"Introduction: The most common cause of the peripheral arterial disease (PAD) is atherosclerosis. PAD is associated with other atherosclerotic diseases such as renal artery stenosis (RAS). Life expectancy decreases in patients with RAS, especially those whose stenosis is above 60% but has not reached the stage of chronic kidney failure. This study aims to determine the prevalence of RAS in PAD patients, the relationship between angiographic scoring system (ANGIO Score), history of hypertension, and diabetes mellitus with the degree of RAS. Method: This research was a cross-sectional study conducted at Cipto Mangunkusumo Hospital from February to May 2019. Patients with a diagnosis of lower extremity PAD and had been assessed with CT angiography examination, were included in this study. The degree of RAS and ANGIO Score were calculated. Sampling was done by the total sampling method. Results Most patients were women 33 (50.8%), while men were 32 (49.2%). 90.8% of the patients had diabetes, while 61.5% of the sample had hypertension. Grade 1 RAS was the most found. There was no correlation between ANGIO Score on age, sex, and diabetes mellitus, but there was a significant relationship with hypertension. There was a relationship between RAS with age and hypertension, but there was no relationship with diabetes mellitus and gender. ANGIO Score and RAS had a significant relationship (p <0.001). Conclusion: There was a relationship between the ANGIO Score and the severity of RAS. The cut-off score of 9 for the ANGIO Score had a sensitivity of 85.7% and a specificity of 61.4% for predicting RAS."

"Aterosklerosis sampai saat ini merupakan penyebab utama morbiditas dan mortalitas di negara maju. Meskipun modifikasi faktor risiko di negara maju telah dapat menurunkan kekerapan aterosklerosis namun penurunan ini mulai menunjukkan grafik yang mendatar. Keadaan ini merangsang para peneliti untuk mencari faktor pajanan lingkungan termasuk faktor infeksi yang dapat mempengaruhi proses aterosklerosis. Telah dilakukan penelitian potong lintang dari bulan Maret 1998 sampai Agustus 1998 terhadap 122 orang pasien yang secara klinis menunjukkan penyakit jantung koroner yang menjalani kateterisasi jantung, terdiri dari 92 orang laki-laki dan 30 orang perempuan dengan rerata umur 55 tahun. Pasien diperiksa secara klinis dan laboratorium (gula darah, kolesterol, trigliserida dan antibodi terhadap C.pneumoniae, Cytomegalovirus dan H.pylori). Pada penelitian ini didapatkan perbedaan kadar kolesterol, trigliserida dan HDL antara kelompok stenosis koroner dan non stenosis. Sedangkan kadar antibodi C.pneumoniae, Cytomegalovirus, H.pylori tidak berbeda bermakna. Penelitian ini belum dapat menyimpulkan pengaruh antibodi terhadap aterosklerosis karena pada kelompok non stenosis tidak dapat disingkirkan kemungkinan terjadinya aterosklerosis mengingat rerata umur subyek penelitian 55 tahun. Penelitian mengenai interaksi infeksi dengan risiko tradisional serta gender dan nutrisi diperlukan untuk mendapat jawaban yang lebih jelas tentang pengaruh infeksi terhadap aterosklerosis. Atherosclerosis is still the chief cause of morbidity and mortality in developed nations. Even though in developed nations the modification of risk factors is able to reduce the prevalence rate of atherosclerosis, such reduction is starting to slow down. Such condition has stimulated researchers to identify environmental exposure, including infection, that can influence the process of atherosclerosis. This cross sectional study was conducted from March to August 1998, on 122 patients that clinically demonstrate coronary heart disease and have underwent cardiac catheterization, 92 males and 30 females with an average age of 55 years. Patients undergo clinical and laboratory evaluation (blood glucose, cholesterol, triglyceride, and antibody for C.pneumoniae. Cytomegalovirus, and H.pylori). We found a significant difference in cholesterol, triglyceride, and HDL levels in those with coronary stenosis and those without. However, we did not find a significant difference in the levels of C.pneumoniae, Cytomegalovirus, and H.pylori antibodies. This study is unable to conclude the influence of these antibodies on atherosclerosis, since in the non-stenosis group, we cannot eliminate the possibility of atherosclerosis, since the average age of study subject is 55 years. Studies on the interaction between infection and traditional risk factors as well as gender and nutrition is needed to find a clear answer of the influence of infection in atherosclerosis. "ABSTRAK
Latar Belakang: Aterosklerosis merupakan penyakit sistemik, bisa terjadi di seluruh pembuluh darah. Pada arteri karotis terjadi penebalan tunika intima yang dapat dideteksi menggunakan penunjang non invasif yaitu dengan ultasound. Pada tungkai dapat dilakukan pemeriksaan non invasif yaitu Ankle-Brachial Index (ABI) dan Toe-Brachial Index (TBI) untuk mengetahui adanya penyakit arteri perifer, dengan asumsi bahwa adanya penurunan ABI atau TBI menunjukkan sudah ada stenosis. Berdasarkan hal ini diduga terdapat hubungan antara ABI dan TBI dengan Carotid Intima Media Thickness (CIMT). Tujuan: Mendapatkan hubungan antara ABI dan TBI dengan CIMT. Metode: Dilakukan studi potong lintang pada 36 pasien diabetes tipe II. Dilakukan pemeriksaan ABI dan TBI bila memenuhi kriteria Penyakit Arteri Perifer, dilanjutkan dengan pemeriksaan ultrasound untuk mengetahui ketebalan tunika intima karotis. Hubungan antara ABI dan TBI dengan CIMT dihitung dengan Spearman. Hasil: Rerata ABI yang diperoleh adalah 0,97 ± 0,15, rerata TBI 0,56 ± 0,1. Nilai tengah CIMT 0,96 (0,77 - 3,60). Tidak terdapat hubungan antara ABI dengan CIMT (r=-259, p=0,127) dan terdapat hubungan negatif bermakna antara TBI dengan CIMT (r=-0,47, p=0,004). Simpulan: Tidak terdapat hubungan antara ABI dengan CIMT. Terdapat hubungan negatif bermakna antara TBI dengan CIMT.

ABSTRACT
Background: Atherosclerosis is a systemic disease that can be found in all arteries. Carotid Intima Media Thickness can be measure with ultrasound. Peripheral Artery Disease can be assessed with Ankle Brachial Index (ABI) And Toe Brachial Index (TBI). Low ABI or TBI can detect stenosis on the lower extremity arteries. Objective:To determine correlation between Ankle Brachial Index And Toe Brachial Index With Carotid Intima Media Thickness Methods: A cross sectional study on type II diabetic patients. Peripheral artery were assessed with Ankle Brachial Index and Toe Brachial Index. Carotid Intima Media Thickness measured with ultrasound. Correlation between ABI and TBI with CIMT were calculated with Spearman correlation test. Results: Mean Ankle Brachial Index were 0,97 ± 0,15, mean Toe Brachial Index 0,56 ± 0,1. Median of Carotid Intima Media Thickness 0,96 (0,77 - 3,60). Correlation between ABI with CIMT (r=-259, p=0,127) and TBI with CIMT (r=-0,47, p=0,004) Conclusions:There were no correlation between Ankle Brachial Index with Carotid Intima Media Thickness. Aterosklerosis adalah fenomena penyempitan arteri karena penumpukan plak di dinding arteri sebagai akibat dari suatu pola hidup tidak sehat. Menurut pendekatan ilmu sosial, pola hidup yang salah dapat ditularkan kepada orang lain disekitarnya. Bentuk penanganan pasien aterosklerosis adalah melalui operasi bypass yang harus dilakukan oleh dokter spesialis dan di rumah sakit tertentu, yang keduanya memiliki jumlah yang terbatas. Jika jumlah pasien dengan komplikasi aterosklerosis terus bertambah, itu akan berdampak pada pengobatan yang tidak lagi optimal. Penelitian ini bertujuan untuk membangun model penyebaran aterosklerosis tanpa dan dengan mempertimbangkan pengaruh keterbatasan sumber daya rumah sakit, yang dikenal sebagai efek saturasi. Kedua model dibentuk dengan membagi kompartemen manusia dalam populasi yang rentan, terinfeksi aterosklerosis, dan penderita aterosklerosis yang menjalani pengobatan. Model-model yang telah dibangun kemudian dianalisis secara analitik dan numerik. Studi analitik dilakukan untuk menemukan dan menganalisis titik keseimbangan, menentukan bilangan reproduksi dasar (R0), dan menyelidiki keberadaan bifurkasi dari model yang dibangun. Bifurkasi maju, mundur dan maju dengan hysteresis muncul dari model yang telah terbentuk. Hasil analitik didukung oleh simulasi numerik terkait elastisitas dan sensitivitas R0 serta simulasi autonomous.

 


Atherosclerosis is a narrowing of the arteries due to the buildup of plaque on the artery walls resulting from an unhealthy lifestyle. According to the social science approach, the wrong lifestyle can be ”infection” to other people. The form of treatment for atherosclerosis patients is through bypass surgery, which must be performed by specialists and in specific hospitals, both of which have a limited number. If the number of patients with atherosclerosis complications continues to increase, it will result in no longer optimal treatment. This study aims to build a model of atherosclerosis spread without and taking into account the effect of limited hospital resources, known as the saturation effect. Both models were formed by dividing the human compartment into populations susceptible, infected with atherosclerosis, and people with atherosclerosis who are undergoing treatment. The models that have built are then analyzed analytically and numerically. Analytical studies carried out to find and analyze the equilibrium point, determine the basic reproduction number (R0), and investigate the existence of a bifurcation of the built model. Forward bifurcation, backward bifurcation, and forward bifurcation with hysteresis appear from the model that has formed. The analytical results supported by numerical simulations related to elasticity and sensitivity of R0 as well as autonomous simulations.

"Latar belakang: Elabela merupakan peptida baru yang ditemukan tahun 2013, bersama dengan apelin bertindak sebagai ligan endogen yang mengikat reseptor angiotensin receptor-like 1 (APJ), diekspresikan di berbagai jaringan tubuh terutama pembuluh darah dan memperlihatkan profil kardiovaskular yang mirip dengan apelin. Studi-studi ekperimental dan klinis telah memperlihatkan hubungan elabela dengan hipertensi, namun belum banyak diketahui mengenai peran elabela dalam proses aterosklerosis pada pasien hipertensi. Tujuan: Mengetahui hubungan konsentrasi peptida elabela dalam plasma dengan aterosklerosis subklinis pada populasi hipertensi. Metode: Sebanyak 104 subyek dengan hipertensi diikutkan dalam studi potong lintang. Konsentrasi elabela diperiksa dengan metode enzyme-linked immunosorbent assay (ELISA) dengan terlebih dahulu melakukan ekstraksi peptida sesuai protokol. Parameter aterosklerosis subklinis dinilai dengan pemeriksaan ketebalan intima-media (KIM) karotis menggunakan alat ultrasonografi. Hasil: Konsentrasi elabela pada kelompok hipertensi derajat 2 lebih rendah dibandingkan kelompok hipertensi stadium 1 (0.14 [0.09, 0.23] ng/ml vs. 0.23 [0.13, 0.45] ng/ml; P = 0.000). Konsentrasi elabela lebih rendah pada kelompok dengan KIM karotis yang meningkat (0.15 [0.10, 0.23] ng/dl vs. 0.24 [0.13, 0.38] ng/dl; P = 0.005). Elabela memiliki korelasi linier negatif bermakna terhadap tekanan darah sistolik (r = -0.340, P = 0.000) dan KIM karotis (r = -0.213; P = 0.030). Konsentrasi elabela yang rendah (cutoff 0.155 ng/dl) berhubungan dengan subkelompok risiko tinggi kardiovaskular (OR 5.0, IK 95% 1.8-13.5, P <0.001). Kesimpulan: Penelitian ini untuk pertama kalinya memperlihatkan hubungan elabela dengan aterosklerosis subklinis terkait hipertensi. Rendahnya konsentrasi elabela dikaitkan dengan patogenesis aterosklerosis terkait hipertensi sehingga menjadikan elabela sebagai kandidat penanda biologis (biomarker) vaskular yang baru.

In multivariate analysis, lower elabela levels were associated with a higher cardiovascular risk group in this study (OR 5.0, 95% CI 1.8-13.5, P < 0.001). Conclusions: This study demonstrated for the first time that circulating elabela declined in a higher stage of hypertension and hypertensive patients with increased carotid IMT, implicating that elabela may be involved in the pathogenesis of hypertension-associated subclinical atherosclerosis"

"ABSTRAK
Penyebab PAD yang paling sering adalah atherosclerosis. PAD berhubungan dengan penyakit atherosklerosis lain seperti renal artery stenosis (RAS). Angka harapan hidup menurun pada pasien yang mengalami RAS, terutama yag stenosisnya diatas 60% namun belum sampai ke tahap gagal ginjal kronik. Dengan penatalaksanaan yang holistik diharapkan angka harapan hidup pasien semakin meningkat.
Tujuan: Untuk mengetahui prevalensi RAS pada pasien PAD serta melihat hubungan antara Angio Score, riwayat hipertensi dan diabetes melitus terhadap derajat RAS yang terjadi.
Metode: Desain yang digunakan adalah desain potong lintang. Penelitian ini dilakukan di RSUPN Dr. Cipto Mangunkusumo selama periode Februari hingga Mei 2019. Penelitian dilakukan di RSUPN Dr. Cipto Mangunkusumo dengan diagnosis lower extremity PAD dan dari pemeriksaan CT Angiografi didapatkan stenosis pada pembuluh darah tungkai, serta tampak arteri renalis pada pemeriksaan CTA yang dilakukan pada pasien tersebut. Pengambilan sampel dilakukan dengan metode total sampling.
Hasil: sampel terbanyak berjenis kelamin wanita (50,8%) sedangkan pria sebanyak 32 orang (49,2%). Sebanyak 90,8% pasien yang diteliti menderita diabetes sedangkan 61,5% dari sampel menderita hipertensi. RAS derajat 1 merupakan yang terbanyak ditemukan. Tidak ada hubungan ANGIO Score terhadap usia, jenis kelamin dan diabetes mellitus, namun ada terhadap hipertensi. Terdapat hubungan antara RAS dengan usia dan hipertensi, namun tidak terdapat hubungan terhadap diabetes mellitus dan jenis kelamin. ANGIO Score dan RAS terdapat hubungan yang bermakna (p<0,001).
Simpulan: Perbandingan ANGIO Score berdasarkan derajat stenosis mendapatkan hasil uji Kruskall Wallis mendapatkan nilai p<0,001 dan dilanjutkan dengan uji Mann Whitney mendapatkan bahwa perbedaan sudah terjadi saat perbandingan derajat stenosis 0 dengan derajat 1 dan seterusnya (p<0,001). Semakin tinggi nilai ANGIO Score (cut off pada score 9), angka spesifitas semakin tinggi.

With holistic management, it is expected that the patient's life expectancy will increase.
Objective: To determine the prevalence of RAS in PAD patients and to see the relationship between Angio Score, history of hypertension and diabetes mellitus to the degree of RAS that occurred.
Method: The design used is a cross-sectional design. This research was conducted at RSUPN Dr. Cipto Mangunkusumo during the period February to May 2019. The study was conducted at RSUPN Dr. Cipto Mangunkusumo with a diagnosis of lower extremity PAD, which obtained an ABI score of <0.9, severe ischemia until both unilateral and bilateral limb necrosis and CT angiography examination found stenosis in the leg veins, and the appearance of the renal artery on CTA examination performed on patients that is. Sampling is done by the total sampling method.
Results: the most samples were female (50.8%) while men were 32 (49.2%). As many as 90.8% of patients studied had diabetes while 61.5% of the samples suffered from hypertension. 1st degree RAS is the most found. There is no relationship between ANGIO Score with age, gender and diabetes mellitus, but there is a hypertension. There is a relationship between RAS and age and hypertension, but there is no relationship to diabetes mellitus and gender. ANGIO Score and RAS have a significant relationship (p <0.001).
Conclusion: The comparison of ANGIO Score based on the degree of stenosis obtained the results of the Kruskall Wallis test obtained a p value of <0.001 and continued with the Mann Whitney test found that the difference had occurred when the ratio of stenosis degrees was 0 with degrees 1 and so on (p <0.001). The higher the ANGIO score (cut off at score 9), the higher the specificity."

"Latar Belakang: Carotid stiffness (CS) merupakan perubahan fungsional pada arteri karotis akibat aterosklerosis. Diabetes mellitus tipe 2 (DMT2) akan mempercepat dan memperburuk aterosklerosis sehingga meningkatkan risiko kejadian kardiovaskular. Sampai saat kini belum ada data di Indonesia tentang CS pada pasien penyakit jantung koroner (PJK) stabil dengan DMT2 yang menggunakan sistem otomatis echotracking ultrasound berbasis frekuensi radio. Penelitian ini bertujuan untuk membandingkan CS pada pasien PJK stabil dengan dan tanpa DMT2.
Metode: Comparative cross-sectional antara kelompok pasien PJK stabil dengan dan tanpa pasien DMT2. Pemeriksaan CS dilakukan dengan posisi pasien berbaring telentang secara non-invasif pada 1 cm sebelum bulbus arteri karotis kiri dan kanan menggunakan automatic echotracking radiofrequency-based ultrasound dengan probe linear 3-13 MHz. Pengukuran CS dilakukan sebanyak enam kali pada masing-masing sisi arteri karotis dengan nilai tertinggi rerata carotid Pulse Wave Velocity (car-PWV) sebagai nilai CS individu.
Hasil: Dari total 42 pasien (21 pasang) yang diperiksa didapatkan nilai rerata car-PWV pasien PJK stabil dengan DMT2 lebih tinggi dibandingkan pasien PJK stabil tanpa DMT2 (9,8±1,3m/s vs 6,7±1,3m/s, p< 0,001).
Kesimpulan: Nilai carotid stiffness pasien PJK stabil dengan DMT2 lebih tinggi dibandingkan pasien PJK stabil tanpa DMT2.

Background: Carotid stiffness (CS) represents the functional changes in carotid arteries due to atherosclerosis. Progression of atherosclerosis was more accelerated in type 2 diabetes mellitus (T2DM) compared to non-diabetic patient, thus increasing the risk of cardiovascular events. Until now there is no data of CS in stable coronary artery disease (CAD) with T2DM in Indonesia using automatic echotracking radiofrequency-based ultrasound. The aim of this study was to compare CS in stable CAD with and without T2DM patient.
Method: Comparative cross-sectional between group of stable CAD with and without T2DM patients. CS was measured in patient lying down non-invasively at 1 cm proximal to bulbus of the left and right carotid artery using automatICechotracking radiofrequency-based ultrasound system, 3-13 MHz linear probe. The highest mean carotid pulse wave velocity (car-PWV) value of six measurements of both side was used as an individual CS.
Result: Total 42 patients (21 pairs) was examined. Mean value of car-PWV stable CAD with T2DM patient is higher than stable CAD without T2DM patient (9.8 1.3 m/s vs. 6.7 1.3 m/s, p<0.001).
Conclusion: Carotid stiffness value of stable CAD with T2DM patient is higher than stable CAD without T2DM patient.

"Latar Belakang: Anti dsDNA merupakan salah satu faktor risiko aterosklerosis yang berasal dari LES dan belum ada penelitian yang melihat hubungan antara kadar anti dsDNA dengan ketebalan tunika intima-media arteri karotis. Penelitian ini bertujuan untuk melihat korelasi antara anti dsDNA dengan ketebalan tunik intima-media arteri karotis.
Metode: Penelitian ini adalah penelitian potong lintang, melibatkan 84 pasien LES dengan kriteria inklusi adalah pasie LES yang memenuhi kriteria diagnosis sesuai dengan ACR 1997 atau SLICC 2012, dan kriteria eksklusi adalah bila terdapat variasi anatomi pembuluh darah yang tidak dapat dilakukan pengukuran. Anti dsDNA diperiksa dengan menggunakan ELISA dan USG Doppler dilakukan pada pasien untuk mengukur ketebalan maksimal tunika intima media arteri karotis (max-IMT). Analisa statistik dilakukan dengan uji parametrik Pearson dan bila tidak memenuhi syarat dilakukan uji non parametrik Spearman.
Hasil: Delapan empat responden (82 perempuan dan 2 laki-laki) dilakukan analisa. Rerata usia pasien 35,5±8,9 tahun dengan 64,3% berusia di bawah 40 tahun, median anti dsDNA 38,9 IU/L(0,9 ? 750 IU/L) dan Median max-IMT adalah 581 μm (385-1800 μm). Terdapat 43 (51,2 %) pasien dengan ketebalan pada tunika intima-media arteri karotis, 36 (42,9%) pasien dengan ketebalan saja, 6 (7,1%) pasien dengan ketebalan pada tunika intimamedia dan plak dan 1 (1,2%) pasien dengan plak di near wall bulbus kiri tanpa disertai dengan ketebalan pada tunika intima-media. Plak terutama ditemukan pada bulbus karotis kanan dan kiri. Berdasarkan uji korelasi speraman's tidak terdapat korelasi antara ati dsDNA dengan ketebalan maksimal tunika intima media arteri karotis. (r = 0,073, p= 0,520).
Kesimpulan: Tidak terdapat korelasi antara anti dsDNA dengan ketebalan tunika intima-media arteri karotis pada pasien LES.

Background: Anti dsDNA is considered as one of SLE-related risk factors for atherosclerosis. The evaluation of Carotid intimal-media thickness has recently became one of the surrogate markers for atherosclerosis. Until now, there hasn't been any study relate the level of anti dsDNA antibody with Carotid intimal-media thickness. This study is conducted to determine the correlation between anti dsDNA and Carotid intima-Media Thickness.
Methods: This is a cross sectional study, 84 SLE patients were included. Patients diagnosed as SLE according to ACR 1997 or SLICC 2012 criteria were included in the study, while SLE patients with anatomical variation which difficult to measured were excluded from this study. Doppler ultrasound was carried out for patients and max-IMT was measured. Anti dsDNA was measured with ELISA.
Study results: Eighty four subjects (82 female, 2 male) were included. Mean age was 35,5 ±8,9 years old, 64,3 % between 18-39 years old, median anti dsDNA level 38,9 IU/L (0,9 - 750 IU), and median max-IMT value was 581 μm. There were 43 (51,2 %) patients Carotid intima-media thickness, 36 (42,9%) patients with increased IMT only, 6 (7,1%) patients with increase IMT and Plaque, and 1 (1,2%) patient with plaque in near wall left bulbus without increased IMT. Based on spearman's correlation test there are no correlation between anti dsDNA and max-IMT (r=-0,073, p= 0,520).
Conclusion: There are no correlation between anti dsDNA level and Carotid intimal-media thickness this study.

"Latar Belakang: Peningkatan prevalensi penderita diabetes melitus (DM) meningkatkan risiko aterosklerosis arteri karotis interna (internal carotid artery, ICA). Di negara maju, 85% kasus stroke terjadi akibat aterosklerosis karotis. Short chain fatty acid (SCFA) adalah produk metabolisme bakteri yang terutama disintesis di usus besar dan berperan mengurangi aktivasi endotel oleh mediator proinflamasi. Sehingga mencegah progresi aterosklerosis ICA. Penelitian ini bertujuan mengetahui hubungan SCFA feses dengan gambaran ultrasonografi ICA pada penderita DM tipe 2 di RS Dr. Cipto Mangunkusumo (RSCM).
Metode: Desain penelitian ini adalah observasional analitik jenis potong lintang. Data diperoleh dari seluruh pasien DM tipe 2 di Divisi Bedah Vaskular dan Endovaskular RSCM. Meliputi kadar SCFA sampel feses dan gambaran ultrasonografi (carotid intima-media thickness (IMT), diameter lumen, peak systolic velocity (PSV), end diastolic velocity (EDV), dan flow volume). Uji korelasi Spearman dilakuan untuk memperoleh koefisien korelasi. Nilai p <0,05 bermakna signifikan.
Hasil: Dari 30 subjek DM tipe 2, terdapat 12 laki-laki (40,0%) dan setengah populasi berusia >60 tahun. Hasil pemeriksaan IMT berhubungan signifikan dengan jenis kelamin (p=0,048). Kadar SCFA berhubungan signifikan dengan usia, yaitu asetat (p=0,029), proprionat (p=0,005), butirat (p=0,039), dan SCFA total (p=0,024). Kadar SCFA valerat berkorelasi signifikan dengan IMT (r = -0,237; p=0,034) dan diameter lumen (r = -0,243; p=0,031).
Kesimpulan: Kadar SCFA feses berkorelasi dengan gambaran ultrasonografi arteri karotis interna. Nilai kadar SCFA feses pada pasien DM tipe 2 di RSCM lebih tinggi dibandingkan penelitian lain. Peningkatan kadar SCFA menurunkan risiko penyempitan arteri sklerosis interna

Background: The increasing prevalence of diabetes mellitus (DM) increases the risk of atherosclerosis of the internal carotid artery (ICA). In developed countries, 85% of stroke cases occur due to carotid atherosclerosis. Short-chain fatty acids (SCFA) are products of bacterial metabolism which are mainly synthesized in the large intestine and play a role in reducing endothelial activation through pro-inflammatory mediators, thus preventing the progression of ICA atherosclerosis. This study aims to determine the correlation between faecal SCFA and ICA ultrasonography in patients with type 2 diabetes at Dr. Cipto Mangunkusumo General Hospital (CMGH).
Methods: This study is cross-sectional. Data were obtained from all type 2 DM patients in the Vascular and Endovascular Surgery Division. Data that were collected included faecal SCFA levels and ultrasonography examination (carotid intima-media thickness (IMT), lumen diameter, peak systolic velocity (PSV), end-diastolic velocity (EDV), and flow volume). Spearman correlation test was conducted to obtain the correlation coefficient. The p-value <0.05 was significant.
Results: Of the 30 subjects, 12 were male (40.0%) and half the population was >60 years old. BMI examination results were significantly related to gender (p=0.048). SCFA levels were significantly related to age, including acetate (p=0.029), proprionate (p=0.005), butyrate (p=0.039), and total SCFA (p=0.024). SCFA valerate levels were significantly correlated with BMI (r = -0.237; p=0.034) and lumen diameter (r = -0.243; p=0.031).
Conclusion: Fecal SCFA levels correlated with ultrasound images of the internal carotid artery. The value of faecal SCFA levels in type 2 DM patients at CMGH was higher than in other studies. Elevated SCFA levels decrease the risk of ICA narrowing or stenosis.
