"Resep merupakan salah satu bagian dari pelayanan kefarmasian. Resep yang baik harus memiliki cukup informasi untuk memungkinkan ahli farmasi mengerti obat apa yang harus diberikan kepada pasien. Penulisan resep yang tidak jelas dapat memicu kesalahan interpretasi yang dapat berefek pada terjadinya kesalahan medikasi (medication error) sehingga berdampak fatal bagi pasien. Pengkajian resep dilakukan untuk meminimalisir kemungkinan timbulnya permasalahan terkait obat. Pengerjaan tugas khusus ini menggunakan metode kualitatif melalui studi literatur sebagai acuan dan metode observasi dengan melakukan pengkajian atau skrining terhadap resep yang ada di Apotek Kimia Farma Salemba. Resep yang diambil bersifat acak yaitu resep pembelian tunai, resep asuransi kesehatan seperti BPJS, Mandiri Inhealth, Pertalife dan resep Klinik Tunggal. Aspek yang dikaji berupa aspek administrasi, farmasetik dan klinik. Kesalahan peresepan terbanyak pada aspek administrasi yaitu tidak dicantumkan keterangan berat badan pasien. Aspek farmasetik yaitu keterangan aturan pakai dan cara penggunaan obat yang belum terdapat pada semua item obat. Sedangkan pada aspek klinis, kesalahan peresepan terbanyak yaitu tidak adanya keterangan lama pemakaian obat.

Prescription is one part of pharmaceutical services. A good prescription should have enough information to enable the pharmacist to understand appropriate medication given to the patient. Unclear prescription can lead to misinterpretation, which result in medication errors that are fatal to the patient. Prescription-screening is done to minimize the possibility of drug-related problems. This assignment uses a qualitative method through literature study as a reference and observation method by assessing or screening existing prescriptions at Kimia Farma Salemba Pharmacy. Prescriptions were taken randomly, cash purchase prescriptions, health insurance prescriptions such as BPJS, Mandiri Inhealth, Pertalife and Tunggal Clinic prescriptions. The aspects studied were administrative, pharmaceutical and clinical aspects. The most common prescribing error in the administrative aspect is that the patient's weight information is not included. The pharmaceutical aspect is the use of medication that are not yet contained in all drug items. While in the clinical aspect, the most prescribing error was the absence of information on the duration of drug use."

"Apotek merupakan fasilitas kesehatan yang melakukan pelayanan yang salah satunya adalah pelayanan untuk farmasi klinik. Kegiatan pelayananan farmasi klinik yang terdapat di Apotek adalah bagian dari kegiatan Pelayanan Kefarmasian yang berhubungan langsung serta bertanggung jawab terhadap pasien. Pengkajian dan pelayanan resep merupakan salah satu pelayanan farmasi klinik yang ada di apotek. Kegiatan pengkajian Resep yang ada di apotek dibagi menjadi tiga, meliputi administrasi, kesesuaian farmasetik dan pertimbangan klinis. Tujuan pengkajian berdasarkan tiga aspek tersebut untuk menghindari terjadinya kesalahan dalam pelayanan obat (medication error). Ada beberapa kesalahan pengobatan atau medication error yang sering terjadi seperti pemberian obat yang tidak tepat, pemberian dosis obat yang salah, kesalahan dalam menulis atau kesamaan bunyi nama obat, kesalahan pada penggunaan serta kesalahan pada perhitungan dosis obat yang diresepkan. Penelitian ini bertujuan untuk melakukan pengkajian 10 resep racikan dan 10 resep non-racikan yang ada di Apotek Kimia Farma 494 Beji Depok pada periode bulan April 2023. Hasil kajian dari 20 resep yang dilakukan, permasalahan yang banyak terjadi terdapat pada aspek klinisnya seperti ada interaksi obat. Terdapat 3 jenis interaksi obat pada resep racikan dan non racikan yaitu interaksi mayor, moderat, dan minor.

Pharmacy is a healthcare facility that provides services, one of which is clinical pharmacy services. Clinical pharmacy services in pharmacies are part of pharmaceutical services directly related to and responsible for patient care. Prescription assessment and service are among the clinical pharmacy services available in pharmacies. Prescription assessment activities in pharmacies are divided into three parts: administrative, pharmaceutical suitability, and clinical consideration. The purpose of assessing prescriptions based on these three aspects is to prevent medication errors. Several common medication errors include incorrect drug administration, wrong drug dosage, errors in writing or sound-alike drug names, errors in drug use, and errors in calculating prescribed drug doses. This study aims to assess 10 compounded and 10 non-compounded prescriptions at Kimia Farma Pharmacy 494 Beji Depok during April 2023. From the study's results, it was found that clinical aspects such as drug interactions were the most prevalent issue. There were three types of drug interactions found in compounded and non-compounded prescriptions: major, moderate, and minor interactions."

"Hipertensi adalah salah satu penyakit yang menjadi masalah kesehatan terbesar di dunia, termasuk di Indonesia dengan prevalensi sebesar 25,8% pada tahun 2013. Apotek Kimia Farma Raya Bogor bekerja sama dengan Rumah Sakit Tugu Ibu dalam pemberian obat pada pasien BPJS rawat jalan. Dalam kajian ini, lima resep antihipertensi dipilih untuk dievaluasi dari aspek administrasi, farmasetis, dan klinis. Hasil pengkajian menunjukkan bahwa resep-resep tersebut memenuhi semua aspek yang diharuskan kecuali informasi berat badan yang tidak ada. Telaah klinis menunjukkan bahwa semua resep tepat indikasi, dosis, dan pemberiannya.

Hypertension is one of the major global health problems, including in Indonesia with a prevalence of 25.8% in 2013. Apotek Kimia Farma Raya Bogor collaborates with Tugu Ibu Hospital in providing medication for BPJS outpatients. In this study, five antihypertensive prescriptions were selected for evaluation from administrative, pharmaceutical, and clinical aspects. The evaluation results indicate that the prescriptions meet all required aspects except for the missing weight information. Clinical review shows that all prescriptions are appropriate in indication, dosage, and administration."

"Pengkajian resep termasuk dalam standar pelayanan kefarmasian di apotek. Kegiatan ini dilakukan untuk menghindari kesalahan pemberian obat (medication error). Apotek Kimia Farma 202 memiliki layanan pengkajian resep yang mencakup aspek administrasi, kesesuaian farmasetik, dan pertimbangan klinis. Diabetes Melitus (DM) adalah penyakit metabolik kronis ketika pankreas tidak mampu memproduksi cukup insulin atau ketika tubuh tidak mampu menggunakan insulin yang diproduksi secara efektif, yang menyebabkan peningkatan kadar gula darah. Terapi DMT2 didasarkan pada kondisi gula darah pasien, faktor pembiayaan, ketersediaan obat, efektivitas, efek samping, serta preferensi pasien. Pengkajian resep dilakukan pada resep yang ditebus di Apotek Kimia Farma 202 pada periode pelaksanaan Praktek Kerja Profesi Apoteker (PKPA), dan dipilih dua resep untuk laporan ini. Pada kedua resep tersebut, terdapat beberapa informasi yang belum tercantum seperti usia, berat badan pasien, dan SIP dokter dalam aspek administratif. Obat-obat yang diberikan tidak menunjukkan interaksi yang merugikan, baik pada resep 1 dan resep 2. Terdapat beberapa obat yang sebaiknya dihindari oleh pasien dengan riwayat hipersensitivitas, sehingga apoteker perlu melakukan konfirmasi terkait riwayat alergi ke pasien atau keluarga pasien. Informasi dari pengkajian aspek kesesuaian farmasetik dan pertimbangan klinis dapat sangat berguna untuk disampaikan saat penyerahan obat, sehingga dapat mengoptimalkan terapi. Pelayanan yang diberikan Apotek Kimia Farma 202 telah berjalan dengan baik. Pengkajian resep perlu terus dilakukan agar kesahalan dalam pengobatan pengobatan dapat dihindari.

Diabetes Mellitus (DM) is a chronic metabolic disease that occurs when the pancreas is unable to produce sufficient insulin or when the body cannot effectively use the insulin produced, leading to an increase in blood sugar levels. The therapy for T2DM is based on the patient's blood sugar condition, financial factors, drug availability, effectiveness, side effects, and patient preferences. Prescription assessment was conducted for prescriptions redeemed at Kimia Farma 202 Pharmacy during the implementation period of the Pharmacist Professional Internship Program, and two prescriptions were selected for this report. In both prescriptions, there was some missing information in the administrative aspect, such as the patient's age, weight, and the doctor's License to Practice. The medications prescribed did not show any harmful interactions, both in prescription 1 and prescription 2. Some drugs should be avoided by patients with a history of hypersensitivity, so pharmacists need to confirm allergy history with the patient or the patient's family. Information from the assessment of pharmaceutical suitability and clinical considerations can be very useful when giving the medication to optimize therapy. The services provided by Kimia Farma 202 Pharmacy have been running smoothly. Continuous prescription assessment is necessary to avoid errors in medication administration."

"Sebagai seorang apoteker, salah satu cara untuk memastikan pasien telah mendapatkan pengobatan yang rasional adalah melalui pengkajian resep, yang meliputi ketepatan penilaian kondisi pasien, tepat diagnosis, tepat indikasi, tepat jenis obat, tepat dosis, tepat cara dan lama pemberian, tepat informasi, dengan memperhatikan keterjangkauan harga, kepatuhan pasien, dan waspada efek samping. Pengkajian resep dilakukan berdasarkan aspek administratif, farmasetik dan klinis. Pengkajian resep dilakukan pada pasien yang menebus obat di Apotek Kimia Farma Dramaga 348 yang memiliki diagnosis diabetes tipe 2, hipertensi, dan diabetes tipe 2 yang disertai dengan hipertensi. Resep-resep yang akan dikaji umumnya berisi obat-obatan rutin pasien yang diberikan selama 1 bulan. Dari hasil kaji resep, Aspek administratif belum memenuhi persyaratan karena kurangnya informasi mengenai berat badan dan alamat pasien. Namun, kekurangan tersebut dapat diatasi dengan mengkonfirmasi kembali ke pasien dan dokter yang bersangkutan. Aspek farmasetik sudah memenuhi persyaratan, sementara aspek klinis belum memenuhi persyaratan karena ada beberapa obat yang saling berinteraksi dan merupakan polifarmasi. Namun, hal tersebut dapat diintervensi dengan mengedukasi pasien untuk menjeda konsumsi pemberian obat dan memotivasi pasien pentingnya untuk meminum obat secara rutin untuk mendapatkan hasil terapi yang optimal.

As a pharmacist, one way to ensure that patients have received rational treatment is through reviewing prescriptions, which includes accurate assessment of the patient's condition, correct diagnosis, correct indication, correct type of drug, correct dosage, correct method and duration of administration, correct information, with pay attention to affordability, patient compliance, and side effects. Prescription reviews are carried out based on administrative, pharmaceutical and clinical aspects. Prescription reviews were carried out on patients who redeemed medicines at Kimia Farma Dramaga 348 Pharmacy who had a diagnosis of type 2 diabetes, hypertension, and type 2 diabetes along with hypertension. The prescriptions that will be studied generally contain the patient's routine medicines that are given for 1 month. From the results of the prescription review, the administrative aspects did not meet the requirements due to lack of information regarding the patient's weight and address. However, this deficiency can be overcome by confirming it again with the patient and the doctor concerned. The pharmaceutical aspect meets the requirements, while the clinical aspect does not meet the requirements because there are several drugs that interact with each other and polypharmacy. However, this can be intervened by educating patients to separate the consume between interacted medications and motivating patients to the importance of taking medication regularly to get optimal therapeutic results."

"Pelayanan obat atas resep dokter merupakan salah satu dari pelayanan kefarmasian di Apotek. Resep yang sesuai harus memuat cukup informasi yang memungkinkan tenaga kefarmasian yang bersangkutan mengerti obat yang akan diberikan kepada pasiennya. Resep juga harus ditulis dengan jelas agar terhindar dari salah persepsi antara dokter dengan tenaga kefarmasian yang bersangkutan. Namun pada kenyataanya, masih ada beberapa permasalahan yang ditemui di dalam peresepan. Permasalahan dalam peresepan obat tersebut disebut juga Medication error. Medication error merupakan kejadian yang dapat merugikan pasien akibat pemakaian obat yang seharusnya dapat dicegah dalam pengobatan. Upaya dalam pencegahan terjadinya kesalahan pemberian obat (Medication error) adalah pengkajian resep pada resep yang diterima dari pasien. Metode penelitian yang digunakan dalam penyusunan laporan tugas khusus ini yaitu pengambilan data secara retrospektif dimana dilakukannya pengamatan dan mencatat terkait dengan aspek administrasi, aspek farmasetik, dan aspek klinis dari resep tersebut dan di ceklis pada form yang telah dibuat. Berdasarkan hasil Praktik Kerja Profesi Apoteker di Apotek Kimia Farma 352 Margonda yang telah dilakukan oleh peneliti dapat disimpulkan bahwa Ketepatan resep pada pasien gastritis berdasarkan aspek administrasi masih terdapat beberapa kekurangan sehingga diperlukan kelengkapan dalam resep pada aspek administrasi, Ketepatan resep pada pasien gastritis berdasarkan aspek farmasetik telah memenuhi persyaratan standar pelayanan kefarmasian, namun terdapat beberapa resep yang masih belum memenuhi persyaratan sehingga kelengkapannya perlu diperhatikan kembali, dan Ketepatan resep pada pasien gastritis berdasarkan aspek klinis telah memenuho persyaratan standar pelayanan kefarmasian seluruhnya sehingga harus dipertahankan seterusnya seperti ini.

Drug service on a doctor's prescription is one of the pharmaceutical services at the pharmacy. An appropriate prescription must contain sufficient information to enable the pharmacist concerned to understand the drug to be given to the patient. Prescriptions must also be written clearly to avoid misunderstandings between the doctor and the pharmacist concerned. But in fact, there are still some problems encountered in the prescription. Problems in prescribing these drugs are also called Medication errors. Medication error is an event that can harm the patient due to the use of drugs that should be prevented in treatment. Efforts to prevent medication errors from occurring are reviewing prescriptions on prescriptions received from patients. The research method used in the preparation of this special assignment report was retrospective data collection where observations and notes were made related to the administrative, pharmaceutical, and clinical aspects of the prescription and checked on the form that had been made. Based on the results of the Pharmacist Professional Work Practice at Kimia Farma 352 Margonda Pharmacy which has been carried out by researchers, it can be concluded that the accuracy of prescriptions for gastritis patients based on administrative aspects still has some deficiencies, so completeness of prescriptions is needed on administrative aspects. Accuracy of prescriptions for gastritis patients based on pharmaceutical aspects has been meet the standard requirements for pharmaceutical services, but there are several prescriptions that still do not meet the requirements so that their completeness needs to be considered again, and the accuracy of prescriptions for gastritis patients based on clinical aspects has fulfilled the standard requirements for pharmaceutical services in its entirety so it must be maintained onwards like this."

"Pelayanan kefarsmasian adalah suatu pelayanan langsung dan bertanggung jawab seorang apoteker kepada pasien yang berkaitan dengan sediaan farmasi dengan maksud mencapai hasil yang pasti untuk meningkatkan mutu kehidupan pasien. Pengkajian resep merupakan suatu proses pemeriksaan resep. Skrining resep dilakukan bertujuan untuk menganalisis adanya masalah terkait obat dan apabila terdapat kesalahan atau kejanggalan dapat dikonsultasikan dengan dokter penulis resep sehingga pasien dapat terhindar dari risiko medication error. Hipertensi adalah suatu keadaan dimana tekanan darah meningkat melebihi batas normal. Faktor resiko yang menyebabkan hipertensi yaitu umur, jenis kelamin, keturunan, obesitas, merokok, kurangi aktivitas fisik, konsumsi garam berlebihan, dyslipidemia. Terapi pada pasien hipertensi bertujuan untuk mencapai dan mempertahankan target tekanan darah. Adanya peningkatan tekanan darah yang terjadi pada pasien hipertensi dapat menurunkan aliran darah ke ginjal. Aliran darah ginjal yang rendah akan menstimulasi reabsorpsi asam urat. Tekanan darah yang semakin tinggi memiliki kemungkinan untuk memperbesar risiko penyakit mikrovaskuler yang dapat memicu iskemia jaringan. Penggunaan obat yang tepat untuk penderita hipertensi diperlukan agar pengobatan pasien menjadi efektif. Penggunaan obat yang tidak efektif dapat mengakibatkan kegagalan terapi.

Prescription review is a process of examining prescriptions. Prescription screening is carried out with the aim of analyzing drug-related problems and if there are errors or irregularities, it can be consulted with the prescribing doctor so that patients can avoid the risk of medication errors. Hypertension is a condition where blood pressure increases beyond normal limits. Risk factors that cause hypertension are age, gender, heredity, obesity, smoking, reduced physical activity, excessive salt consumption, dyslipidemia. Therapy for hypertensive patients aims to achieve and maintain target blood pressure. Increased blood pressure that occurs in hypertensive patients can reduce blood flow to the kidneys. Low renal blood flow will stimulate uric acid reabsorption. Increasing blood pressure has the potential to increase the risk of microvascular disease that can trigger tissue ischemia. The use of appropriate drugs for hypertensive patients is needed so that patient treatment is effective. Ineffective use of drugs can result in therapy failure."

"Pengkajian resep dilakukan untuk menganalisa adanya masalah terkait obat, bila ditemukan masalah terkait obat harus dikonsultasikan kepada dokter penulis resep. Apoteker harus melakukan pengkajian resep sesuai persyaratan administrasi, persyaratan farmasetik, dan persyaratan klinis. Praktik Kerja Profesi Apoteker (PKPA) di Apotek dilaksanakan selama 1 bulan, dimulai dari 1 April – 29 April 2022 di Apotek Kimia Farma 382 K2. Metode Pelaksanaan dilakukan secara deskriptif, kemudian dipilih sebanyak 10 (satu) resep untuk dianalisa. Berdasarkan dari resep tersebut terdapat duplikasi atau polifarmasi, hal ini dapat memicu terjadi medication error, mengingat informasi yang terdapat dalam aspek klinis berperan penting dalam rasionalitas terapi pengobatan pasien, seperti indikasi, aturan pakai, dosis, efek samping, kontra indikasi dan mekanisme kerja yang dapat mempengaruhi adanya interaksi obat pada resep pasien. Hal ini dapat terjadi karena dokter kurang berhati-hati dalam menulis resep yang diberikan, sehingga terjadi kesalahan pemberian (medication error). Untuk dapat mengatasi hal tersebut dapat diganti dengan obat alternatif lain yang tidak menimbulkan efek medication error sehingga aman diberikan kepada pasien agar mendapatkan hasil yang maksimal.

The implementation method was carried out descriptively, then 10 recipes were selected for analysis. Based on these prescriptions, there is duplication or polypharmacy, this can trigger medication error, considering that information contained in clinical aspects plays an important role in the rationality of patient treatment therapy, such as indications, rules of use, doses, side effects, contra-indications and mechanisms of action which can affect drug interactions in patient prescriptions. This can occur because doctors are less careful in writing the prescriptions given, resulting in medication error. To be able to overcome this, it can be replaced with other alternative drugs that do not cause the effect of medication errors so that it is safe to give to patients in order to get maximum results."

"Peraturan Menteri Kesehatan RI Nomor 73 tahun 2016 menetapkan standar pelayanan kefarmasian di Apotek, yang bertujuan untuk meningkatkan mutu kehidupan pasien. Pelayanan kefarmasian telah berkembang dari orientasi produk menjadi berorientasi pada pasien. Pengkajian resep adalah aspek operasional penting dalam pelayanan kefarmasian, yang bertujuan untuk mengidentifikasi dan menyelesaikan permasalahan terkait obat sebelum disiapkan. Dalam pengkajian resep, terdapat tiga aspek yang diperhatikan: administratif, farmasetik, dan klinis. Tujuan penelitian ini adalah melakukan pengkajian pada resep pasien dengan diagnosis osteoarthritis di Apotek Kimia Farma 049 untuk mengidentifikasi kemungkinan kesalahan pengobatan dan memberikan saran. Ditemukan bahwa resep pasien osteoarthritis memiliki beberapa permasalahan administratif, seperti ketidaklengkapannya, tetapi dari segi farmasetik dan klinis sudah sesuai. Kesimpulan dari pengkajian adalah bahwa meskipun ada permasalahan administratif pada resep, obat yang diresepkan sudah sesuai dengan tatalaksana osteoarthritis. Saran yang diberikan adalah untuk memperhatikan aspek administratif yang kurang lengkap pada resep dan melakukan konfirmasi kepada pasien atau dokter jika diperlukan.

Minister of Health Regulation No. 73 of 2016 sets the standard for pharmaceutical services in pharmacies, aimed at improving the quality of patient life. Pharmaceutical services have evolved from product-oriented to patient-oriented. Prescription assessment is a crucial operational aspect in pharmaceutical services, aiming to identify and resolve medication-related issues before preparation. In prescription assessment, three aspects are considered: administrative, pharmaceutical, and clinical. The aim of this study is to assess prescriptions for patients diagnosed with osteoarthritis at Kimia Farma Pharmacy 049 to identify potential medication errors and provide recommendations. It was found that prescriptions for osteoarthritis patients had some administrative issues, such as incompleteness, but were adequate in terms of pharmaceutical and clinical aspects. The conclusion of the assessment is that despite administrative issues in prescriptions, the prescribed medications are suitable for osteoarthritis management. Recommendations include paying attention to incomplete administrative aspects in prescriptions and confirming with patients or doctors if necessary."

Pengkajian resep dan simulasi konseling dilakukan terhadap lima resep obat. Pembahasan ini dilakukan untuk memberikan gambaran pelaksanaan kegiatan pengkajian resep dan konseling di Apotek Kimia Farma 456 Cinere. Metode yang dilakukan dimulai dari pemilihan resep sebanyak 5 (lima) resep dengan penyakit yang berbeda antara satu dengan yang lainnya, melakukan pengkajian resep yang meliputi pengkajian administratif, farmasetik, dan klinis, serta membuat dialog konseling dari masing- masing resep tersebut sebagai gambaran pelaksanaan konseling di apotek.
