"ABSTRAK

Diabetes melitus merupakan penyakit kronik yang dapat menyebabkan berbagai komplikasi. Salah satu komplikasi yang banyak ditemukan adalah neuropati perifer. Neuropati perifer menyebabkan perubahan pada biomekanik pasien sehingga terjadi keterbatasan mobilitas fisik yang dapat menurunkan kemampuan melakukan aktivitas fisik pasien diabetes melitus tipe 2 dengan neuropati perifer. Penelitian ini bertujuan untuk mengetahui faktor-faktor penghalang yang berhubungan dengan aktivitas fisik pasien diabetes melitus tipe 2 dengan neuropati perifer. Penelitian ini merupakan penelitian analisis korelatif dengan desain cross sectional pada 77 orang sampel di Poliklinik Rumah Sakit Umum Daerah Pasar Minggu DKI Jakarta. Uji statistik yang digunakan untuk melihat hubungan dua variabel adalah chi square dan uji multivariat dengan logistik berganda untuk melihat faktor yang paling dominan. Hasil penelitian ini menunjukkan tidak terdapat hubungan antara nyeri neuropati, sensasi kaki, dan deformitas kaki dengan aktivitas fisik jalan kaki dan peregangan statis. Malas dan pengetahuan berhubungan dengan aktivitas fisik peregangan statis pada responden, dimana malas merupakan faktor dominan. Sedangkan pengetahuan, status fungsional, dan rasa takut berhubungan dengan aktivitas fisik jalan kaki responden, dimana status fungsional merupakan faktor dominan. Aktivitas fisik jalan kaki dan peregangan statis merupakan jenis aktivitas fisik yang direkomendasikan pada pasien diabetes melitus tipe 2 dengan neuropati perifer, dimana jalan kaki merupakan aktivitas fisik yang paling banyak dilakukan pasien karena sederhana, tidak membutuhkan alat, mudah, dan dapat dilakukan kapan saja.


ABSTRACT

 


Diabetes mellitus is a chronic disease that can cause various complications. One of complication that commonly found is peripheral neuropathy. Peripheral neuropathy causes changes in the biomechanics, resulting limited physical mobility which can reduce the ability to perform physical activity in type 2 diabetes melitus patients with peripheral neuropathy. This study aimed to determine the barriers related to physical activity in type 2 diabetes mellitus with peripheral neuropathy. This study was a correlative analysis study with a cross sectional design in 77 people sampled at government hospital of Pasar Minggu Jakarta. The statistical test used to see the relationship between two variables is chi square and multivariate test with multiple logistics to see the most dominant factors. The results indicate that there is no relationship between neuropathic pain, foot sensation, and foot deformity with physical activity of walking and static stretching. Laziness and knowledge are related to the physical activity of static stretching, which laziness is the dominant factor. While knowledge, functional status, and fear are related to the physical activity of walking, which functional status is the dominant factor. Physical activity of walking and static stretching are type of physical activity that recommended in type 2 diabetes mellitus with peripheral neuropathy, where walking is a the most frequent of physical activity in patients as it is simple, no tools requirement,  easy, and can be done at any time.

Konsep dasar teori Buteyko adalah mengajarkan cara bernapas yang benar pada pasien asma. Tujuan penelitian ini yaitu mengidentifikasi pengaruh latihan pernafasan Buteyko terhadap nilai saturasi oksigen perifer dan arus puncak ekspirasi pada pasien asma. Desain penelitian yang digunakan adalah kuasi eksperimen, pretest dan posttest dengan kelompok kontrol. Responden penelitian berjumlah 24 orang, terdiri dari 12 orang yang melakukan latihan pernapasan Buteyko pada kelompok intervensi dan 12 orang pada kelompok kontrol. Hasil penelitian menunjukkan terdapat pengaruh yang bermakna terhadap arus puncak ekspirasi pada kelompok intervensi p value = 0.001 . Namun tidak terdapat pengaruh yang bermakna terhadap saturasi oksigen perifer p value = 0,082 . Hasil penelitian ini menunjukkan bahwa latihan pernapasan Buteyko dapat diberikan sebagai terapi pelengkap, bagian dari perawatan pada pasien asma. The basic concept of the Buteyko theory is taught how to breathe correctly in asthma patients. The purpose of this study is to identify the Buteyko breathing exercises influence on the value of peripheral oxygen saturation and peak expiratory flow in patients with asthma. The research design is quasi experimental, pretest and posttest with control group. Respondents totaling 24 people, consisting of 12 people doing Buteyko breathing exercises in the intervention group and 12 in the control group. The results showed a significant effect on peak expiratory flow in the intervention group p value 0.001 . However, there is no significant effect on peripheral oxygen saturation p value 0.082 . "Latar belakang. Salah satu komplikasi paling umum dari diabetes mellitus (DM) adalah penyakit arteri perifer (PAD). Diperkirakan PAD mempengaruhi sebanyak 20% orang di atas 65 tahun. Banyak faktor yang berhubungan dengan terjadinya PAD. Golongan darah merupakan faktor risiko yang dikatakan mempengaruhi keparahan PAD namun belum banyak diteliti. Penelitian ini bertujuan untuk mengevaluasi hubungan golongan darah ABO dengan derajat keparahan PAD pada pasien DM tipe II.
Metode. Studi cross-sectional dilakukan pada pasien DM tipe II yang didiagnosis dengan PAD dan datang ke Rumah Sakit Nasional Cipto Mangunkusumo, Indonesia selama periode Januari 2022 hingga Juni 2022. Diagnosis PAD didasarkan pada pengukuran indeks pergelangan kaki-brakialis. (ABI). Tingkat keparahan PAD dikelompokkan menjadi PAD ringan (ABI 0,7-0,9) dan PAD sedang-berat (ABI <0,7). Pasien dikategorikan menurut golongan darah ABO menjadi golongan darah O dan golongan darah non-O (A, B, dan AB).
Hasil. Sebanyak 366 subjek dilibatkan dalam penelitian ini (A = 108, B = 52, AB = 12, O = 194). Tidak ada perbedaan kejadian PAD pada pasien PAD golongan darah O dan non golongan darah O (p = 0,780). PAD lebih parah pada golongan darah non-O (p = 0,041). Faktor risiko PAD yang lebih berat adalah periode diabetes yang lebih lama (OR 10,325 (95% CI 5,108-20,871), p < 0,001) dan hipertensi (OR 4,531 (95% CI 1,665-
12,326), p < 0,003).
Kesimpulan. Golongan darah ABO tidak berhubungan dengan terjadinya PAD. Golongan darah non-O dikaitkan dengan PAD yang lebih buruk di antara pasien DM tipe II. Faktor risiko lain untuk PAD yang lebih parah adalah periode diabetes dan hipertensi yang lebih lama.

Introduction. One of the most common complications of diabetes mellitus (DM) is peripheral artery disease (PAD). It is estimated that PAD affects as many as 20% of people over 65 years. Many factors are associated with the occurrence of PAD. Blood type is a risk factor that is said to influence the severity of PAD but has not been widely studied. This study aims to evaluate the relationship between ABO blood group type and the severity of PAD in DM type II patients.
Method. A cross-sectional study was performed on DM type II patients who was diagnosed with PAD and came to Cipto Mangunkusumo National Hospital, Indonesia during the period of January 2022 to June 2022. The diagnosis of PAD was based on the measurement of ankle-brachial index (ABI). The severity of PAD was grouped into mild PAD (ABI 0.7-0.9) and moderate-severe PAD (ABI <0.7). The patients were categorized according to the ABO blood group into O blood type and non-O (A, B, and AB) blood type
Results. A total of 366 subjects were included in the study (A = 108, B = 52, AB = 12, O= 194). There was no difference of PAD occurrence in O blood type and non-O blood type PAD patients (p = 0.780). The PAD was more severe in non-O blood type (p = 0.041). The risk factors of more severe PAD were longer period of diabetes (OR 10.325 (CI95% 5.108-20.871), p < 0.001) and hypertension (OR 4.531 (CI95% 1.665-12.326), p

Salah satu komplikasi dari Peripheral Arterial Disease (PAD) adalah kerusakan tungkai bawah hingga tidak
dapat digunakan untuk beraktifitas. PAD disebabkan oleh gangguan vaskular yang menyebabkan atherosklerotik
pada vaskular di bagian distal. Suatu penelitian kohort di Swedia menyatakan bahwa penyembuhan primer, laju
amputasi dan mortalitas pada pasien PAD berhubungan dengan derajat insufisiensi vaskular. Dengan demikian perlu dilakukan penelitian untuk mengetahui karakteristik vaskular pada kasus PAD secara kuantitatif maupun kualitatif serta hubungannya dengan nilai Ankle Brachial Index (ABI) dan gambaran gelombang Doppler Ultrasonography(DUS). Metode penelitian menggunakan studi potong lintang. Hasil penelitian didapatkan
pasien PAD dengan nilai ABI rata-rata 0,7 dengan gambaran gelombang DUS yang berubah dari trifasik
menjadi non-tirfasik dominan pada arteri infrapopliteal. ABI di bawah 0.9 menunjukkan perubahan pada gelombang DUS dari arteri femoralis hingga a.dorsalis pedis dengan nilai p <0.05. Faktor-faktor risiko yang paling tampak adalah usia di atas 45 tahun yang menunjukkan peningkatan risiko PAD. Selain itu hubungan
ABI dan faktor ?faktor risiko menunjukkan bahwa hiperlipidemia dan diabetes melitus menunjukkan hubungan signifikan dengan p<0,05. Sedang pada hubungan DUS dan faktor-faktor risiko hanya usia >45 tahun yang
tampak secara statistik signifikan meski secara klinis faktor-faktor risiko yang lain menunjukkan jumlah
persentase yang diatas 50%. Disimpulkan bahwa dengan mengetahui nilai ABI dapat ditentukan derajat
keparahan PAD dan juga dengan mengetahui gelombang DUS saja maka dapat diketahui oklusi di tingkat segmen arteri yang mana.Selain itu, usia di atas 45 tahun, hiperlipidemia dan juga diabetes melitus merupakan faktor risiko yang penting untuk terjadinya atherosklerosis dan penyumbatan pada distal arteri. Penggunaan DUS untuk menentukan tingkat penyumbatan sangat informatif dalam hal penggambran gelombang, aman dan murah sehingga dapat menetukan tindakan selanjutnya.ABSTRACT One of the complications of Peripheral Arterial Disease (PAD) is the damage that can be made to the lower

from triphasic to non-triphasic dominantly seen in infrapopliteal arteries. ABI less than 0.9 has a significant value towards changes in the spectral waveform from the femoral artery to the dorsal pedis artery. The risk factors such as age above 45 years old shows a significant relationship with the increase in PAD risk (p <0.05). Besides that, p <0.05 was also seen in hyperlipidemia and diabetes mellitus patients with PAD. As for the
relation between DUS and the risk factors, age above 45 years old was seen significant statistically eventhough
clinically all risk factors showed a percentage above 50%. It is concluded that by knowing the ABI value, the
degree of severity of PAD can be acknowledge and by knowing the waveform of DUS only one can know the
level of occlusion in an arterial segment.
