"Latar belakang: Penyakit Kawasaki adalah penyebab utama penyakit jantung didapat pada anak, yang merupakan suatu vaskulitis sistemik akut. Penyakit ini berhubungan dengan luaran aneurisme arteri koroner, yang dapat dicegah dengan pemberian imunoglobulin intravena (IGIV). Terapi baku emas pada penyakit Kawasaki adalah IGIV dosis tinggi (2 g/kgBB). Namun, IGIV dosis medium (1 g/kgBB) merupakan terapi berbiaya lebih rendah dan mungkin memiliki efikasi yang sama. Melalui penelitian ini, kami mengevaluasi keberhasilan terapi IGIV dosis 1 g/kgBB.
Metode: Studi kohort retrospektif multisenter dari data rekam medis dengan total 507 pasien dengan penyakit Kawasaki komplit. Penelitian dilakukan di Rumah Sakit Umum Pusat Nasional Cipto Mangunkusumo dan Kawasaki Center, Indonesia dari Januari 2012 hingga Januari 2022. Pasien yang mendapatkan terapi IGIV dengan dosis 1 g/kgBB didefinisikan sebagai grup A, dan pasien yang mendapatkan terapi IGIV dengan dosis 2 g/kgBB didefinisikan sebagai grup B. Karakteristik dasar subjek dibandingkan antar-kelompok tersebut; demografi, hasil laboratorium, keterlibatan mukokutan, hari demam saat diberikan IGIV, durasi demam pasca-IGIV, lama rawat, dan temuan aneurisme arteri koroner dari ekokardiografi pada periode follow-up.
Hasil: Sebanyak 24 pasien (grup A), mendapatkan IGIV dosis medium (1 g/kgBB). Sementara itu, sebanyak 483 pasien (grup B) mendapatkan IGIV dosis tinggi (2 g/kgBB). Distribusi usia dan jenis kelamin, nilai leukosit dan trombosit, hari demam saat diberikan IGIV, durasi demam pasca-IGIV, dan lama rawat tidak berbeda bermakna antar-kelompok (p >0,05). Semua pasien mengalami keterlibatan mukokutan. Berat badan menurut umur (WAZ) dan nilai CRP antar-kelompok bermakna secara statistik (p <0,05). Aneurisme arteri koroner tidak ditemukan pada pasien di grup A dan pada 9 pasien (1,9%) di grup B pada periode follow-up (p >0,05).
Simpulan: Terapi dengan dosis inisial IGIV 1 g/kgBB untuk pasien dengan penyakit Kawasaki menunjukkan keberhasilan yang sama dengan IGIV dosis tinggi (2 g/kgBB). Hal ini dapat menjadi opsi terapi bagi negara berkembang.

Background: Kawasaki disease (KD), the leading cause of acquired heart disease in children, is an acute childhood systemic vasculitis. It is associated with coronary artery aneurysms (CAA), that could be prevented by intravenous immunoglobulin (IVIG) administration. High-dose IVIG (2 g/kg) is usually given in the treatment of Kawasaki disease (KD). However, medium-dose IVIG (1 g/kg) is a low-cost treatment and may have the same efficacy. We aim to determine whether the treatment with IVIG at an initial dose of 1 g/kg is effective for preventing CAA.
Methods: A multicenter retrospective cohort study was conducted. A total of 507 patients with complete KD who were treated with high-dose and medium-dose immunoglobulin at Cipto Mangunkusumo Hospital and Kawasaki Center, Indonesia from January 2012 to January 2022 were enrolled. Patients treated with a single infusion of medium-dose IVIG (1 g/kg) were defined as group A, and patients treated with high-dose IVIG (2 g/kg) were defined as group B. Patient characteristics were compared between the two groups; demographic features, laboratory findings, mucocutaneous involvement, day of fever, duration of fever after treatment, length of stay, and rates of CAA from echocardiography during the follow-up period.
Results: Medium-dose IVIG was given in 24 patients (group A). High-dose IVIG was given in 483 patients (group B). Age and gender distributions, white blood cell and platelet counts, day of fever when IVIG was administered, duration of fever after IVIG treatment, and length of stay did not differ significantly between the two groups (p >0.05). All patients had mucocutaneous involvement. Median of WAZ was higher in group A (+0,35 vs -0,26; p <0.05). Median of concentrations of C-reactive protein was higher in group B (59,5 mg/L vs 81 mg/L; p <0.05). Coronary artery aneurysms were not found in group A and in 9 patients (1.9%) in group B during the follow-up period (p >0.05).
Conclusion: Treatment of KD with IVIG at an initial dose of 1 g/kg could show the same effectiveness as the high-dose IVIG (2 g/kg) and might be an option for low- and middle-income country.

"Kejadian malnutrisi pada balita menjadi perhatian besar karena menyangkut investasi sumber daya manusia. Indonesia menghadapi triple burden status gizi balita yang menjadi beban negara. Berbagai upaya dilakukan untuk menurunkan prevalensi kurang gizi balita. Growth faltering sebagai indikator awal risiko terjadinya stunting menjadi titik awal intervensi intensif dilakukan untuk mencegah stunting. Upaya mengatasi growth faltering dilakukan melalui intervensi spesifik terutama pemberian makan bayi dan anak pada baduta. Penelitian ini bertujuan untuk mengidentifikasi efektivitas brief intervention terhadap praktik pemberian makan responsif pada bayi growth faltering usia 6-23 bulan. Desain penelitian ini adalah pre-experimental study dengan sampel 29 responden di kelompok kontrol (mendapatkan intervensi konseling pemberian makan bayi dan anak/PMBA dan 27 responden kelompok intervensi (mendapatkan intervensi konseling PMBA dan brief intervention). Hasil penelitian ini menunjukkan bahwa kelompok yang mendapatkan intervensi cenderung meningkatkan skor total pemberian makan responsif dan skor active feeding, meskipun tidak ditemukan signifikansi (pValue > 0,05); sedangkan pada kelompok kontrol selisih skor menunjukkan penurunan. Hasil ini menunjukkan bahwa tidak ada perbedaan bermakna antara kelompok yang mendapat intervensi konseling PMBA dengan kelompok yang mendapatkan intervensi PMBA dan brief intervention “Mentari”. Rekomendasi pelayanan menunjukkan bahwa konseling PMBA tetap dapat menjadi intervensi mengubah praktik pemberian makan.

The incidence of malnutrition in children under five is a big concern because it involves investing in human resources. Indonesia faces a triple burden on the nutritional status of children under five. Various efforts were made to reduce the prevalence of malnutrition. Growth faltering as an early indicator of the risk of stunting is the starting point for intensive interventions to prevent stunting. Efforts to overcome growth faltering are carried out through specific interventions, especially infant and young child feeding practices. This study aims to identify the effectiveness of the brief intervention on responsive feeding practices in growth-faltering infants aged 6-23 months. The design of this study was a pre-experimental study with a sample of 29 respondents in the control group (getting infant and young child feeding counselling interventions/IYCF and 27 respondents in the intervention groups (getting IYCF counselling interventions and brief intervention). The results of this study showed that the group that received the intervention tended to improve the total responsive feeding score and active feeding score, although no significance was found (pValue > 0.05); Meanwhile, in the control group, the difference in scores showed a decrease. These results showed no significant difference between the group that received IYCF counselling intervention and the group that received IYCF intervention and brief intervention. Service recommendations suggest that IYCF counselling can still be an intervention to change feeding practices."

"Latar belakang: Pandemi coronavirus disease 2019 (COVID-19) secara global menyebabkan pemerintah Indonesia untuk menerapkan Pembatasan Sosial Berskala Besar (PSBB) sebagai upaya pencegahan transmisi COVID-19. Kebijakan ini berdampak pada perubahan gaya hidup anak melalui penutupan sekolah dan fasilitas rekreasional sehingga terjadi penurunan aktivitas fisik dan perubahan pola tidur anak. Penelitian ini bertujuan untuk mengetahui hubungan tingkat aktivitas fisik dengan gangguan tidur pada anak usia sekolah dasar di masa pandemi COVID-19.
Metode: Penelitian ini menggunakan desain studi cross-sectional yang dilakukan pada 437 subjek. Penelitian ini menggunakan data sekunder tahun 2020 yang menggunakan Physical Activity Questionnaire for Older Children (PAQ-C) dan Children Sleep Habits Questionnaire-Abbreviated (CSHQ-A). Data yang diperoleh dianalisis menggunakan uji Chi-square.
Hasil: Hasil penelitian menunjukkan sebanyak 60,87% subjek tidak aktif dan sebanyak 73,23% subjek mengalami gangguan tidur selama pandemi COVID-19. Hasil uji analisis statistik menunjukkan tidak terdapat hubungan bermakna antara tingkat aktivitas fisik dengan gangguan tidur (p=0,248).
Kesimpulan: Sebagian besar anak usia sekolah dasar tidak aktif melakukan aktivitas fisik, kurang tidur pada hari biasa, tidur cukup pada akhir pekan, dan mengalami gangguan tidur selama pandemi COVID-19. Tidak terdapat hubungan bermakna antara tingkat aktivitas fisik dengan gangguan tidur pada anak usia sekolah dasar selama pandemi COVID-19.

Introduction: Coronavirus disease 2019 (COVID-19) globally affected the Indonesian government to implement the large-scale social restriction to prevent the COVID-19 transmission. The policy has altered children’s lifestyles through the closure of schools and recreational facilities which caused the decline in children’s physical activity level and the alteration of children’s sleep pattern. This study aims to determine the relationship between elementary school-aged children’s physical activity level and sleep disorders during the COVID-19 pandemic.
Method: This study was conducted with a cross-sectional design on 437 subjects. The research used secondary data collected in 2020 to obtain the data through the Physical Activity Questionnaire for Older Children (PAQ-C) and the Children Sleep Habits Questionnaire-Abbreviated (CSHQ-A). The data were analyzed using the Chi-square analysis test.
Result: This study showed that 60,87% of the subjects were not physically active and 73,23% of the subjects had sleep disorders. The statistical analysis test showed no significant relationship (p 0.05) between children’s physical activity level and sleep disorders.
Conclusion: The majority of elementary school-aged children were not physically active and had sleep disorders during the COVID-19 pandemic. There was not a significant relationship between elementary school-aged children’s physical activity level and sleep disorders during the COVID-19 pandemic.

"Abstrak
Upper Respiratory Infection (URI) is a major cause of morbidity and mortality of infants and toddlers in developing countries. The high infant morbidity and mortality rates in Indonesia are associated with the low exclusive breastfeeding ability. Breast milk is a natural drink for newborns in the first month of life that is beneficial not only for the babies, but also for mothers. The aim of study was to determine exclusive breastfeeding and decrease in incidence of URI among infants aged 6-12 months. This study was conducted by using case control design. Samples were taken by using cluster random sampling. Subject of study consisted of 162 cases and 162 control with infants aged 6-12 years. Cases were 162 infants aged 6-12 months suffering from URI within one last month and taken by mothers to primary health care that was selected location of study, while control was mothers who took their infants aged 6-12 months who did not suffer from URI within one last month to the selected primary health care. Data analysis included univariate, bivariate, stratification, and multivariate analysis with logistic regression. Results of study found that infants who were not exclusively breastfed were 1.69 times (95% CI: 1.02-2.80) more at risk of increasing URI incidence compared to infants who were breastfed exclusively after controlled by smoker in house and immunization variables. Health promotion of 6-month exclusive breastfeeding, provision of immunization, and anti-smoking program need to be implemented continuously to decrease the rates of morbidity and mortality due to URI disease."
