"Discharge planning dapat menurunkan angka rawatan ulang. Penelitian ini bertujuan mengidentifikasi hubungan implementasi sistem jenjang karir dan fungsi manajemen dengan pelaksanaan discharge planning. Desain penelitian deskriptif korelasi dengan pendekatan cross sectional. Sampel penelitian adalah perawat dan dokumen discharge planning masing-masing sebanyak 121. Cara pengambilan data menggunakan simple random sampling dan proportional sampling. Analisis data menggunakan korelasi Spearman. Hasil penelitian menunjukkan ada hubungan implementasi sistem jenjang karir (p=0,001), penilaian kompetensi (p=0,001), kewenangan klinik (p=0,001), pengembangan profesional berkelanjutan (p=0,001), dan fungsi manajemen (0,001) dengan pelaksanaan discharge planning. Hasil penelitian ini merekomendasikan perlunya pelatihan discharge planning bagi perawat untuk meningkatkan kompetensi perawat dalam melaksanakan discharge planning.

This study aimed to identify relationship of implementation nursing career ladder system and management function to the implementation of discharge planning. Design research was descriptive correlation with cross sectional approach. Data were collected through primary data as much as 121 nurses and also secondary data from patient medical records related to discharge planning documentation. The sampling technique was simple random sampling and proportional sampling. Data were analyzed by spearman correlation. The result showed a significant correlation between implementation of career ladder system (p=0,001), competency assessment (p=0,001), clinical privileges (p=0,001), continuing professional development (p=0,001) and management function (p=0,001) and the implementation of discharge planning. It is recommended to the need for discharge planning training for nurse to improve the competency of nurses in implementing discharge planning.

"Sistem jenjang karir keperawatan yang direncanakan dan diimplementasikan dapat memengaruhi proses kehidupan dan masa depan perawat. Penelitian ini bertujuan mengidentifikasi faktor dominan yang memengaruhi efektifitas pelaksanaan program sistem jenjang karir dengan implementasi manajemen nyeri di rumah sakit. Disain penelitian menggunakan deskriptif korelasi dengan pendekatan cross sectional. Data dikumpulkan melalui data primer sebanyak 121 perawat pelaksana serta data sekunder dokumen rekam medis pasien berisi catatan keperawatan terkait manajemen nyeri yang terdiri dari pengkajian, intervensi dan re-evaluasi. Tehnik pengambilan sampel adalah simple random sampling. Analisis menggunakan regresi linier berganda. Hasil penelitian didapatkan bahwa variabel implementasi sistem jenjang karir yang paling berpengaruh terhadap implementasi manajemen nyeri oleh perawat klinik adalah pengembangan profesional berkelanjutan (PPB). Peneliti merekomendasikan perlunya perencanaan terstruktur melalui analisa kebutuhan training bagi masing-masing perawat.

Career ladder system which its planning and implementation affected nurse?s life and future. This study aimed to identify the dominant factors of career ladder system program to the implementation of pain management in the hospital. The design research was descriptive correlative with cross sectional approach. Data were collected through primary data as much as 121 nurses and also secondary data from patient medical records related to pain management nursing documentation included assessment, intervention, and re-evaluation. The sampling technique was simple random sampling. Data were analyzed by multiple linear regression. The result showed that the variables of the career ladder system implementation that most affected on the pain management implementation by clinical nurses is continuing professional development (CPD). It is recommended to make structured planning through training needs analysis for each nurse.

"Penerapan discharge planning yang efektif terbukti dapat memberikan manfaat, baik pada pasien maupun rumah sakit seperti dapat mengurangi risiko readmisi. Discharge planning merupakan bagian dari proses keperawatan. Peran perawat dalam pelaksanaan discharge planning ini sangat penting. Pada ibu postpartum, penerapan discharge planning sangat perlu dilakukan. Hal ini dikarenakan pada ibu postpartum mengalami perubahan peran sehingga pengetahuan, keterampilan, dan perilaku dalam perawatan di rumah perlu dimiliki. Namun, beberapa penelitian menyatakan bahwa masih banyak perawat yang tidak menerapkan discharge planning. Selain tidak menerapkan, perawat juga seringkali tidak melakukan proses discharge planning secara menyeluruh dan kontinu. Metode penulisan yang digunakan yaitu case report¸dimana pada tahap awal penulis mengidentifikasi masalah dengan observasi dan wawancara langsung. Kemudian, penulis mencari evidence based practice berupa model IDEAL dalam discharge planning untuk diterapkan dan menganalisis evaluasi setelah model ini diterapkan. Lalu, penulis menyusun pembahasan dan rekomendasi ke pihak ruangan berdasarkan hasil studi kepustakaan. Hasil laporan pada kasus ini mengidentifikasi bahwa penerapan discharge planning pada ibu postpartum di ruangan masih belum optimal. Beberapa perawat masih belum memahami terkait proses discharge planning. Salah satu hal yang menyebabkan masalah ini yaitu belum adanya aturan yang mengatur tentang discharge planning di ruangan sehingga diperlukan penerapan model IDEAL sebagai acuan untuk discharge planning di rumah sakit.

Implementing effective discharge planning has been proven to provide benefits, both for patients and hospitals. Discharge planning is part of the nursing process and the role of nurses in discharge planning is significant. In postpartum mothers, the implementation of discharge planning is essential because postpartum mothers experience a change in their role so knowledge, skills, and behavior in-home care need to be possessed. However, several studies state that there are still nurses who do not implement discharge planning. In addition to not implementing it, nurses also often do not carry out the discharge planning process comprehensively and continuously. The writing method used is a case report, where in the initial stage the author identifies problems through direct observation and interviews. Then, the author looks for evidence-based practice in the form of an IDEAL model in discharge planning to be applied and analyzes the evaluation after this model is applied. Then, the author compiles a discussion and recommendations to the ward based on the results of a literature study. The results of the report in this case identify that the implementation of discharge planning in postpartum mothers in the ward is still not optimal. Some nurses still do not understand the discharge planning process. One of the things that causes this problem is the absence of regulations governing discharge planning in the room, so it is necessary to apply the IDEAL model as a reference for discharge planning in hospitals. "

"Sistem jenjang karir perawat yang diterapkan di Rumah Sakit dapat meningkatkan kinerja, kepuasan, dan profesionalisme perawat, sehingga mutu pelayanan kesehatan menjadi meningkat. Penelitian ini bertujuan untuk mengetahui gambaran persepsi perawat tentang peningkatan jenjang karir dan untuk mengidentifikasi hubungan karakteristik perawat dengan persepsi tentang jenjang karir juga dengan sub variabel jenjang karir (pengembangan karir, penghargaann pengakuan, promosi dan tantangan). Desain penelitian ini adalah deskiptif dengan pendekatan potong lintang (cross-sectional) menggunakan sampel perawat klinik di Rumah Sakit Krakatau Medika Cilegon sebanyak 133 responden yang dipilih dengan teknik Proposional sampling. Penelitian dianalisis dengan cara chi squere.
Hasil penelitian menemukan terdapat hubungan bermakna antara usia dengan persepsi perawat tentang pengakuan (p= 0.034), terdapat hubungan bermakna antara lama kerja dengan persepsi tentang pengakuan (p= 0.031: OR= 0.423). Hasil penelitian ini merekomendasikan program jenjang karir yang dapat diterapkan untuk menciptakan profesi keperawatan yang Profesional. Mengimplementasikan sistem jenjang karir institusi pelayanan harus menyiapkan perangat sistem jenjang karir yang akan digunakan, merencanakan kapan target untuk mulai dilaksanakan dan panduan untuk kedepan agar sistem jenjang karir dapat berlaku untuk saat ini dan masa yang akan datang.

Nursing career ladder system which is implemented at the hospital can improve the nurses’ performance, satisfaction and professionalism, so that the quality of health services can be improved. This study aimed to describe the nurses’ perception of career ladder promotion and to identify the correlation between the nurses’ characteristics and their perceptions of career ladder and its sub variables (career development, reward, recognition, promotion and challenge). This research was a descriptive study with cross sectional design. The sample was 133 clinical nurses at the Krakatau Medika Cilegon Hospital who was selected by proportional sampling technique. The study was analyzed using Chi Square.
The study found that there was a significant correlation between the nurses’ age and their perception of career recognition (p = 0.034), there was a significant correlation between the length of employment and the perception of career recognition (p = 0.031: OR = 0.423). The results of this study recommended a career ladder program that can be applied to create a professional nursing profession. The implementation of career ladder program in health services institutions needs a preparation of a career ladder system that will be used, a plan of time when the system has to be implemented and a guide for a future so that the career ladder system can be applied now and in the future.

"Discharge planning yang dilakukan secara optimal telah diteliti dapat meminimalisir readmisi, memperpendek waktu rawat inap dan meningkatkan kepuasan pasien dan profesional kesehatan. Metode penulisan yang digunakan yaitu Case Report, dengan tahap pertama mengidentifikasi masalah dengan observasi dan wawancara langsung. Kedua, mengidentifikasi tingkat kepuasan dan pemahaman pasien. Ketiga, menganalisis penerapan dengan studi kepustakaan. Keempat, menyusun pembahasan dan rekomendasi ke pihak ruangan. Hasil laporan kasus ini mengidentifikasi masih kurang optimalnya penerapan discharge planning sehingga perlunya penerapan strategi IDEAL untuk mendapatkan intervensi perawatan yang paripurna seperti pada visi rumah sakit.

Discharge planning carried out optimally has been researched to minimize readmissions, shorten hospitalization time and increase patient and health professional satisfaction. The writing method used is a Case Report, with the first stage identifying problems through direct observation and interviews. Second, identify the level of patient satisfaction and understanding. Third, analyze the application using literature study. Fourth, prepare discussions and recommendations to the room. The results of this case report identify that the implementation of discharge planning is still less than optimal, so it is necessary to implement the IDEAL strategy to obtain complete care interventions as in the hospital vision.

"Pendahuluan: Penerapan jenjang karir di sebuah rumah sakit akan memberikan dampak baik untuk semua tenaga kesehatan yang ada termasuk perawat. Namun penerapan ini harus bersinergi antara antusiasme perawat dan komite keperawatan serta dukungan penuh dari pimpinan rumah sakit dan pengetahuan perawta itu sendiri.
Metode: Desain penelitian ini menggunakan korelasi dengan rancangan penelitian cross-sectional. Sampel diambil dengan teknik simple random sampling terhadap 156 perawat pelaksana. Pengumpulan menggunakan kuesioner dalam bentuk google form. Analisa data univariate skala numerik dan analisa data bivariate menggunakan uji parametrik.
Hasil: Gambaran implementasi di rumah sakit didapatkan 93,27 % di rumah sakit. Hasil penelitian ini juga menunjukkan ada hubungan yang signifikan antara umur, lama kerja, tingkat pendidikan, pengetahaun, dukungan organisasi dan dukungan atasan dengan p=0,001. Variabel factor independent mempengaruhi factor dependet dengan 52,1 %. Simpulan: Hasil penelitian ini dapat disimpulkan bahwa menunjukkan ada factor yang mempengaruhi yang signifikan karakteristik perawat (umur, lama kerja dan tingkat pendidikan), pengetahuan tentang jenjang karir, dukungan organisasi, dan dukungan atasan.

Introduction: The application of career paths in a hospital will have a good impact on all existing health workers, including nurses. However, this application must synergize between the enthusiasm of the nurses and the nursing committee and the full support of the hospital leadership and the knowledge of the nurses themselves.
Method: The design of this study uses correlation with a cross-sectional research design. Samples were taken by simple random sampling technique from 156 practicing nurses. Collection using a questionnaire in the form of google form. Numerical scale univariate data analysis and bivariate data analysis using parametric test.
Results: The description of implementation in the hospital was found to be 93.27% in the hospital. The results of this study also showed that there was a significant relationship between age, length of work, level of education, knowledge, organizational support and superior support with p=0.001. The independent factor variables affect the dependent factor by 52.1%.
Conclusion: The results of this study can be concluded that there are factors that significantly influence the characteristics of nurses (age, length of work and level of education), knowledge about career paths, organizational support, and superior support

"Tesis ini membahas Perencanaan dan Pengembangan Karir Anggota Polri yang ditugaskan di Kementrian dan Lembaga Negara. Permasalahan perencanaan karir individu dan organisasi sertapengembangan karir menjadi fokus peneliti. Metode penelitian kualitatif dengan hasil bahwa anggota Polri telah membuat perencanaan karir untuk kembali ke Polri dengan alasan ingin mensinergikan pengalaman bertugas diluar struktur untuk kemajuan Polri, namun belum sinergis dengan perencanaan karir Polri dan adanya desain pengembangan karir anggota Polri guna menunjang pelaksanaan tugas. Kesimpulan: belum ada sistem perencanaan karir yang terintegrasi bagi anggota yang ditugaskan di luar struktur dengan perencanaan karir di Polri, sehingga berdampak pada pengembangan karir anggota.

The focus of this study is the career planning and development of the Police Officer on Special duties at Government Institutions. The problems are the individual and organization career planning and career development. This is qualitative research. The result are the officers as informan have designed career planning to return back to Police Institution because they will develop Police institution by their capability during at Government Institution. However it is not matched with organizational (Police) career planning. There is career development program. The conclution : no integrated career planning system that manage the officer who posted at government institutions and at Police that will effect to police career development.

"Stroke menimbulkan gangguan neurologic sehingga menyebabkan perubahan kualitas hidup. Kualitas hidup adalah sehat fisik, mental, social dan terlepas dari penyakit. Pemulihan stroke agar tercapainya kualitas hidup yang baik dibutuhkan peran serta tenaga kesehatan. Discharge planning adalah mempersiapkan pasien untuk mendapatkan kontinuitas perawatan dalam proses penyembuhan maupun dalam mempertahankan derajat kesehatan. Desain penelitian Quasy Experimental. Bertujuan membandingkan pengaruh pemberian discharge planning terstruktur di RS Al-Islam dengan discharge planning rutin di RS Al-Ihsan Bandung terhadap kualitas hidup pasien stroke iskemik. Populasi adalah pasien stroke iskemik. Sampel secara consecutive admission berdasarkan estimasi proporsi sebanyak 44 orang. Instrument untuk kualitas hidup The MOS (SF-36). Berisi 36 pertanyaan; Fungsi fisik, peranan fisik, rasa nyeri, kesehatan umum, fungsi social, vitalitas, kesehatan mental, peranan emosi, ringkasan fisik dan mental. Skor 0-100. Hasil uji statistic discharge planning terstruktur berpengaruh secara bermakna terhadap kualitas hidup pasien stroke iskemik setelah mempertimbangkan umur, jenis kelamin, tingkat pendidikan pada nilai p<0,001 nilai koefisien beta 3,008 dengan nilai Relatif Risk (RR) 20,25 yang menunjukkan pasien stroke iskemik yang dilakukan discharge planning terstruktur memiliki peluang 20 kali lebih besar untuk memiliki perubahan kearah kualitas hidup yang lebih baik dibandingkan tanpa dilakukan discharge planning.

Stroke cause neurologic disorders that cause changes in quality of life. Quality of life is a healthy physical, mental, social, and regardless of the disease. Stroke recovery in order to achieve good quality of life required the participation of health personnel. Discharge planning is to prepare patients for continuity of care in a healing process and in maintaining health. Quasy Experimental research design. Aiming to compare the effect of discharge planning is structured in RS Al-Islam with the routine discharge planning in hospitals of Al-Ihsan Bandung to quality of life of patients with ischemic stroke. The population was patients with ischemic stroke. Samples are based on estimates of the proportion of consecutive admissions of 44 people. Instrument for quality of life of the MOS (SF-36). Contains 36 questions; physical function, physical role, pain, general health, social functioning, vitality, mental health, role emotional, physical and mental summary. Score 0-100. The results of statistical tests structured discharge planning significantly affects the quality of life of ischemic stroke patients after considering age, sex, educational level on the p-value <0.001 3.008 beta coefficient value with the value Relative Risk (RR) 20.25 indicating ischemic stroke patients who do structured discharge planning to have 20 times greater chance to have a better quality of life compared with no discharge planning done."

"Intervensi keperawatan merupakan bagian dari proses discharge planning. Proses discharge planning yang meliputi edukasi, konseling dan koordinasi tim diharapkan dapat meningkatkan self efficacy pada pasien gagal jantung. Tujuan penelitian ini adalah untuk mengetahui pengaruh discharge planning terhadap self-efficacy pada pasien gagal jantung. Penelitian ini adalah penelitian kuantitatif dengan desain quasi eksperimen dengan rancangan pre-test and post-test nonequivalent with control group design. Besar sampel 21 responden (11 responden kelompok intervensi dan 10 responden kelompok kontrol).
Hasil penelitian menunjukkan perbedaan selisih self efficacy antara kelompok kontrol dan intervensi (p=0,02) walaupun tidak terdapat perbedaan self efficacy antara kelompok kontrol dan intervensi secara statistik (p=0,515). Hal ini bisa terjadi karena minimnya jumlah sampel sehingga mengakibatkan power penelitian yang setelah dihitung ulang adalah sebesar 55%. Namun demikian dapat disimpulkan secara klinis bahwa discharge planning mempengaruhi nilai self efficacy selisih antara kelompok kontrol dan intervensi. Rekomendasi hasil penelitian ini perlu adanya penelitian ulang dengan jumlah sampel yang lebih banyak.

Nursing intervention is a part of discharge planning process. Discharge planning process that consist of education, conseling and tim coordination can be expected to improve self efficacy in heart failure patients. This study aims to examine the effect of discharge planning on self efficacy in heart failure patients. This study was a quantitative research with quasi experiment, using pre-test and post-test nonequivalent with control group design. The number of samples was 21 respondents (11 respondent in intervention group and 10 respondent in control group).
The result showed that there was significant mean difference in self efficacy on both groups after providing discharge planning intervention (p=0,02) although there was no difference in self efficacy on both groups (p=0,515). This could be caused by small sample size that make this research power 55%. However, there was conclusion that discharge planning influence self efficacy mean diferenceon both groups. This study recommends further research to examine the effect of discharge planning on self efficacy with larger samples size.
