"Latar Belakang. Pasien kanker usia lanjut dapat memiliki luaran yang buruk akibat intoleransi terhadap terapi standar, hingga mortalitas, terutama pada stadium lanjut (III dan IV). Memprediksi mortalitas merupakan hal yang penting dalam menentukan pemberian terapi. Hingga saat ini ECOG (Eastern Cooperative Oncology Group) masih digunakan di RSUPN dr. Cipto Mangunkusumo (RSCM). Geriatric 8 (G8) merupakan salah satu instrumen penapis yang dikembangkan untuk memprediksi frailty. Namun, beberapa studi menunjukkan bahwa G8 dapat memprediksi mortalitas.
Tujuan. Penelitian ini bertujuan untuk menilai performa ECOG dan G8 dalam memprediksi mortalitas 6 bulan pada pasien usia lanjut dengan kanker stadium III dan IV.
Metode. Metode penelitian ini adalah kohort retrospektif, menggunakan data rekam medis pasien berusia > 60 tahun dengan kanker stadium III dan IV yang belum atau sudah menjalani terapi kanker, di Poliklinik Hematologi Onkologi Medik RSCM sejak Oktober 2019-Maret 2021. Peneliti menilai ECOG dan G8, serta melakukan follow-up dalam 6 bulan untuk mengetahui luaran subjek berupa meninggal atau tidak. Peneliti menganalisis uji kalibrasi dengan Hosmer-Lemeshow dan uji diskriminasi dengan Area Under the Curve (AUC).
Hasil. Peneliti mendapatkan 230 subjek. Proporsi mortalitas dalam 6 bulan adalah 40,9%, dengan kanker terbanyak adalah kanker kepala dan leher (11,3%). ECOG memiliki performa kalibrasi yang baik (p = 0,085), dan performa diskriminasi yang baik (AUC 0,705; IK95% 0,570 - 0,841), sensitivitas 70,2%, dan nilai prediksi negatif (NPN) 49,3%. Performa kalibrasi skor G8 menunjukkan hasil yang juga baik (p = 0,687), dan performa diskriminasi yang baik (AUC 0,777; IK95% 0,683 - 0,872), sensitivitas 89,4% dan NPN 76%.
Kesimpulan. Skor G8 memiliki performa kalibrasi dan diskriminasi yang lebih baik untuk memprediksi mortalitas 6 bulan pada pasien usia lanjut dengan kanker stadium III dan IV.

Background: Elderly cancer patients can have a poor outcome due to intolerance to standard therapy, up to mortality, especially in advanced stages (III and IV). Predicting mortality is important to determine treatment. ECOG is still being used at dr. Cipto Mangunkusumo General Hospital (RSCM). G8 is one of the screening instruments developed to predict frailty. However, several studies have shown that the G8 can predict mortality.
Aim. To assess the performance of ECOG and G8 in predicting six months mortality in elderly cancer patients with stage III and IV.
Methods: In this study, a retrospective cohort design was used using medical records of patients aged > 60 years old with stage III and IV cancer who had not or had undergone cancer therapy at the Medical Oncology Hematologic Polyclinic of RSCM since October 2019-March 2021. Collected data were analyzed using the Hosmer-Lemeshow test for the calibration performance and the Area Under the Curve (AUC) test for the discrimination performance in predicting mortality.
Results. Among 230 subjects, mortality in 6 months was 40.9%, with the most common types of cancer was head and neck cancer (11.3%). ECOG had good calibration (p = 0.085), and good discrimination performance (AUC 0.705; 95% CI 0.570 - 0.841). G8 also had good calibration (p = 0.687), and good discrimination performance (AUC 0.777; 95% CI 0.683 - 0.872). G8 had higher sensitivity and negative predictive value than ECOG (89,4% vs 70,2%; 76% vs 49,3%).
Conclusion. G8 has better calibration and discrimination performance for predicting six months mortality in elderly cancer patients with stages III and IV.
