"ABSTRAK
Pusat Kesehatan Masyarakat sebagai salah satu jenis fasilitas pelayanan kesehatan tingkat
pertama memiliki peranan penting dalam sistem kesehatan nasional. Praktek Kerja Profesi
Apoteker yang dilakukan di Puskesmas Kecamatan Kemayoran ditujukan untuk memahami tugas
dan fungsi apoteker dalam melaksanakan Pelayanan Kefarmasian di Puskesmas. Pelayanan
Kefarmasian adalah suatu pelayanan langsung dan bertanggung jawab kepada pasien yang
berkaitan dengan sediaan farmasi dengan maksud mencapai hasil yang pasti untuk meningkatkan
mutu kehidupan pasien. Suatu sarana pelayanan kefarmasian tempat dilaksanakannya
dilakukannya praktek kefarmasian oleh Apoteker adalah apotek. Aktivitas pelayanan kefarmasian
di Puskesmas dilakukan secara menyeluruh yang meliputi aktivitas promotif, preventif, kuratif
dan rehabilitatif kepada masyarakat di wilayah kerja puskesmas yang bersangkutan. Tugas
khusus yang diberikan pada kegiatan Praktek Kerja kali ini adalah membuat pengamatan Terhadap
Data Pelayanan Obat yang Rasional di Puskesmas.

"ABSTRAK
Praktek Kerja Profesi (PKP) di apotek bertempat di Puskesmas Cipayung. Kegiatan ini
berlangsung dari tanggal 3 sampai dengan tanggal 29 Juni-7 Juli 2015. PKP di puskesmas
bertujuan agar mahasiswa apoteker mengerti peranan Apoteker Pengelola Apotek (APA),
memiliki wawasan tentang pelaksanaan pekerjaan kefarmasian, dan memiliki gambaran
nyata akan permasalahan pekerjaan kefarmasian yang terjadi di apotek. Berdasarkan
kegiatan PKP yang dilakukan, APA di puskesmas Cipayung secara umum telah
melaksanakan tugas dan tanggung jawab dalam pengelolaan apotek terkait kegiatan teknis
kefarmasian dan kegiatan nonteknis kefarmasian sesuai dengan peraturan perundangundangan
yang berlaku. Mahasiswa apoteker telah berhasil memperoleh wawasan
mengenai kegiatan rutin yang dilakukan di puskesmas. Kegiatan teknis kefarmasian yang
dilakukan di puskesmas meliputi perencanaan, pengadaan, penerimaan, penyimpanan,
pendistribusian, pengendalian, pengelolaan psikotropika. Kegiatan pelayanan klinik yang
telah dilakukan adalah pengekajian resep, pemberian informasi obat. Masalah yang terjadi
di Puskesmas Cipayung adalah belum dilaksanakannya konseling, ronde, pelaporan efek
samping dan pemantauan terapi obat.
ABSTRACT Work Practice Profession (PKP) in pharmacies located in RSUPN Dr. Cipto
Mangunkusumo. This activity took place from September 7 until 3 November 2015. The
PFM in hospital aims to make students understand the role of pharmacists Pharmacists and
have insight into the implementation of pharmacy work, and have a real picture of the
issues that occur in pharmacy jobs pharmacy. Based on the activities undertaken PKP in
general have been carrying out the duties and responsibilities in the management of
activities related pharmacy pharmaceutical technical and nontechnical pharmacy activities
in accordance with the legislation in force. Student pharmacists have managed to gain
insights into the routine activities performed in the clinic. Pharmaceutical technical
activities undertaken in the clinic include the selection, planning, procurement, receipt,
storage, distribution, control, management of narcotic drugs and psychotropic ,. Clinical
service activities that have been carried out is the assessment prescription, medical history
taking, monitoring of drug therapy, Drug Information services, counseling, round / visite,
monitoring of drug side effects, drug use evaluation ,. Problems that occur in the hospital
Dr. Cipto Mangunkusumo is not yet implemented monitoring of drug levels in the blood.

"ABSTRAK
Praktek Kerja Profesi di Puskesmas Kecamatan Palmerah dilaksanakan dengan tujuan untuk mengetahui dan memahami peran apoteker di Puskesmas, baik dalam bidang manajemen pengelolaan perbekalan farmasi maupun dalam hal pelayanan farmasi klinik. Peran apoteker dalam manajemen meliputi perencanaan, pengadaan, penerimaan, penyimpanan, pemusnahan, pengendalian, pencatatan dan pelaporan. Sedangkan pelayanan farmasi klinik meliputi pengkajian resep, dispensing, pelayanan informasi obat, dan konseling. Tugas khusus yang diberikan adalah mengenai pembuatan leaflet informasi obat untuk pasien dengan penyakit tidak menular seperti diabetes, asam urat, hipertensi, adn kolesterol. Leaflet berisi informasi obat dan tambahan informasi untuk pasien mengenai bagaimana cara mengendalikan penyakitnya. Leaflet ini dibuat untuk memberikan informasi kepada pasien mengenai pengobatannya dan mengedukasi pasien tentang pengobatan yang diterimanya sehingga diharapkan kualitas hidup pasien dapat meningkat.ABSTRACT Profession Internship at Puskesmas Kecamatan Palmerah was held to learn and understand the role of pharmacist in community health center, especially to manage a pharmacy and to deliver pharmaceutical care. Pharmacist has role to manage the pharmacy, such as planning, supplying, receipt, saving, annihilation, controlling, and reporting. On the other hand, pharmacist also has to deliver a pharmaceutical care, such as receipt assessment, dispensing, drug information, and patient counseling. Praktek Kerja Profesi Apoteker di Puskesmas Palmerah bertujuan untuk memahami peran dan tanggung jawab Apoteker dalam melaksanakan pelayanan kefarmasian di Puskesmas, memahami kegiatan pelayanan kefarmasian yang ada di Puskesmas baik kegiatan manajerial maupun pelayanan farmasi klinis, dan menambah pengetahuan dan keterampilan calon Apoteker dalam melaksanakan pelayanan kefarmasian di Puskesmas. Pelayanan Kefarmasian Puskesmas Palmerah secara garis besar terdiri dari dua kegiatan yaitu manajemen perbekalan farmasi dan kegiatan farmasi klinik. Kegiatan Manajemen perbekalan farmasi di Puskesmas Kecamatan Palmerah secara garis besar sudah berjalan dengan baik sesuai standar pelayanan kefarmasian dari Kementerian Kesehatan Republik Indonesia, hanya saja masih ada kekurangan fasilitas penyimpanan perbekalan farmasi. Kegiatan farmasi klinik di Puskesmas Kecamatan Palmerah terdiri dari penyiapan obat, penyerahan obat, dan pemberian informasi obat. Tugas khusus yang diberikan yaitu berupa pembuatan media pelayanan informasi obat tentang cara penggunaan obat luar. bertanggung jawab menyelenggarakan pembangunan kesehatan di suatu wilayah kerja. Peran
apoteker di Puskesmas adalah mengelola perbekalan farmasi dan pelayanan farmasi klinik. Praktek
Kerja Profesi Apoteker di Puskesmas Kecamatan Kembanganr bertujuan agar calon apoteker dapat
memahami peranan, tugas, dan tanggung jawab apoteker dalam praktek pelayanan kefarmasian,
memiliki pengetahuan, keterampilan, sikap perilaku serta wawasan dan pengalaman nyata untuk
melakukan praktek profesi dan pekerjaan kefarmasian di Puskesmas, melihat dan mempelajari
strategi dan pengembangan praktek profesi Apoteker di Puskesmas, memiliki gambaran nyata
tentang permasalahan praktek dan pekerjaan kefarmasian di Puskesmas, berkomunikasi dan
berinteraksi dengan tenaga kesehatan lain yang bertugas di Puskesmas. Kegiatan yang dilakukan
yaitu melakukan pelayanan farmasi klinik, seperti pemberian informasi obat, konseling, serta
berdiskusi tentang pengelolaan perbekalan farmasi dan tugas khusus. Puskesmas Kecamatan
Kembangan perlu menambah sumber daya manusia yakni apoteker sehingga pekerjaan
kefarmasian dapat dilakukan secara maksimal.ABSTRACT Puskesmas is unit of implementation public health office in regency which has
responsibility to organize health development in a working area. Pharmacist at Puskesmas has role
to manage pharmaceutical supply and clinical pharmacy service. The purposes of Internship
Program in Puskesmas Kecamatan Sawah Besar are for understanding about the roles, jobs, and
responsibilities of pharmacist candidate in pharmaceutical practice, having knowledge, skills,
attitudes, perception, and real experiences in Puskesmas pharmaceutical practices, finding strategic
ways to develop the role of Puskesmas pharmaceutical and having a real illustration about
problems in pharmaceutical practices, communicating and interacting with another Puskesmas
health workers. The activities were doing clinical pharmacy services, Puskesmas Keliling, and
discussed about management of pharmacy and special task. Puskesmas Kecamatan Sawah Besar
need to add human resources like pharmacist so that pharmaceutical practice can be done well."

"Telah dilakukan Praktek Kerja Profesi Apoteker (PKPA) di Puskesmas Kecamatan Kembangan Jakarta Barat. Kegiatan PKPA ini berlangsung selama tiga minggu dari tanggal 4 sampai dengan tanggal 24 Agustus 2015. Tujuan dari kegiatan PKPA ini adalah agar mahasiswa Apoteker mampu memahami peranan, tugas dan tanggung jawab Apoteker dalam praktek pelayanan kefarmasian di Puskesmas sesuai dengan ketentuan perundangan dan etika farmasi yang berlaku dan agar mahasiswa Apoteker memiliki pengetahuan, keterampilan, sikap perilaku, serta wawasan dan pengalaman nyata untuk melakukan praktik profesi dan pekerjaan kefarmasian di Puskesmas. Berdasarkan kegiatan PKPA yang dilakukan, diketahui bahwa Apoteker di Puskesmas Kecamatan Kembangan bertanggung jawab atas kegiatan pengelolaan obat dan bahan medis habis pakai dan pelayanan farmasi klinik. Pelayanan kefarmasian di Puskesmas Kecamatan Kembangan mangacu pada Permenkes No. 30 Tahun 2014 tentang standar pelayanan kefarmasian di Puskesmas.
Profession Internship at Puskesmas Kecamatan Kembangan Jakarta Barat has been done for three weeks from August 4 th until 24 th 2015. Puskesmas is community health centers. Profession Internship at this area was intended to make apothecary student understand the role and responsibility of pharmacist accordances to the statutory provisions and ethics, have insight into the implementation of pharmaceutical practice in Puskesmas, and know the issues in pharmaceutical practice in Puskesmas. Based on the activities, pharmacist in Puskesmas have been carrying out the duties and responsibilities in management of pharmaceutical preparation and consumable medical supply and in practice of clinical pharmacy. Pharmacy services in Puskesmas is carried out refers to Peraturan Menteri Kesehatan No. 30 Tahun 2015 about the standard of pharmacy services at Puskesmas."

Praktek Kerja Profesi Apoteker di Suku Dinas Kesehatan Kota Administrasi Jakarta Timur dan Puskesmas Kecamatan Kramat Jati bertujuan untuk memahami peranan serta tugas pokok dan fungsi apoteker di Suku Dinas Kesehatan Jakarta Timur mampu memahami peranan tugas dan tanggung jawab Apoteker dalam praktek kefarmasian serta mengamati pelaksanaan kegiatan farmasi klinis yang telah dan belum dilakukan di Puskesmas Kecamatan Kramat Jati Peran apoteker di Suku Dinas Kesehatan Kota Administrasi Jakarta Timur adalah sebagai pengelola Sub Seksi Farmasi Makanan dan Minuman Apoteker di Puskemas Kecamatan Kramat Jati bertugas dan bertanggung jawab melakukan pelayanan kefarmasian berupa pengelolaan sediaan farmasi dan pelayanan farmasi klinik sesuai Peraturan Menteri Kesehatan No 30 Tahun 2014 tentang Standar Pelayanan Kefarmasian Di Puskesmas Kegiatan Farmasi Klinis yang telah dilakukan di Puskesmas Kecamatan Kramat Jati adalah Pengkajian resep penyerahan obat dan pemberian informasi obat Pelayanan Informasi Obat hanya dilakukan jika ada permintaan pasien Sedangkan kegiatan yang belum dilakukan antara lain konseling ronde Monitoring Efek Samping Obat MESO Pemantauan Terapi Obat dan Evaluasi Penggunaan Obat Dikarenakan Puskesmas Kramat Jati merupakan puskesmas pelayanan rawat jalan maka kegiatan ronde tidak dilakukan Tugas khusus yang diberikan bertujuan untuk menganalisis laporan serta mengkaji perencanaan obat dan perbekalan farmasi di Puskesmas Kecamatan Kramat Jati Berdasarkan pengamatan sebaiknya perlu ditingkatkan jumlah SDM terutama apoteker dan asisten apoteker di Puskesmas Kegiatan pelayanan farmasi klinik juga harus bisa dijalankan semua agar apoteker dapat memiliki peran penting di masyarakat ABSTRACT Profession Internship at Suku Dinas Kesehatan Kota Administrasi Jakarta Timur and Puskesmas Kecamatan Kramat Jati aims to comprehend the role the main task and the function of pharmacist at Suku Dinas Kesehatan Jakarta Timur able to comprehend their role task and responsibility in pharmaceutical care and keep track of clinical pharmacy rsquo s activity that has and hasn rsquo t yet been held at Puskesmas Kecamatan Kramat Jati The role of pharmacist at Suku Dinas Kesehatan Kota Administrasi Jakarta Timur is to become a Sub Section of Pharmacy Food and Drink rsquo s manager Pharmacist at Puskemas Kecamatan Kramat Jati is on duty of and responsible to do the pharmaceutical care that consists of pharmaceutical dosage form rsquo s management and the service of clinical pharmacy that equals to Peraturan Menteri Kesehatan No 30 Tahun 2014 about Standar Pelayanan Kefarmasian di Puskesmas Clinical pharmacy rsquo s activity that has been held at Puskesmas Kecamatan Kramat Jati are prescription rsquo s screening delivery of medicine and drug information rsquo s service The drug information rsquo s service were only given if there was a request from patient Besides the activity that has not been done yet are counseling ward round Adverse Drug Reaction ADR rsquo s Monitoring Therapeutic Drug Monitoring and drug use evaluation Puskesmas Kramat Jati is an outpatient rsquo s primary health care so that ward round patient are never been held forever Special task that was given aims to analyze the report and screen the drug rsquo s planning and inventory of pharmacy at Puskesmas Kecamatan Kramat Jati Based on the observation the amount of human resource should be developed especially for pharmacist and assistant pharmacist in primary health care Clinical pharmacy rsquo s activity should be all held in order to pharmacist have the important role in public "

Praktek kerja profesi Puskesmas Kecamatan Tambora Jakarta Barat periode bulan Oktober tahun 2015 bertujuan untuk memahami peranan, tugas dan tanggung jawab Apoteker dalam praktek pelayanan kefarmasian di Puskesmas sesuai dengan ketentuan perundang-undangan dan etika farmasi yang berlaku dan dalam bidang kesehatan masyarakat, memahami dan memiliki pengetahuan, keterampilan, sikap perilaku serta wawasan dan pengalaman nyata untuk melakukan praktek profesi dan pekerjaan kefarmasian di Puskesmas, melihat dan mempelajari strategi dan pengembangan praktek profesi Apoteker di Puskesmas, memahami gambaran nyata tentang permasalahan praktek dan pekerjaan kefarmasian di Puskesmas serta mampu berkomunikasi dan berinteraksi dengan tenaga kesehatan lain yang bertugas di Puskesmas.
ABSTRACT The aims of apothecary profession internship program at Tambora District Community Health Centers in West Jakarta on March 2015 are to understand the role , duties and responsibilities in pharmaceutical care at Community Health Centers in accordance with the provisions and ethics pharmacy that applies in Community Health Centers, perceive and have knowledge, skill, behavior and attitude insight and experience to do the practice of a profession and pharmaceutical care in Community Health Centers, look and study the development of strategies and practice of the profession of pharmacists at Community Health Centers, having an example of the problems and work of pharmaceutical care in Community Health Centers and able to communicate and interact with other health workers working there.
