"Praktek Kerja Profesi (PKP) di puskesmas bertempat di Puskesmas Kecamatan Ciracas Jakarta Timur. Kegiatan ini berlangsung selama tiga minggu dari tanggal 1 sampai dengan tanggal 25 October 2015. PKP di puskesmas bertujuan agar mahasiswa program studi apoteker memahami peranan, tugas dan tanggung jawab apoteker dalam praktek pelayanan kefarmasian di Puskesmas sesuai dengan ketentuan perundangan dan etika farmasi yang berlaku, dan dalam bidang kesehatan masyarakat, memahami dan memiliki pengetahuan, keterampilan, sikap perilaku serta wawasan dan pengalaman nyata untuk melakukan praktek profesi dan pekerjaan kefarmasian di Puskesmas, serta melihat dan mempelajari strategi dan pengembangan praktek profesi Apoteker di Puskesmas.Berdasarkan kegiatan PKP yang dilakukan, dapat disimpulkan bahwa Apoteker di Puskesmas Kecamatan Ciracas memiliki tanggung jawab terhadap pengelolaan sediaan farmasi dan pelayanan farmasi klinik, meliputi pengkajian resep, penyerahan obat, pemberian informasi obat, dan pelayanan informasi obat. Pengetahuan, keterampilan, sikap perilaku serta wawasan dan pengalaman nyata untuk melakukan praktek profesi dan pekerjaan kefarmasian di Puskesmas telah diperoleh penulis. Berdasarkan pengamatan selama pelaksanaan PKPA, Puskesmas Kecamatan Ciracas secara umum telah menerapkan seluruh aspek pelayanan farmasi klinik dengan baik. Masalah yang terjadi di Puskesmas Kecamatan Ciracas berupa kurangnya tenaga Apoteker yang tidak seimbang dengan jumlahnya pasien. Selain itu, perlu diadakannya ruang konseling agar kegiatan konseling berjalan dengan baik.
Profession Internship in community health center located in Ciracas, East Jakarta. This activity lasted for three weeks from March 1st to October 25th 2015. Profession Internship in the community health center aims to make apothecary student understand the role, duties and responsibilities of a pharmacist in the practice of pharmacy services at the community health center in accordance with the provisions of the legislation and pharmaceutical ethics applicable, and in the field of public health, understand and have the knowledge, skills, attitudes and behaviors as well as insight into the real experience to practice apothecary profession and work in health centers, as well as see and learn strategies and the development of professional practice of pharmacists in community health center. Based on profession internship activities undertaken, it can be concluded that pharmacists in Ciracas sub-district health centers have responsibility for the management of pharmaceutical and clinical pharmacy services, including assessment prescription, drug delivery, give the information of drugs, and drug information services. Knowledge, skills, attitudes and behaviors as well as insight into the real experience to practice apothecary profession and work in health centers has been obtained by the authors. Based on observations during the implementation, Ciracas sub-district health centers in general have implemented all aspects of clinical pharmacy services properly. Problems that occur in Ciracas sub-district health center in the form of shortage of pharmacists is not balanced with the amount of patients. In addition, the holding space needs counseling so that counseling activities run well."

"ABSTRAK
Praktek kerja profesi (PKP) ini bertempat di Suku Dinas Kesehatan Kota Administrasi Jakarta Timur dan Pusat Kesehatan Masyarakat (Puskesmas) Kecamatan Cipayung. PKP ini bertujuan agar mahasiswa memahami, peranan, tugas dan tanggung jawab Apoteker, memiliki pengetahuan, keterampilan, sikap perilaku serta wawasan dan pengalaman nyata untuk melakukan praktek kefarmasian, mempelajari strategi dan pengembangan praktek profesi Apoteker, memiliki gambaran nyata tentang permasalahan praktek kefarmasian, serta mampu berkomunikasi dan berinteraksi dengan tenaga kesehatan lain di Puskesmas. Kegiatan PKP ini berlangsung dari tanggal 5 sampai 30 Oktober 2015 dengan tugas khusus rekapitulasi laporan pemakaian dan lembar permintaan obat (LPLPO), penggunaan obat rasional, penggunaan obat generik, jumlah kunjungan resep, dan data tenaga kefarmasian periode Januari-Agustus 2015 di Puskesmas Kecamatan Cipayung Kota Administrasi Jakarta Timur. Berdasarkan kegiatan PKP yang dilakukan, Apoteker di Puskesmas Kecamatan Cipayung secara umum telah melaksanakan tugas dan tanggung jawab dalam menjalankan pekerjaan kefarmasian. Mahasiswa apoteker telah berhasil memperoleh wawasan mengenai kegiatan yang dilakukan di Puskesmas. Kegiatan praktek kefarmasian dalam bidang manajemen yang dilakukan di puskesmas meliputi perencanaan, permintaan, penerimaan, penyimpanan, pendistribusian, pengendalian, pencatatan, pelaporan dan pengarsipan, serta pemantauan pengelolaan dan evaluasi pengelolaan. Pelayanan Farmasi klinik yang telah dilaksanakan di Puskesmas Kecamatan Cipayung adalah pengkajian resep, penyerahan obat, pemberian informasi obat dan konselingABSTRACT Profession Internships were held at regional health department of east jakarta and cipayung district community health centers. Profession Internships were intended to make student understood the role, duties, and responsibilities of pharmacist, had insights, knowledges, skills, attitude behavior, and real experiences to do pharmaceutical practices, learned strategy and practical development of pharmacist profession, had a real description about the issues of pharmaceutical, and had ability to communicate and interact with other medical personnel at community health centers. This activity was held from 5th to October 30th 2015 with special assignment of usage report recapitulation and sheet of drug request, rational drug usage, generic drug usage, the number of prescription visit, and pharmaceutical staffs on January-August 2015 period at community health centers of Cipayung District, East Jakarta. Based on the activities, pharmacist at community health centers of Cipayung District generally had done the duties and responsibilities of pharmaceutical practices. Apothecary student had succeed obtaining the insights about activities in community health centers. Pharmaceutical practices in management at community health centers included planning, request, reception, storage, distribution, controling, recording, reporting, archiving, along with management monitoring and management evaluation. Clinical pharmacy services which had been done at community health centers are prescription assessment, drug handover, drug information distribution, and counseling."

"ABSTRAK
Suku Dinas Kesehatan Kota merupakan unit kerja Dinas Kesehatan pada Kota
Administrasi. Suku Dinas Kota dipimpin oleh seorang Kepala Suku Dinas yang
secara teknis dan administrasi berkedudukan di bawah dan bertanggung jawab
kepada Kepala Dinas, serta secara operasional berkedudukan di bawah dan
bertanggung jawab kepada Walikota. Jakarta Timur memiliki 88 puskesmas yang
terdiri dari 10 Puskesmas Kecamatan dan 78 Puskesmas Kelurahan. Puskesmas
Kecamatan membawahi beberapa Puskesmas Kelurahan. Puskesmas Kecamatan
yang terletak di wilayah Jakarta Timur yaitu Puskesmas Kecamatan Makasar,
Matraman, Jatinegara, Cakung, Duren Sawit, Kramat Jati, Pulogadung, Pasar
Rebo, Ciracas, dan Cipayung. Pengadaan obat di Puskesmas dilakukan oleh
Kepala Apotek Puskesmas kepada Suku Dinas Kota Jakarta Timur dengan
mengajukan formulir LPLPO (Laporan Pemakaian dan Lembar Permintaan Obat).
Perencanaan kebutuhan Obat dan Bahan Medis Habis Pakai di Puskesmas
mengacu pada Daftar Obat Esensial Nasional (DOEN) dan Formularium
Nasional. Pelaporan Penggunaan Obat Rasional (POR) dilakukan setiap bulan
dengan tujuan untuk melihat kerasionalan peresepan obat, terutama pada pasien
ISPA non pneumonia, diare non spesifik, dan myalgia.ABSTRACT Tribe Health Department is working units in the city Health Department official

the aim to see the rationality of prescribing drugs, especially in patients ISPA non
pneumonia, non-specific diarrhea and myalgia."

Puskesmas merupakan unit pelaksana teknis dinas kesehatan kabupaten/kota yang bertanggung jawab menyelenggarakan pembangunan kesehatan di suatu wilayah kerja. Peran apoteker di Puskesmas adalah mengelola perbekalan farmasi dan pelayanan farmasi klinik. Praktek Kerja Profesi Apoteker di Puskesmas Kecamatan Kembangan bertujuan agar calon apoteker dapat memahami peranan, tugas, dan tanggung jawab apoteker dalam praktek pelayanan kefarmasian, memiliki pengetahuan, keterampilan, sikap perilaku serta wawasan dan pengalaman nyata untuk melakukan praktek profesi dan pekerjaan kefarmasian di Puskesmas, melihat dan mempelajari strategi dan pengembangan praktek profesi Apoteker di Puskesmas, memiliki gambaran nyata tentang permasalahan praktek dan pekerjaan kefarmasian di Puskesmas, berkomunikasi dan berinteraksi dengan tenaga kesehatan lain yang bertugas di Puskesmas. Praktek kerja profei di puskesmas kecamatan cipayung, Jakarta Timur periode Maret 2016 bertujuan untuk memahami peranan, tugas dan tanggung jawab apoteker dalam praktek pelayanan kefarmasian di puskesmas sesuai dengan ketentuan perundang-undangan dan etika farmasi yang berlaku dan dalam bidang kesehatan masyarakat, memahami dan memiliki pengetahuan, keterampilan, sikap perilaku serta wawasan dan pengalaman nyata untuk melakukan praktek profesi dan pekerjaan kefarmasian di puskesmas, melihat dan mempelajari strategi dan pengembangan praktek profesi apoteker di puskesmas, memahami gambaran nyata tentang permasalahan praktek dan pekerjaan kefarmasian di puskesmas serta mampu berkomunikasi dan berinteraksi dengan tenaga kesehatan lain yang bertugas di puskesmas. Praktek kerja profesi dilakukan selama empat minggu dengan tugas khusus yaitu Rekapitulasi Laporan Pemakaian dan Lembar Peminatan Obat (LPLPO), Penggunaan Obat Rasional (POR), PEnggunaan Obat Generik di Puskesmas Kecamatan Cipayung periode Januari-Februari 2016.
The aim of apothecary profession internship program at Cipayung District District Community Health Centers on March 2016 are to understand the role, duties and responsibilities in pharmaceutical care at Community Health Center in accordance with the provisions of legislations and ethics pharmacy that applies in community health, perceive and have knowledge, skill, behavior and attitude insight and experience real to do practice of a profession and pharmaceutical care in Community Health Centers, look and study the development of strategies and practice of the profession of pharmacists at Community Health Centers, understand a real picture of the problems and work practices pharmaceutical in Community Health Centers and able to communicate and interact with other health workers working there. The apothecary profession internship program conducted during four weeks in a special assignment that is Recapitulation of LPLPO, POR, Generic Medicine Used Reports in Cipayung District Community Health Centers on January-February 2016."

"ABSTRAK
Pelayanan kefarmasian merupakan bagian integral dari sistem pelayanan kesehatan
termasuk di dalamnya pelayanan kefarmasian di Puskesmas yang merupakan unit
pelaksana teknis dinas kesehatan kabupaten/kota. Praktek Kerja Profesi Apoteker
di Suku Dinas Kesehatan Kota Administrasi Jakarta Timur bertujuan untuk
memahami tugas dan fungsi Suku Dinas Kesehatan Kota Jakarta Timur dan juga
memahami tugas dan fungsi dari bagian seksi sumber daya kesehatan. Sedangkan
tujuan dari tugas khusus adalah untuk mengetahui tugas pokok dan fungsi tenaga
farmasi di puskesmas, memahami bagaimana pengelolaan obat, pelaksanaan
pelayanan farmasi klinik, laporan pemakaian dan lembar permintaan obat dan
penggunaan obat rasional (POR) di Puskesmas Kecamatan Pulogadung. Peran
apoteker di Suku Dinas Kesehatan Kota Administrasi Jakarta Timur adalah sebagai
pengelola Sub Seksi Farmasi, Makanan dan Minuman, yang salah satu tugasnya
adalah pengelolaan laporan tentang pengunaan obat dari seluruh Puskesmas di
wilayah Jakarta Timur. Sementara itu Apoteker di puskemas bertugas dan
bertanggungjawab dalam melakukan pelayanan kefarmasian, baik dalam
pengelolaan sediaan farmasi maupun pelayanan farmasi klinik. ABSTRACT Pharmacy services are an integral part of the health care system including pharmacy

understand the duties and functions of the parts of the section of health resources.
While the purpose of the special task is to determine the duties and functions of
pharmacy staff in health centers, to understand how the drug management, the
implementation of clinical pharmacy services, usage reports and sheets demand for
drugs and rational drug use (POR) in the sub-district Puskesmas Pulogadung. The
role of the pharmacist in Suku Dinas Kesehatan Jakarta Timur is as manager of Sub
Section Pharmaceutical, Food and Beverage, which is one of its tasks is the
management reports on the use of drugs of all health centers in East Jakarta. While
the pharmacist in Puskesmas has duties and responsiblilities to perform pharmacy
services, both in the management of pharmaceutical and clinical pharmacy services.;Pharmacy services are an integral part of the health care system including pharmacy
