"ABSTRAK
Praktek Kerja Profesi di industri farmasi dilaksanakan di Departemen Process Development PT Kalbe Farma, Tbk. pada bulan Juli - Agustus 2015. PKP bertujuan agar mahasiswa apoteker mengerti peranan, tugas dan tanggung jawab apoteker, memiliki wawasan tentang pelaksanaan pekerjaan kefarmasian, memahami penerapan CPOB, dan memiliki gambaran nyata tentang permasalahan pekerjaan kefarmasian di industri farmasi. Agar lebih memahami tetang peran apoteker maka mahasiswa di berikan tugas khusus untuk membantu menyempurnakan dokumen Prosedur Pengemasan Induk sediaan solida agar tetap sesuai dengan ketentuan CPOB. Berdasarkan kegiatan PKP yang dilakukan, dapat diketahui dan disimpulkan bahwa peran apoteker tidak hanya menjadi personil kunci namun berperan disetiap bagian yang berhubungan dengan kualitas produk baik secara langsung maupun tidak langsung seperti peran apoteker di Departemen Process Development. kemampuan dan ketrampilan yang harus dimiliki seorang apoteker khsusnya dibagian pengembangan pengemasan yaitu mampu menerapkan ketentuan CPOB kedalam Prosedur Pengemasan Induk. Permasalahan pekerjaan kefarmasian yang dihadapi khususnya dibagian pengembangan kemasan adalah menentukan jenis bahan kemasan primer dan sekunder dalam penyesuaiannya terhadap stabilitas dari produk yang akan dikemas. Di dalam penyemprunaan dokumen Prosedur Pengemasan Induk hal-hal yang perlu ditambahkan berupa penambahan kolom ?pelaksana dan pemeriksa?, instruksi serah terima saat proses tertentu, instruksi rekonsiliasi, instruksi pemeriksaan kesesuaian seting mesin, dan layout coding area.
ABSTRACT Profession Internship at pharmaceutical industry was held at Process Development Department of PT Kalbe Farma, Tbk. This activity was held from July until August 2015. Profession internship at pharmaceutical industry was intended to make apothecary student understand the role of pharmacist, have insight into the implementation of pharmaceutical practice, understand the implementation of Cara Pembuatan Obat yang Baik (CPOB), and know the issues in pharmaceutical practice in pharmaceutical industry. To imporve the knowledge of apothecary student about play role of pharmacist, they gave task to help imporvement packaging procedure document solid dosage form. Based on the activities, role of pharmacists in industry not only as key person but also as pharmacist who work at process development departement. the ability that should apothecary have especially in packaging development can implement CPOB in packaging procedure document. The issues that faced by apothecary in packaging development are to determine kind of primary and secondary packaging material that suitable their subtance stability. To improve the packaging procedur document can add several point, such as adding column of operator and inspector, instruction of certain handover, instruction of reconciliation, instruction of setting machine conformity and lay out of coding area.

Praktek Kerja Profesi di Apotek Hidup Baru dilaksanakan dengan tujuan untuk mengetahui dan memahami peran apoteker di apotek, baik dalam bidang manajemen apotek maupun dalam hal pelayanan farmasi klinik. Peran apoteker dalam manajemen apotek meliputi perencanaan, pengadaan, penerimaan, penyimpanan, pemusnahan, pengendalian, pencatatan dan pelaporan. Sedangkan pelayanan farmasi klinik meliputi pengkajian resep, dispensing, pelayanan informasi obat, dan konseling. Tugas khusus yang diberikan adalah pembuatan leaflet asam urat sebagai media promosi kesehatan di apotek. adalah Mengetahui kelengkapan resep pasien pediatrik dengan penyakit ISPA di
Apotek Hidup Baru periode 1 Agustus hingga 24 Agustus 2017 ditinjau dari
persyaratan administrasi, farmasetik dan klinis
ABSTRACT
Internship at Hidup Baru Pharmacy aims to make prospective pharmacists
understand the duties and responsibilities of pharmacists in management of
pharmacy and perform pharmaceutical practices in accordance with applicable
legislation and ethics. This internship also aims to make prospective pharmacists
have insight, knowledge skill, and practical experience as well as an overview of
pharmaceutical practice problem in pharmacy. The specific assignment given is
entitled Study of Pediatric Prescription with Upper Respiratory Tract Disease in
Apotek Hidup Baru Period of August 1st to August 29th 2017. The purpose of this
specific assignment is to know the completeness of pediatric prescritption with
upper respiratory tract disease in Apotek Hidup Baru period of August 1st to
August 29th 2017 that reviewed from administrative pharmacetic and clinial
requirement."

"Praktik Kerja Profesi di Apotek Axia dilaksanakan dengan tujuan untuk mengetahui dan memahami peran apoteker di apotek, baik dalam bidang manajemen apotek maupun dalam hal pelayanan farmasi klinik. Peran apoteker dalam manajemen apotek meliputi perencanaan, pengadaan, penerimaan, penyimpanan, pemusnahan, pengendalian, pencatatan dan pelaporan. Sedangkan pelayanan farmasi klinik meliputi pengkajian resep, dispensing, pelayanan informasi obat, dan konseling. Tugas khusus yang diberikan adalah mengenai penerapan praktek pelayanan farmasi klinik yang bertanggung jawab beserta tinjauan mengenai beberapa permasalahan yang mungkin timbul dalam pelayanan farmasi klinik di apotek. Selama pengamatan praktek kerja dapat diketahui bahwa pelayanan farmasi klinik di apotek-apotek belum banyak dilakukan seluruhnya.

Apoteker memiliki peran dan tanggung jawab yang sangat penting di apotek, yakni sebagai
pemimpin dan pemberi arahan seluruh kegiatan teknis kefarmasian dan kegiatan non-teknis
kefarmasian seperti pengelola dan penggerak bisnis di apotek, serta sebagai pengambil keputusan.
Pelayanan Kefarmasian adalah suatu pelayanan langsung dan bertanggung jawab kepada pasien
yang berkaitan dengan sediaan farmasi dengan maksud mencapai hasil yang pasti untuk
meningkatkan mutu kehidupan pasien. Suatu sarana pelayanan kefarmasian tempat
dilaksanakannya dilakukannya praktek kefarmasian oleh Apoteker adalah apotek. Dalam
melakukan kegiatan kefarmasian di Apotek, Apoteker juga dituntut untuk memiliki wawasan,
pengetahuan, keterampilan, pengalaman praktis di Apotek untuk dapat mengetahui gambaran
permasalahan yang sering dihadapi serta strategi dalam pengembangan praktek kefarmasian.
Praktek Kerja Profesi Apoteker yang dilakukan di Apotek Kimia Farma No.7 Bogor dilakukan
selama satu bulan dengan tugas khusus yang diberikan yaitu pengkajian.ABSTRACT Pharmacists have an important roles and responsibilities formanaging the pharmacy store, such as
a leader and giver of direction about technical and non-technical pharmaceutical activities such as

"ABSTRAK
Praktek Kerja Profesi (PKP) di apotek bertempat di Apotek Hidup Baru. Kegiatan ini berlangsung selama empat minggu dari tanggal 5 sampai dengan tanggal 30 Oktober 2015. PKP di apotek bertujuan agar mahasiswa apoteker mengerti peranan Apoteker Penanggung Jawab Apotek (APA), memiliki wawasan tentang pelaksanaan pekerjaan kefarmasian, dan memiliki gambaran nyata akan permasalahan pekerjaan kefarmasian yang terjadi di apotek. Berdasarkan kegiatan PKP yang dilakukan, Standar pelayanan farmasi di Apotek sebagian besar sudah diterapkan di apotek Hidup Baru meliputi pengelolaan perbekalan sediaan farmasi dan alat kesehatan, hal ini dilakukan untuk meningkatkan mutu pelayanan kefarmasian di apotek Hidup Baru yang berorientasi kepada keselamatan pasien. Untuk meningkatkan pemahaman, mahasiwa diberikan tugas khusus yaitu membuat standar prosedur operasional pelayanan resep yang kemudian di implementasikan di apotek Hidup Baru. Berdasarkan hasil implementasi SPO pelayanan resep, pelayanan resep telah dilakukan menjadi lebih teratur dan jelas mengenai pembagian tugas dalam melakukan penyiapan resep serta lebih teratur serta terdokumentasi dengan baik.
ABSTRACT
Profession Internship at apotek was held at apotek Hidup Baru. This activity was held for four weeks from October 5th until October 30th 2016. Profession Internship at apotek was intended to make apothecary student understand the role of pharmacist, have insight into the implementation of pharmaceutical practice, and know the issues in pharmaceutical practice in pharmacy. Based on the activities, most of pharmaceutical care standard at apotek been applied at apotek hidup baru, include management of phramaceutical adn medical device. This activities intendeed to increase pharmaceutical care quality at apotek Hidup Baru based on patient oriented. To improve understanding of apothecary student, they gave task to make standard operational procedure of prescription service. Based on implementation of standard operational procedure prescription service, it been more documented and tidier."

Praktek Kerja Profesi Apoteker di Apotek Kimia Farma 202 bertujuan untuk
memahami peran dan tanggung jawab Apoteker dalam melaksanakan pelayanan
kefarmasian di Apotek, memahami sistem dan manajemen yang ada di Apotek
baik sistem perbekalan kefarmasian maupun pelayanan farmasi klinis, serta
menambah pengetahuan dan keterampilan calon Apoteker dalam melaksanakan
pelayanan kefarmasian di Apotek. Apotek Kimia Farma 202 Depok telah
melaksanakan pengelolaan perbekalan kefarmasian dan pelayanan farmasi klinik
dengan cukup baik. Tugas khusus yang diberikan tentang penyakit hipotiroid dan
analisa resep obat hipotiroid di Apotek Kimia Farma 202. Tugas khusus ini
diberikan agar mahasiswa memahami sistem dan fungsi hormon tiroid pada tubuh
manusia, memahami penyakit hipotiroid dan terapi yang diberikan, serta
memahami rasionalitas resep berdasarkan pengkajian resep yang berkaitan dengan
penyakit tiroid.
