"Latar Belakang. Menurut SDKI tahun 2007, AKI 228 per 100.000KH dan AKB 34 per 1000KH sementara target MDG‟s AKI 102 per 100.000 KH dan AKB 23 per 1000KH. Untuk mempercepat pencapaian target MDG‟s maka diluncurkan program Jampersal sesuai dengan surat edaran yang dikeluarkan Menteri Kesehatan nomor TU/Menkes/E/391/11/2011 tentang Jaminan Persalinan, tanggal 22 Februari 2011. Kabupaten Lebak mendapatkan alokasi dana sebesar Rp. 5.470.545.000,- untuk 16.870 ibu bersalin. Namun dana program tersebut hanya terserap Rp. 3,9 milyar atau sekitar 71,2 % dengan cakupan sebesar 11.137 ibu bersalin atau 68,3%.
Metode. Jenis Penelitian ini adalah desain kualitatif. Arah penelitian ini mengenai Implementasi Kebijakan Program Jampersal di Kabupaten Lebak Propinsi Banten Tahun 2011. Metode analisa yang digunakan adalah content analysis berdasarkan triangulasi metode, triangulasi sumber dan triangulasi data.
Hasil. Hasil analisa yang didapat menunjukkan bahwa implementasi kebijakan dilakukan sudah berjalan dengan baik, hanya saja masih ada hambatan terkait Kendala seperti terhambatnya laporan ke Dinas Kesehatan Propinsi, rendahnya tarif, ketersediaan fasilitas, sebagian bidan desa yang tidak berada di tempat dan geografis.

Background. According IHDS in 2007, Maternal Mortality Rate is 228/ 100.000 life birth and Infant Mortality Rate is 34/1000 life birth, while MDG's target is Maternal Mortality Rate is 102/100.000 life birth and Infant Mortality Rate is 23/1000 life birth. To achieve MDG's target therefore Jampersal had been launched. According to circular issued by Ministry of Health No. TU/Menkes/E/391/II/2011 about Jampersal, on February 22nd 2011. In Lebak Regency had fund allocation 5.470.545.000,- for 16.870 maternal but that fund only absorb for 3,9 billion or about 71,2 % of 11.137 maternal or 68,3%.
Methods. This research is qualitative design research. It about the Implementation of Jampersal Policy in Lebak regency, Banten Province in 2011. Analysis method that being used is content analysis with triangulation method, source and data.
Result. Analysis results obtained showed that the implementation of the policy has been running very well, but there are any constraints such as delays in reporting to the Provincial Health Office, the low rates, availability of facilities, many midwives who are not stay in her and geography."

"[ABSTRAK
Intelligent Transportation System (ITS) adalah salah satu teknologi yang mengintegrasikan antar sistem informasi dan teknologi komunikasi dengan infrastruktur transportasi, kendaraan, dan pengguna jalan. Salah satu implementasi teknologi Intelligent Transportation System (ITS) adalah Vehicular Ad Hoc Network (VANET). VANET merupakan sistem komunikasi kendaraan yang mendukung untuk komunikasi Vehicle to Infrastructure (V2I) dan Vehicle to Vehicle (V2V). Sebagai bagian dari Intelligent Transportation System (ITS), komunikasi kendaraan dalam jaringan VANET dapat lebih efektif dalam menghindari kecelakaan dan kemacetan lalu lintas dari pada jika setiap kendaraan mencoba untuk memecahkan masalah ini secara individual.
Standar IEEE 1609.4 didefinisikan sebagai mode operasi Multikanal jaringan VANET pada lapisan Medium Access Control (MAC) yang terdiri dari tujuh kanal frekuensi yang berbeda, yaitu satu kanal CCH178 akan dialokasikan untuk Control Channel (CCH), yang digunakan sebagai kanal publik untuk aplikasi keamanan yang relevan di jalan. Enam kanal ya ng lainnya dialokasikan untuk Service Channel (SCH), yang digunakan sebagai kanal untuk menangani layanan multimedia dan yang tidak berhubungan dengan keamanan di jalan. Salah satu permasalahan dalam penjaminan kinerja pada IEEE 1609.4 adalah tingginya mobilitas node kendaraan dan perubahan lintasan yang berbeda. Hal ini menyebabkan delay yang tinggi dan throughput yang rendah. Peningkatan kinerja pada standar IEEE 1609.4 dapat dilakukan dengan optimasi pada proses sinkronisasi interval kanal CCH dan SCH.
Pada disertasi ini dikembangkan model baru Markov chain yang bertujuan untuk meningkatkan kinerja sistem koordinasi kanal dinamis pada standar multikanal IEEE 1609.4 terhadap pengaruh anomali kinerja, slot anomali, efek Doppler, fading Nakagami dan AWGN. Perbaikan kinerja yang dilakukan terhadap pengaruh diatas adalah dengan menggunakan nilai awal optimal Contention Window (CW). Penentuan nilai awal CW akan mempengaruhi kinerja yang dihasilkan pada model Markov chain yang digunakan. Nilai awal optimal CW didapatkan dari hasil distribusi node di setiap zone dengan menggunakan distribusi Poisson.
Dari hasil simulasi dan evaluasi kinerja yang dihasilkan, dapat dianalisa bahwa model DCF yang diajukan pada disertasi ini dapat menurunkan nilai delay transmisi CCH terhadap adanya kanal propagasi Nakagami dengan rata-rata (mean) sebesar 16.84 %. Selanjutnya, dapat disimpulkan bahwa kinerja yang dihasilkan pada model Markov chain dengan menggunakan nilai awal optimal CW didapatkan meningkatkan nilai Aggregate Throughput sebesar 42.53% dibandingkan dengan model yang diajukan oleh Wang. Sedangkan model DCF yang diajukan meningkatkan nilai probabilitas transmisi paket WAVE Service Advertisement (WSA) terhadap fenomena anomalous slot dengan persentase kenaikan rata-rata (mean) sebesar 11.35 %. Selanjutnya, dapat dianalisa bahwa model DCF yang diajukan meningkatkan nilai interval waktu dari akses contention kanal CCH terhadap efek Doppler dengan persentase kenaikan rata-rata (mean) sebesar 11.31 %;

ABSTRACT
Intelligent Transportation System (ITS) is one of the technologies that integrate information systems and communication technologies with transportation infrastructures, vehicles and road users. One implementation of the Intelligent Transportation System (ITS) is Vehicular Ad Hoc Network (VANET). VANET is a vehicle communication system which supports Vehicle to Infrastructure (V2I) and Vehicle to Vehicle (V2V) communication. As a part of the Intelligent Transportation System (ITS), vehicles communication in VANET networks can be more effective in avoiding accidents and traffic congestion than if each vehicle try to solve this problem individually.
The IEEE 1609.4 standard is defined as the multichannel operation mode of VANET on Medium Access Control (MAC) layer. One of the problems in guaranteeing the performance of the IEEE 1609.4 is the high vehicular node mobility and different trajectory changes. These cause high delay and low throughput. Services Quality assurance to the IEEE 1609.4 standard can be done using optimizing the synchronization process of CCH and SCH channel intervals so that delay can be reduced and throughput saturation of SCH channel can be increased.
In this dissertation a new model of the Markov chain will be developed which aims to evaluate the performance of dynamic channel coordination system on the IEEE 1609.4 multichannel standard against performance anomalies influences, slot anomalies, the Doppler Effect, Nakagami fading and Additive White Gaussian Noise (AWGN). The performance improvements that is done to the effect above is to use the optimal initial value of Contention Window (CW). This is consistent with previous studies that have been done, the determination of the initial value of Contention Window (CW) will affect the resulting performance of the used Markov chain model. Optimal initial value Contention Window (CW) is obtained from the distribution of nodes in each zone by using the Poisson distribution.
From the simulation and performance evaluation results, it can be concluded that the DCF model in this dissertation can reduce the CCH transmission delay against the propagation channel Nakagami with an average of 16.84%. Moreover, it can be concluded that the performance of the resulting Markov chain model using the optimal initial value obtained CW increases value Aggregate Throughput of 42.8% against the effects of the anomaly performance. Meanwhile, the probability of packet transmission WSA influenced by anomalous slot with the percentage of mean increases approximately 11.35 %. "Penelitian ini bertujuan untuk mengetahui gambaran persalinan operasi sesar pada pasien BPJS Kesehatan di Rumah Sakit X menurut faktor risiko yang mendorong terjadinya persalinan operasi sesar. Penelitian ini menggunakan jenis penelitian kuantitatif dengan metode cross sectional. Pendekatan secara kuantitatif menggunakan data sekunder yang didapatkan dari data rekam medis persalinan pada pasien BPJS Kesehatan di Rumah Sakit X, jumlah sampel sebanyak 240 responden. Faktor risiko pada ibu hamil meliputi umur ibu berisiko (< 21 tahun dan > 35 tahun) sebesar 28,8%, tinggi badan ibu berisiko (≤ 145 cm) sebesar 5,4%, jarak kehamilan berisiko (≤ 24 bulan) sebesar 25,4%, paritas yang berisiko (> 3 persalinan) sebesar 5%, graviditas yang berisiko (> 3 persalinan) sebesar 20,4%, proporsi ibu memiliki riwayat kegagalan kehamilan 22,9%, dan proporsi ibu memiliki riwayat operasi sesar sebesar 51,7%. Faktor risiko pada janin meliputi berat badan bayi berisiko (< 2500 dan > 4000 gram) sebesar 11,7%, proporsi denyut jantung janin tidak normal (< 120 dan > 160 kali/menit) sebesar 3,8%, umur kehamilan berisiko (< 37 dan ≥ 42 minggu) sebesar 5%, letak janin kondisi sungsang sebesar 5,8% dan kondisi lintang sebesar 2,9%. Kemudian, pasien BPJS Kesehatan di Rumah Sakit X yang memiliki faktor komplikasi obstetri 42,9%.

This study aims to determine the description of cesarean delivery in BPJS Health patients at Hospital X according to the risk factors that encourage cesarean delivery. This research uses quantitative research with cross sectional method. The quantitative approach uses secondary data obtained from medical records of childbirth in BPJS Health patients at Hospital X, the number of samples is 240 respondents. Risk factors for pregnant women include the age of the mother at risk (< 21 years and > 35 years) of 28.8%, the height of the mother at risk (≤ 145 cm) of 5.4%, the distance between pregnancy at risk (≤ 24 months) of 25, 4%, parity at risk (> 3 deliveries) by 5%, gravidity at risk (> 3 deliveries) by 20.4%, proportion of mothers having a history of pregnancy failure 22.9%, and proportion of mothers having a history of cesarean section 51, 7%. Risk factors for the fetus include at-risk baby weight (< 2500 and > 4000 grams) of 11.7%, proportion of abnormal fetal heart rate (< 120 and > 160 beats/minute) of 3.8%, gestational age at risk (< 37 and 42 weeks) by 5%, the position of the fetus in the breech condition by 5.8% and the latitude by 2.9%. Then, BPJS Health patients at Hospital X who have obstetric complications factors are 42.9%."

"Jaminan persalinan merupakan jaminan yang digunakan untuk pemeriksaan kehamilan, pertolongan persalinan, pelayanan nifas, pelayanan KB pasca persalinan dan pelayanan bayi baru lahir. Pembiayaannya dijamin oleh pemerintah dan dilaksanakan sejak bulan April 2011. Tujuan penelitian menjelaskan gambaran pelaksanaan program Jampersal di puskesmas di Kabupaten Lombok Timur meliputi input (peserta, informasi, peraturan), proses (pelayanan peserta, administrasi, pembayaran) dan output (gambaran sikap bidan puskesmas terhadap program Jampersal). Metode penelitian adalah kualitatif dengan subjek penelitian bidan puskesmas dan sebagai triangulasi adalah Pengelola Jampersal Puskesmas dan Pengelola Jampersal di Kabupaten. Pengumpulan data melalui wawancara mendalam.
Hasil penelitian menunjukkan bahwa pelaksanaan jaminan persalinan di Kabupaten Lombok Timur terdapat beberapa kendala, berdasarkan input, banyak peserta yang tidak memiliki KTP, sosialisasi masih kurang dan peraturan sering berubah. Berdasarkan proses, pelayanan peserta terkendala dengan kurangnya obat-obatan dan bahan habis pakai, bidan terbebani dengan administrasi yang terlalu banyak, pembayaran klaim sering terlambat dan banyak potongan. Bidan setuju dan mendukung program Jampersal namun perlu perbaikan dalam pelaksanaannya di masa datang. Dari hasil penelitian ini diharapkan Pengelola Jampersal bisa lebih komunikatif agar tidak terjadi salah interpretasi dalam pelaksanaan jampersal.

Childbirth assurance is an assurance used for pregnancy check up, childbirth aid, parturition service, post childbirth (family birth planning), and newborn service. The financing is guaranteed by government since april 2011. The purpose of this research is to describe the implementation of jampersal program in public health centre in east lombok which includes input (participants, information, regulation), process (service for participants, administration, payment) and output (description of midwives response to jampersal program). The research method is qualitatif with midwives as research subject, and to be the triangulation is the administrator of jampersal program in the regency. The data collecting is through deep interviews.
The research result shows that the implementation of the program in East Lombok faces some problems. Based on the input, lots participants do not have identity card, lack of socialization, and the regulation often changes. From process side, service for participants is found lack of medicine and disposable materials. Midwives are overburdened with too many administrations, and late payment with a lot of cuts. Midwives agree and support for jampersal program, but the program itself needs improvement in its implementation for the future. The research results expects that the jampersal administrator needs to be more communicative, in order to avoid misinterpretation in jampersal implementation.

"Program Jaminan Persalinan merupakan suatu terobosan untuk menurunkan Angka Kematian Ibu (AKI) dan Angka Kematian Bayi (AKB) sehingga dapat mempercepat capaian target Millenium Development Goals (MDGs). Penelitian ini bertujuan untuk menganalisa implementasi kebijakan program Jampersal di Kota Bekasi. Metode penelitian yang digunakan adalah pendekatan kualitatif dan pengumpulan data menggunakan wawancara mendalam dan telaah dokumen. Hasil penelitian menunjukan bahwa pelaksanan program belum berjalan optimal sebagaimana yang diharapkan, sehingga menyebabkan cakupan masih rendah. Agar implementasi program Jampersal dapat berjalan dengan optimal diperlukan koordinasi Dinas Kesehatan Kota Bekasi dengan Pemerintah Daerah dan lintas sektor terkait seperti Ikatan Bidan Indonesia (IBI). Selain itu perlu melakukan monitoring dan evaluasi rutin ke RSUD Kota Bekasi, Puskesmas dan Bidan Praktik Mandiri.

Delivery Security Program is a breakthrough to reduce Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) in order to accelerate the achievement of the Millennium Development Goals (MDGs). This study aims to analyze the implementation of program policies Delivery Security in Bekasi. The research method used was a qualitative approach and data collection using in-depth interviews and document review. The results showed that the optimal conduct of the program has not run as expected, resulting in coverage is still low. Delivery Security program implementation in order to run optimally required coordination with the City Health Office Bekasi local government and across relevant sectors such as the Indonesian Midwives Association (IBI). In addition to the need to perform regular monitoring and evaluation to Bekasi City Hospital, health centers and midwives Independent Practice."
