"Pelayanan kesehatan di rumah sakit kepada masyarakat memerlukan tenaga kesehatan, salah satunya adalah Apoteker. Apoteker memiliki tugas dalam melakukan praktik pekerjaan kefarmasian di rumah sakit yang berpartisipasi langsung sebagai profesional yang memahami mengenai obat dan alat kesehatan. Selain itu, Apoteker profesional juga memiliki fungsi penting di Apotek sebagai penanggung jawab Apotek untuk menyalurkan obat yang berkhasiat, bermutu, dan aman. Penyaluran obat tersebut diregulasi oleh Badan POM, sehingga Apoteker profesional harus memiliki pengetahuan dan pengalaman yang berkaitan dengan Badan POM. Untuk menjadi apoteker yang profesional sebelum masuk dunia kerja, perlu adanya pengalaman dalam bentuk praktik kerja profesi apoteker sebagai modalnya. Praktik Kerja Profesi Apoteker dilaksanakan di Rumah Sakit Universitas Indonesia (RSUI) periode Januari – Februari 2022, Badan POM periode Maret 2022, dan Apotek Kimia Farma Margonda periode Mei 2022. Melalui proses PKPA tersebut, calon apoteker profesional diharapkan mendapatkan wawasan, pengetahuan, keterampilan, dan pengalaman baru yang tidak ditemukan di luar PKPA dan menjadi bekal dan modal yang sesuai dalam melakukan pekerjaan kefarmasian.

Health services in hospitals to the community require health workers, one of which is a pharmacist. Pharmacists have a duty to doing practice pharmacy in hospitals that participate directly as professionals who understand drugs and medical devices. In addition, professional pharmacists also have an important function in pharmacies as the person in charge of pharmacies to distribute drugs that are efficacious, quality, and safe. The distribution of these drugs is regulated by the FDA, so professional pharmacists must have knowledge and experience related to the FDA. To become a professional pharmacist before entering the world of work, it is necessary to have experience in the form of working practice as a pharmacist. The Professional Pharmacist Practice is held at the University of Indonesia Hospital (RSUI) for the period January – February 2022, the FDA for the March 2022 period, and Kimia Farma Margonda Pharmacy for the May 2022 period. Through the PKPA process, prospective professional pharmacists are expected to gain insight, knowledge, skills, and new experiences that are not found outside of PKPA and become appropriate provisions and capital in carrying out pharmaceutical work."

"Salah satu unsur penting dalam pelayanan kesehatan yaitu penggunaan obat yang rasional demi tercapainya kesehatan pasien. Peresepan berkualitas bertujuan untuk mewujudkan penggunaan obat yang rasional, hal tersebut meliputi tepat indikasi, tepat pasien, tepat dosis, tepat obat, dan tepat interval waktu penggunaan. Di seluruh dunia, sekitar 50% dari semua obat yang diresepkan, dibagikan, dan dijual secara tidak tepat, serta separuh dari semua pasien gagal meminum obatnya dengan benar. Formularium Rumah Sakit merupakan daftar obat dan kebijakan penggunaan obat yang disepakati staf medis, disusun oleh Komite/Tim Farmasi dan Terapi, yang ditetapkan oleh direktur/kepala rumah sakit. Formularium Rumah Sakit menjadi kendali mutu dan kendali biaya obat yang akan memudahkan pemilihan obat yang rasional, mengurangi biaya pengobatan, dan mengoptimalkan pelayanan kepada pasien. Berdasarkan evaluasi yang dilakukan terhadap peresepan obat di RSUI periode Januari – Desember 2020, hasil kajian menujukkan bahwa belum terpenuhinya standar pelayanan minimal pelayanan kefarmasian. Peresepan yang sesuai dengan formularium RSUI baru mencapai 56,5%. Pada penelitian ini bertujuan untuk mengevaluasi kesesuaian peresepan dan jenis obat di RSUI dengan formularium RSUI pada periode Januari – Juni 2021. Penyakit kronis merupakan penyakit dengan durasi panjang dengan proses penyembuhan atau pengendalian kondisi klinis yang pada umumnya berkembang secara lambat dan diakibatkan oleh faktor genetik, fisiologis, lingkungan, dan perilaku. Contoh dari penyakit kronis yaitu penyakit kardiovaskular (jantung dan stroke), diabetes, kanker, dan penyakit respiratori kronis. Penyakit kronis, khususnya diabetes dan hipertensi memerlukan monitoring dan pengelolaan secara terus menerus seumur hidup. Jenis obat untuk diabetes dan hipertensi tergolong banyak, sehingga memerlukan strategi terapi yang tepat untuk menjamin penggunaan obat yang rasional agar keberhasilan terapi dapat tercapai. Salah satu peran Apoteker di apotek yang bertujuan untuk mengoptimalkan terapi obat dalam pengelolaan penyakit kronis adalah dengan melakukan kajian terhadap keseluruhan terapi obat yang digunakan oleh pasien pada seluruh tingkat pelayanan kesehatan. Penelitian ini bertujuan unutk mengevaluasi kesesuaian penggunaan obat antidiabetes dan antihipertensi dengan algoritma terapinya pada pasien di Apotek Kimia Farma Condet.

One of the important elements in health services is the rational use of medicines to achieve patient health. Quality prescribing aims to realize the rational use of medicines, this includes the correct of indication, patient, dose, medicine, and the time interval for use. In worldwide, approximately 50% of all medicines are prescribed, dispensed and sold inappropriately, and half of all patients fail to take their medicines correctly. The Hospital Formulary is a list of drugs and its use policies, agreed upon by medical staff, compiled by the Pharmacy and Therapy Committee/Team, which is determined by the director/head of the hospital. The hospital formulary provides quality control and drug cost control which will facilitate rational drug selection, reduce medical costs and optimize service to patients. Based on an evaluation carried out on drug prescribing at RSUI for the period January – December 2020, the results of the study show that minimum service standards for pharmaceutical services have not been met. Prescriptions in accordance with the RSUI formulary have only reached 56.5%. This study aims to evaluate the compatibility of prescribing and types of drugs at RSUI with the RSUI formulary in the period January – June 2021. Chronic diseases are defined as diseases of long duration with a process of healing or controlling clinical conditions which generally develop slowly and caused by genetic, physiological, environmental and behavioral factors. Examples of chronic diseases are cardiovascular disease (heart disease and stroke), diabetes, cancer and chronic respiratory disease. Chronic diseases, especially diabetes and hypertension, require continuous monitoring and management throughout life. There are many types of medicines for diabetes and hypertension, so appropriate therapeutic strategies are needed to ensure the rational use of medicines to achieve therapeutic success. One of the roles of pharmacists in pharmacies which aims to optimize drug therapy in the management of chronic diseases is to conduct studies on all drug therapy used by patients at all levels of health services. This study aims to evaluate the suitability of the use of antidiabetic and antihypertensive drugs with the therapeutic algorithm for diabetes and hypertension patients at Kimia Farma Condet Pharmacy."

"Profesi apoteker sangat memegang peranan penting dalam pekerjaan kefarmasian. Pekerjaan Kefarmasian adalah pembuatan termasuk pengendalian mutu Sediaan Farmasi, pengamanan, pengadaan, penyimpanan dan pendistribusi atau penyaluranan obat, pengelolaan obat, pelayanan obat atas resep dokter, pelayanan informasi obat, serta pengembangan obat, bahan obat dan obat tradisional. Calon apoteker memiliki kewajiban untuk melakukan praktik profesi sebagai gambaran untuk memahami peran apoteker dan mengasah kompetensi sebelum berpraktik kerja. Praktik kerja Profesi Apoteker dilaksanakan di Puskesmas Setiabudi Jakarta pada periode Februari 2022, di Industri Farmasi PT. Kalbio Global Medika pada periode Maret – April 2022, dan Apotek Kimia Farma 152 Pasar Minggu pada periode Mei 2022. Proses PKPA ini diharapkan dapat mempersiapkan calon apoteker melalui wawasan dan keterampilan yang sesuai untuk melakukan peran pekerjaan kefarmasian.

The Practice was carried out at the Setiabudi Public Health Center, Jakarta in the period of February 2022, at the Pharmaceutical Industry of PT. Kalbio Global Medika in the period March – April 2022, and Apotek Kimia Farma 152 Pasar Minggu in the period May 2022. This PKPA process is expected to prepare prospective pharmacists through the appropriate insight and skills to perform the role of pharmacy practice."

"Apoteker merupakan salah satu tenaga kesehatan yang memiliki tugas dan tanggung jawab serta perannya sangat dibutuhkan dalam pengelolaan obat baik dalam rangka produksi maupun pelayanan kepada masyarakat luas. Pelayanan kefarmasian untuk masyarakat secara langsung dapat dilakukan di apotek, rumah sakit, maupun puskesmas. Sedangkan dalam rangka melakukan pengembangan serta produksi obat dengan kualitas yang telah terstandar dapat dilakukan di industri farmasi. Bertujuan agar terlatihnya calon apoteker dalam melakukan tugas kefarmasian serta meningkatkan pemahaman terhadap peran, fungsi, posisi, dan tanggung jawab apoteker sehingga diperlukan praktik langsung di lapangan dengan melakukan Praktik Kerja Profesi Apoteker. Dengan dilakukannya praktik kerja diharapkan apoteker dapat mengetahui tugas serta tanggung jawabnya baik secara langsung maupun tidak langsung kepada masyarakat luas.

The aim is to train prospective pharmacists in carrying out pharmaceutical duties and increase understanding of the roles, functions, positions, and responsibilities of pharmacists so that direct practice is needed in the field by conducting Pharmacist Professional Work Practices. Work Practices are carried out at the Roxy Klender Pharmacy during the period of February 2022 and PT. Soho Pharmasi Industry during the period March – April 2022. By doing the work practice, it is hoped that pharmacists can know their duties and responsibilities both directly and indirectly to the wider community."

"Profesi Apoteker di Indonesia memiliki standar kompetensi yang bertujuan untuk memastikan bahwa Apoteker memiliki kompetensi untuk menjalankan peran sebagai Apoteker. Keterampilan untuk melaksanakan praktik kefarmasian perlu dilatih selama masa pendidikan apoteker, sehingga Apoteker dapat menjalankan praktik kefarmasian sesuai dengan ketentuan praktik kefarmasian dan peraturan perundang-undangan. Pelaksanaan Praktik Kerja di Apotek, Badan Pengawas Obat dan Makanan, serta industri farmasi diharapkan dapat memberikan pengembangan wawasan, ilmu pengetahuan, serta kompetensi bagi calon apoteker dalam melaksanakan praktik kefarmasian di masa mendatang.

The Professional Pharmacist Practice Act (PKPA), which is intended to help prospective pharmacists recognize and enhance their competencies, is one method of enhancing pharmacists' professional skills. PT. Kalbio Global Medika, Jagakarsa District Health Center, and Kimia Farma Dramaga Pharmacy are among the institutions that work collaboratively with the Pharmacist Professional Study Program, Faculty of Pharmacy, University of Indonesia prospective pharmacists can take the PKPA examination between February and May 2022. Prospective pharmacists must acquire the necessary insight, knowledge, skills, and experience to practice pharmacy through PKPA in pharmacies, health centers, and these industries."

"Peresepan obat yang ada di rumah sakit harus sesuai dengan Fornas dan Formularium Rumah Sakit (FRS) dengan target 100% sesuai dengan FRS dan memenuhi standar minimal kesesuaian peresepan sebesar ≥80%. Kesesuaian peresepan berdasarkan formularium dapat meningkatkan kualitas layanan dan ketersediaan obat bagi pasien serta dapat meningkatkan mutu pelayanan kesehatan melalui peningkatan efektivitas dan efisiensi pengobatan sehingga tercapai penggunaan obat yang rasional. Berdasarkan hasil analisis di RSUI, terdapat 189 jumlah item obat yang diresepkan dari 50 resep pasien selama periode September-Oktober 2023 dengan rincian 171 item obat sesuai dengan Fornas dan 18 item obat tidak sesuai dengan Fornas. Oleh karena itu, persentase kesesuaian peresepan obat pasien BPJS dengan Fornas sudah melampaui standar indikator mutu pelayanan kesehatan RS karena menunjukkan kesesuaian item obat yang diresepkan terhadap Fornas 2021 sebesar 90,48%. Diharapkan instalasi farmasi RSUI dapat memonitoring secara berkala minimal satu bulan sekali terkait peresepan obat di seluruh departemen di RS serta mengingatkan kepada dokter untuk meresepkan obat sesuai dengan daftar obat yang ada di Fornas. Selain itu, peran apoteker sangat diperlukan untuk meningkatkan pelayanan kefarmasian di RSUI dengan aktif memberikan edukasi serta pengenalan kepada pasien maupun dokter terkait obat-obatan yang terdapat dalam Fornas.

Prescription of drugs in the hospital must be following the Fornas and Hospital Formulary (FRS) with a target of 100% by the FRS and meet the minimum standard of prescribing suitability of ≥80%. The suitability of prescribing based on the formulary can improve the quality of service and availability of drugs for patients and can improve the quality of health services through increasing the effectiveness and efficiency of treatment to achieve rational use of drugs. Based on the results of the analysis at RSUI, there were 189 total drug items prescribed from 50 patient prescriptions during the September-October 2023 period with details of 171 drug items by Fornas and 18 drug items not following the Fornas. Therefore, the percentage of conformity of BPJS patient drug prescriptions with Fornas has exceeded the standard of hospital health service quality indicators because it shows that the conformity of the prescribed drug items to Fornas 2021 is 90.48%. It is hoped that the RSUI pharmaceutical installation can monitor regularly at least once a month regarding drug prescriptions in all departments in the hospital and remind doctors to prescribe drugs according to the list of drugs in Fornas. In addition, the role of pharmacists is needed to improve pharmaceutical services at RSUI by actively providing education and introduction to patients and doctors regarding drugs contained in Fornas."

"Pelayanan kefarmasian di rumah sakit harus mematuhi standar pelayanan minimal yang ditetapkan, termasuk kesesuaian peresepan dengan Formularium Nasional. Penelitian ini bertujuan mengevaluasi kesesuaian peresepan obat di depo farmasi rawat jalan Rumah Sakit Universitas Indonesia (RSUI) dengan Formularium Nasional pada April 2023. Hasil menunjukkan bahwa dari 390 item obat yang diresepkan, 256 item (65,64%) sesuai dengan Formularium Nasional, sedangkan 134 item (34,36%) tidak sesuai. Peresepan BPJS yang tidak sesuai mayoritas berasal dari klinik anak, penyakit dalam, neurologi, dan pulmonologi. Keseluruhan peresepan BPJS yang tidak sesuai dengan Formularium Nasional adalah 27.749 obat (18,98%). Penelitian ini mengindikasikan bahwa kesesuaian peresepan obat di RSUI belum mencapai standar 100% yang diharapkan, disebabkan oleh ketidaktersediaan beberapa obat dalam Formularium Nasional. Disarankan agar dokter lebih memperhatikan formularium dalam penulisan resep untuk meningkatkan kesesuaian dan efisiensi biaya.

This study aims to evaluate the conformity of drug prescriptions at the outpatient pharmacy depot of the University of Indonesia Hospital (RSUI) with the National Formulary in April 2023. The results showed that of the 390 prescribed drug items, 256 items (65.64%) were by the National Formulary, while 134 items (34.36%) were not in accordance. The majority of inappropriate BPJS prescriptions came from pediatric clinics, internal medicine, neurology, and pulmonology. The total number of BPJS prescriptions that were not by the National Formulary was 27,749 drugs (18.98%). This study indicates that the conformity of drug prescriptions at RSUI has not reached the expected 100% standard, due to the unavailability of several drugs in the National Formulary. It is recommended that doctors pay more attention to the formulary in writing prescriptions to improve conformity and cost efficiency."
