"Ketidakakuratan pemberian tekanan cuff pada trakeostomi dapat menyebabkan komplikasi pada trakea. Pasien dengan pneumonia cenderung mengalami gagal nafas sebagai akibat dari proses inflamasi di parenkim paru. Pasien dari ruang IGD dengan penumonia yang terpasang trakeostomi membutuhkan perawatan lanjutan di ruang ICU untuk penggunaan ventilasi mekanik, penggunaan trakeostomi ataupun endotracheal tube yang lama dapat pengalami komplilkasi terjadinya iskemia pada trakea. Manajemen pencegahan komplikasi akibat tekanan cuff penting dilakukan oleh perawat diruang IGD ataupun di ICU. Tekanan cuff trakea di pasang bertujuan untuk mencegah hilangnya volume tidal selama ventilasi tekanan positif, meminimalkan aspirasi paru sekresi lambung dan oral, memfasilitasi pemantauan pernapasan. Salah satu intervensi keperawatan yang dilakukan yaitu memastikan tekanan cuff pada trakeostomi berada di range yang direkomendasikan, tekanan manset cuff harus >25 cmH2O untuk mencegah aspirasi dan <30 cmH2O untuk menghindari kerusakan pada struktur di sekitarnya. Obstruksi aliran darah dapat terjadi ketika tekanan cuff melebihi tekanan perfusi kapiler, yang dapat menyebabkan iskemia mukosa trakea. Pemantauan ini harus dilakukan setiap 6- 8 jam atau setiap shift , untuk mengecek terjadinya adanya kebocoran atau tekanan yang terlalu kencang.

Inaccuracies of applying cuff pressure to the tracheostomy can lead to complications in the trachea. Patients with pneumonia are predisposed to respiratory failure as a result of inflammatory processes in the pulmonary parenchyma. Patients from the emergency room with penumonia attached to the tracheostomy require further care in the ICU room for the use of mechanical ventilation, the use of tracheostomy or endotracheal tube which can long experience complications of ischemia in the trachea. Management of prevention of complications due to cuff pressure is important for nurses in the emergency room or in the ICU. Tracheal pressure cuff in pairs aims to prevent loss of tidal volume during positive pressure ventilation, minimize pulmonary aspiration of gastric and oral secretions, facilitate monitoring of breathing. One of the nursing interventions carried out is to ensure the cuff pressure in the tracheostomy is in the recommended range, the cuff cuff pressure must be >25 cm H2O to prevent aspiration and <30 cm H2O to avoid damage to the surrounding structures. Obstruction of blood flow can occur when the cuff pressure exceeds the capillary perfusion pressure, which can lead to ischemia of the tracheal mucosa. This monitoring must be done every 6-8 hours or every shift, to check for leaks or pressure that is too tight."
