"Pendahuluan: Perhitungan luas area luka bakar/total burn surface area (TBSA) tidak mungkin dilakukan secara eksak. Perbedaan estimasi TBSA sering dijumpai, bahkan di antara para ahli luka bakar. Namun, perhitungan ini merupakan langkah yang sangat penting untuk menentukan jumlah cairan resusitasi yang akan diberikan. Kebutuhan akan bantuan computer dalam proses perhitungan ini dirasakan perlu, walaupun penggunaan grafik Lund-Browder telah lama digunakan dalam praktik klinis. Burn Case 3DTM adalah sebuah aplikasi peranti lunak baru yang dapat digunakan untuk membantu perhitungan TBSA. Tujuan dari studi ini adalah untuk membandingkan validitas dan reliabilitas di antara kedua metode ini untuk menghitung TBSA dalam praktik klinis.Metode: Dua orang evaluator menghitung TBSA dari 20 set foto digital pasien Unit Luka Bakar Rumah Sakit Cipto Mangunkusumo yang didapatkan dari bank data Divisi Bedah Plastik. Metode perhitungan yang digunakan adalah grafik Lund-Browder dan Burn Case 3DTM. Validitas dan reliabilitas kedua metode ini akan diukur menggunakan grafik Bland Altman. Rata-rata perbedaan pengukuran dapat diterima bila berada di bawah 5%.Hasil. Validitas BurnCase 3DTM dibandingkan dengan grafik Lund-Browder dalam menghitung total luas luka bakar adalah sangat baik (Beda Rerata -0,96 (IK95% -0,36 sd 2,28); limit kesesuaian -4,69 sd 6,61; ICC=0,997 (IK95% 0,992 sd 0,999). Reliabilitas perhitungan luas luka bakar dengan grafik Lund-Browder menurut beberapa parameter adalah sebagai berikut: beda rerata -0,025 (IK95% -1,47 hingga 1,42); limit kesesuaian -6,22 hingga 6,17; ICC = 0,996 (IK95% 0,990 hingga 0,998). Reliabilitas penghitungan luka bakar dengan BurnCase 3DTM adalah sebagai berikut: beda rerata -0,71 (IK95% -1,59 hingga 0,18); limit kesesuaian -4,48 hingga 3,07; ICC = 0,999 (IK95% 0,996 hingga 0,999).Kesimpulan. Validitas BurnCase 3DTM dalam menghitung luas luka bakar dibandingkan dengan grafik Lund- chart sebagai alat referensi adalah sangat baik dengan nilai ICC sebesar 0,997. BurnCase 3DTM terbukti reliabel secara klinis dan statistik sebagai alat untuk menghitung luas luka bakar.

Introduction. Calculating the total burn surface area (TBSA) is never an exact measure. High deviation of TBSA estimation is common even among the burn specialist. However, this is a very critical step in determining the amount of initial fluid resuscitation to be administered. The need for computer assisted calculation is consider even the Lund-Browder chart has long been used in the clinical setting as a guide to estimate TBSA. A more recent software application is available to aid this estimation, the BurnCase 3DTM. This study aims to compare the validity and reliability of the two tools in calculating TBSA in the clinical setting.

Methods. The TBSA of twenty set of digital pictures of Burn Unit Cipto Mangunkusumo Hospital patients, extracted from the Plastic Surgery division database, covering the patients’ whole body is assessed by 2 assessors using (1) The Lund-Browder chart as the reference, and (2) The BurnCase 3DTM as a new measurement tools. The validity and reliability of each estimated values from both device will be measured using Bland Altman test. The mean difference assumed acceptable if less than 5 percent.

Results. The validity of the BurnCase 3DTM compared to the Lund-Browder chart to calculate total burn surface area (TBSA) according to various parameters is as follow: mean difference -0,96 (CI95% -0,36 to 2,28); limit of agreement -4,69 to 6,61; ICC = 0,997 (CI95% 0,992 to 0,999). The inter-rater reliability of TBSA calculation using the Lund Browder chart is as follow: mean difference -0,025 (CI95% -1,47 to 1,42); limit of agreement -6,22 to 6,17; ICC = 0,996 (CI95% 0,990 to 0,998). The inter-rater reliability of TBSA calculation using The BurnCase 3DTM is as follow: mean difference -0,71 (CI95% -1,59 to 0,18); limit of agreement -4,48 to 3,07; ICC = 0,999 (CI95% 0,996 to 0,999).

Conclusion. The BurnCase 3DTM is a valid tool to calculate TBSA when compared to the Lund Browder chart as the reference measurement tool to calculate total burn surface area (TBSA) with an ICC value of 0,997. The BurnCase 3DTM is proved clinically and statistically reliable as a measurements tool to calculate TBSA."
