"PURPOSE: Surgical site infection (SSI) is one of the most common healthcare-associated infections (HAIs); however, SSI after hepatobiliary and pancreatic surgery (HBPS) has not been well investigated in a large cohort of patients. This study analyzed the factors associated with SSI following HBPS in Japan, using a Japanese national database.
METHODS: Data on HBPS performed between 2012 and 2014 were extracted from a national monitoring system for HAI: The Japan Nosocomial Infections Surveillance. Using multivariate logistic regression, I assessed the factors associated with SSI.
RESULTS: The cumulative incidence of SSI following HBPS was 15.6% (2873/18,398). The incidence of SSI after pancreatoduodenectomy was 28.0%, which was significantly higher than that after liver resection and other types of HBPS (8.8 and 15.5%, respectively). Among the four traditional risk factors, the American Society of Anesthesiologists score was ineffective for predicting SSI in the final model of all three types of surgery. Additional risk factors were identified, including age and male gender.
CONCLUSIONS: The incidence of and factors associated with SSI after the three types of HBPS analyzed differed significantly. To accurately compare hospital performance in relation to SSI following HBPS, the operative procedure category in the surveillance system must be divided into three types."

"PURPOSE: To clarify the risk factors for complications after diverting ileostomy closure in patients who have undergone rectal cancer surgery.
METHODS: The study group comprised 240 patients who underwent a diverting ileostomy at the time of lower anterior resection or internal anal sphincter resection, in our department, between 2004 and 2015. Univariate and multivariate analyses of 18 variables were performed to establish which of these are risk factors for postoperative complications.
RESULTS: The most common complications were intestinal obstruction and wound infection. Univariate analysis showed that an age of 72 years or older (p = 0.0028), an interval between surgery and closure of 6 months or longer (p = 0.0049), and an operation time of 145 min or longer (p = 0.0293) were significant risk factors for postoperative complications. Multivariate analysis showed that age (odds ratio, 3.4236; p = 0.0025), the interval between surgery and closure (odds ratio, 3.4780; p = 0.0039), and operation time (odds 2.5179; p = 0.0260) were independent risk factors.
CONCLUSIONS: Age, interval between surgery and closure, and operation time were independent risk factors for postoperative complications after diverting ileostomy closure. Thus, temporary ileostomy closure should be performed within 6 months after surgery for rectal cancer."

"PURPOSE: To identify the possible roles of carcinoembryonic antigen (CEA) testing after liver resection for synchronous colorectal liver metastasis (CLM).
METHODS: The subjects of this retrospective study were patients who underwent complete resection of primary tumors and synchronous CLM between 1997 and 2007 at 20 institutions in Japan. We studied the associations between perioperative CEA levels and the characteristics of recurrence.
RESULTS: Recurrence was detected during the median follow-up time of 52 months in 445 (73.7%) of the total 604 patients analyzed. A postoperative CEA level >5 ng/ml was an independent predictor, with the highest hazard ratio (2.25, 95% confidence interval 1.29-3.91, P = 0.004). A postoperative CEA level >5 ng/ml had a specificity of 86.2% and a positive predictive value of 84.2% for recurrence. Patients with a high postoperative CEA level had a significantly higher recurrence rate, with a shorter time until recurrence and a higher frequency of multiple metastatic sites than those with a low postoperative CEA level. Among the patients with recurrence, 173 (52.7%) had an elevated CEA level (>5 ng/ml) when recurrence was detected.
CONCLUSIONS: A postoperative CEA level >5 ng/ml was an independent predictor of recurrence; however, CEA testing was not a reliable surveillance tool to identity recurrence after liver resection."

"PURPOSE: A triplet regimen of docetaxel, cisplatin, and S-1(DCS) is highly effective against metastatic gastric cancer. We performed this study to clarify the safety and efficacy of surgical resection in patients with initially unresectable gastric cancer, after down-staging or disease control was achieved by DCS chemotherapy.
METHODS: The subjects of this retrospective study were 31 consecutive patients with initially unresectable gastric cancer, who underwent surgical resection between October, 2006 and December, 2012, after downstaging or disease control was achieved by DCS chemotherapy. We evaluated the clinicopathological factors and clinical outcomes and assessed radiographic response based on the RECIST criteria, not by central review.
RESULT: Before DCS chemotherapy, 18 patients had extraregional lymph node metastasis, 5 had liver metastasis, 8 had macroscopic peritoneal metastasis, and 8 had pancreatic head invasion. Twenty-three (74.2%) of the 31 patients underwent R0 resection. Postoperative morbidity and mortality rates were 16.1 and 0%. During chemotherapy, grade 3/4 toxicities included neutropenia (54.8%), leukopenia (32.3%), and anemia (16.1%). Median progression-free survival and median overall survival (OS) were 42.1 and 56.1 months, respectively. These results were similar for all patients, except those with locally advanced disease alone. In the multivariate analysis for OS, ypN remained an independent negative prognostic factor (p = 0.018).
CONCLUSION: Surgical resection after DCS chemotherapy for initially unresectable gastric cancer was safe and provided a reasonable R0 resection rate and good midterm survival."

"PURPOSE: To assess the safety and feasibility of laparoscopic gastrectomy (LG) for gastric cancer patients with a history of abdominal surgery (HAS).
METHODS: This retrospective study analyzed data collected from gastric cancer patients with HAS, who underwent LG between 2004 and 2015. We compared the clinicopathological features that correlated with conversion to open surgery and the development of severe postoperative complications (Clavien-Dindo classification of grade III or higher).
RESULTS: Of the 41 patients identified, 6 (14.6%) required conversion to open surgery. The incidence of conversion to open surgery was associated with a history of lower gastrointestinal tract surgery (p = 0.009), attempted laparoscopic total gastrectomy (p = 0.002), and excessive blood loss (p < 0.001). Severe postoperative complications developed in six patients (14.6%). Although the development of complications was associated with high postoperative serum C-reactive protein, the type of past abdominal surgery was not significantly correlated with severe complications.
CONCLUSIONS: LG was feasible for gastric cancer patients with a HAS, but for those with a history of lower abdominal surgery or those who require total gastrectomy, surgeons should carefully consider the indications for LG."

"PURPOSE: Familial adenomatous polyposis (FAP)-associated desmoid tumor (DT) is sometimes life threatening. However, the optimal treatment for DTs has not been established. The aim of this study was to analyze the outcomes of surgical and pharmacological treatments for DT in Japanese FAP patients.
METHODS: We retrospectively reviewed the data of 303 patients who underwent colectomy for FAP between 2000 and 2012. We analyzed 41 patients with DTs in which the location was apparent. The selection of treatment for intra-abdominal DTs was also evaluated according to Churchs classification.
RESULTS: Surgery was frequently used to treat extraabdominal DTs. Multimodal treatments, including surgery, and the administration of non-steroidal antiinflammatory drugs, hormonal therapy, and chemotherapy were widely used for intra-abdominal DTs. The most effective pharmacological treatment was cytotoxic chemotherapy, which was associated with a response rate of 45.5% and a disease control rate of 72.7%. After a median follow-up period of 53.0 months, the 5-year DT-specific survival rate in patients with stage IV disease was 71.4%; in contrast, the rate in patients with other stages was 100%. Four-stage IV patients died of DT due to uncontrollable rapid progression. No cytotoxic chemotherapy was administered; however, incomplete resection was performed in three cases.
CONCLUSION: Our findings will provide clues that may help physicians in selecting the optimal strategy for this rare disease."

"Latar belakang : Infeksi daerah operasi (IDO) merupakan salah satu infeksi terkait perawatan di rumah sakit, dan meningkatkan morbiditas, mortalitas dan biaya perawatan di rumah sakit. IDO pasca operasi jantung masih merupakan masalah serius. Prevalensi IDO pasca operasi jantung berkisar 0,25 sampai 6%. Banyak faktor risiko yang dapat meningkatkan kejadian IDO. Baik faktor risiko pre-operatif, peri-operatif, intra-operatif maupun pasca-operatif. Faktor usia, status nutrisi, tindakan transfusi, lama rawat inap sebelum dilakukan tindakan dan ketepatan pemberian antibiotik profilaksis dapat menjadi faktor risiko yang memengaruhi kejadian IDO paska operasi jantung.
Tujuan : Mengetahui faktor-faktor risiko yang meningkatkan kejadian IDO operasi jantung anak dan kesintasan pada anak.
Metode : Penelitian kohort retrospektif dengan rancangan penelitian potong lintang yang mengalami IDO pada operasi jantung di RSCM. Data penelitian diambil dari rekam medis. Data yang dikumpulkan adalah usia, status nutrisi, tindakan transfusi, lama rawat inap pasien sebelum dilakukan tindakan operasi dan ketepatan pemberian antibiotik profilaksis terhadap kejadian IDO pasca operasi jantung. Data tersebut kemudian dianalisis dengan analisis univariat, bivariat dan analisis multivariat.
Hasil : Jumlah subyek yang direkrut sebesar 360 subyek, prevalensi IDO sebesar 13,8%. Faktor risiko usia tidak memengaruhi kejadian IDO dengan p=0,178 RR 0,54(0,217-1,327) pada kelompok umur 0-1 tahun, p=0,415 RR 0,72(0,331 – 1,578) pada kelompok usia 1-5 tahun dan p=0,205 RR 0,27(0,035 – 2,052) pada kelompok usia 5 – 10 tahun. Status nutrisi tidak memengaruhi kejadian IDO dengan p= 0,287 RR0,75(0,436-1,278). Lama rawat inap sebelum tindakan operasi tidak memengaruhi kejadian IDO dengan p=0,324 RR 0,772 (0,662-1,292). Ketepatan pemberian antibiotik profilaksis tidak memengaruhi kejadian IDO p=0,819 RR 1,011(0,918-1,114).
Simpulan : Faktor risiko usia, status nutrisi, lama rawat inap sebelum tindakan, ketepatan antibiotik profilaksis tidak memengaruhi kejadian IDO pada operasi jantung anak.

Background : Surgical site infection (SSI) is one of the hospital associated infections, and increases morbidity, mortality and hospital care costs. SSI Post cardiac surgery is still a serious problem. The prevalence of SSI post cardiac surgery ranges from 0.25 to 6%. Many risk faktors can increase the incidence of IDO. Faktors such as age, nutritional status, transfusion , length of hospitalization before surgery and accuracy of prophylactic antibiotik administration can be risk faktors that affect the incidence of IDO after cardiac surgery.
Aime : to investigate the risk faktors in pediatric cardiac surgery that will increase the incidence of SSI and to improve the survival of the child after cardiac surgery.
Method : Retrospective cohort study with cross-sectional research design that undergoes Surgical site infection in cardiac surgery at RSCM. The research data is taken from medical records. The data collected are age, nutritional status, transfusion procedure, length of hospitalization of the patient before surgery and accuracy of prophylactic antibiotik administration against the incidence of postoperative SSI cardiac surgery. The data were then analyzed by univariate, bivariate and multivariate analysis.Result : The number of subjects recruited was 360 subjects, the prevalence of SSI was 13.8%. Age risk factors did not affect the incidence of SSI with p=0.178 RR 0.54(0.217-1.327) in the age group 0-1 years, p=0.415 RR 0.72(0.331 – 1.578) in the age group 1-5 years and p=0.205 RR 0.27(0.035 – 2.052) in the age group 5 – 10 years. Nutrient status does not affect the incidence of SSI with p= 0.287 RR0.75(0.436-1.278). The length of hospitalization prior to surgery did not affect the incidence of SSI with p=0.324 RR 0.772 (0.662-1.292). The accuracy of prophylactic antibiotik administration did not affect the incidence of IDO p=0.819 RR 1.011(0.918-1.114).
Conclusion : risk faktors such as Age, nutritional status, length of hospitalization before treatment, accuracy of prophylactic antibiotiks do not affect the incidence of IDO in pediatric cardiac surgery.

"PURPOSE: Laparoscopic fundoplication (LF) has become a standard operative procedure for GERD-related diseases in Japan, although meta-analyses have mainly evaluated findings from Western countries. The propensity score matching method was used to compare and investigate the treatment outcomes of two fundoplication procedures (the Nissen and Toupet methods).
METHODS: Among 474 patients who underwent initial LF from December 1994 to April 2016, we extracted 401 cases (Nissen: 92 cases, Toupet: 309 cases), excluding 73 patients in whom follow-up was insufficient. We then matched 126 of these patients (63 per group).
RESULTS: The esophageal acid reflux time (%) was 12.2:2.8, being higher in the Nissen group than in the Toupet group (p < 0.001). Regarding the surgical outcome, the amount of bleeding was higher in the Nissen group (p = 0.001), and the number of hospitalization days following surgery was longer (p = 0.003). Furthermore, a significantly rate of postoperative difficulty in swallowing (%) was observed in the Nissen group, at 13:0 (p = 0.004). The recurrence rate (%) was 8:3, with no difference between the two groups (p = 0.243).
CONCLUSIONS: Although there was no marked difference in the recurrence rate between the two procedures, postoperative dysphagia was observed at a higher frequency with the Nissen method than the Toupet method."

"PURPOSES: The purpose of this study was to evaluate the influence of comorbidities on the surgical outcomes of early cholecystectomy for acute cholecystitis.
METHODS: Data were retrospectively collected for patients with acute cholecystitis who underwent early cholecystectomy. Patients were separated into three groups based on the cholecystitis severity grade, and the surgical outcomes of early cholecystectomy were analyzed. Patients with mild and moderate cholecystitis were subdivided into a comorbidity group (n = 10) and a non-comorbidity group (n = 83).
RESULTS: There were 57 (55.3%) patients with mild cholecystitis, 36 (35.0%) with moderate cholecystitis, and 10 (9.7%) with severe cholecystitis. The surgical outcomes were significantly worse for patients with severe cholecystitis than for patients with mild or moderate cholecystitis. There were no postoperative deaths after cholecystectomy. There were no significant differences in the complication rate (P = 0.629), conversion rate (P = 0.114), or intraoperative blood loss (P = 0.147) between the comorbidity and non-comorbidity groups.
CONCLUSION: Our findings suggest that early cholecystectomy can be performed safely for patients with mild and moderate cholecystitis even if comorbidities are present. Early cholecystectomy may be an alternative treatment strategy for patients with severe cholecystitis who are candidates for anesthesia and surgery."

"Purpose: The body mass index (BMI) before lung transplantation (LT) is a benchmark of the post-LT survival. The aim of the study is to determine the BMI inadequate for the post-LT survival.
Methods: We examined the survival after LT in patients grouped into the following BMI categories: <18.5 kg/m2 (underweight), 18.5-24.9 kg/m2 (normal weight), 25-29.9 kg/m2 (overweight), and ≥30.0 kg/m2 (obese) according to the World Health Organization (WHO) criteria. A more detailed categorization was made for further evaluation of the underweight group: mild (17.0 ≤ BMI < 18.5 kg/m2) and severely underweight (BMI <17.0 kg/m2).
Results: There was no statistically significant difference in the post-LT survival between underweight and normal-weight patients (5-year survival: 78.7 vs. 76.1%). Patients with BMI <17.0 kg/m2 had a worse prognosis than those with 17.0 ≤ BMI < 18.5 kg/m2 (5-year survival: 70.3 vs. 90.0%).
Conclusions: Standard BMI categorization per the WHO criteria is inadequate for determining the post-LT survival, especially in underweight patients. For the nutritional evaluation of underweight pre-LT patients, BMI <17.0 kg/m2 should be used instead of BMI <18.5 kg/m2."
