"Tuberkulosis (TBC) merupakan salah satu penyakit menular yang menjadi permasalahan kesehatan global. Indonesia menempati peringkat kedua kasus TBC tertinggi di dunia setelah India, diikuti Cina, menjadikan penyakit TBC sebagai isu kesehatan yang mendesak untuk segera ditangani. Namun, pemerintah Indonesia baru mengadopsi kebijakan penanggulangan TBC melalui Peraturan Presiden (Perpres) No. 67 Tahun 2021, meskipun bantuan dari Global Fund telah tersedia sejak tahun 2003. Kelambanan ini mengindikasikan terdapat dinamika ekonomi politik internasional dan nasional dalam merespon isu tersebut. Penelitian ini bertujuan menganalisis faktor-faktor penyebab kelambanan tersebut dengan menggunakan teori difusi norma dari Finnemore dan Sikkink (1998). Pendekatan kualitatif diterapkan dengan metode studi kasus dan process tracing . Data diperoleh melalui analisis dokumen resmi, kajian literatur dan wawancara mendalam. Hasil penelitian menunjukkan bahwa adopsi kelambanan hadirnya kebijakan pemerintah dalam penanganan TBC dipengaruhi oleh lemahnya kesadaran politik, hambatan birokrasi, dan prioritas kebijakan yang berubah-ubah. Proses difusi norma melalui tahapan emergence, cascade, dan internalization memakan waktu lama akibat resistensi internal dan pandemi COVID-19. Namun, tekanan internasional dari WHO, Global Fund, dan USAID ditambah advokasi dari masyarakat sipil, akhirnya mendorong pemerintah untuk menerbitkan Perpres No. 67 Tahun 2021. Implementasi kebijakan ini mencerminkan keberhasilan internalisasi norma kesehatan global dalam waktu yang lama. Selain itu, implementasinya masih menghadapi tantangan koordinasi dan pendanaan. Pendekatan multisektoral dan berbasis komunitas menjadi kunci keberhasilan penanggulangan TBC di Indonesia.

Tuberculosis (TB) is one of the infectious diseases that constitutes a global health issue. Indonesia ranks second in the world for the highest TB cases following India and ahead of China, making TB an urgent health issue that requires immediate attention. However, the Indonesian government only adopted a TB eradication policy through Presidential Regulation (Perpres) No. 67 of 2021, despite support from the Global Fund being available since 2003. This delay indicates the presence of international and national political-economic dynamics in responding to the issue. This research aims to analyze the factors causing the delay using the norm diffusion theory by Finnemore and Sikkink (1998). A qualitative approach was employed, utilizing case study and process tracing methods. Data were gathered through the analysis of official documents, literature reviews, and in-depth interviews. The findings reveal that the delay in adopting government policies to address TB was influenced by weak political awareness, bureaucratic obstacles, and shifting policy priorities. The norm diffusion process, encompassing the stages of emergence, cascade, and internalization, took a prolonged time due to internal resistance and the COVID-19 pandemic. However, international pressure from WHO, the Global Fund, and USAID, combined with civil society advocacy, eventually pushed the government to issue Perpres No. 67 of 2021. The implementation of this policy reflects the successful internalization of global health norms over an extended period. Additionally, its implementation continues to face challenges related to coordination and funding. A multisectoral and community-based approach remains key to the successful eradication of TB in Indonesia."
