"Karsinoma kolorektal adalah salah satu masalah kesehatan yang sering terjadi di daerah perkotaan, karena masyarakat perkotaan cenderung beresiko lebih besar untuk mengalami karsinoma kolorektal yang berhubungan dengan usia, diet, dan gaya hidup modern. Jenis karsinoma yang paling sering dialami adalah adenokarsinoma rekti. Klien dengan adenokarsinoma rekti membutuhkan tindakan pembedahan kolostomi karena perlunya dibuat stoma sebagai pengganti fungsi anus. Klien dengan stoma beresiko mengalami komplikasi jika tidak dilakukan perawatan stoma dan perisromal dengan baik. Salah satu cara terbaik untuk pencegahan dini komplikasi adalah memberikan edukasi yang tepat pada klien dan keluarga mengenai perawatan stoma. Dengan demikian, tujuan penulisan ini adalah untuk melakukan analisis evidence based mengenai edukasi perawatan stoma dalam mencegah komplikasi gangguan integritas kulit peristomal. Hasil dari pemberian edukasi perawatan stoma pada pasien ini terbukti efektif dalam mencegah terjadinya komplikasi. Rekomendasi penulisan ini adalah agar perawat sangat perlu memberikan edukasi perawatan stoma sebagai upaya pencegahan dini komplikasi pasca pembedahan kolostomi.

Colorectal carcinoma is one of the health problems that often occur in urban areas, because urban societies tend to be at greater risk for experiencing colorectal carcinoma associated with age, diet, and modern lifestyle. The most often experienced type of colorectal carcinoma is rectal adenocarcinoma. Clients with rectal adenocarcinoma require colostomy to create stoma that will be functioned like anus. Clients with a stoma have a risk of complications if the stoma or peristomal skin is not treated properly.

One of the best ways for early prevention of complications is to provide appropriate stoma care education to clients and their relatives. Thus, the purpose of this paper is to analyze the evidence based on stoma care education in preventing peristomal skin complications.

The result of stoma care education is effective in preventing the occurrence of complications. Then, recommendation of this paper is that it is necessary for nurses to educate clients about their stoma care as early prevention of post surgical complications colostomy."

"Atresia ani merupakan malformasi kongenital pada anorektum. Klien dengan atresia ani biasanya dilakukan kolostomi. Kolostomi seringkali mengalami iritasi pada kulit peristomal. Penulisan Karya Ilmiah Akhir Ners ini bertujuan untuk memberikan gambaran asuhan keperawatan pada klien dengan atresia ani dengan kolostomi yang memiliki kondisi iritasi pada kulit peristomal. Salah satu intervensi untuk menghindari komplikasi lebih lanjut yaitu dengan perawatan stoma. Perawatan stoma dilakukan dengan cara menilai iritasi kulit dengan ostomy skin tools, melakukan intervensi, dan pemantauan kondisi kulit. Setelah dilakukan perawatan stoma selama 3 hari ditemukan hasil berkurangnya kemerahan pada kulit dan hilangnya iritasi dari tiga lokasi menjadi hanya satu lokasi iritasi. Hasil karya ilmiah ini merekomendasikan pihak rumah sakit untuk mengembangkan asuhan keperawatan khususnya pada klien dengan kerusakan integritas kulit untuk meningkatkan kualitas pelayanan.

Atresia ani was a congenital malformation in anorectum. Clients with atresia ani usually had colostomy procedure. Colostomy often experienced for peristomal skin irritation. The purpose of this study was provided an overview of nursing care to clients with atresia ani who had peristomal skin irritation. One intervention to avoid further complications was by stoma care. Stoma care was done by assessing skin irritation with ostomy skin tools, interventions, and monitoring the condition of the skin. After stoma care for 3 days found resulted in redness less and reduce irritation location from three locations into one location. From the result of this study recommend the hospital need to develop a nursing care on clients with skin integrity damage to improve the quality of service."

"Kanker rektum merupakan jenis kanker yang sering terjadi di masyarakat saat ini. Saat individu terdiagnosis kanker rektum, eliminasi fekal dapat terganggu sehingga diperlukan alternatif untuk eliminasi berupa pembuatan kolostomi. Tindakan kolostomi merupakan pengalaman baru bagi pasien sehingga pasien membutuhkan dukungan untuk penyesuaian diri bahkan sejak awal perawatan. Tujuan penulisan ialah menganalisis pelaksanaan edukasi perencanaan pulang yang dilakukan oleh perawat pada pasien dengan rencana pembuatan kolostomi. Perencanaan pulang telah dilakukan dalam bentuk edukasi dengan beragam strategi edukasi dan berlangsung pada periode pre-operatif dan periode post-operatif. Evaluasi pelaksanaan edukasi pada periode pre-operatif ialah pasien memahami kondisinya, siap menjalani prosedur pembedahan, serta mengetahui kondisinya setelah operasi. Evaluasi edukasi pada periode post-operatif ialah pasien memahami praktik perawatan stoma serta bentuk penyesuaian aktivitas sehari-hari dengan kolostomi. Rekomendasi penulisan perlunya pelaksanaan edukasi perencanaan pulang pada pasien rencana pembuatan stoma sejak awal perawatan dengan materi yang terstruktur dan strategi yang beragam.

Rectal cancer is a common type of cancer in today's society. Patient with rectal cancer has impaired elimination and need another alternative to maintain elimination function by formatting a colostomy. Colostomy is a new experience for patients so that they need support for adjustment from the beginning of treatment. The purpose of writing is to analyze the implementation of discharge planning education for patient with colostomy surgery schedule. Discharge planning education is done with several strategies and from the preoperative until postoperative period.
