"ABSTRAKPengisian rekam medik merupakan kegiatan yang sangat penting untuk memberikan pelayanan yang baik kepada pasien dan data rekam medik sangat diperlukan untuk kepentingan manajemen rumah sakit, pasien, dan petugas kesehatan sendiri. Pengelolaan rekam medik yang dilakukan secara seksama dan lebih profesional merupakan salah satu pelayanan yang dapat menunjang pemberian pelayanan medik yang cepat, tepat, dan akurat. Tujuan penelitian ini adalah untuk mengetahui faktor-faktor yang mempengaruhi perilaku dokter spesialis surgical dalam kelengkapan pengisian resume medik pasien rawat inap di RSUP Fatmawati tahun 2014. Penelitian ini merupakan jenis penelitian analitik dengan pendekatan kuantitatif dan kualitatif. Sedangkan desain penelitiannya adalah cross sectional. Sampel pada penelitian ini adalah total sampel 73 orang dokter spesialis bedah surgical ( THT, mata, bedah, orthopedi, kebidanan ). Analisis menggunakan uji Chi-Square dan regresi logistik berganda.Hasil penelitian ada hubungan yang signifikan antara monitoring, beban kerja, pengetahuan, variabel pelatihan dan kompensasi dengan perilaku dokter spesialis pegisian resume medik di RSUP Fatmawati. Variabel kompensasi merupakan variabel paling dominan memiliki hubungan secara signifikan dengan perilaku dokter spesialis surgical dalam pengisian resume medik di RSUP Fatmawati. Pihak manajemen perlu meningkatkan pengawasan (monitoring) terhadap pengisian resume medis bekerjasama dengan tim komite medik. Kendala-kendala yang ditemukan dalam pengisian resume medis dibahas dan diselesaikan bersama dengan melibatkan seluruh dokter.

ABSTRACT

Completion of medical records is an activity that is very important to provide good service top patient and medical records management is indispensable for the benefit of hospitals, patients, and health workers themselves. Management of medical records that done carefully and professionally is one of the care that can support the provision of medial services that is fast, precise, and accurate. The purpose of this study was to determine the factors that influence the behavior of the completeness of inpatient discharge summaries in Fatmawati Hospital in 2014. This research is analytic study with quantitative and qualitative approaches. While the sampla of 73 surgical surgeons (ENT, eye, surgery, orthopaedic, obstetrics). Analysis using Chi-square test and multiple logistic regression.

The results of the study there was a significant correlation between monitoring, workload, knowledge, training and compensation variables with the completion behavior of discharge summary by surgeons in Fatmawati Hospital. Variable compensation is the most dominant variable has a significant correlation with the behavior of surgical specialists in medical resume completion. The management needs to improve supervision the completion of medical resume working together with a team of medical committee. Barriers found in medical resume completion discussed and solved together by all the doctor."

"Setiap rumah sakit harus menerapkan sistem keselamatan pasien yang mana salah satunya adalah ketepatan prosedur obat dan operasi dengan melakukan pengkajian awal pasien, termasuk RSUP Fatmawati. Penelitian ini bertujuan untuk menganalisis kelengkapan pengisian pengkajian awal dokter pada pasien rawat inap di RSUP Fatmawati Tahun 2015. Jenis penelitian ini adalah kuantitatif dan kualitatif dengan metode observasi dan wawancara mendalam. Hasil penelitian didapat rata-rata kelengkapan pengkajian awal dokter pasien rawat inap sebesar 65%. Banyaknya kolom yang harus diisi, alokasi waktu dokter untuk pelayanan pasien rawat inap, dan feedback langsung dari pihak manajemen kepada dokter yang belum optimal dianggap memengaruhi kelengkapan pengisian pengkajian awal dokter pasien rawat inap RSUP Fatmawati.

Every hospital should implementing patient safety which is one of them is ensure correct- site, correct-procedur, correct-patient surgery by doing initial patient asessment. The aim of this research is to analyze the physician's early assessment completeness of inpatients of RSUP Fatmawati in 2015. This research use both quantitative and qualitative by using observation and in-depth interview method. The result of this research found that the average of physician‟s early assessment completeness of inpatients is 65%. The number of required fields, doctor‟s time allocation for inpatients services, and management's feedback to the physicians had not been optimal were considered affecting the physician‟s early assessment completeness of inpatients of RSUP Fatmawati."

"ABSTRACTInfeksi luka operasi merupakan infeksi yang terjadi 30 hari pasca operasi. Angka kejadian ILO di RSUP Fatmawati dari tahun 2014-2016 mengalami peningkatan dari 0,74 menjadi 1,24. Tahun 2016 0,3 dari total angka kejadian ILO berasal dari pasien kebidanan dan kandungan. Tujuan dari penelitian ini adalah merumuskan upaya perbaikan pencegahan kejadian infeksi luka operasi sehingga diharapkan dapat mengurangi kejadian infeksi luka operasi di RSUP Fatmawati. Penelitian ini merupakan jenis penelitian operasional yang bersifat kuantitatif dan kualitatif. Metode yang digunakan adalah telaah data sekunder, wawancara mendalam, observasi dan telaah dokumen. Hasil penelitian menunjukkan bahwa ILO masih terjadi di RSUP Fatmawati walaupun rumah sakit sudah mempunyai SOP terkait ILO untuk melakukan pencegahan dan pengendalian ILO, karena ILO bisa terjadi dari sisi kondisi pasien dan sistem pelayanan rumah sakit. Maka dari itu diperlukan kerjasama dan komitmen antar seluruh pihak yang berkaitan dengan pencegahan dan pengendalian ILO, yakni Instalasi Rawat Inap Teratai, Instalasi Bedah Sentral, KPPI dan Komite Mutu serta diperlukan penelitian lanjutan untuk membahas faktor lain yang tidak terdapat dalam penelitian ini.

ABSTRACTSurgical site infections is infection that occurs within 30 days after the operation. The incidence rate of SSI at RSUP Fatmawati has increased from 0.74 to 1.24 in 2014 2016. In 2016, there is 0.3 of total incidence SSI that came obstetric and gynaecological patients. The aim of this study is formulate improved preventive incidence of SSIs so that can reduce incidence of SSIs at RSUP Fatmawati. This research is an operational research, which use quantitative and qualitative approach. The method are secondary data review, observation, interviews, and documents review. The results showed that there is still incidence of SSIs at RSUP Fatmawati despite having Standard Operating Procedure SOP for prevention and controlling of SSIs, because SSIs caused of patients condition and hospital service system. Therefore it is necessary to do cooperation and commitment among people concerned in prevention and controlling of SSIs, i.e., Central Surgical Installation and Teratai rsquo s Inpatient Installation, IPCN, and Quality Committe, and further research is needed to discuss other factors not used in this study."

"Penelitian ini membahas kepatuhan dokter dalam pengisian formulir instruksi medis farmakologis di Rawat Inap RSUP Fatmawati tahun 2017. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang mempengarui tingkat kepatuhan dokter dalam pengisian formulir instruksi medis farmakologis, menggunakan metode penelitian kuantitatif dengan pendekatan cross sectional. Data diperoleh menggunakan bantuan kuesioner dan melakukan telaah dokumen terhadap formulir instruksi medis pada rekam medis pasien pulang rawat inap dengan menggunakan daftar tilik. Hasil uji statistik chi square didapat nilai alpha p 0.05 artinya tidak memiliki hubungan yang signifikan dengan kepatuhan dokter dalam mengisi formulir instruksi medis farmakologis.

This research discusses about doctoral compliance on filling Medical Instructions Pharmacological Inpatient Form RSUP Fatmawati 2017. This research aimed to determine the factors that affect the level of doctoral compliance on filling medical instructions pharmacological form used quantitative research method with cross sectional approach. Data obtained using questionnaire support and conducted documents review of Medical Instructions Pharmacological Form on medical record of patient who go home after inpatient by using checklist. Based on the result of statics test chi square obtained alpha value p p 0.05 , it means no significant relationship with doctoral compliance on filling medical instructions form."

"Rumah sakit sebagai penyedia jasa pelayanan kesehatan harus terus meningkatkan mutu dari pelayanan kesehatannya salah satunya dapat dilihat dari mutu rekam medis. Mutu rekam medis yang baik dapat dilihat dari kelengkapan pengisian berkas rekam medis. Tujuan dari penelitian ini adalah untuk mengetahui gambaran kelengkapan pengisian berkas rekam medis pasien rawat inap di RSAU dr.Esnawan Antariksa Tahun 2014. Penelitian ini bersifat deskriptif observasional dengan pendekatan kuantitatif dan kualitatif. Sampel penelitian ini adalah berkas rekam medis pasien rawat inap bulan Januari dan Februari tahun 2014 yang telah dikembalikan ke Bagian Rekam medis sebanyak 90 berkas rekam medis. Teknik sampling yang digunakan dengan sampel acak sistematis. Hasil penelitian menunjukkan bahwa rata - rata kelengkapan pengisian rekam medis sudah cukup lengkap namun masih kurang dari standar kelengkapan yang ditetapkan Depkes RI sebesar 100%. Disamping itu, faktor pendukung kelengkapan pengisian rekam medis yang terdiri dari sumber daya manusia, sarana dan prasarana, bahan, metode dan biaya masih kurang mendukung kegiatan kelengkapan pengisian berkas rekam medis pasien rawat inap. Oleh karena itu, untuk mendapatkan angka kelengkapan pengisian rekam medis 100% sesuai dengan standar Depkes maka perlu dilakukan pengembangan kemampuan tenaga medis dan paramedis dengan pengadaan pelatihan mengenai kelengkapan pengisian rekam medis, penambahan jumlah petugas rekam medis, sosialisasi SOP kelengkapan pengisian rekam medis pemberian sanksi dan reward serta pengembangan teknologi rekam medis yang terkomputerisasi untuk meningkatkan mutu dari rekam medis pasien rawat inap.

Hospitals as health care providers should continue to improve the quality of health care one of which can be seen from the quality of medical records. Good quality of medical record can be seen from filling completeness of medical record document. The purpose of this study is to describe the completeness filling of the inpatients medical record document in RSAU dr.Esnawan Antariksa of 2014. This is a descriptive observational study with quantitative and qualitative approaches. The sample was medical record document of inpatients in January and February of 2014 that has been restored a total of 90 medical records. The sampling technique used by systematic random sampling.

The results showed that the averages of completeness filling inpatients medical records are quite complete, but still less than the standard set by Ministry of Health Republic of Indonesia completeness of 100%. In addition, the supporting factors of completeness filling inpatients medical records consisting of human resources, facilities, materials, methods and costs are still lacking. Therefore, to obtain medical record numbers 100% of filling completeness in accordance with the standards of the Department of Health is necessary for the development of medical and paramedical personnel capability with procurement training on completeness filling of medical records, increasing the number of medical records clerk, socialization SOP completeness filling of medical records, sanctions and reward as well as the development of a computerized medical record technology to improve the quality of inpatients medical record."

"[ABSTRAKTesis ini membahas analisis kelengkapan rekam medis rawat inap RSKO Jakartatahun 2014. Penelitian ini adalah penelitian mixed method yaitu penelitian secarakuantitatif dan kualitatif. Penelitian kualitatif digunakan untunk mengetahui nilaikelengkapan rekam medis, dan penelitian kualitatif untuk menggali informasiterhadap input, proses, dan output. "Tesis ini membahas tentang kelengkapan pengisian resume medis (diagnosis utama, diagnosis sekunder, prosedur utama) terhadap kesesuaian standar Tarif INA-CBGs di Instalasi rawat inap Teratai Rumah Sakit Umum Pusat Fatmawati. Penelitian menggunakan pendekatan kuantitatif dan kualitatif dengan desain studi cross sectional.Hasil penelitian masih ditemukannya ketidaklengkapan pengisian resume medis terkait variabel diagnosis utama, diagnosis sekunder, dan prosedur utama, sehingga menyebabkan potensi ketidaksesuaian standar tarif INA-CBGs. This research discussed on the completeness of medical resume (primary diagnostic, secondary diagnostic and major procedure) in consistency with INACBGs costing at Teratai Inpatient Instalation Central General Hospital (RSUP). This research used mix methods approach with cross sectional design.

This research found that there is still incompleteness in filling medical records especially for primary diagnostic, secondary diagnostic and major procedure that potentially may cause inconsistency with INA-CBSs costing. The incompleteness were caused by many factors, and this research suggest to conduct evaluation and socialization of the Standard Procedure Operational (SPO), the implementation of reward and punishment system, monitoring and evaluation on medical record forms, addition of book or list of diagnostic code, upgrading of INA-CBGs software."

"Penelitian ini membahas kelengkapan resume medis rawat inap di RS Umum Hermina Depok. Tujuan penelitian ini adalah mendapatkan gambaran hasil analisis kelengkapan resume medis rawat inap. mendapatkan gambaran hasil analisis kelengkapan resume medis rawat inap. Jenis penelitian ini menggunakan kuantitatif dengan desain studi cross sectonal dan kualitatif dengan wawancara mendalam dan terstruktur menggunakan data primer dan data sekunder. Hasil penelitian ini adalah kurangnya pengetahuan, kurang sesuainya sanksi dan insentif, instrumen yang penting, format instrument kolom, huruf, perlengkapan, uji coba, revisi, sosialisasi, kelengkapan, ketidaklengkapan dan dokter penanggung jawab resume medis rawat inap.. Hasil penelitian didapatkan rata-rata resume medis rawat inap lengkap sebesar 84,6% dan yang tidak lengkap sebesar 15,4%. Hal ini menunjukkan bahwa standar kelengkapan resume medis rawat inap masih belum sesuai RS Umum Hermina Depok sebesar ≥ 95%. Kurangnya sosialisasi terkait peraturan resume medis kepada para dokter merupakan salah satu hambatan dalam pengisian resume medis rawat inap masih belum lengkap.

This research discuss about the completeness of Inpatient Medical Resume at Hermina Depok Hospital. The purpose of this research is to get the illustration of analysis the completeness of inpatient medical resume. The research uses quantitative method with cross sectional design and qualitative by in-depth interview and structural interview by using both primer and secondary data. The result of this research are less knowledge, inappropriate sanction and incentive, important items. Column instrument format, character, tools, trial test, revision, socialization, completeness, incompleteness, and physician responsible due to inpatient medical resume. Concludes that the average of completeness inpatient medical resume is about 84,6% and discompleteness medical resume is about 15,4%. It shows that the completeness of inpatient medical resume yet with the standard of Hermina Hospital Depok which is about >95%. The minimum socialization about the rule of medical resume to the physician become one of obstacle when filling inpatient medical resume still not complete."

"RSUP Fatmawati sebagai rumah sakit pendidikan kelas A dituntut untuk memiliki kualitas dokumentasi yang baik. Penelitian ini bertujuan untuk mengetahui gambaran kelengkapan sisi dokumen rekam medis pasien rawat inap kebidanan November-Desember 2014. Jenis penelitian ini adalah kuantitatif dan kualitatif dengan metode observasi dan wawancara mendalam.Hasil penelitian didapatkan rata-rata rekam medis pasien rawat inap kebidanan yang masuk dalam kategori lengkap sebesar 5%. Rata-rata kelengkapannya adalah 85,67% dan masuk dalam kriteria baik. Kompleksnya variabel dalam formulir rekam medis, dukungan sarana prasarana masih terdapat kekurangan serta SDM yang kurang juga menjadi hambatan dalam pengisian rekam medis secara lengkap.

Fatmawati Hospital as educational hospital type A required to have a good quality in patient documentation. The objective of this research is to identify the completeness of obstetric inpatient medical record in Fatmawati Hospital November-December 2014. This research use both quantitative and qualitative method by using observation and in-depth interview.

The result of this research found that average of obstetric inpatient medical record that included in complete category was 5%. About 85,67% complete medical record can be said in to good criteria. Medical record's variabels are too complex, lack of infrastructure support, and lack of human resources."

Jenis penelitian ini adalah kuantitatif dan kualitatif dengan metode observasi dan wawancara mendalam. Hasil penelitian didapatkan rata-rata rekam medis pasien rawat inap adalah 57,8%. Tidak adanya pelatihan pengisian rekam medis merupakan salah satu hambatan dalam pengisian rekam medis secara lengkap.
