"Falls are a serious consequence of declining physical function. Otago exercise is a strength and balance training program designed to prevent falls and enhance health status among the elderly. This study aimed to investigate the effect of a 12-
week Otago exercise intended to reduce the risk of falls and health status among the elderly with chronic illness in the social elderly institution. This quasi-experimental study employed a pre- and post-test design using a control group. The study involved an intervention group (21 respondents) and a control group (21 respondents). The sample used in this study were elderly living in a social elderly institution. The sample was selected using simple random sampling. The data
were analyzed using Mann–Whitney test, independent t-test, and Chi-square test. Otago exercise significantly reduced
the respondents’ risk of falling and enhanced their health status. Significant differences were observed between the two
groups in terms of the risk of fall (p= 0.041) and health status (p= 0.011). Otago exercise significantly improves the health status and reduces the risk of falling among elderly with chronic illness. The exercise can be recommended for older adults with chronic illness in social elderly institutions and communities."

"[Jatuh merupakan salah satu masalah yang serius pada lansia. Gangguan
keseimbangan tubuh merupakan salah satu penyebab jatuh yang cukup penting. Penelitian ini bertujuan mengetahui pengaruh latihan keseimbangan lansia selama 8 minggu terhadap keseimbangan tubuh, risiko jatuh, status kesehatan dan kualitas hidup lansia. Penelitian quasi experimental ini terdiri dari dua kelompok; 30 lansia sebagai kelompok kontrol dan 30 perlakuan. Tekhnik pengambilan sampel yang digunakan adalah multistage random sampling. Hasil penelitian
menunjukkan latihan keseimbangan lansia berpengaruh signifikan meningkatkan keseimbangan tubuh (p=0,0005), status kesehatan (p=0,0005), kualitas hidup (p=0,0001) dan menurunkan risiko jatuh (p=0,0005). Hasil uji mancova menunjukkan pengaruh LKS lansia diperkuat oleh usia (p=0,0005). Latihan keseimbangan lansia dapat digunakan sebagai salah satu upaya pencegahan gangguan keseimbangan tubuh, risiko jatuh dan upaya meningkatkan status
kesehatan dan kualitas hidup pada lansia di komunitas. Penelitian selanjutnya dapat melakukan penelitian pada kelompok usia yang berbeda untuk mengetahui keefektifan latihan keseimbangan lansia;Falls are the one of serious problem among elderly. "Peristiwa jatuh pada lansia berdampak pada kondisi fisik lansia. Penulisan karya ilmiah ini bertujuan untuk memaparkan asuhan keperawatan pada lansia dengan masalah risiko jatuh. Risiko jatuh dan keseimbangan lansia dalam penelitian ini menggunakan pengukuran Morse Falls Scale (MFS), Time Up and Go Test (TUG) Berg Balance Scale (BBS). Salah satu intervensi keperawatan yang dapat dilakukan adalah program latihan keseimbangan dengan metode Otago Exercise Program (OEP).
Hasil dari latihan selama enam kali dibuktikan dengan hasil MFS sebelum latihan skornya 65 dan setelah latihan 45. Pengukuran TUG sebelum latihan 35 detik, sementara itu setelah latihan 26 detik. Keseimbangan lansia sebelum latihan skornya adalah 19, sedangkan setelah latih terjadi peningkatan signifikan dengan skor 29. Latihan keseimbangan menggunakan metode Otago Exercise Program terbukti mampu untuk mengurangi risiko jatuh, meningkatkan keseimbangan, kekuatan otot, dan gaya berjalan lansia.

Falling events in the elderly have an impact on the physical condition of the elderly. The writing of this scientific paper aims to describe nursing expectations in the elderly with the problem of falling risk. The risk of falling and elderly balance in this study uses the Morse Falls Scale (MFS), Time Up and Go Test (TUG) and Berg Balance Scale (BBS) measurements. One nursing intervention that can be done is a balance training program using the Otago Exercise Program (OEP) method.
The results of the six-time workout are proven by the MFS results before the practice score is 65 and after the practice 45. TUG measurement before exercise 35 seconds, meanwhile after training 26 seconds. The elderlys balance before the practice score is 19, whereas after training there is a significant increase with a score of 29. Balance exercises using the Otago Exercise Program method have proven able to reduce the risk of falls, increasing balance, muscle strength, and the gait of the elderly.

"Lansia mengalami penurunan fungsi organ tubuh. Salah satunya yaitu perubahan pada sistem muskuloskeletal yang dapat mempengaruhi terjadinya jatuh pada lansia. Kejadian jatuh merupakan masalah kesehatan lansia di seluruh dunia dan penyebab utama terkait dengan mortalitas dan morbiditas. Penulisan karya ilmiah ini bertujuan untuk memaparkan asuhan keperawatan pada lansia yang memiliki risiko jatuh. Metode untuk mengatasi risiko jatuh adalah asuhan keperawatan program latihan otago. Instrumen pengkajian risiko jatuh yang digunakan yaitu Morse Falls Scale MFS , Timed Up and Go Test TUGT dan Berg Balance Scale BBS . Hasil yang didapatkan yaitu adanya perubahan yang cukup signifikan setelah dilakukan intervensi program latihan otago. Hasil pemeriksaan TUGT dari 31 detik menjadi 25 detik dan hasil pemeriksaan BBS dari skor 33 menjadi 41. Selain itu, selama dilakukan intervensi ketiga klien tersebut tidak mengalami jatuh. Program latihan otago ini dapat dilakukan oleh perawat untuk meningkatkan kekuatan, keseimbangan, dan mobilitas lansia agar meminimalisir risiko jatuh pada lansia.
Elderly experience decrease organ function. One of them is a change in the musculoskeletal system that influence of falls in elderly. Fall is worldwide health problem among older people and a major cause associated with mortality and morbidity. The purpose of this scientific article is to explain nursing care of elderly with risk of falls. The method to solve risk of falls was using nursing care such as otago exercise program. Assessment instrument used are Morse Falls Scale MFS , Timed Up and Go Test TUGT dan Berg Balance Scale BBS . The obtained result shows quite significant change after otago exercise program. TUGT score from 31 seconds to 25 seconds, BBS score increases from 33 to 41. Moreover, during intervention otago exercise program, client not to falls. The otago exercise program can be implemented by nurse to improve strength and mobility on elderly in order to minimize risk of falls on elderly."

"Status nutrisi merupakan kondisi status kesehatan individu yang banyak menjadi masalah pada lanjut usia. Masalah status nutrisi menyebabkan dampak buruk seperti kejadian jatuh yang berdampak pada kematian. Tujuan penelitian ini untuk mengetahui hubungan status nutrisi dengan risiko jatuh pada lansia di Kota Depok. Penelitian ini menggunakan pendekatan desain cross sectional dengan jumlah sampel sebanyak 111 responden. Instrumen yang digunakan untuk mengukur status nutrisi berupa Mini Nutrition Assasment MNA dan risiko jatuh berupa Morse Fall Scale MFS . Hasil penelitian didapat ada hubungan bermakna antara status nutrisi dengan risiko jatuh p = 0,000 dengan OR= 3,8 2,6-5,8 . Penelitian selanjutnya diharapkan mampu mengembangkan intervensi peningkatan status nutrisi untuk mencegah risiko jatuh pada lansia.

Nutritional status is a condition of the health status of individuals who are a lot of problems in the elderly. Problems of nutritional status cause adverse effects such as fall events that can cause death. The purpose of this study to determine the relationship of nutritional status with the risk of falling in the elderly in Depok City. This research uses cross sectional design approach with the number of samples is 111 respondents. Instruments used to measure the nutritional status of the Mini Nutrition Assassment MNA and the risk of falling Morse Fall Scale MFS . The result showed that there was significant correlation between nutritional status and risk of fall p 0,000 with OR 3,8 2,6 5,8 . Subsequent research is expected to develop improved interventions of nutritional status to prevent the risk of falling in the elderly.

"Penurunan fungsi fisiologis akibat penuaan pada lansia berdampak pada risiko jatuh. Berdasarkan hasil observasi selama tiga minggu praktik di STW Ria Pembangunan Cibubur, kejadian jatuh dari bulan Januari sampai dengan September 2023 sebanyak 22 kali kejadian jatuh beberapa diantaranya jatuh berulang. Risiko jatuh merupakan peningkatan potensial jatuh sehingga dapat menyebabkan cedera fisik. Risiko jatuh dapat diminimalisir dengan latihan peningkatan kekuatan otot. Salah satu intervensi latihan peningkatan kekuatan otot yaitu strengthening exercises. Latihan ini dilakukan selama dua minggu dengan frekuensi enam kali dalam seminggu. Hasil dari latihan strengthening exercises menunjukkan peningkatan kekuatan otot yang signifikan yaitu semula menjadi . STW Ria Pembangunan Cibubur diharapkan dapat memberikan perhatian lebih terhadap permasalahan risiko jatuh dengan melakukan latihan peningkatan kekuatan otot. Tidak hanya senam fisik biasa melainkan latihan yang dikolaborasikan dengan peningkatan kekuatan otot seperti strengthening exercises, agar kejadian jatuh tidak terjadi maupun tidak berulang dan risiko jatuh menurun.

Decreased physiological function due to aging in the elderly has an impact on the risk of falls. Based on the results of observations during three weeks of practice at STW Ria Pembangunan Cibubur, the incidence of falls from January to September 2023 was 22 falls, some of which were repeated falls. The risk of falling is an increase in the potential to fall so that it can cause physical injury. The risk of falling can be minimized by training to increase muscle strength. One of the training interventions to increase muscle strength is strengthening exercises. This exercise is carried out for two weeks with a frequency of six times a week. "Tesis ini disusun untuk mengetahui efektifitas alat ukur Modified Mobility Interaction Fall Chart (Modified MIF Chart), sebagai alat ukur untuk menapis risiko jatuh, khususnya pada populasi lansia di panti wreda. Desain penelitian adalah cohort prospective dengan menilai komponen performa fisik menggunakan Stop walking when talking (SWWT) dan Difference Time Up and Go (DiffTUG), komponen fungsi visual yang menilai ketajaman penglihatan dengan Snellen chart, dan komponen kognisi yang dinilai menggunakan Montreal Cognitive Assesment versi Bahasa Indonesia (MoCA-INA) pada awal studi dan ditentukan tingkat risiko jatuh. Didapatkan subjek penelitian (n=111) yang menyelesaikan Modified MIF Chart kemudian dilakukan observasi kejadian jatuh selama tiga bulan. Terdapat 12 (10,8%) kejadian jatuh dari seluruh subjek. Terdapat perbedaan bermakna (p=0,038) antara kelompok risiko jatuh tinggi yang mengalami kejadian jatuh sebanyak 8 (18,6%) dibandingkan 4 kejadian jatuh (5,9%) pada kelompok risiko jatuh rendah dengan AUC 0,657 (95% CI: 0,49-0,82). Didapatkan sensitifitas dan spesifisitas Modified MIF chart secara berurutan adalah 64,6% dan 66,7%. Kesimpulan penelitian ini adalah Modified MIF Chart dapat digunakan sebagai alat penapis risiko jatuh pada lansia di panti wreda, tetapi tetap perlu memperhatikan faktor-faktor risiko jatuh internal dan eksternal lain yang belum dinilai oleh Modified MIF Chart.

This thesis is designed to determine the effectiveness of the Modified Mobility Interaction Fall Chart (Modified MIF Chart) as a tool to screen the risk of falls, especially in the elderly population in nursing homes. The research design was a prospective cohort by assessing the physical performance components using Stop walking when talking (SWWT) and Difference Time Up and Go (DiffTUG), a visual function component that assessed visual acuity using a Snellen chart, and a cognitive component assessed using the Montreal Cognitive Assessment. Indonesian language version (MoCA-INA) at the start of the study and the level of risk of falling was determined. Obtained research subjects (n = 111) who completed the Modified MIF Chart then observed the fall for three months. There were 12 (10.8%) incidence of falls for all subjects. There was a significant difference (p = 0.038) between the high risk group who experienced falls as much as 8 (18.6%) compared to 4 falls (5.9%) in the low risk group with AUC 0.657 (95% CI: 0, 49- 0.82). The sensitivity and specificity of the Modified MIF chart are 64.6% and 66.7%, respectively. The conclusion of this study is that the Modified MIF Chart can be used as a means of screening for the risk of falls in the elderly in nursing homes, but still needs to consider other internal and external risk factors that have not been assessed by the Modified MIF Chart."

"Penurunan fungsi fisiologis akibat penuaan pada lansia berdampak pada risiko jatuh. Berdasarkan hasil observasi selama praktik tujuh minggu, kejadian jatuh yang sering ditemukan di panti sebanyak sebelas kejadian jatuh, dan lima diantaranya mengalami jatuh berulang. Selain itu dari sebelas kejadian jatuh tersebut, satu diantaranya harus dirujuk ke rumah sakit karena mengalami penurunan kesadaran. Risiko jatuh merupakan peningkatan potensial jatuh, sehingga menyebabkan cedera fisik dan dapat diatasi dengan latihan kekuatan otot berupa intervensi floor-seated exercise program. Asuhan keperawatan untuk mengurangi risiko jatuh dengan floor-seated exercise program dilakukan selama 5 minggu dalam frekuensi 4 kali seminggu pada 3 klien kelolaan. Hasil menunjukkan terjadi peningkatan fungsional kekuatan otot menggunakan manual muscle test pada kedua ektremitas pada klien keloaan utama yaitu semula 4444 4444  menjadi 5554 5544.  Pihak panti diharapkan dapat memberikan dapat memberikan perhatian lebih pada masalah kesehatan lansia dengan memberikan sarana prasarana maupun peningkatan motivasi dalam aktivitas fisik yang dapat menurunkan kejadian jatuh pada lansia. Intervensi berupa floor-seated exercise program juga dapat dilakukan oleh perawat maupun mahasiswa praktikan dalam mengatasi lansia dengan masalah keperawatan risiko jatuh secara optimal.

Decreasing physiological function due to aging in the elderly has an impact on the fall risk. Based on the results of observations during the seven-week practice, the fall events that are often found in the orphanage were eleven falls, and five of them experienced repeated falls. In addition, of the eleven falls, one of them had to be referred to the hospital because of a decrease in consciousness. The fall risk is an increase in the potential for falls, causing physical injury and can be overcome by muscle strength training in the form of floor-seated exercise program interventions. Nursing care to reduce the fall risk with a floor-seated exercise program is carried out for 5 weeks in a frequency of 4 times a week in 3 managed clients. The results showed that there was an increase in functional muscle strength using the manual muscle test on both extremes on the main client client, namely 4444 4444   to  5554 5544. PSTW is expected to be able to give more attention to elderly health problems by providing infrastructure and increasing motivation in physical activities that could reduce fall in the elderly. Interventions in the form of floor-seated exercise programs can also be carried out by nurses and nursing students to elderly with fall risk nursing problems optimally."

"Lansia mengalami penurunan fungsi fisiologis pada sistem musculoskeletal dikarenakan proses
menua. Perubahan sistem ini menimbulkan masalah risiko jatuh pada lansia. Risiko jatuh dapat
meningkatkan kerentanan kejadian jatuh, sehingga dapat menyebabkan cedera fisik. Penulis
ini bertujuan untuk menganalisis intervensi unggulan dalam mengatasi risiko jatuh pada lansia
yaitu intervensi Multifactorial Exercise dengan metode Stay and Independent for Life (SAIL).
Intervensi ini merupakan kombinasi antara latihan kekuatan otot dan keseimbangan dilakukan
selama 2 minggu dalam frekuensi 2 kali seminggu pada klien kelolaan. Hasil implementasi
akan dievaluasi menggunakan pengukuran Morse Falls Scale (MFS), Time Up and Go Test
(TUG) dan Berg Balance Scale (BBS). Hasil menunjukan peningkatan pada pengukuran TUG
yang melihat keseimbangan dalam pergerakan mobilitas klien, dan didapatkan skor 33 detik
menjadi 31 detik setelah intervensi. Pengukuran keseimbangan klien dengan BBS
menunjukkan peningkatan skor dari 14 menjadi 16. Rekomendasi penulis perlu ditambahkan
intervensi Multofactorial Exercise sebagai variasi dalam Terapi Aktivitas Kelompok yang
sudah dilakukan secara rutin. Manfaatnya agar lansia tidak cepat bosan dengan latihan yang
sedang diterapkan, selain itu variasi latihan dapat memberikan hasil yang optimal.

The elderly experience a decrease in physiological function in the musculoskeletal system due
to aging. This system change raises the risk of falling in the elderly. The risk of falling can
increase the susceptibility to falls, which can cause physical injury. This author aims to analyze
leading interventions in overcoming the risk of falling in the elderly, namely the Multifactorial
Exercise intervention with the Stay and Independent for Life (SAIL) method. This intervention
is a combination of muscle strength and balance exercises carried out for 2 weeks in a
frequency of 2 times a week on managed clients. Implementation results will be evaluated using
Morse Falls Scale (MFS), Time Up and Go Test (TUG) and Berg Balance Scale (BBS)
measurements. The results show an increase in TUG measurements that see balance in the
movement of client mobility, and obtained a score of 33 seconds to 31 seconds after the
intervention. Measurement of client balance with BBS shows an increase in score from 14 to
16. The authors recommendation needs to be added to the intervention of Multofactorial
Exercise as a variation in Group Activity Therapy that has been done routinely. The benefit is
that the elderly do not get bored quickly with the exercise being applied, besides that variations
of the exercise can provide optimal results."

"Lansia merupakan individu tahap akhir perkembangan yang memiliki beberapa perubahan pada sistem tubuhnya. Sistem muskuloskeletal ialah sistem yang identik dengan perubahan yang sering terjadi pada lansia. Perubahan pada sistem tersebut berdampak pada menurunnya kekuatan otot, tulang dan sendi yang dapat mengakibatkan lansia mengalami jatuh. Penulisan karya ilmiah ini bertujuan untuk memaparkan hasil asuhan keperawatan pada lansia yang memiliki masalah resiko jatuh. Format pengkajian morse fall scale (MFS) dan Timed up and Go test (TUG) digunakan untuk mengukur resiko jatuh klien. Salah satu intervensi keperawatan yang dapat diberikan untuk menangani masalah resiko jatuh ialah dengan latihan keseimbangan. Metode square stepping exercise (SSE) dipilih sebagai metode latihan keseimbangan yang digunakan. Hasil yang didapatkan selama memberikan asuhan kepererawatan menggunakan metode ini didapatkan peningkatan gaya berjalan klien yang lebih cepat saat dilakukan pengukuran awal dan akhir, lebih mahir melakukan gerakan latihan dan peningkatan kemampuan perpindahan kaki dari tiap pijakan latihan.  Latihan ini direkomendasikan untuk diterapkan di nursing home sebagai aktifitas latihan rutin untuk mencegah resiko jatuh lansia.

 


The elderly was the individuals which in the final stages of development. Musculosceletal system is a system that identical to frequent changes in the elderly. Changes in that system, have an impact on decreasing the strength of muscles, bones and joints which can cause elderly to fall. This paper aims to explain the results of nursing care to elderly who have a risk of fall. Morse fall scale (MFS) and Timed up and Go test (TUG) were used to measure risk of fall. Balance exercise is one of nursing intervention that can deal to risk of fall. The square stepping exercise (SSE) method was chosen as the balance exercise method used. The result obtained that there was increased gait during initial and final measurements; more skillfull when performing exercises movement and increased ability to move each steps. This exercise is recommended to be applied at nursing home as a new activities to prevent the risk of fall in elderly."
