"ABSTRAK
Praktek kerja profesi apoteker di rumah sakit pusat angkatan darat (rspad) Gatot soebroto
dilakukan selama periode September hingga oktober 2015. Praktek kerja ini bertujuan untuk
memahami peran dan tanggung jawab apoteker di rumah sakit sebagai bagian dari upaya dalam
meningkatkan pengetahuan, keterampilan, dan kemampuan calon apoteker sehingga nantinya
dapat menerapkan pelayanan kefarmasian di rumah sakit sesuai ketentuan yang berlaku serta
mampu berinteraksi langsung dengan pasien maupun bekerja sama dengan profesi kesehatan
lainnya. Tugas khusus yang diberikan pada praktek kerja profesi adalah analisa masalah terkait
obat pada pasien selulitis di ruang perawatan umum lantai V RSPAD Gatot Soebroto. Pemberian
tugas khusus bertujuan untuk melakukan pemantauan terapi obat pada pasien sehingga mampu
menganalisa masalah terkait obat yang diberikan pada pasien selulitis dan mampu memberikan
intervensi dan saran pengobatan terhadap terapi obat yang diberikan. Analisa dilakukan dengan
cara mengkaji data yang diperoleh dari hasil observasi langsung terhadap pasien, catatan
perkembanagan pasien terintegrasi (CPPT), hasil wawancara dengan pihak-pihak terkait serta
penelusuran literatur terkait. Analisa masalah dilakukan dengan menggunakan metode klasifikasi
menurut PCNE (Pharmaceutical Care Network Europe Foundation) V 6.2 2003-2010 dengan
temuan masalah pada domain penyebab : p1.1, p1.4, p5.1 dan p8.1. Intervensi yang dilakukan
selanjutnya sesuai klasifikasi PCNE adalah i1.2, i1.3, i2.1, i2.2, i2.4, i3.4, i3.5 dan i3.6. ABSTRACT Profession Internship at Indonesia Army Central Hospital (RSPAD) Gatot Soebroto had been held
on the periode of September-Oktober 2015. The internship aims to learn and understand the role
and responsibilities of pharmacists in hospital as part of efforts to enhance knowledge , skills , and
the prospective pharmacists which in turn can apply the pharmacy service in hospital based on
regulation and able to interact directly with patients and cooperate with other health professional.
The special assignment given to analysis drug related problem (DRP) in patients cellulitis in a
medical room PU floor V, Indonesia Army Central Hospital Gatot Soebroto. The special
assignments aims to monitor drug therapy in patients order to be able to analyze the drug related
problem being provided in patients cellulitis and able to provide intervention and treatment
suggestions.The analysis done by means of studying the data obtained from the results of direct
observation towards patients, integrated patient development note (CPPT) , interviewing the
parties involved and searching for the related literature. The analysis was conducted using the
classification methods according to PCNE (pharmaceutical care network europe foundation) v 6.2
2003-2010 with the findings problems in domain cause: p1.1, p1.4, p5.1 and p8.1.then the
interventions done in accordance PCNE classifications are i1.2, i1.3, i2.1, i2.2, i2.4, i3.4, i3.5 and
i3.6. "

Praktek kerja profesi PKP di Rumah Sakit Pusat Angkatan Darat RSPAD Gatot Soebroto Ditkesad bertujuan untuk memahami peranan tugas dan tanggung jawab Apoteker di Rumah Sakit sesuai dengan ketentuan dan etika pelayanan kefarmasian pada khususnya dan pelayanan kesehatan pada umumnya memiliki wawasan pengetahuan keterampilan dan pengalaman praktis untuk melakukan praktek kefarmasian di Rumah Sakit serta memiliki gambaran nyata tentang permasalahan praktek kefarmasian terutama dalam berinteraksi baik dengan pasien maupun dengan tenaga kesehatan lain Berdasarkan kegiatan PKP di RSPAD pengelolaan sediaan farmasi alat kesehatan dan bahan medis habis pakai dilaksanakan di instalasi farmasi rumah sakit dengan sistem satu pintu Pelayanan farmasi klinis masih belum berjalan optimal akibat kurangnya tenaga Apoteker yang berfokus pada pelayanan farmasi klinik Pemantauan kadar obat dalam darah adalah pelayanan farmasi klinis yang belum dilaksanakan Peran Apoteker di RSPAD perlu ditingkatkan melalui kerjasama dan komunikasi dengan tenaga kesehatan lain Kegiatan PKP di RSPAD berlangsung selama dua bulan dari tanggal 1 September sampai dengan tanggal 31 Oktober 2015 dengan tugas khusus yaitu analisa masalah terkait obat terhadap pasien neonatus kurang bulan kurang masa kehamilan dan bayi berat lahir rendah dengan sepsis neonatorum di instalasi rawat inap perinatal resiko tinggi RSPAD Gatot Soebroto Dikesad periode 21 ndash 27 Oktober 2015 ABSTRACT Profession internship at the Gatot Subroto Army Central Hospital was itended to make apotechary students understand the roles and responsibilities of Hospital Pharmacists in accordance with the rules and ethics of pharmaceutical care in particular and healthcare in general have the insight knowledge skills and practical experience of pharmaceutical practice and know the issues of pharmaceutical care in Hospital especially in their interaction with the patients and with other health professionals Based on the activities management of pharmaceutical and medical devices in Gatot Subroto Army Central Hospital is responsibility of hospital pharmacy by one door system Clinical pharmacy services is still not optimal due to the lack of the number of pharmacists that focus on clinical pharmacy services Monitoring of drug levels in the blood is a clinical pharmacy services that have not been implemented here Role of Pharmacists needs to be improved through cooperation and communication with other health professionals Profession internship at the Gatot Subroto Army Central Hospital was held for two months from September 1st until October 31st 2015 in a special assignment that is identification of drug related problems of neonates with less gestation low birth weight and neonatal sepsis in high risk perinatal unit of Gatot Subroto Army Central Hospital on period October 21st until October 27th 2015 "

Rumah sakit merupakan salah satu tempat dilakukannya pelayanan medis atau kesehatan bagi
masyarakat. RSPAD Gatot Soebroto merupakan rumah sakit tipe A Pendidikan yang merupakan
rumah sakit rujukan dari rumah sakit dibawahnya. Salah satu faktor yang sangat mempengaruhi
mutu dari rumah sakit adalah pelayanan kefarmasian. Praktek Kerja Profesi Apoteker yang
dilakukan di Rumah Sakit Umum Pusat Fatmawati ditujukan untuk memahami tugas dan fungsi
apoteker dalam melaksanakan Pelayanan Kefarmasian di Rumah Sakit yang meliputi pengelolaan
Sediaan Farmasi dan Bahan Medis Habis Pakai, serta pelayanan farmasi klinik. Untuk melakukan
pelayanan kefarmasian di Rumah Sakit, khususnya dalam bidang farmasi klinis maka Apoteker
perlu untuk terus mengikuti perkembangan pengetahuan serta peraturan yang berlaku. Strategi
pengembangan praktek kefarmasian di RSPAD Gatot Soebroto adalah dengan melakukannya
kegiatan penyuluhan rutin terkait obat untuk para tenaga kefarmasian maupun tenaga kesehatan
lainnya.

Telah dilakukan Praktek Kerja Profesi Apoteker (PKPA) di Rumah Sakit Pusat
Angkatan Darat Gatot Soebrot (RSPAD Gatsoe) Ditkesad. Kegiatan PKPA ini
berlangsung selama dua bulan dari tanggal 1 September sampai dengan tanggal 30
Oktober 2015. Tujuan dari kegiatan PKPA ini adalah agar mahasiswa Apoteker
mampu memahami peranan, tugas, dan tanggung jawab Apoteker di Rumah Sakit
sesuai dengan ketentuan dan etika pelayanan farmasi khususnya dan pelayanan
kesehataan umumnya dan agar mahasiswa Apoteker memiliki wawasan,
pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan praktek
kefarmasian di Rumah Sakit. Berdasarkan kegiatan PKPA yang dilakukan,
diketahui bahwa Apoteker di RSPAD Gatsoe Ditkesad bertanggung jawab atas
kegiatan pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai
dan pelayanan farmasi klinik. Pelayanan kefarmasian di Puskesmas Kecamatan
Kembangan mangacu pada Permenkes No. 58 Tahun 2014 tentang standar

"ABSTRAK
Praktek Kerja Profesi di Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Ditkesad dilaksanakan dengan tujuan untuk mengetahui dan memahami peran dan tanggung jawab apoteker sebagai pengelola instalasi farmasi, baik dalam bidang manajemen instalasi farmasi maupun dalam hal pelayanan farmasi klinik. Peran apoteker dalam manajemen instalasi farmasi yaitu pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai meliputi pemilihan, perencanaan, pengadaan, penerimaan, penyimpanan, pendistribusian, pemusnahan, pengendalian, pencatatan dan pelaporan. Sedangkan pelayanan farmasi klinik yang telah dilakukan yaitu pengkajian dan pelayanan resep, penelusuran riwayat penggunaan obat, rekonsiliasi obat, pelayanan informasi obat, konseling, visite, pemantauan terapi obat, monitoring efek samping obat, dan dispensing sediaan steril. Tugas khusus yang diberikan adalah analisis drug related problem pada penderita rawat inap dengan diagnosa fraktur mandibula sinistra dengan riwayat HIV/AIDS dan gangguan mental organik akibat toksoplasmosis di lantai 4 perawatan umum Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Ditkesad untuk dapat memberikan rekomendasi terkait terapi pasien kepada tenaga kesehatan terkait.ABSTRACT Profession Internship at Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Ditkesad was held to learn and understand the role and responsibility of pharmacist as a pharmacy manager, especially to manage a pharmacy or to deliver pharmaceutical care. "Rumah Sakit adalah institusi pelayanan kesehatan yang menyelenggarakan pelayanan kesehatan perorangan secara paripurna yang menyediakan pelayanan rawat inap, rawat jalan, dan gawat darurat. Tujuan Praktek Kerja Profesi Apoteker adalah agar mahasiswa memahami peranan, tugas, dan tanggung-jawab apoteker di rumah sakit sesuai dengan ketentuan dan etika pelayanan kefarmasian. Pelayanan Kefarmasian di RSPAD Gatot Soebroto Ditkesad mengacu kepada PMK No. 58 tahun 2014 tentang Standar Pelayanan Kefarmasian di Rumah Sakit. Kegiatan pelayanan farmasi klinik yang sudah terlaksana meliputi pengkajian dan pelayanan resep; penelusuran riwayat penggunaan; Konseling; PIO; rekonsiliasi obat; dispensing sediaan steril; Evaluasi penggunaan obat, Pemantauan Terapi Obat dan monitoring efek samping obat. Tugas khusus yang dikerjakan yaitu Analisis Masalah Terkait Obat pada Pasien Prolonged Fever e.c Tuberkulosis Paru di Ruang Perawatan Anak Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Ditkesad.

Pharmacist internship program was established to provide pharmacy students understanding their roles, duties and responsibilities of a pharmacist accordance to pharmaceutical law and code of ethics. Pharmaceutical Services at Central army Hospital Gatot Soebroto Ditkesad refers to Regulation of Minister of Health No. 58 year 2014 about Standardization of Pharmaceutical Services at the Hospital. Clinical pharmacy Service which has been done include assessing prescriptions medical record Counseling PIO medication reconciliation aseptic dispensing Evaluation of drugs use, Drug Therapy Monitoring and monitoring of drug side effects. A special assignment given to author was analysis Drug Related Problems in Patients with Prolonged Fever e.c Tuberculosis in Children 39 s inpatient installation Central Army Hospital Gatot Soebroto Ditkesad."
