"Praktik Spesialis Keperawatan Medikal Bedah Kekhususan Respirasi di RSUP Perasahabatan Jakarta merupakan aplikasi peran dan fungsi sebagai ners spesialis dengan menerapkan Model Adaptasi Roy dalam memberikan asuhan keperawatan pada pasien dengan gangguan sistem respirasi baik pada pasien kelolaan utama maupun 30 pasien resume. Selain sebagai pemberi asuhan keperawatan peran perawat spesialis respirasi adalah menerapkan tidakan berbasis bukti ilmiah pada area respirasi yaitu dengan memberikan terapi suara berbasis alam dalam mengurangi kecemasan, agitasi, dan stress pada pasien terpasang ventilator mekanik. Peran selanjutnya dari perawat spesialis respirasi adalah melakukan inovasi melalui analisi situasi dan studi literature dalam hal ini adalah penerapan The Respiratory Distress Observation Scale (RDOS) dalam menilai risiko terjadinya distress pernapasan. Hasil praktik menunjukan bahwa teori Model Adapatasi Roy dalam memberikan asuhan keperawatan pada pasien dengan gangguan sistem respirasi dan suara berbasis alam cukup efektif dalam mengurangi kecemasan, agitasi, dan stress pada pasien terpasang ventilator mekanik. Serta RDOS dapat diterapkan pada pasien dengan gangguan respirasi dalam menilai risiko distress pernapasan.
The Practice of Medical Nursing Specialists Specialized in Respiration in RSUP Perasahabatan Jakarta is the application of roles and functions as specialist nurses by applying Roy's Adaptation Model in providing nursing care to patients with respiratory system disorders in both the main managed patient and 30 resume patients. Aside from being a nursing care provider, the role of respiration specialist nurses is to apply scientific evidence-based nursing actions to the area of ​​respiration by providing nature-based sound terapi to reduce anxiety, agitation, and stress in patients with mechanical ventilators. The next role of respiration specialist nurses is to innovate through situation analysis and literature studies in this case is the application of The Respiratory Distress Observation Scale (RDOS) in assessing the risk of respiratory distress. Practical results show that Roy's adaptation theory in providing nursing care to patients with respiratory and natural-based sound disorders is quite effective in , agitation, and stress in patients with mechanical ventilators. As well as RDOS can be applied to patients with respiratory disorders in assessing the risk of respiratory distress."

"Praktik residensi keperawatan medikal bedah adalah bagian dari pendidikan profesi yang mempunyai tujuan untuk menghasilkan perawat spesialis. Perawat spesialis diharapkan dapat melaksanakan perannya dalam memberikan pelayanan keperawatan pada kasus dengan gangguan sistem pernapasan. Peran tersebut sejalan dengan kompetensi yang harus dicapai selama praktik residensi berupa pemberian asuhan keperawatan, penerapan praktik keperawatan berbasis bukti dan proyek inovasi. Pemberian asuhan keperawatan diterapkan kepada satu pasien kelolaan utama dengan kasus PPOK dan 30 kasus resume menggunakan pendekatan model adaptasi Roy. Penerapan tindakan keperawatan berbasis bukti ilmiah yang dilakukan yaitu pemberian posisi condong kedepan dan Latihan nafas pursed lips breathing pada pasien dyspnea dengan penyakit paru. Proyek inovasi yang diterapkan yaitu Pengembangan Self Manajemen Dengan Video Latihan Breathing Exercise : ACBT Dalam Meningkatkan Airway Clearance Pada Pasien Dengan Penyakit Paru Obstruksi Kronis (PPOK). Hasil analisis praktik menunjukkan bahwa model adaptasi Roy dapat digunakan sebagai pendekatan dalam asuhan keperawatan pasien dengan gangguan sistem pernapasan untuk mengembangkan mekanisme adaptif pasien. Pemberian posisi condong kedepan dan Latihan nafas pursed lips breathing dapat menurunkan sesak napas pada pasien dengan penyakit paru. Selain itu, Latihan Breathing Exercise : ACBT Dalam Meningkatkan Airway Clearance Pada Pasien mendapat evaluasi yang positif dan dapat diterapkan di tatanan klinis sebagai inovasi pengkajian dan manajemen keperawatan mandiri pada pasien PPOK selama di rumah.
Medical surgical nursing residency practice is part of professional education that aims to produce specialist nurses. Specialist nurses are expected to carry out their role in providing nursing services in cases with respiratory system disorders. The role is in line with the competencies to be achieved during residency practice in the form of providing nursing care, implementing evidence-based nursing practice and innovation projects. Nursing care was applied to one primarily managed patient with a case of COPD and 30 resume cases using Roy's adaptation model approach. The application of scientific evidence-based nursing actions carried out is the provision of a forward leaning position and pursed lips breathing exercises in dyspnea patients with lung disease. The innovation project applied is the Development of Self Management with Video Breathing Exercise: ACBT in Increasing Airway Clearance in Patients with Chronic Obstructive Pulmonary Disease (COPD). The results of the practice analysis show that Roy's adaptation model can be used as an approach in nursing care of patients with respiratory system disorders to develop patient adaptive mechanisms. Forward leaning and pursed lips breathing exercises can reduce shortness of breath in patients with lung disease. In addition, Breathing Exercise: ACBT in Increasing Airway Clearance in Patients received a positive evaluation and can be applied in clinical settings as an innovation in self-assessment and nursing management in COPD patients while at home."

"Penyakit Paru Obstruksi Kronis (PPOK) merupakan penyakit paru yang menetap yang dapat dicegah dan diobati, ditandai dengan adanya keterbatasan aliran udara yang menetap dan bersifat progresif dikarenakan abnormalitas saluran napas dan/atau alveolus yang biasanya disebabkan oleh pajanan gas atau partikel berbahaya. Laporan ini dimaksudkan untuk menganalisis pemberian Asuhan Keperawatan dengan penerapan Model Adaptasi Roy pada pasien kelolaan utama dan 30 pasien resume dengan menggunakan metode studi kasus, laporan tentang penerapan latihan pernapasan untuk mengurangi sesak dan meningkatkan toleransi aktivitas pada pasien PPOK dan penerapan RDOS untuk menilai tanda-tanda distres pernapasan. Hasil menunjukkan masalah keperawatan yang muncul adalah hambatan pertukaran gas, nutrisi kurang dari kebutuhan tubuh, ketiddakberdayaan dan ketidakefektifan manajemen kesehatan. Model Adaptasi Roy dapat diterapkan dalam asuhan keperawatan pada pasien PPOK untuk mampu beradaptasi terhadap penyakitnya.
Chronic Obstructive Pulmonary Disease (COPD) is a persistent lung disease that can be prevented and treated, characterized by limited persistent and progressive air flow due to airway abnormalities and / or alveoli which is usually caused by exposure to harmful gases or particles. This report aims to analyze the apllication of Roy Adaptation Model to primary care nursing and the 30 patient resumes using case study methods, the application of respiratory training to reduce dispnea and increase activity tolerance in COPD patients and the application of RDOS to assess the signs of respiratory distress. The results showed nursing problems arise were impaired gas exchange, imbalanced nutrition less than body requirements, powerlessness and ineffective health management. Roy  Adaptation Model can be applied in nursing care for patients with COPD  to adapted the ability of their the disease."

"Praktik spesialis keperawatan medikal bedah merupakan rangkaian dari pendidikan ilmu
keperawatan yang dilaksanakan dalam tatanan pelayanan kesehatan. Selama menjalani proses residensi, residen berperan menjadi Clinical Care Manager yang bertugas sebagai pemberi asuhan keperawatan, sebagai peneliti, konsultan keperawatan bagi staf keperawatan, serta pendidik. Guna mencapai peran tersebut, residen menerapkan teori keperawatan model adaptasi Roy sebagai alat untuk mempermudah pemberian asuhan keperawatan dalam membantu individu, baik dalam kondisi sakit maupun pada proses penyembuhan dan peningkatan pemeliharaan kesehatan di setiap mode fisiologis, konsep diri, fungsi peran dan interdependen secara sistematis. Pada kasus kelolaan dengan tuberkuloma paru diperoleh hasil bahwasanya ada perbaikan kondisi setelah pemberian asuhan keperawatan selama 15 hari dan hasil pada 30 kasus resume pasien dengan penyakit sistem pernapasan terbanyak adalah tuberkulosis paru. Penerapan Evidence Based Nursing menggunakan terapi Eye Movement Training (EMT) didapatkan hasil bahwa kelompok intervensi EMT memiliki nilai perubahan kualitas tidur rerata lebih tinggi sebelum dan sesudah diberikan terapi dibandingkan kelompok kontrol. Pada proyek inovasi, dilakukan program peningkatan kemampuan perawat dalam melakukan perawatan trakeostomi pada pasien dewasa dengan 6 elemen bundle trakeostomi diperoleh hasil bahwa perawat ruangan merasa materi yang diberikan sangat bermanfaat dan mampu laksana sehingga tindak lanjutnya adalah terbentuknya booklet perawatan trakeostomi dan teraplikasi dalam Electronic Medical Record (EMR).
Medical-surgical nursing specialist practice is a series of nursing science education carried out in health care settings. During the residency process, the resident acts as a Clinical Care Manager in charge of providing nursing care, as a researcher, nursing consultant for nursing staff, and educator. In order to achieve this role, residents apply the Roy adaptation model nursing theory as a tool to facilitate the provision of nursing care in helping individuals, both in sickness and in the process of healing and improving health maintenance in each physiological mode, self-concept, role function and interdependence systematically. In the case of management with pulmonary tuberculosis, the results showed that there was an improvement in condition after providing nursing care for 15 days and the results in 30 cases of patient resumes with the most respiratory system diseases were pulmonary tuberculosis. The application of Evidence Based Nursing using Eye Movement Training (EMT) therapy found that the EMT intervention group had a higher mean sleep quality change value before and after therapy than the control group. In the innovation project, a programme to improve the ability of nurses to perform tracheostomy care in adult patients with 6 elements of the tracheostomy bundle was carried out. The results showed that the room nurses felt the material provided was very useful and applicable so that the follow-up was the formation of a tracheostomy care booklet and applied in the Electronic Medical Record (EMR)."

Keperawatan respirasi merupakan salah satu area khusus dimana perawat memiliki peran dalam memberikan asuhan keperawatan berdasarkan kebutuhan pasien. Perawat spesialis respirasi memiliki peran lebih yang berbeda, dengan menerapkan konsep dan teori keperawatan dalam penanganan kasus pada pasien gangguan pernapasan. Peran ners spesialis sebagai klinisi ahli, pendidik, manajer kasus, konsultan, dan peneliti untuk merencanakan atau memperbaiki kualitas keperawatan bagi klien dan keluarganya. Teori keperawatan yang digunakan dalam merawat pasien (satu kasus kelolaan dan 30 kasus resume) dengan menggunakan model adaptasi Roy. Peran sebagai peneliti diterapkan dengan melakukan latihan breathing retraining pada pasien asma. Sementara peran untuk memperbaiki kualitas pelayanan salah satunya adalah menerapkan inovasi bagi pasien tuberkulosis dalam mencegah terjadinya putus obat.

mengelola kasus utama pada pasien tuberkulosis dan diabetes melitus tipe 2 serta

keperawatan yang diberikan. Masalah keperawatan utama yang paling sering
muncul pada pasien dengan gangguan sistem respirasi yaitu bersihan jalan napas

telah menerapkan intervensi keperawatan berdasarkan pembuktian yaitu latihan
pernapasan diafragma pada pasien penyakit paru obstruktif kronis (PPOK). Inovasi
keperawatan yang dilakukan yaitu diskusi interaktif pada pasien tuberkulosis untuk
meningkatkan kepatuhan pasien dalam menjalankan pengobatan tuberkulosis
sampai tuntas.
ABSTRACT
Residential clinical practice medical surgical nursing specializing at implementing
three of primary roles of nurse specialists including case managers, healthcare
providers and innovators. As a case managers, the specialist nurse managed primary
cases of patient with tuberculosis and type-2 diabetes mellitus along with 30 cases
of resume with Roy Adaptation Model approach. The most often found nursing
diagnosis during the clinical practice were ineffective airway clearance and
ineffective breathing pattern. As a healthcare provider, the specialist nurse have
implemented an evidence based nursing intervention which diaphragmatic
breathing exercises for patients with chronic obstructive pulmonary disease
(COPD). Innovation in nursing performed was interactive discussion involving
patients as obedience encouragement to tuberculosis medication until the
completion of the medication regimen"

"Pelaksanaan praktek residensi keperawatan medical bedah peminatan imunologi ditujukan supaya perawat residen mampu memberikan asuhan keperawatan spesialis dalam menerapkan evidence-based nursing (EBN) yang berperan sebagai inovator dan edukator di pelayanan keperawatan. Peran perawat residen sebagai pemberi asuhan keperawatan dilakukan dengan menerapkan teori adaptasi Roy pada pasien dengan HIV dan AIDS dengan TB Paru dan 30 pasien HIV dan AIDS dengan infeksi oportunistik lainnya. Masalah keperawatan yang paling banyak didapatkan yaitu risiko ketidakseimbangan elektrolit, intervensi keperawatan yang dilakukan adalah pemantauan elektrolit dengan tujuan keseimbangan elektrolit meningkat. Penerapan EBN pemberian probiotik yogurt pada pasien dengan HIV, AIDS dan keganasan yang menunjukkan kandidiasis oral berupa plak putih pada lidah. Probiotik yogurt diberikan 2 kali sehari selama 5 hari dengan penilaian evaluasi skor tongue coating Miyazaki, hasil menunjukkan 45,6% terjadi penurunan skor dari grade 3 menjadi grade 0. Proyek inovasi pemberian edukasi dengan materi infeksi oportunistik yang menunjukkan pengetahuan dan sikap ODHA meningkat setelah pemberian edukasi.

The role of the resident nurse as a care giver is carried out by applying Roy's adaptation theory to patients with HIV and AIDS with pulmonary TB and 30 HIV and AIDS patients with other opportunistic infections. The most commonly encountered problem is the risk of going to electrolyte court, the engagement intervention for the risk of electrolyte stress is electrolyte monitoring with the aim of increasing electrolyte balance. Application of EBN giving probiotic yogurt to patients with HIV, AIDS and malignancy showing oral candidiasis in the form of white plaque on the tongue. Probiotic yogurt was given 2 times a day for 5 days with an evaluation of the Miyazaki tongue coating score, the results showed a 45.6% decrease in score from grade 3 to grade 0. An innovation project providing education with material on opportunistic infections which shows that the knowledge and attitudes of PLWHA have increased after providing education."

Abstrak

Praktek klinik lanjut selama praktek residensi keperawatan pada sistem Urologi bertujuan agar dapat memberikan asuhan keperawatan, menerapkan Evidence Based Nursing (EBN) serta mampu berperan sebagai innovator di ruang perawatan. Peran pemberi asuhan keperawatan dilakukan dengan menggunakan model adaptasi Roy pada pasien dengan Karsinoma Ginjal dan 30 pasien lainnya dengan berbagai gangguan sistem urologi. Perilaku maladaptif paling banyak adalah mode adaptasi fisiologi, yaitu kelebihan volume cairan. Intervensi keperawatan berupa manajemen keseimbangan cairan untuk meningkatkan adaptasi pasien dalam keseimbangan cairan. Penerapan EBN dengan music intradialytic pada tiga pasien hemodialisis menunjukkan keefektifan musik intradialytic dalam menurunkan tingkat kecemasan. Program inovasi keperawatan berupa penerapan program peningkatan adekuasi hemodialisis pada pasien ESRD di Instalasi Hemodialisis.

Abstract

Advanced clinical practice during nursing residency practice in the Urology system aims to provide nursing care, implement Evidence Based Nursing (EBN) and be able to act as an innovator in the nursing center. The role of the nursing care provider was performed using Roy's adaptation model in patients with Renal Carcinoma and 30 other patients with various urological system disorders. The most inefektive behavior is the physiological adaptation mode, which is excess fluid volume. Nursing interventions take the form of fluid balance management to improve patient adaptation in fluid balance. The application of EBN with intradialytic music in six patients undergoing hemodialysis therapy demonstrated the effectiveness of music in reducing anxiety levels. The nursing innovation program is in the form of implementing a hemodialysis adequacy program in ESRD patients at the Hemodialysis Installation.

"Praktik spesialis Keperawatan Medikal Bedah peminatan sistem perkemihan ini bertujuan untuk melakukan asuhan keperawatan medikal bedah dengan pendekatan Model Adaptasi Roy pada pasien gangguan sistem perkemihan dan menerapkan peran sebagai care provider. Sebagai penyedia asuhan keperawatan dilakukan kepada 30 orang pasien dengan gangguan sistem perkemihan. Peran sebagai peneliti yaitu menerapkan evidence based nursing dengan membuktikan akupresur sebagai intervensi yang dapat menurunkan keluhan insomnia pada pasien yang menjalani hemodialisis. Peran sebagai inovator melalui pemberian edukasi mengenai gagal ginjal kronik dan penanganan komplikasi akut hemodialisis melalui media video. Hasil praktik ini menunjukkan bahwa Model Adaptasi Roy cocok digunakan pada pasien gangguan sistem perkemihan. Intervensi akupresur bermanfaat untuk mengatasi insomnia dan video edukasi terkait penanganan komplikasi akut hemodialisis bermanfaat untuk meningkatkan pengetahuan pasien dan dapat diterapkan oleh perawat di ruangan hemodialisis.

Role as a researcher in the application of evidence-based nursing by proving acupressure as an intervention that can reduce insomnia in patients undergoing hemodialysis. Role as an innovator through the provision of education about chronic renal failure and treatment of acute hemodialysis complications through video media. The results of this practice show that Roy's Adaptation Model is well suited for urinary system disorders patients. Acupressure interventions effectively for treating insomnia and educational videos related to the treatment of acute hemodialysis complications are significant for improving patient knowledge and can be applied by nurses in the hemodialysis unit. "

"Praktik Spesialis Keperawatan Medikal Bedah peminatan respirasi memiliki tujuan untuk mencapai kompetensi Ners Spesialis Respirasi. Pencapaian kompetensi dilakukan melalui 3 bentuk kegiatan, antara lain: pengelolaan 30 pasien gangguan sistem respirasi dengan menggunakan pendekatan teori model Adaptasi Roy, pengelolaan kasus kompleks dengan pendekatan teori model Adaptasi Roy, penerapan Tehnik Pernapasan Buteyko pada pasien asma sebagai suatu praktik keperawatan berbasis bukti (Evidence Based Practice Nursing), dan pelaksanaan inovasi keperawatan: pembuatan WSD Pionir dengan sistem 1 botol sebagai upaya peningkatan kualitas pelayanan dan keamanan pasien (patient safety). Praktik dilaksanakan selama 9 bulan di RSUP Persahabatan sebagai rumah sakit pusat rujukan nasional penyakit paru. Ners spesialis respirasi mencapai kompetensi sebagai expert practice, educator, innovator/agent of change, researcher, dan consultant selama praktik di rumah sakit. Hasil dari pencapaian kompetensi didapatkan kesimpulan bahwa teori model Adaptasi Roy dapat diterapkan dengan efektif pada kasus-kasus gangguan respirasi terutama kasus TB Paru, Kanker Paru, dan pasien Pneumonia. Penerapan teori Adaptasi Roy pada pasien dengan ventilator sulit diterapkan terutama pada mode pangkajian konsep diri, fungsi peran, dan interdependensi. Pelaksanaan EBNP tehnik pernapasan Buteyko selama 1 bulan memberikan pengaruh secara signifikan pada peningkatan kualitas hidup pasien asma, nilai Arus Puncak Respirasi (APE), dan nilai control pause (CP). Pembuktian keefektifan botol WSD Pionir terhadap kualitas dan kemananan pasien akan diuji secara ilmiah melalui proses penelitian.
The residency practicum of medical surgical nursing with a specialization in respiratory aimed to facilitate residents in achieving the competences as Respiratory Nurse Specialists. After the completion of the residency practicum, students would be able to achieve the competences as clinical experts, educators, innovators/ change agents, researchers, and consultants in respiratory care. Three major activities during the practicum were: 1) the application of Roy adaptation model- based nursing care and complex case management to 30 patients with respiratory disorders, 2) the application of Buteyko breathing method to asthmatic patients as an evidence- based nursing practice, and 3) the implementation of innovative project: the creation of water- seal drainage- pioneer with one- bottle system as an effort to improve patient safety and quality care. The practicum was completed in 9 months at Persahabatan Hospital as a top referral hospital for respiratory care. Results of the practicum activities revealed that Roy adaptation model was effective to improve patient safety as well as quality care to patients with respiratory disorders, particularly patients with pulmonary tuberculosis, lung cancer, and pneumonia. In addition, the results showed that the application of Buteyko breathing method for a month significantly enhanced the quality life of asthmatic patients, increased the respiratory peak flow and control pause values. Further studies, nonetheless, are required to examine the effectiveness of WSD pioneer to the improvement of patient safety and quality care."
