"Halusinasi dan isolasi sosial sering muncul secara bersamaan. Dampak dari halusinasi yang tidak ditangani adalah perilaku mencederai diri sendiri, dan lingkungan, sedangkan isolasi sosial yang tidak ditangani dapat memicu gejala positif dan berkontribusi pada kekambuhan. Penelitian ini bertujuan untuk mendapatkan gambaran pengaruh Cognitive Behaviour Therapy dan Cognitive Behavioural Social Skills Training pada gejala klien. Desain quasi eksperimental digunakan dengan jumlah sampel 56 responden. Hasil penelitian menemukan penurunan gejala kognitif, afektif dan perilaku lebih besar pada klien yang mendapatkan terapi daripada mereka yang tidak mendapatkan. Terapi ini direkomendasikan pada klien halusinasi dan isolasi sosial.

Hallucination and social isolation often occur simultaneously. Impact of dishandled hallucinations is injuring self and environment, while dishandled social isolation will trigger negative sign to possitive sign and contribute in client recurrence. The goal of this study was identify the influences of therapy for client symptomps. Quasi-experimental designs used with sample of 56 respondents. The study found a decrease symptoms of hallucination and social isolation who have CBT and CBSST than the clients that did not receiving therapy. Cognitive, affective, attitude of symptomps hallucination and social isolation have increased significantly. It is recommended for clients with hallucinations and social isolation.

"ABSTRAK
Skizofrenia adalah ganguan jiwa yang dimanifestasikan dengan penurunan dan ketidakmampuan berkomunikasi, gangguan realita, afek tumpul, gangguan kognitif serta kesulitan melakukan aktivitas sehari-hari. Tanda dan gejala negatif yang muncul mengakibatkan isolasi sosial, dan tanda gejala positif yang muncul mengakibatkan halusinasi. Tujuan penanganan kasus ini adalah diketahuinya perubahan tanda gejala dan kemampuan klien isolasi sosial dan halusinasi setelah diberikan tindakan keperawatan ners, social skill training dan cognitive behaviour therapy. Desain penulisan adalah studi kasus dengan responden empat orang. Penanganan kasus tentang topik yang sama sudah pernah dilakukan, namun yang membedakan dengan kasus ini adalah pendekatan teori yang digunakan yaitu teori adaptasi Stuart dan interpersonal Peplau. Data dikumpulkan sebelum dan sesudah klien diberikan tindakan keperawatan ners, social skill training dan cognitive behaviour therapy. Hasil penanganan kasus menunjukan bahwa terjadi penurunan tanda gejala isolasi sosial dan halusinasi serta peningkatan kemampuan klien bersosialisasi, kognitif dan perilaku setelah diberikan tindakan keperawatan ners, social skill training dan cognitive behaviour therapy.ABSTRACT
Schizophrenia is a mental disorder manifested by decreased and inability to communicate, reality disorder, dull affects, cognitive impairment and difficulty performing daily activities. Negative signs and symptoms that result in social isolation, and signs of positive symptoms that appear to cause hallucinations. The purpose of this case is to know the change of symptom signs and ability of social isolation client and hallucinations after given nursing action, social skill training and cognitive behavior therapy. The design of writing is a case study with four respondents. Handling cases on the same topic has been done, but what distinguishes this case is the theoretical approach used is Stuart 39 s adaptation and interpersonal theory of Peplau. Data were collected before and after clients were given nursing actions ners, social skill training and cognitive behavior therapy. The results of case handling showed that there was a decrease of symptoms of social isolation and hallucinations as well as increased ability of client socializing, cognitive and behavior after given nursing action, social skill training and cognitive behavior therapy."

Penerapan Cognitive Behaviour Social Skill Training dilakukan pada 17 klien di ruang Yudistira mulai 9 September- 12 November 2013.
Hasil terapi Cognitive Behaviour Social Skill Training meningkatkan fungsi kognitif dan perilaku sosial serta menurunkan tanda dan gejala pada klien yang mengalam halusinasi dan isolasi sosial. Berdasarkan hasil penelitian perlu direkomendasikan bahwa terapi Cognitive Behaviour Social Skill Training dapat dijadikan standar terapi spesialis keperawatan jiwa yang dapat digunakan pada klien yang mengalam halusinasi dan isolasi sosial.

Application of cognitive behaviour social skills training therapy was done to 17 clients since 9 September-12 November 2013.
Finding was revealed cognitive behaviour social skills training exactly effective to improves cognitive function and social behavior and reduce the signs and symptoms of hallucinations. Base on this finding, recommended cognitive behaviour social skills training become to specialist standard therapy in psychiatric nursing and may used for hallucinations and social isolation clients.

"Skizofrenia adalah kondisi kecatatan yang kronik dan persisten, ditandai dengan gejala positif dan negatif. Diagnosis keperawatan yang muncul adalah risiko perilaku kekerasan dan isolasi sosial. Tujuan karya ilmiah akhir spesialis ini adalah untuk mengetahui perubahan tanda dan gejala serta kemampuan klien risiko perilaku kekerasan dan isolasi sosial setelah mendapatkan tindakan keperawatan ners dan tindakan keperawatan ners spesilais terapi kognitif perilaku dan latihan keterampilan sosial. Metode yang digunakan adalah desain operational riset, jumlah sampel sebanyak 30 orang klien skizofrenia paranoid dengan diagnosis keperawatan risiko perilaku kekerasan dan isolasi sosial. Hasil penelitian ini didapatkan tanda gejala risiko perilaku kekerasan dan isolasi sosial menurun (p-value<0.05) dan kemampuan meningkat (p-value<0.05) setelah pemberian tindakan keperawatan ners, terapi kognitif perilaku dan latihan keterampilan sosial. Kesimpulan, kombinasi tindakan keperawatan ners, terapi kognitif perilaku dan latihan keterampilan sosial direkomendasikan untuk diberikan pada diagnosis risiko perilaku kekerasan dan isolasi sosial.

The purpose of the scientific work of this specialist are to know the change in the signs of symptoms and the ability of clients the risk of behavioral violence and social isolation after obtaining the of nursing generalis and nursing specialist actions of the Cognitive Behaviours Therapy and Social Skills Training. The method using the design of operational research, the number of samples of 30 patients schizophrenia paranoid with the diagnosis of the risk of violent behavior and social isolation. The results of this study were obtained signs of risk of violent behavior and social isolation (p-value of < 0.05) and increased capability (P-value of < 0.05) after the administration of nursing action, cognitive behaviour therapy, and social skills training. Conclusions, the combination of nursing action, behavioral Mind therapy and social skills therapy are recommended to be administered on the diagnosis of risk of violent behavior and social isolation"

Pikiran negative pada klien harga diri rendah kronik menyebabkan klien sulit berpikir jernih dan logis terhadap diri sendiri. Tujuan penulisan ini untuk memberikan gambaran pengaruh penerapan terapi pikiran perilaku dan psikoedukasi keluarga pada klien harga diri rendah kronik. Metode yang digunakan adalah deskriptif analitik dengan pendekatan teori adaptasi Roy dan model stres dan adaptasi Stuart. Tindakan keperawatan terapi pikiran perilaku diberikan pada 20 klien dimana 15 keluarga klien diberikan psikoedukasi keluarga. "ABSTRAK
Skizofrenia merupakan penyakit neurobiologis berat dan terus menerus yang mengakibatkan terganggunya kehidupan individu. Tanda gejala yang terjadi klien berupa berupa gejala positif dan gejala negatif dengan diagnosis keperawatan harga diri rendah kronik dan risiko perilaku kekerasan. Tujuan karya ilmiah akhir spesialis ini adalah untuk mengetahui perubahan tanda gejala dan kemampuan klien harga diri rendah kronik dan risiko perilaku kekerasan setelah mendapatkan tindakan keperawatan ners generalis dan ners spesialis : terapi kognitif perilaku dan latihan asertif. Asuhan medik adalah pemberian antipsikotik dan asuhan keperawatan adalah tindakan ners dan ners spesialis. Metode karya ilmiah akhir yang digunakan adalah riset operational dengan jumlah sampel 30 klien. Hasil penelitian ini didapatkan bahwa tanda dan gejala harga diri rendah kronik dan risiko perilaku kekerasan menurun secara bermakna (p-value ≤ 0.05) dan kemampuan klien meningkat secara bermakna (p-value ≤ 0.05) setelah pemberian tindakan keperawatan ners generalis dan ners spesialis terapi kognitif perilaku dan latihan asertif. Pemberian kombinasi tindakan ners, ners spesialis terapi kognitif perilaku dan latihan asertif sangat direkomendasikan untuk penatalaksanaan diagnosis keperawatan harga diri rendah kronik dan risiko perilaku kekerasan.

ABSTRACT
Schizophrenia is a severe and continuous neurobiological disease that results in disruption of individual life. Signs and symptoms that occur on clients are in the form of positive symptoms and negative symptoms with a nursing diagnosis chronic low self-esteem and the risk of violent behavior. The purpose of this specialist s final scientific work is to determine changes in signs and symptoms and the client's ability who have chronic low self-esteem and the risk of violent behavior after obtaining nursing actions by generalist nurses and specialist nurses : cognitive behaviour therapy and assertiveness training. Medical care is the provision of antipsychotics and nursing care is the action of nurses and specialist nurses. The final scientific method used is operational research with a sample of 30 clients. The results of this study showed that the signs and symptoms of chronic low self-esteem and the risk of violent behavior decreased significantly (p-value ≤ 0.05) and the ability of clients increased significantly (p-value ≤ 0.05) after giving generalist nursing actions and specialist nursing actions cognitive behaviour therapy and assertiveness training. Giving a combination of actions by nurses, specialist nurses cognitive behaviour therapy and assertiveness training is highly recommended for the management of nursing diagnoses of chronic low self-esteem and the risk of violent behavior."

"Isolasi sosial adalah salah satu gejala negatif dari skizofrenia (Stuart, 2009) yang paling banyak ditemukan di ruang Bratasena. Isolasi sosial dapat diartikan sebagai keadaan seorang individu yang mengalami penurunan atau bahkan sama sekali tidak mampu berinteraksi dengan orang lain disekitarnya, mungkin merasa ditolak, tidak diterima, kesepian, dan tidak mampu membina hubungan yang berarti dengan orang lain (Keliat, dkk, 2011). Social skills training merupakan salah satu pendekatan psikoedukasional untuk memperbaiki kekurangan pada beberapa kemampuan interpersonal dalam berinteraksi dengan orang lain (Stuart &Laraia, 2005). Cognitive behavior therapy merupakan salah satu bentuk psikoterapi yang didasarkan pada teori bahwa tanda dan gejala fisiologis berhubungan dengan interaksi antara pikiran, perilaku dan emosi (Pedneault, 2008).
Tujuan penulisan karya ilmiah akhir ini adalah menggambarkan penerapan terapi social skills training dan cognitive behavior therapy pada klien isolasi sosial dengan pendekatan Model Hubungan Interpersonal Peplau. Penerapan social skills trainingdilakukan pada 26 kliendan cognitive behavior therapydilakukan pada 15 orang klien di ruang Bratasena pada kurun waktu 20 Pebruari - 20 April 2012.
Hasil terapi Social skills training sangat efektif pada 26 klien isolasi sosialdengan menunjukkan peningkatan dalam berkomunikasi, baik secara verbal maupun non verbal. Terapi Cogntive behavior therapy juga menunjukkan efektifitasnya dimana sebanyak 15 klien mampu menunjukkan kemampuan mengubah pikiran otomatis yang negatif terhadap diri, orang lain, dan lingkungannya. Berdasarkan hasil di atas perlu direkomendasikan bahwa social skills trainingdan cognitive behavior therapydapat dijadikan standar terapi spesialis keperawatan jiwa dan perlu disosialisasikan pada seluruh tatanan pelayanan kesehatan.

Social isolation is one of the negative symptoms of skizofrenia (Stuart, 2009) is mostcommonly found in the Bratasena. Social isolation can be interpreted as the state of an individual who had a reduction or even not at all able to interact with others around them, may feel rejected, not accepted, lonely, and unable to establish meaningful relationships with others (Keliat, et al, 2011). Social skills training is one approach psikoedukasional to correct deficiencies in some interpersonal skills in interacting with others (Stuart &Laraia, 2005). Cognitive behavior therapy is a form of psychotherapy that is based on the theory that the physiological signs and symptoms associated with the interaction between thoughts, behaviors and emotions (Pedneault, 2008).
Purpose of this final scientific work is to describe the application of social skills training therapy and cognitive behavior therapy in client's social isolation with Peplau Interpersonal Relations Model approach. Application of social skills training carried out at 26 clients and cognitive behavior therapy performed on 15 clients in the Bratasena during the period 20 February - 20 April 2012.
Social skills training outcomes are very effective in social isolation with 26 clients showed an increase in communication, both verbal and non verbal. Cogntive therapy behavior therapy also showed its effectiveness in which as many as 15 clients were able to demonstrate the ability to change negative automatic thoughts to yourself, others and the environment. Based on the above results need to be recommended that social skills training and cognitive behavior therapy can be used as standard therapy nursing specialists need to be socialized to whole structure of health services.

"Klien gangguan jiwa cenderung tidak produktif dan berpenghasilan mempengaruhi stigma sehingga menyebabkan Harga Diri Rendah (HDR) dan isolasi sosial. Klien memerlukan pemulihan jangka panjang, yang dipersulit insight buruk.
Tujuan: menjelaskan penerapan Acceptance and Commitment Therapy (ACT), Cognitive Behavior Social Skills Training (CBSST) menggunakan pendekatan Model Adaptasi Roy (MAR) dan Model Tidal. MAR sebagai kerangka utama dan Teori Model Tidal pada proses kontrol koping. Analisis dilakukan pada 32 klien ACT+CBSST, dan 22 klien CBSST.
Hasil: ACT+CBSST memperbaiki insight dan meningkatkan keterampilan sosial, perilaku adaptif meningkatkan penurunan tanda gejala HDR kronis dan isolasi sosial.
Saran: kombinasi ACT+CBSST pilihan pada awal program pemulihan.

Clients of mental disorders tend to be non- productive and non-income that affect stigma the impact Low Self-Esteem (LSE) and social isolation. The client required a long-term recovery, which compounded a poor insight.
Objective: explained the application of Acceptance and Commitment Therapy (ACT), Cognitive Behavior Social Skills Training (CBSST) by using the approaches of Roy Adaptation Model (RAM) and the Tidal Model. RAM as the main framework and the Tidal Model Theory as coping control process. Analysis was performed on 32 ACT + CBSST clients, and 22 CBSST client.
Results: ACT improved insight and increased social skills, improved adaptive behavior and decreased in signs and symptoms of chronic LSE and social isolation.
Recomendation: the combination of ACT + CBSST choice at the beginning of the recovery program.

Latar belakang: Angka Skizofrenia di Jawa Tengah melebihi dari rata-rata di Indonesia yaitu sebesar 0,25%. Gejala skizofrenia digolongkan sebagai gejala positif dan negatif. Gejala positif yaitu halusinasi dan defisit fungsi kognitif. Selain fungsi kognitif, isolasi sosial juga dapat mempengaruhi motivasi. Motivasi merupakan perilaku untuk memperoleh kesenangan dan kepuasan setelah melakukan sesuatu sesuai dengan kemampuannya. Hal ini akan berdampak juga pada fungsi sosial klien.

Tujuan: untuk mengetahui pengaruh latihan keterampilan sosial terhadap fungsi kognitif, motivasi, dan fungsi sosial pada klien dengan isolasi sosial; penelitian ini juga untuk mengetahui perbedaan fungsi kognitif, motivasi, dan fungsi sosial pada kelompok intervensi dan kelompok kontrol setelah kelompok intervensi diberikan LKS.
Metode: Penelitian kuantitatif dengan kuasi ekperimen menggunakan dua kelompok yaitu kelompok intervensi dan kontrol. Setiap kelompok terdiri atas 36 responden pada kelompok intervensi dan kontrol.
Hasil: Ada pengaruh latihan keterampilan sosial terhadap fungsi kognitif, motivasi, dan fungsi sosial pada klien dengan isolasi sosial; fungsi kognitif, motivasi, dan fungsi sosial meningkat secara signifikan pada kelompok intervensi; berdasarkan dari uji statistik menunjukkan motivasi mengalami peningkatan yang paling rendah.
Rekomendasi: Perawat spesialis memberikan latihan keterampilan sosial di rumah sakit sehingga perlu ditambahkan jumlah perawat spesialis di rumah sakit.

Background: Schizophrenia rates in Central Java exceed the average in Indonesia which is 0.25%. Symptoms of schizophrenia are classified as positive and negative symptoms. Positive symptoms are hallucinations and cognitive function deficits. Besides cognitive function, social isolation can also influence motivation. Motivation is a behavior to obtain pleasure and satisfaction after doing something according to his abilities. This will also have an impact on the client`s social functions.
Objective: to determine the effect of social skills training on cognitive functions, motivation, and social functions on clients with social isolation; this study was also to determine differences in cognitive function, motivation, and social function in the intervention group and the control group after the intervention group was given LKS.
Method: Quantitative research with quasi-experiment using two groups namely intervention and control groups. Each group consisted of 36 respondents in the intervention and control groups.
Results: There is the influence of social skills training on cognitive functions, motivation, and social functions on clients with social isolation; cognitive function, motivation, and social function increased significantly in the intervention group; based on the statistical test shows the motivation experienced the lowest increase.
Recommendation: Specialist nurses provide social skills training at the hospital so that the number of specialist nurses in the hospital needs to be added.
