"The purpose of this research was to develop anti-cytokine-based treatment using extract of Plectranthus amboinicus
applied to gout arthritis (GA) patients. The research was quasi experimental, with a pretest-posttest randomized control
group design. The samples were GA patients in the Outpatient Installation of Internal Medicine in General Hospital
Haji, Surabaya. The sample was comprised of 30 respondents. The respondents were divided into a treatment group and
a control group. The treatment group was asked to take medicine from the hospital, coupled with P. amboinicus extract
capsules, for 7 days, during which time patients’ joint inflammation was observed. The control group was provided with
only medication from the hospital, and their joint inflammation was likewise observed. Blood samples were taken
before and after treatment, to measure the levels of blood urea nitrogen (BUN) and creatinine, as well as the
concentrations of TNF-α and IL-1β. There was a decrease in BUN and creatinine levels in the control group, but it was
not significant, decreasing by 3% and 8%, respectively. The treatment group also showed elevated levels of BUN and
creatinine, which also was not significant at 3% and 7%, respectively. There was a decrease in the concentration of
TNF-α in the control group by 9% and 22%. The concentration of IL-1β in the control group increased by 18%,
whereas,in the treatment group, it decreased by 3%; however, the decreases in both groups were not significant.

"Tanaman Jintan (Plectranthus amboinicus) dikenal sebagai tanaman bangun-bangun, dikenal sebagai salah satu tanaman berkhasiat obat bagi masyarakat Indonesia. Penelitian ini bertujuan menganalisis aktivitas ekstrak daun jintan dan mengetahui efek toksisitas akut pada tikus putih yang diinduksi arthritis. Ekstrak dari daun jintan segar disarikan dengan metode maserasi ethanol 96%, dan diencerkan dengan larutan CMC-Na. Tikus putih Wistar jantan dan betina, umur 2-3 bulan dibagi 5 kelompok: Kontrol, induksi arthritis (P1), induksi artritis dan ekstrak daun jintan dosis 19 g/kgBB (P2), induksi artritis dan ekstrak daun jintan dosis 38 g/kgBB (P3) dan kelompok obat allopurinol 2,5 mg/kgBB (P4). Seluruh kelompok tikus diinduksi arthritis menggunakan uric acid 2% dan oxonic acid 1,5% per oral selama 15 hari berturut-turut. Setelah terbentuk lesi arthritis, diberikan ekstrak daun jintan secara intra peritoneal selama 7 hari. Sampel serum darah diambil sebelum dan sesudah perlakuan untuk mengukur konsentrasi monosodium urea (MSU). Uji toksisitas akut menggunakan 4 kelompok tikus putih Wistar jantan dan betina yang diberi ekstrak daun jintan mulai dosis 1900 mg/kg BB, 3800 mg/kgBB dan 5000 mg/kgBB. Hasil penelitian menunjukkan ekstrak daun jintan secara kualitatif mempunyai kandungan senyawa Flavonoid, Saponin, Polifenol, Terpen dan Antrakuinon. Uji aktivitas ekstrak daun jintan memperlihatkan penurunan konsentrasi MSU (p<0,05) pada kelompok P2 dan P3, sedangkan pada kelompok P1, P4 dan kontrol tidak ada perbedaan (p>0,05) sebelum dan sesudah perlakuan. Uji toksisitas akut ekstrak daun jintan tidak menimbulkan kematian 50% (LD50) dan tidak menimbulkan gejala toksik, gangguan syarafi dan penurunan aktivitas pada semua kelompok perlakuan sehingga ekstrak daun jintan dapat digolongkan sebagai bahan yang ?praktis tidak toksik.

Anti Inflammation Effects and Acute Toxicity of Jintan Leaves (Plectranthus amboinicus) Extract on Arthritis Induced Rats. Jintan plant (Plectranthus amboinicus) is known as bangun-bangun plant and known as one of medicinal plants for Indonesian people. The purposes of this study were to analyze jintan leaves extract activity and to understand acute toxicity effect on arthritis induced rats. Extract were obtained from fresh jintan leaves by ethanol 96% maceration method, then diluted with CMC-Na. White rats strain wistar aged 2?3 months were divided into 5 groups: Control; treatment arthritis induced (P1), treatment arthritis induced given extract dose of 19 g/kg BW (P2); treatment arthritis induced given extract dose of 38 g/kgBW (P3) and treatment group with allopurinol dose of 2.5 mg/kg BW. Arthritis induced was done by uric acid 2% and oxonic acid 1.5% intraperitoneal for 15 days. After formed a lesion arthritis, jintan extract and allopurinol were given for 7 days. Blood serum sample were collected before and after treatment to measure Monosodium Urea (MSU) concentration. Acute toxicity test using 4 groups of Wistar rats given jintan extract starting dose of 1900 mg/kgBW, 3800 mg/kgBW and 5000 mg/kgBW. The results showed that jintan extract contain relative fraction of flavonoid, saponin, polyphenol, terpen and antraquinon. Activity test of jintan extract showed decrease concentration of MSU (p<0.05) in group P2 and P3, while in group P1, P4, and control no differences (p>0.05) before and after treatment. Acute toxicity test showed no lethal dose 50% and there were no toxic symptoms of neurological disorders and physical activity disturbance in all treatment group, so that jintan leaves extract can be classified as ?practically nontoxic? herbal plant."

"Peningkatan konsentrasi kristal monosodium urea pada sendi dan jaringan menunjukkan adanya keradangan Gout artritis (GA). Angka insiden dan prevalensi GA tersebar di negara berkembang sebesar 2?15%. Di Indonesia, prevalensi GA sekitar 29% dan sering terjadi pada suku Minahasa, Toraja dan Batak. Penelitian ini bertujuan untuk menganalisis aplikasi minyak atsiri kunyit sebagai anti-radang pada penderita GA dengan diet tinggi purin serta mengukur mediator
seluler tumor necrosis factor-α (TNF-α). Desain penelitian adalah randomized pretest-posttest control group design dengan pemberian secara single blind. Tes GCMS dilakukan untuk mengetahui komponen aktif minyak atsiri. Sampel penelitian ini adalah pasien baru GA di RS Haji Surabaya. Selama tujuh hari, kelompok perlakuan diberi minyak atsiri kunyit dengan dosis 25 mg/kg BB, sedangkan kelompok kontrol diberi indometasin dengan dosis 150 mg/kg BB. Sampel darah diambil sebelum dan sesudah perlakuan. Minyak atsiri kunyit mempunyai empat fraksi komponen aktif. Terdapat penurunan kadar urea darah pada kelompok perlakuan (p = 0,001) dan kelompok kontrol (p = 0,007). Terdapat penurunan konsentrasi pelepasan TNF-α, tetapi penurunan ini tidak berbeda secara signifikan pada kelompok kontrol dan perlakuan.
Increased concentrations of crystal monosodium urea at joint and soft tissue represent induced of inflammation at gout arthritis (GA). Incidence and prevalence GA disseminate wide in developed countries in Asian range from 2-15% and In Indonesia, GA prevalence was 29% and mostly found in Minahasa, Toraja, and Batak ethnics. This research was aimed to analyse application of curcuma domestica volatile oil as anti inflammation agent on gout arthritis patient who has high purin diet and to assess specific cellular mediator Tumor Necrosis Factor-α (TNF-α). The design of the study was randomized pretest-posttest control group design with single blind treatment. The GCMS test was performed to identify active component in volatile oil. The sample was the new gout arthritis patient in Haji Public Hospital Surabaya. For a week, treatment group was assigned with volatile oil with dose 25 mg/kg body weight and the control group was given indometasin 150 mg/kg body weight. Blood samples were taken before and after treatment. Volatile oil of curcuma domestica (Curcuma domestica, val) has four fraction of active component. There was
decreasing in blood urea level in treatment group (p = 0.001) and control group (p = 0.007). Both in control and treatment group, there was also decreasing in TNF-α, however it was not statistically significant. "

"Background: Diabetes Mellitus is a chronic disease characterised by elevated levels of blood glucose known as
hyperglycaemia. Diabetes is due to impaired insulin action in the metabolism of glucose and can result in impaired
wound healing. Excessive production of pro-inflammatory cytokines, an increased number of macrophages and
neutrophils, and decreased levels of transforming growth factor ? beta 1 (TGF-β1) serum can be characteristic of
impaired wound healing. This study aims to determine the effects of squid extract on certain wound parameters such as
levels of tumour necrosis factor ? alpha (TNF-α), and TGF-β1 serum and the number of macrophages and neutrophils.
Methods: This was a post-test only, randomized controlled group study that was conducted on male Wistar rats.
Experimental animals were divided into 6 groups; (1) normal wound with standard diet, (2) diabetic wound with
standard diet, (3) diabetic wound with chitosan supplement, (4) diabetic wound given squid extract orally once a day,
(5) diabetic wound given squid extract orally twice a day, and (6) diabetic wound given squid extract orally once every
two days. Levels of TNF-α and TGF-β1 serum were observed using Enzyme-Linked Immunosorbent Assay.
Haematocylin and eosin staining was used to observed macrophage and neutrophil counts. All data was analysed
statistically by one-way analysis of variance. Results: TNF-α serum levels showed a significant decrease (p < 0.05) in
subjects that received squid extract orally once every two days. The mean levels of TGF-β1 showed no significant
differences. The mean number of macrophage cells showed a significant decrease (p < 0.05) in all treatment groups.
The mean number of neutrophil cells also showed significant decrease (p < 0.05) in all treatment groups. Conclusions:
Squid extract is effective in lowering the TNF-α serum levels and the number of macrophages and neutrophils cells in
Wistar rats. However, there were insignificant findings on increasing levels of TGF-β1 serum. This data suggests that
squid extract is most effective during the inflammatory phase of wound healing which takes places about 2-4 days after
wound creation."

"Periodontal status is a periodontum condition evaluated by using plaque index, calculus index, gingival index and pocket index. The main mediator of periodontum inflammation is IL-113 examined by ELISA method. There is an evaluation of PMN s in periodontum inflammation, but the leucotoxin as well as the protease in turn lowers the PMN phagocytotic function. Phagocytotic function was measured by flowcytometry. The aim of the study was to evaluate the high risk factors of being type 2 DM. A diagnostic study was conducted by using cross-sectional design on 45 controlled DM (CDM) subjects, 45 uncontrolled DM (UCDM) subjects in the Metabolic Endocrinology Clinic Cipto Mangunkusumo Hospital Jakarta, as compared to 45 non-DM control subjects. The result of multivariate analysis showed that patients of older age (>54 years old), low periodontum status (periodontal index >1.80), high IL-113 level (>23.70 pg/mL), and low PMN phagocytotic function <<53.47%), were significantly at high risk of having DM compared to non-DM (p<0.05). Lower periodontum status showed an increase in IL-113 level, decrease PMN phagocytotic function, and consequently, an increase in the risk of being type 2 DM."

"ABSTRAK
Background Gout arthritis associates with many comorbodities such as hyperuricemia, hypertension, hyperglycemia, obesity, and dyslipidemia, which are also factors for the development of/or predisposition factors of chronic kidney diseases . Albuminuria is a predictor factors for CKD. Screening for albuminuria is needed to be done in patients with high risk of CKD. This research was conducted to examine the prevalence of albuminuria and the associated factors in gout arthritis patients.Methods This research was a cross sectional study from gout arthritis patients medical records in Cipto Mangunkusumo Hospital. We included all gout patients who treated within 2011 until 2015. Subjects with chronic kidney disease, in kidney replacement therapy, hypertension > 10 years, and diabetes > 5 years were excluded. Albuminuria was determined by urine dipstick result of protein > 1. Factors associated were age, sex, hyperuricemia, hypertension, stage of hypertension, hyperglycemia, obesity, dyslipidemia, uric acid level, and body mass index. Data associated with the factors were recorded and the associations were tested with chi square, fisher exact, or independent t test.Results from 54 subjects included in this research, the prevalence of albuminuria was 20.4 percent . There were no significant associations between all factors and albuminuria tested by chi square and fishers exact test. Independent t tests results also showed no significant associations between all the factors and albuminuria Conclusion The prevalence of albuminuria in gout arthritis patient was 20.4 percent. There were no significant associatons between age, sex, hyperuricemia, hypertension, hyperglycemia, obesity, dyslipidemia, uric acid level, and body mass index tested with albuminuria in gout arthritis patients in Cipto Mangunkusumo Hospital from 2011 until 2015."

"Hail penelitian ini tidak menunjukkan adanya penurunan kadar asam urat darah (AUD) yang bermakna. Hal ini menunjukkan bahwa diet rendah purin selama 5 hari tidak banyak mempengaruhi kadar asam urat darah. Demikian pula usaha untuk menilai pengaruh diet rendah purin 5 hari terhadap penggolongan kadar asam urat darah tidak menunjukkan perbedaan bermakna (p > 0.05). Salah satu faktor yang dapat menyebabkan hal ini adalah karena diet rendah purin 5 hari hanya dapat menghilangkan pengaruh purin eksogen dalam pembentukan asam urat tubuh. Pengaruh eksogen ini sebenarnya tidak begitu besar yaitu kurang lebih hanya sekitar 20% dari seluruh pembentukan asam urat dalam tubuh. Dengan demikian diet rendah purin 5 hari belum dapat memperlihatkan penurunan asam urat darah yang nyata.

This study did not show a significant decrease in blood uric acid (AUD) levels. This suggests that a low-purine diet for 5 days has little effect on blood uric acid levels. Similarly, efforts to assess the effect of a 5-day low-purine diet on classification Blood uric acid levels show no difference mean (p > 0.05). One factor that can cause this is because a 5-day low-purine diet can only eliminate the influence of exogenous purines in the body's uric acid formation. This exogenous influence is actually not so great, which is approximately only about 20% of the total formation of uric acid in the body. Thus, a 5-day low-purine diet has not been able to show a decrease in blood uric acid which is real."

"ABSTRAK
Background: a freshwater fish Ophiocephalus striatus or known locally to Indonesian as haruan,can potentially increases IGF-1 and albumin levels in elderly patients with hypoalbuminemia due to the contents of amino acids, fatty acids, vitamins, and minerals. This study was conducted to investigate the effect of Ophiocephalus striatus extract on the level of IGF-1 and albumin in elderly patients with hypoalbuminemia due to the contents of amino acids, fatty acids, vitamins, and minerals. This study was conducted to investigate the effect of Ophiocephalus striatus extract on the level of IGF-1 and albumin in elderly patients with hypoalbuminemia.
Methods: the study is a double-blind randomized controlled trial involving malnourished elderly inpatients (≥60 years old) recovering from acute condition before hospital discharge, with Mini Nutritional Assessment score ≤23.5 and albumin level <3.5 g/dL. A total of 109 subjects were randomly divided into two groups: one group received 10 g Ophiocephalus striatus extract per day for 14 days and another group received placebo. Albumin and IGF-1 levels were obtained before and after intervention.
Results: ninety subjects completed the study (extract group=45 subjects; placebo group =45 subjects) for 14 days. The median of age were 69 (64;75) years and the male to female ratio were 2 : 3. The changes of IGF-1 and albumin levels from before to after intervention between extract group compared to placebo group were 14.7 (0.30;31.5) ng/mL vs 1.0 (-6;13.15) ng/mL (p=0.002) and 0.5 (0.15;0.70) g/dL vs 0.10 (0.0;0.50) g/dL (p=0.003), respectively. There were significant differences in the improvement of IGF-1 and albumin levels between extract and placebo group.
Conclusion: supplementation of Ophiocephalus striatus extract was associated with a significant increase in IGF-1 and albumin levels in elderly patients with hypoalbuminemia."
