"Abstrak
Physical excercises are hoped to improve functional capacity and quality of life of patients with chronic obstructive pulmonary disease (COPD). This study is aimed to assess the differences between treadmill and ergocycle exercise on changes in functional capacity and quality of life in patients with stable COPD.
This is an experimental study with complete randomization and repeated pre and post-intervention observation. The study was conducted on 44 subjects with COPD who were presented at Persahabatan Hospital, Medical Rehabilitation Clinic, consisting of 22 subjects undergoing ergocycle exercise and 22 subjects undergoing treadmill exercise test. Functional capacity was assessed by using the 6 minutes walking test (6 MWT) performed at week I, V and IX. Quality of life was measured using the St. George Respiratory Questionnaire (SGRQ) at week I and IX. Training program was conducted for 8 weeks with exercise dosage based on the results of exercise test on week I. Treadmill and ergocycle exercise produce significant improvement in both the 6 MWT and SGRQ test since week I to IX. But in comparison, treadmill exercise improves 6 MWT distance better than ergocycle consistently at week I-V, V-IX and I-IX (p < 0.001). For the SGRQ score, both exercises did not differ significantly.
Conclusion: Treadmill exercise is associated with significantly better improvement in functional capacity compared to ergocycle exercise in stable COPD subjects. Concerning quality of life, both exercises gave an equivalent improvement in stable COPD subjects."

"Tujuan: Penelitian ini bertujuan untuk menilai perbedaan latihan jentera dan sepeda statis terhadap perubahan kapasitas fungsional dan kualitas hidup pada penderita Penyakit Paru Obstruktif Kronik (PPOK) kondisi stabil.
Metode: Metode penelitian eksperimental dengan jumlah sampel 44 orang, terdiri dari 22 orang dengan latihan sepeda statis dan 22 orang dengan latihan jentera yang datang ke poli Rehabilitasi Medik RS Persahabatan. Penilaian kapasitas fungsional menggunakan metode Uji Jalan 6 Menit (UJ6M) dilakukan minggu I, V dan IX. Penilaian kualitas hidup diukur menggunakan St. George’s Respiratory Questionnaire (SGRQ) dilakukan minggu I dan IX. Program latihan dilakukan selama 8 minggu.
Hasil: Latihan jentera dan sepeda statis menghasilkan perbaikan signifikan baik dalam hal hasil uji jalan 6 menit dan SGRQ sejak minggu I sampai IX. Namun dalam perbandingan latihan yang memberikan hasil terbaik, jentera meningkatkan jarak tempuh jalan 6 menit lebih baik dibandingkan sepeda statis secara konsisten pada minggu I–V, V-IX dan I-IX (p <0,001). Untuk nilai SGRQ, hasil kedua latihan tidak berbeda signifikan.
Kesimpulan: Kelompok latihan jentera memiliki peningkatan kapasitas fungsional yang lebih besar dan berbeda bermakna dibandingkan kelompok latihan sepeda statis pada subjek PPOK stabil. Kelompok latihan jentera memiliki peningkatan kualitas hidup yang tidak berbeda bermakna dibandingkan kelompok latihan sepeda statis pada subjek PPOK stabil.

"Latar Belakang: Perubahan sistemik pada PPOK menyebabkan terjadinya disfungsi otot yang berhubungan dengan penurunan fungsi keseimbangan. Gangguan keseimbangan menimbulkan konsekuensi terhadap kejadian jatuh. Penambahan latihan keseimbangan pada PPOK dapat meningkatkan fungsi keseimbangan, namun belum menjadi standar tatalaksana pada program rehabilitasi PPOK. Latihan ketahanan dengan menggunakan jentera dan sepeda statis menunjukkan adanya peningkatan nilai uji fungsi keseimbangan pasien PPOK, namun belum ada penelitian yang membandingkan antara kedua latihan tersebut dalam meningkatkan fungsi keseimbangan pasien PPOK.
Tujuan: Menilai efek latihan jentera dan latihan sepeda statis selama delapan minggu terhadap perbaikan fungsi keseimbangan pasien PPOK.Metode. Uji klinis teracak terhadap pasien PPOK stabil grup A,B,C dan D pada usia 55-80 tahun. Subjek dibagi dalam dua kelompok, yaitu kelompok jentera dan kelompok sepeda statis. Kedua kelompok mendapat program rehabilitasi paru selama delapan minggu. Dilakukan evaluasi fungsi keseimbangan dengan menggunakan Berg Balance Scale BBS pada awal penelitian, 4 dan 8 minggu setelah mulai penelitian.
Hasil: Terdapat 16 subjek PPOK yang menyelesaikan penelitian. Didapatkan peningkatan yang signifikan secara statistik pada nilai BBS baik pada latihan jentera maupun pada latihan sepeda statis setelah delapan minggu latihan dengan nilai akhir BBS 51,88 dan 50,25 secara berurutan. Tidak didapatkan perbedaan yang bermakna secara statistik antara selisih peningkatan nilai BBS latihan jentera dan latihan sepeda statis dengan nilai tengah 3,00 dan 3,50 secara berurutan.
Kesimpulan: Nilai BBS menunjukkan perbaikan bermakna secara statistik baik pada latihan jentera maupun sepeda statis. Tidak terdapat perbedaan efek yang bermakna secara statistik antara latihan pada kedua kelompok tersebut.

Background: Systemic changes in COPD result in muscle dysfunction that associated with decreased balance function. Impaired balance has consequences for falling events. The addition of balance exercises to COPD can improve balance function, but it has not yet become the standard treatment for COPD rehabilitation programs. Endurance exercises using treadmill and static cycle show an increase in balance function test of COPD patients, but no studies have compared the two exercises in order to improve the balance function of COPD patients.
Aim: To assess the effects of treadmill and static cycle exercise for eight weeks on improving balance function of COPD patients. Method. Randomized Clinical trials of stable COPD patients on A, B, C and D group at age 55 80 years. Subjects were divided into two groups, treadmill and static cycle group. Both groups received pulmonary rehabilitation program for eight weeks. Evaluation of balance function using Berg Balance Scale BBS at the beginning of the study, 4 and 8 weeks after the study.
Results: There were 16 subjects of COPD who completed the study. There was a statistically significant increase in the value of BBS in both treadmill and static cycle group after eight weeks of exercise with a final BBS score of 51.88 and 50.25 respectively. There was no statistically significant difference between the improvement value of BBS in treadmill and static cycle exercise with median values of 3.00 and 3.50 respectively.
Conclusion: The BBS score showed statistically significant improvements in treadmill and static cycles exercise. There was no statistically significant different effect of exercises in both groups.

"Pasien penderita penyakit pant obstruktif kronik ( PPOK ) tampaknya mendapatkan manfaat dari program rehabiltiasi paru. Penelitian ini mengkaji manfaat program rehabilitasi paru pada pasien rawat jalan yang menderita PPOK, dengan menggunakan St George Respiratory Questionnaire (SGRQ) dan six min walking distance test (6MWD), yang mengukur kualiti hidup kesehatan dan toleransi latihan fungsional sebagai hasil pengukuran utama. Penelitian ini merupakan penelitian prospektif, terbuka, acak dengan kelompok kontrol paralel yang diberikan program rehabilitasi pasien rawat jalan pada 56 pasien penderita PPOK (52 orang laki-laki dan 4 orang perempuan). Kelompok aktif(n= 27) diberikan program edukasi dan latihan selama 6 minggu. Kelompok kontrol (n= 29) diperiksa secara rutin sebagai pasien medis rawat jalan. SGRQ dan 6MWD ditakukan pada saat awal penelitian dan setelah 6 minggu. Didapatkan hasil SGRQ dan 6MWD sebelum dan sesudah terapi. Berdasarkan statistik, SGRQ menurun dan skor 6MWD meningkat secara signifikan pada kelompok aktif dibandingkan kelompok kontrol. Disimpulkan bahwa program selama 6 minggu pada pasien rawat jalan ini secara signifikan telah rneningkatkan kualiti hidup dan kapasitas fungsional pasien PPOK derajat ringan hingga sedang. Patients with chronic obstructive pulmonary disease (COPD) have been shown to be benefit from pulmonary rehabilitation programs. We assessed an entirely outpatient-based program of pulmonary rehabilitation in patients with COPD, using the Si George's Respiratory Questionnaire (SGRQ) and six minutes walking distance test (6MWD) (which measures health-related quality of life and functional exercise tolerance) as the primary outcome measure. We undertook a randomized, opened, prospective, parallel-group controlled study of outpatient rehabilitation program in 56 patients with COPD (52 men and 4 women). The active group (n~27) took part in a 6-weeks program of education and exercise. The control group (n=29) were reviewed routinely as medical outpatients. The SGRQ and 6MWD were administered at study entry and after 6 weeks. Outcome with SGRQ and 6MWD before and after therapy was performed. Decrease score SGRQ and increase 6MWD in both groups of study, it was analyzed by statistic study and in active group the decrease of SGRQ and the increase of 6MWD was statistically significant. In conclusion 6-weeks outpatient-based program significantly improved qualify of life and functional capacity in mild-to-moderate COPD patient. (Med J Indones 2006; 15:165-72)"

"Rehabilitasi pasien penyakit paru obstruktif kronik (PPOK) yang stabil bertujuan untuk mengurangi sesak, meningkatkan toleransi latihan, dan meningkatkan status kesehatan. Pada dekade terakhir banyak dikembangkan teknik terapi mandiri salah satunya adalah menggunakan perangkat positive expiratory pressure (PEP). Penelitan ini bertujuan untuk menilai efektivitas latihan pernapasan menggunakan perangkat PEP pada volume ekspirasi paksa detik pertama (VEP1), kapasitas fungsional, dan kualitas hidup pada pasien PPOK. Desain penelitian yang digunakan yaitu studi intervensional prospektif yang membandingkan efek sebelum dan sesudah latihan pernapasan menggunakan PEP selama 8 minggu. Subjek yang menyelesaikan penelitian sebanyak 20 orang. Latihan pernapasan dilakukan dua kali sehari dengan durasi 15 menit pada masing-masing sesi latihan. Tekanan yang digunakan disesuaikan dengan kemampuan pasien yaitu inspirasi berbanding ekspirasi 1:3. Tekanan ditentukan sebelum memulai latihan dan dievaluasi setiap dua minggu. Hasil keluaran yang dinilai adalah nilai VEP1, kapasitas fungsional yang diukur dengan kecepatan berjalan dalam uji jalan 4 meter dan kualitas hidup yang diukur dengan kuesioner St. George s Respiratory Questionnaire (SGRQ). Nilai VEP1 sebelum intervensi adalah 1369,5±569,63 ml dan sesudah sebesar 1390±615,01 ml (p=0.585). Kecepatan berjalan sebelum intervensi 1,43±0.31 m/s dan sesudah 1,56±0,40 m/s (p=0.248). Skor kuesioner SGRQ domain gejala terdapat penurunan dari rerata 44,00±17,88% menjadi 25,31±14,06% (p=0.000), domain aktivitas dari rerata 54,22±28,18% menjadi 40,38±24,25% (p=0.006), domain dampak dari 32,83% (0,00-67,46) menjadi 16,32% (0,00-61,33) (p=0.002), dan skor total dari 39,46% (6,30-75,42) menjadi 25,96% (5,24-61,34) (p=0.001). Peningkatan kecepatan berjalan dan perbaikan skor SGRQ memenuhi nilai minimum clinically important difference (MCID). Latihan pernapasan menggunakan perangkat PEP selama 8 minggu dapat meningkatkan nilai VEP1, kapasitas fungsional dan kualitas hidup pasien PPOK.

Rehabilitation of stable chronic obstructive pulmonary disease (COPD) aims to reduce dyspnoe, increase exercise tolerance, and improve health status. In the last decade, many independent therapy techniques have been developed, one of them is positive expiratory pressure (PEP). The aim of this study was to assess the effectiveness of breathing exercises using a PEP device on Forced Expiratory Volume in 1 second (FEV1), functional capacity, and quality of life in COPD patients. The study design was a prospective interventional studies that compared the effects before and after breathing exercises using PEP for 8 weeks. Twenty subjects completed the study. Breathing exercises were carried out twice a day, 15 minutes duration at each session. The pressure used is adjusted to the patient's ability, reaching inspiration to expiration ratio of 1:3. Pressure was determined before starting the exercise and evaluated every two weeks. The outcome were FEV1, functional capacity measured by walking speed in the 4 meter gait speed assesment and quality of life as measured by the St. George s Respiratory Questionnaire (SGRQ). FEV1 ​​before intervention were 1369,5±569,63 ml and after 1390±615,01 ml (p=0.585). Walking speed before intervention was 1,43±0,31 m/s and after 1,56±0,40 m/s (p=0,248). The symptom domain SGRQ questionnaire score has a decrease from 44,00±17,88% to 25,31±14,06% (p=0,000), the activity domain from 54,22±28,18% to 40,38±24,25% (p=0.006), the impact domain of 32,83% (0,00-67,46) to 16,32% (0,00-61,33) (p=0,002), and the total score of 39,46% (6,30-75,42) to 25,96% (5,24-61,34) (p=0.001). Increase in walking speed and SGRQ score exceed the minimum clinically important difference (MCID). Breathing exercises using a PEP device for 8 weeks can increase FEV1, functional capacity and quality of life of COPD patients."

"Tujuan: Mengetahui perbandingan efek latihan isokinetik dan isometrik terhadap nyeri, kekuatan otot dan kemampuan fungsional pada pasien osteoarthritis lutut.
Disain: Eksperimental paralel.
Subjek: 28 orang pasien berusia antara 50-64 tahun, dibagi secara acak menjadi dua kelompok.
Tempat: Bagian Ilmu Kedokteran Fisik dan Rehabilitasi. Perjan RS Dr. Hasan Sadikin. Bandung.
Intervensi: Pasien menjalani program latihan isokinetik atau isometrik selama 6 minggu.
Parameter: VAS, peak torque, indeks Lequesne yang diukur setiap minggu.
Hasil: Kedua kelompok menunjukkan penurunan yang bermakna pada intensitas nyeri (p<0,001) dan indeks Lequesne (p<0,001), juga peningkatan yang bermakna pada peak torque (p<0,001) setelah 6 minggu. Tidak terdapat perbedaan yang bermakna antara kedua kelompok.
Kesimpulan: Kedua jenis latihan berguna pada pasien osteoarthritis berusia lanjut Pada kelompok isokinetik tidak didapatkan subjek yang mengeluh nyeri yang bermakna.

Objective: To compare the effect of isokinetic and isometric strengthening exercise on pain, strength and functional capacity of patients with knee osteoarthritis.
Design: Experimental parallel.
Participants: 28 patients, age 50-64 years, were randomly assigned into two groups.
Setting: Department of Physical Medicine and Rehabilitation. Hasan Sadikin Hospital Bandung.
Interventions: Patients received either a regimen of isokinetic exercise or a regimen of isometric exercise for 6 weeks.
Main outcome measure : VAS, peak torque and Lequesne index were measured each week.
Result: Both training groups showed significant decrease in pain score (pc0, 001) and Lequesne index (p<0, 001) and an increase in peak torque (p<0,001). However there is no significant difference of those parameters between groups.
Conclusion: Both exercises can benefit elderly patients with knee osteoarthritis as shown by the increase of strength and functional capacity. In the isokonetic group there were no subjects who experienced an increase in pain.

Background: obesity and osteoarthritis have strong inter-relationship with multi-factorial mechanism that
caused pain and leads to decreased quality of life. Exercise has been identified as prevention and management
against obesity and joint pain. This systematic review aims to assess the effect between exercises compared to diet
group for chronic pain management, physical and mental function in obese patients with chronic musculoskeletal
problem. Methods: we performed a systematic search of Randomized Control Trial studies from Cochrane CentralRegister of Controlled Trials (CENTRAL); MEDLINE; EBSCO; SCIENCEDIRECT (Elsevier); SCOPUS, grey
literature, trial registry, ongoing study for published studies, and from the ClinicalTrial.gov, thesis of rehabilitation
medicine in RSCM, and proceeding books for unpublished studies that was last updated on November 2016. Risk
of bias was assessed using Cochrane risk-of-bias tool and data were analyzed using Review Manager 2014.
Results: one study showed no difference in pain reduction (assessed using VAS) between two groups. Two studies
showed improvement in physical function measured using 6MWT in exercise group at 6 and 18 months with mean
difference 28.12 [11.20, 45.04] and 26.21 [9.01, 43.41]. There was no significant effects observed for Mental and
Physical Function based on SF-36 after 6 months (1 study) and 18 months (2 studies) observation, with mean
difference 1.10 [-0.79, 2.99] and 0.08 [-1.53, 1.70] respectively for Mental Function score and -0.30 [-2.54, 1.94]
and -0.36 [-2.30, 1.57] respectively for Physical Function score. Conclusion: exercise can improve physical
function objectively, but could not reduce pain in obese patients with chronic musculoskeletal problem subjectively.

"Angka kematian mendadak semakin meningkat setiap tahunnya. Kebanyakan diperkirakan akibat serangan jantung atau penyakit jantung koroner. Frekuensi nadi pemulihan dan kapasitas fungsional merupakan prediktor risiko kematian akibat gangguan jantung. Penelitian dilakukan untuk mengetahui gambaran frekuensi nadi pemulihan dan kapasitas fungsional pada pasien dengan penyakit jantung koroner.
Metode Penelitian ini adalah deskriptif dilakukan dengan melakukan pemantauan terhadap 100 pasien penyakit jantung koroner yang menjalani pemeriksaan treadmill.Frekuensi nadi pemulihan diukur pada menit pertama setelah selesai pemeriksaan lalu dilakukan analisis kategori kapasitas fungsional.
Hasil penelitian dari 53 laki-laki dan 47 perempuan yang mengikuti penelitian dengan rentang usia 40-78 tahun, hanya 31% yang mengalami gangguan frekuensi nadi pemulihan dan 44% yang mengalami gangguan kapasitas fungsional. Penelitian ini merekomendasikan pengkajian frekuensi nadi pemulihan dan kapasitas fungsional perlu dilakukan sebagai dasar dalam memberikan edukasi.

Sudden death rate is increasing every year. Most expected cause is heart attack or coronary artery disease. Heart rate recovery and functional capacity as predictor of risk of death from cardiac event. The study was conducted to reveal the heart rate recovery and functional capacity in patients with coronary artery disease.
The method of study is descriptive, it was done by monitoring 100 coronary artery disease patients who underwent treadmill test. Heart rate recovery measured in the first minute after the treadmill test is completed and then the analysis of functional capacity categories was done.
The results of 53 men and 47 women who followed the study with age range 40-78 years, only 31% of patients were susceptible to abnormal heart rate recovery and 44% of patients were impaired functional capacity. The study recommend that ssessment of heart rate recovery and functional capacity needs to be done as a basis for providing education.
