an effort to prevent medication errors, one of which is the occurrence of drug interactions and unwanted side effects. Observations related to side effects and drug interactions for medicines prescribed by patients were carried out on five concoction polypharmacy prescriptions given as of January 2023. The prescribed medicines usually consist of five or more active ingredients of the drug which are compounded to be made into other dosage forms which is more practical for patients to use. Several prescriptions for polypharmacy concoctions that have been observed have the potential to cause drug interactions, both interactions at the intermediate level and major level and some unwanted side effects can arise from drug use, either as a result of concomitant use with other drugs or as a result of long- term use."

"Polifarmasi merupakan penggunaan bersamaan enam obat atau lebih oleh seorang pasien. Semakin banyak obat yang digunakan maka semakin tinggi potensi interaksi yang terjadi dan dapat menyebabkan efek samping yang berbahaya. Tujuan dari penelitian ini adalah mengetahui resiko efek samping dan potensi interaksi obat pada resep polifarmasi nonracikan. Identifikasi terkait potensi efek samping dan interaksi obat dinilai berdasarkan database dari Medscape Drug Interaction Checker, RxList Drug Interaction Checker, drugs.com Drug Interaction Checker, dan WebMD Drug Interaction Checker. Pada resep polifarmasi nonracik pertama ditemukan potensi efek samping berupa ketidakseimbangan elektrolit, hipotensi, hiperurisemia, asam urat, sakit kepala, dan mual/muntah serta terdapat dua potensi interaksi major dan empat potensi interaksi moderate. Sementara pada resep polifarmasi nonracik kedua ditemukan potensi efek samping berupa pencernaan yang terganggu, mengantuk, tremor otot rangka, peningkatan BUN atau kreatinin, dan bronkospasme serta terdapat lima potensi interaksi moderate. Berdasarkan mekansimenya, mayoritas potensi interaksi obat yang ditemukan adalah interaksi farmakodinamik (45.4%). Sementara berdasarkan tingkatannya, mayoritas potensi interaksi obat yang ditemukan adalah interaksi moderate (81.8%).

Polypharmacy is the concomitant use of six or more drugs by a patient. The more drugs used, the higher the potential for interactions that occur and can cause dangerous side effects. The purpose of this study was to determine the risk of side effects and potential drug interactions in nonconcocted polypharmacy prescriptions. Identification of potential side effects and drug interactions is assessed based on databases from Medscape Drug Interaction Checker, RxList Drug Interaction Checker, drugs.com Drug Interaction Checker, and WebMD Drug Interaction Checker. In the first non-mixed polypharmacy prescription, potential side effects were found in the form of electrolyte imbalance, hypotension, hyperuricemia, gout, headache, nausea, and vomiting, and there were two potential major interactions and four potential moderate interactions. While the second non-mixed polypharmaceutical prescription found potential side effects in the form of disturbed digestion, drowsiness, skeletal muscle tremors, increased BUN or creatinine, and bronchospasm, there were five potential moderate interactions. Based on the mechanism, the majority of potential drug interactions found were pharmacodynamic interactions (45.4%). Based on the level, the majority of potential drug interactions found were moderate (81.8%)."

"Apoteker dituntut harus memiliki kompetensi yang memadai dalam menjalankan tugas dan tanggung jawab kefarmasian di apotek. Oleh karena itu, calon apoteker harus dipersiapkan dengan baik sebelum memegang tugas dan tanggung jawab tersebut. Salah satu program untuk mempersiapkan calon apoteker yaitu melalui Praktik Kerja Profesi Apoteker (PKPA). Pelaksanaan PKPA dilakukan mulai tanggal 3-31 Juli 2023 di Apotek Roxy Depok. Metode yang digunakan dalam penyusunan laporan tugas khusus ini menggunakan metode berupa studi literatur dan penelusuran terkait interaksi dan efek samping obat yang dapat terjadi pada resep polifarmasi pada periode bulan Juli 2023. Berdasarkan 5 resep yakng dikaji, hasil penelitian menunjukkan adanya interaksi yang bersifat moderat pada resep 2,4 dan 5. Oleh karena itu interaksi tersebut perlu dimonitor agak tidak membahayakan keselamatan pasien. Apoteker memiliki peran penting untuk memberikan konseling bagi pasien yang mendapatkan resep polifarmasi serta berusahan untuk melakukan kerja sama yang baik dengan dokter pembuat resep dalam rangka memberikan pengobatan yang efektif dan aman bagi pasien.Peran apoteker dalam meminimalkan risiko obat yang membahayakan bagi pasien sangat diperlukan. Apoteker dapat memaksimalkan konseling terhadap pasien untuk menyampaikan efek samping yang dapat terjadi terkait dengan obat serta penanganannya. Selain itu apoteker memiliki peran penting untuk mencegah adanya interaksi yang membahayakan pasien terkait polifarmasi yakni dengan memaksimalkan kolaborasi dengan dokter pembuat resep terkait pengobatan yang efektif bagi pasien.

Therefore, prospective pharmacists must be well-prepared before assuming these tasks and responsibilities. One program aimed at preparing prospective pharmacists is through the Pharmacist Professional Internship Program (PKPA). The PKPA was conducted from July 3-31, 2023, at Roxy Pharmacy Depok. The method used in preparing this specific task report employed literature review and investigation methods related to drug interactions and side effects that may occur in polypharmacy prescriptions during the July 2023 period. Based on the analysis of five prescriptions, the study results showed moderate drug interactions in prescriptions 2, 4, and 5. Therefore, these interactions need to be monitored to avoid endangering patient safety. Pharmacists play a crucial role in counseling patients receiving polypharmacy prescriptions and strive to collaborate effectively with prescribing doctors to ensure effective and safe treatment for patients. The pharmacist's role in minimizing the risk of dangerous drug effects for patients is essential. Pharmacists can maximize counseling for patients to convey potential drug-related side effects and their management. Additionally, pharmacists play a critical role in preventing dangerous interactions related to polypharmacy by maximizing collaboration with prescribing doctors regarding effective patient treatment."

"Pelayanan kefarmasian di apotek saat ini berorientasi pada kebutuhan pasien berdasarkan kondisi klinis atau penyakit. Pada beberapa kasus, pasien yang menebus resep pada apotek memiliki jumlah obat melebihi satu atau resep polifarmasi sehingga apoteker berperan dalam pengkajian dan analisis terkait efek samping dan interaksi dari obat-obat setiap bulan. Pengkajian dilakukan pada dua resep polifarmasi bulan Februari 2023 di Apotek Roxy Depok secara klinis melalui literatur klinis kemudian dilanjutkan dengan analisis mekanisme masing-masing obat dan mekanisme interaksi terhadap obat-obat lainnya terhadap kondisi klinis pasien sehingga diperoleh rekomendasi peresepan polifarmasi. Berdasarkan proses tersebut, resep polifarmasi 1 diperoleh 4 jenis efek samping yang tidak diresepkan obat lain dan 6 jenis interaksi obat yang perlu diedukasi serta resep polifarmasi 2 diperoleh 3 jenis efek samping yang tidak diresepkan obat lain, 1 jenis efek samping yang diresepkan obat lain, dan 1 jenis interaksi obat yang perlu diedukasi.

Pharmaceutical services in pharmacies are currently oriented towards the needs of patients based on clinical conditions or diseases. In some cases, patients who fill prescriptions at the pharmacy have more than one drug or polypharmacy prescriptions, so pharmacists play a role in assessing and analyzing the side effects and interactions of drugs every month. The review was conducted on two polypharmacy prescriptions in February 2023 in Roxy Pharmacy Depok clinically through clinical literature then continued with the analysis of the mechanism of each drug and the mechanism of interaction with other drugs against the patient's clinical condition to obtain recommendations for polypharmacy prescribing. Based on this process, polypharmacy prescription 1 obtained 4 types of side effects that were not prescribed other drugs and 6 types of drug interactions that needed to be educated and polypharmacy prescription 2 obtained 3 types of side effects that were not prescribed other drugs, 1 type of side effect that was prescribed other drugs, and 1 type of drug interaction that needed to be educated."

"Meningkatnya kejadian interaksi obat pada pola peresepan dapat disebabkan banyaknya obat yang sering digunakan / polifarmasi. Polifarmasi dapat menimbulkan risiko yaitu terjadinya interaksi obat. Interaksi obat dapat menyebabkan beberapa hal seperti penurunan efek terapi, peningkatan toksisitas atau efek farmakologis yang tidak diharapkan. Pengumpulan data didasarkan pada resep rawat jalan dengan skrining 50 resep polifarmasi yang kemudian dipilih resep polifarmasi pada pasien dengan diagnosa penyakit kardiovaskuler. Resep polifarmasi tersebut diidentifikasi melalui studi pustaka menggunakan pustaka drug interaction checker seperti Stockley’s Drug Interaction, Lexicomp Drug Interactions, Medscape, dan Drugs.com. Peresepan polifarmasi dari 3 resep menunjukkan pada R1 potensi interaksi obat pada kategori minor, pada R2 potensi interaksi obat yaitu kategori mayor dan R3 potensi interaksi obat yaitu kategori minor sebanyak 1 dan kategori moderate sebanyak 4, dengan setiap resep memiliki beberapa efek samping yang perlu diperhatikan. Langkah untuk meminimalkan interaksi obat dan efek samping yang mungkin terjadi yaitu dengan menghindari penggunaan obat secara bersamaan yang memiliki potensi menimbulkan interaksi, memberikan jeda penggunaan obat 2 jam, serta melakukan monitoring gejala klinis atau data laboratorium pasien terkait efek dari penggunaan obat.

The increasing incidence of drug interactions in prescribing patterns can be caused by the large number of drugs frequently used / polypharmacy. Polypharmacy can pose a risk, namely drug interactions. Drug interactions can cause several things such as decreased therapeutic effect, increased toxicity or unexpected pharmacological effects. Data collection was based on outpatient prescriptions by screening 50 polypharmacy prescriptions which then selected polypharmacy prescriptions in patients diagnosed with cardiovascular disease. These polypharmacy prescriptions were identified through literature studies using drug interaction checker libraries such as Stockley's Drug Interaction, Lexicomp Drug Interactions, Medscape, and Drugs.com. Prescribing polypharmacy from 3 prescriptions shows that in R1 the potential for drug interactions is in the minor category, in R2 the potential for drug interactions is in the major category and in R3 the potential for drug interactions is in the minor category of 1 and the moderate category is 4, with each prescription having several side effects that need to be taken into account. Steps to minimize drug interactions and side effects that may occur are by avoiding simultaneous use of drugs that have the potential to cause interactions, giving a 2 hour break from drug use, and monitoring the patient's clinical symptoms or laboratory data regarding the effects of drug use."
