Pengkajian nyeri merupakan bagian dari manajemen nyeri yang harus diperhatikan. Pengkajian nyeri yang akurat sangat penting untuk manajemen nyeri yang efektif. Pada area intensive care pengkajian nyeri menjadi sulit dilakukan karena adanya beberapa hambatan, salah satunya adalah adanya gangguan komunikasinya atau pasien tidak dapat melaporkan rasa nyeri secara verbal. Perawat sebagai tenaga kesehatan sudah seharusnya tetap melakukan pengkajian nyeri pada pasien di area intensive care, namun pengkajian nyeri pada pasien di area intensive care masih belum seluruhnya dilakukan. Tujuan penelitian ini adalah untuk melihat gambaran sikap dan perilaku perawat dalam melakukan pengkajian nyeri di ruang perawatan intensive care. Metode yang digunakan pada penelitian ini adalah survey deskriptif menggunakan pendekatan cross sectional. Sampel dalam penelitian ini sebanyak 80 perawat yang bekerja di ruang intensif yang diambil secara total sampling. Hasil penelitian menunjukkan bahwa 51,25% responden memiliki sikap positif dan 57,50% responden memiliki perilaku yang baik dalam melakukan pengkajian nyeri di ruang intensif. Rekomendasi dari penelitian ini adalah perlunya meningkatkan pengetahuan dan motivasi perawat melalui pendidikan dan pelatihan terkait pengkajian nyeri sehingga manajemen nyeri di ruang intensif dapat lebih adekuat.

 


Pain assessment is a part of pain management that must be considered. Accurate pain assessment is essential for effective pain management. In the intensive care area, pain assessment becomes difficult because of several obstacles, one of which is the communication disorder or the patient cannot report pain verbally. Nurses as health workers should still carry out pain assessments in patients in intensive care areas, but assessments of pain in patients in intensive care areas are still not fully carried out. The purpose of this study was to describe the attitudes and behavior of nurses in conducting pain assessments in the intensive care room. The method used in this study is a descriptive survey using a cross sectional approach. The sample in this study were 80 nurses working in the intensive room who were taken by total sampling. The results showed that 51.25% of respondents had a positive attitude and 57.50% of respondents had good behavior in conducting pain assessments in the intensive room. The recommendation of this study is the need to increase the knowledge and motivation of nurses through education and training related to pain assessment so that pain management in intensive rooms can be more adequate

"Infeksi aliran darah banyak terjadi pada pasien yang terpasang kateter vena sentral. Infeksi sistemik mengakibatkan lama rawat karena memerlukan terapi pengobatan yang panjang dan berdampak pada mahalnya biaya perawatan. Perilaku perawat yang sesuai dengan standar prosedur operasional dapat mencegah terjadinya infeksi aliran darah mulai dari pemasangan, perawatan, dan pencabutan kateter vena sentral.
Tujuan penelitian ini adalah mengetahui gambaran perilaku perawat dalam pencegahan infeksi aliran darah melalui kateter vena sentral di ruang perawatan intensif. Penelitian ini merupakan penelitian deskriptif dengan metode cross sectional terhadap 107 perawat di sebuah RS di Jakarta diambil dengan teknik simple random sampling.
Hasil penelitian menunjukkan bahwa perawat yang berperilaku baik dalam pencegahan infeksi aliran darah melalui kateter vena sentral sebesar 54,2%. Hasil penelitian ini merekomendasikan penambahan informasi dan pelatihan mengenai CVC bundle pada perawat serta penyusunan dan sosialisasi standar prosedur operasional tentang perawatan dan pencabutan kateter vena sentral, termasuk desinfektan yang digunakan.

The infections in bloodstream often occur in patient with Central Venous Catheter (CVC). Systemic infection can result in longer hospitalization due to requiring lengthy treatment therapy and has an impact on the higher cost of treatment expenses. Appropriate nurse behavior standards of operational procedures can prevent bloodstream infections starting from the CVC insertion preparation, CVC maintenance, and CVC removal.
The purpose of this research was to describe the behavior of nurses in the prevention central line associated with bloodstream infections in intensive care. This research is a descriptive research using cross sectional method involving 107 nurses in the hospital in Jakarta. The respondents were taken by using simple random sampling.
The result of this study, showed that the behavior of nurses in the prevention of bloodstream infections related to Central Venous Catheter is good behavior 54,2%. This research recommends the importance of the provision of information and training on CVC bundle in nurses, as well as the promotion of standards operational procedures about CVC maintenance and removal, including disinfectant used."

"ABSTRAK
Perilaku caring adalah wujud nyata dari dukungan perawat baik fisik maupun
emosional dalam memberikan asuhan keperawatan pada pasien dengan tujuan untuk
memberikan perlindungan, membantu meningkatkan kesehatan dan merupakan sifat
dasar dari keperawatan. Sifat ini yang membedakan perawat dengan profesi lain.
Penelitian yang dilakukan di ruang ICU anak RS Pusat Jantung dan Pembuluh Darah
Harapan Kita ini merupakan penelitian kuantitatif dengan metode survei deskriptif,
yang memiliki tujuan untuk melihat sejauh mana perilaku caring perawat ICU anak
terhadap respon nyeri anak toddler pada saat pencabutan selang WSD berdasarkan
penilaian yang dilakukan secara observasi. Sampel dalam penelitian ini sebanyak 37
perawat pelaksana ICU anak yang diambil secara simple random sampling.
Instrumen yang digunakan oleh peneliti berdasarkan 10 karatif Watson yang telah
dilakukan uji validitas dengan pakar ilmu keperawatan dan Uji rialibilitas interrater.
Hasil penelitian menunjukkan 73% responden memiliki perilaku caring tinggi.
Untuk meningkatkan penerapan perilaku caring di RSJPDHK peningkatan
pengetahuan melalui pelatihan dan pendidikan menjadi rekomendasi dalam
penelitian ini.

ABSTRACT
Caring behavior is real manifestation of nurse supports either physical or emotional in
giving nursing care for the patients in purpose of giving protection, helping to
improve the health and is the fundamental nature ofnursing, this trait that
distinguishes the nurses with other professions. The research which is conducted at
the children?s ICU of National cardiovaskuler center harapan kita hospital is a
quantitative research with descriptive survey method, which has the goal to see haw
far the caring behavior of a nurse based on the assessment by observing the nurse
while doing collaboration of removal WSD tube. The samples in this research are 37
of children?s ICU nurses which is taken in simple random sampling. The instruments
used created by the researcher based on 10 caratif factors of Watson which has done
the validity test with the sign nursing expert and inter-reater reliability twice. The
result showed 73% of respondents have high caring behavior. Need to enhance
through training and education to improve the applicaton of caring behavior be the
purpose of this research."

"Pengukuran kinerja perawat dalam sistem layanan kesehatan merupakan hal yang tidak terpisahkan dalam upaya meningkatkan kualitas dan citra keperawatan. Ruang perawatan intensif sebagai bagian dari layanan kesehatan yang memiliki karakteristik ruang perawatan berbeda dengan unit lainnya, hingga saat ini belum memiliki instrumen untuk mengukur kinerja perawat pelaksana. Tujuan penelitian terwujudnya instrumen pengukuran kinerja perawat pelaksana ruang perawatan intensif rumah sakit di Indonesia. Penelitian menggunakan desain research and development, dengan jumlah responden sebanyak 722 perawat pelaksana yang ditentukan dengan teknik total sampling. Penilaian kinerja perawat pelaksana diperoleh melalui penilaian diri sendiri, atasan, dan sejawat. Hasil penelitian berdasarkan uji pakar didapatkan nilai Content Validity Index 0.78 untuk keterwakilan, dan 0.70 untuk kejelasan. Nilai validitas dan reliabilitas instrumen berdasarkan uji Alpha-Cronbach untuk penilaian diri sendiri antara 0.25-0.56, dengan reliabilitas 0.86, untuk penilaian pimpinan diperoleh nilai antara 0.73-0.93, dengan realibilitas 0.95, dan penilaian sejawat nilainya antara 0.52-0.83 dengan reliabilitas 0.97. Hasil uji Goodness of Fit menyatakan model fit dengan data, ditunjukkan dengan nilai RMSEA (≤005) untuk evaluasi diri 0.045, atasan 0.05, dan sejawat 0.075. Selanjutnya, hasil penghitungan koefisien loading factor untuk penilaian diri sendiri diperoleh 14 butir pernyataan valid (> 0.5), sedangkan atasan dan sejawat hanya 7 butir pernyataan yang valid. Hasil tersebut menunjukkan semua butir valid mengukur satu hal yang sama, yaitu kinerja perawat pelaksana. Simpulan penelitian bahwa instrumen yang telah dikembangkan dan diberi nama Instrumen WIDA (Work performance Instrument of intensive care nurses Direct Assessment) ini dapat digunakan untuk mengukur kinerja perawat pelaksana di ruang perawatan intensif rumah sakit. Rekomendasi ditujukan kepada Direktorat Keperawatan Kementrian Kesehatan, instrumen WIDA dapat digunakan sebagai alat pengukur kinerja perawat di ruang intensif berbagai rumah sakit di Indonesia.

The assessment of nursing work performance in health care system may influence the improvement of quality and image of nursing working performance. Intensive care unit has specific service that differs from general and other unit in the hospital, and yet has not had a nursing work performance tool to measure their staff within the unit. The purpose of this study was to establish a Nursing Work Performance assessment tool in Intensive Care in Indonesia. This study used a research and development design, and 722 staff nurses were recruited as respondents using a total sampling method. The tool was developed for three types of assessment head nurse, peer & self evalution. Based on an expert review, it was found that. Content Validity Index score is 0.78 for representation, and 0.70 for clarity. The validity score Alpha- Cronbach) for self assessment between 0.25-0.56, and reliability of 0.86. The head nurse assessment had scores between 0.73-0.93, and the reliability of 0.95, whereas the peer assessment score between 0.52-0.83, and the reliability 0.97. The result of Goodness of Fit showed that RMSEA (≤0.05) for self assessment is 0.045, the head nurse assessment is 0.05, and the peer assessment is 0.075. Further,the result of loading factor for self assessment found 14 statement items are valid (> 0.5), whereas the head nurse and the peer assessment are only 7 statement items valid. Those valid items measure the same variable which is nursing work performance. Conclusion: this study had succeeded to establish a valid instrument to measure nursing work performance in intensive care which named as WIDA (Work performance Instrument of intensive care nurses Direct Assessment). A recomendation is directed to Directorate of Nursing at Ministry of Health Republic of Indonesia to utilize the instrument of WIDA as a tool to measure Nursing work performance in variety hospital in Indonesia."

"Ruangan perawatan intensif yang kompleks dan dilengkapi berbagai peralatan medis berbasis alarm berisiko menyebabkan perawat mengalami alarm fatigue akibat paparan alarm secara terus-menerus. Penelitian ini memberikan gambaran kejadian alarm fatigue berdasarkan karakteristik responden di ruang intensif RS X Jakarta. Penelitian ini merupakan penelitian kuantitatif dengan desain pendekatan cross- sectional. Sampel penelitian berjumlah 256 perawat dengan rentang usia ≤ 25 - ≥ 46 tahun di seluruh ruang perawatan intensif dewasa RSUPN Dr. Cipto Mangunkusumo. Sampel didapatkan dengan teknik total sampling jenis non random sampling. Penelitian ini menggunakan kuesioner yang telah diuji reliabilitas dengan nilai Cronbach Alpha 0,744. Hasil penelitian dianalisis menggunakan analisis statistik deskriptif dan dilakukan uji normalitas Kolmogorov-Smirnov dengan hasil p value 0,028 (P< 0,05). Pengkategorian dilakukan dengan cut-off poin, nilai median 44. Rekomendasi berkaitan dengan penelitian ini adalah dengan teridentifikasinya gambaran alarm fatigue berdasarkan karakteristik responden untuk selanjutnya dilakukan pelatihan manajemen alarm sebagai pembekalan kepada perawat.

An intensive care unit that is complex and equipped with various alarm-based medical equipment is at risk of causing nurses to experience alarm fatigue due to continuous exposure to alarms. This study provides an overview of the incidence of alarm fatigue based on the characteristics of respondents in the intensive care unit of Hospital X Jakarta. This study is a quantitative study with a cross-sectional approach design. The study sample amounted to 256 nurses with an age range of ≤ 25 - ≥ 46 years in all adult intensive care units of Dr. Cipto Mangunkusumo Hospital. The sample was obtained using a total sampling technique of non-random sampling type. This study used a questionnaire that had been tested for reliability with a Cronbach Alpha value of 0.744. The results were analyzed using descriptive statistical analysis and the Kolmogorov-Smirnov normality test with a p-value of 0.028 (P < 0.05). Categorization was done with cut-off points, a median value of 44. Recommendations related to this study are the identification of a picture of alarm fatigue based on the characteristics of respondents for further alarm management training as a provision to nurses."

"Pasien yang dirawat diruang perawatan intensif banyak terpasang alat-alat invasif yang dapat membuat pasien merasa tidak nyaman, nyeri dan kadang gelisah. Kolaborasi pemberian sedasi merupakan salah satu intervensi yang dapat memberikan rasa nyaman. Perawat berperan penting dalam pemantauan sedasi sehingga perlu paham terkait manajemen sedasi di ruang rawat intensif. Penelitian ini bertujuan untuk mengetahui hubungan pengetahuan perawat dengan praktik pemberian sedasi di ruang intensif di salah satu rumah sakit rumah sakit tipe A di Jakarta. Metode yang digunakan adalah cross sectional study dengan 80 responden, alat ukur yang digunakan adalah Nurses Knowledge of Sedatives, Sedation Assessment and Management Questionnaire dan Nurses Sedation Practice Scale Questionnaire. Hasil analisis one way anova menunjukkan bahwa terdapat perbedaan yang signifikan pada rata-rata praktik pemberian sedasi oleh perawat menurut tingkat pengetahuan perawat dengan p Value = 0,000. Penelitian ini merekomendasikan bahwa peningkatan pengetahuan perawat dapat dilakukan melalui program pendidikan dan pelatihan yang bertujuan untuk meningkatkan pengetahuan terkait pengkajian dan manajemen sedasi.

Patients who are treated in intensive care unit are predominantly equipped by invasive devices which initiate discomfort, pain and restlessness to the patients. Collaboration in offering sedation is one of the intervention that can provide a feeling of comfort. Nurses play an important role in monitoring sedation hence they need to understand the sedation management in the intensive care unit. This study aimed to determine the correlation of nurses' knowledge with the practice of providing sedation in the intensive care unit in a type A hospital in Jakarta. The research methodology used a cross sectional study with 80 respondents and used the Nurses Knowledge of Sedatives, Sedation Assessment and Management Questionnaire and Nurses Sedation Practice Scale Questionnaire. The results of the one way anova analysis showed that there was a significant difference in the average sedation practice by nurses according to the level of knowledge of nurses with p Value = 0.000. This study recommends that increasing the knowledge of nurses can be done through education and training programs that aims to increase the knowledge related to sedation assessments and managements."

Perawat yang mengalami alarm fatigue membahayakan keselamatan pasien karena dapat menyebabkan kematian sebagai dampak mengabaikan alarm kegawatdaruratan. Pengelolaan alarm dilakukan dengan manajemen kebisingan namun belum semua rumah sakit menerapkannya. Penelitian ini bertujuan untuk mengidentifikasi hubungan manajemen kebisingan dengan alarm fatigue pada perawat di ruang perawatan intensif. Rancangan penelitian ini adalah penelitian kuantitatif dengan pendekatan cross sectional pada dua kelompok sampel yang dipilih dengan teknik total sampling. Hasil penelitian menunjukkan bahwa nilai alarm fatigue lebih rendah di ruang intensif yang telah melaksanakan manajemen kebisingan dibandingkan yang belum menerapkan. Penerapan manajemen kebisingan hanya sebesar 38,3-62,75% dengan rerata tingkat alarm fatigue 29,387%. Faktor yang paling berpengaruh dengan alarm fatigue adalah jenis kelamin (perempuan lebih berisiko mengalami alarm fatigue). Semakin baik penerapan manajemen kebisingan area kamar pasien akan semakin menurunkan tingkat alarm fatigue perawat (p=0,001; r=-0,240). Pimpinan rumah sakit diharapkan dapat meningkatkan penerapan manajemen kebisingan area kamar pasien dan mengevaluasi penerapan manajemen keseluruhan dalam rangka mencegah alarm fatigue lebih dini pada perawat di ruang intensif.

 


Nurses who undergo alarm fatigue put patient safety at risk as it can lead to death as an impact of ignoring emergency alarms. Alarm management is implemented with noise management, but not all hospitals have done the same. This research aimed to identify the relationship between noise management and alarm fatigue on nurses at intensive care unit. The research design was quantitative research with cross sectional approach done in the two-sampled group`s selected using total sampling technique. The results of the research indicated that the alarms fatigue was lower in intensive care unit, which did implement the noise management than those, which did not. The implementation of noise management was only 38.3-62.75% with the average of alarm fatigue level of 29.387%.  The most influential factor with alarm fatigue was gender (women are more at risk of undergoing alarm fatigue). The better the implementation of noise management in patient`s room area, the lower the level of alarm fatigue on nurse will be (p=0.001; r = -0.240). Hospital leaders are expected to be able to improve the implementation of noise management in the patient`s room area and evaluate the implementation of overall management for the early prevention of alarm fatigue on nurses at intensive care unit.

"Unit perawatan intensif menjadi tempat kerja yang dapat menyebabkan stres kerja bagi perawat. Tujuan penelitian adalah mengetahui gambaran tingkat stres kerja perawat di Unit Perawatan Intensif Neonatal dan Pediatrik RSUPN Dr Cipto Mangunkusumo. Penelitian dilakukan secara cross sectional dengan desain deskriptif sederhana pada 75 perawat.
Hasil penelitian memperoleh sebagian besar responden mengalami tingkat stres sedang yang didominasi perawat perempuan (75,7%), perawat yang telah menikah (73,6%), tingkat pendidikan diploma(76,6%), status kepegawaian PNS (81,6%) dan mayoritas gaji responden antara Rp 3.000.000- Rp 4.000.000. Program untuk menurunkan tingkat stres perawat diperlukan agar perawatan kepada pasien menjadi optimal.

Intensive care unit is workplace that can lead to job stress for nurses. The research aim is to determine the stress level overview of nurses working in the Neonatal Intensive Care Unit and Pediatric RSUPN Dr Cipto Mangunkusumo. The study was conducted by cross sectional with a simple descriptive design to 75 nurses.
The results showed the majority of respondents experienced stress levels are predominantly female nurses (75.7%), nurses who have been married (73.6%),level of education diploma (76.6%), employment status of civil servants (81.6% )and the majority of respondents salary between Rp 3,000,000 to Rp 4,000,000. The program is to reduce the stress level required nursing care to patients in order to be optimal.

"Terbatasnya waktu respon, penurunan daya tahan tubuh, serta banyaknya prosedur invasif yang dilakukan perawat kepada pasien menjadi penyebab tingginya risiko infeksi terkait pelayanan kesehatan di instalasi gawat darurat dan ruang perawatan intensif. Insiden infeksi terkait pelayanan kesehatan dapat dicegah dan dihindari melalui penerapan kepatuhan kewaspadaan standar. Namun, tingkat kepatuhan kewaspadaan standar pada perawat masih tergolong rendah. Perilaku kepatuhan kewaspadaan standar pada perawat dapat dipengaruhi oleh beberapa faktor, seperti faktor demografi, predisposisi, pemungkin, dan penguat. Penelitian ini bertujuan untuk mengidentifikasi faktor-faktor yang berhubungan dengan kepatuhan kewaspadaan standar pada perawat. Desain riset menggunakan deskriptif korelasi dengan pendekatan cross sectional. Penelitian ini melibatkan 100 perawat ruang perawatan intensif dan IGD yang dipilih dengan menggunakan metode convenience sampling. Data penelitian dikumpulkan dengan menggunakan kuesioner. Nilai kepatuhan kewaspadaan standar perawat relatif cukup rendah yaitu sebesar 54.66 (SD = 4.68) atau sekitar 67.89 persen dari total nilai kepatuhan tertinggi. Hasil penelitian lebih lanjut menunjukkan bahwa usia (p = 0.939), lama bekerja (p = 0.564), jenis kelamin (p = 0.064), tingkat pendidikan (p = 0.870), unit kerja (p = 0.078), jenjang karir (p = 0.919), pelatihan (p = 0.065), pengetahuan (p = 0.137), sikap (p = 0.738), ketersediaan fasilitas (p = 0.810), standar prosedur operasional (p = 0.229), dan dukungan atasan (p = 0.436) tidak memiliki hubungan yang bermakna. Hanya efikasi diri (p = 0.009) yang memiliki hubungan yang bermakna dengan kepatuhan kewaspadaan standar. Penelitian ini menyimpulkan bahwa efikasi diri merupakan faktor yang esensial untuk meningkatkan kepatuhan kewaspadaan standar. Hasil penelitian ini merekomendasikan rumah sakit untuk menyelenggarakan atau menggiatkan program atau aktivitas yang mampu meningkatkan efikasi diri perawat agar tingkat kepatuhan kewaspadaan standar perawat dapat meningkat.

The limited response time, decreased immune system, and many invasive procedures performed by nurses to the patient are responsible for the high risk of healtcare-associated infections (HAIs) in the emergency and intensive care units. The incidence of HAIs can be prevented and avoided through compliance on standard precautions. However, the level of nurses compliance on standard precautions is still low. The compliance on standar precautions can be influenced by several factors, such as demographic, predisposing, enabling, and reinforcing factors. This study aimed to identify factors affecting nurses compliance on standard precautions.  This study used descriptive correlation with cross sectional approach. The study involved 100 intensive care and emergency care nurses who were selected using convenience sampling method. Data were collected using questionnaires. The study revealed that the mean score of nurses compliance on standard precaution was 54.66 (SD = 4.68) or 67.89 percent of the total correct score. The results of the study, furthermore, showed that there was no significant correlation between age (p = 0.939), working experience (p = 0.564), gender (p = 0.064), level of education (p = 0.870), working unit (p = 0.078), level of career (p = 0.919), training (p = 0.065), knowledge (p = 0.137), attitude (p = 0.738), facility (p = 0.810), standard operational procedure (p = 0.229), managerial support (p = 0.436) with standard precaution compliance. Only self-efficacy showed significant correlation with standard precaution compliance (p = 0.009). The study concluded that self- efficacy could increase nurses compliance on standard precautions. The results of the study recommended the hospitals conducting programs or activities that may enhance nurses efficacy, because it can improve nurses compliance on standar precautions."
