"This paper investigates caste /ethnicity based inequity in women's health service utilization, particularly focusing on antenatal care (ANC) in the socioculturally complex patriarchal context of Nepal. Numerous studies worldwide have examined the effects of various factors contributing to antenatal care. However, much less is known about the influence of caste/ ethnicity on women's health-care utilization in Nepal. Using the nationally representative Nepal Demographic Health Survey (NDHS) Data 2011, a multilevel logistic regression was run with results suggesting that both non-economic (caste/ethnicity) and economic (household wealth) factors influence women's health-care utilization. First, women who belong to a disadvantaged caste/ethnicity such as the Hill Janajafi, Hill and Terai Dalit and Muslims are significantly less likely to make four plus antenatal care visits compared to the advantaged Bahun/Chhetri mothers. Second, mothers who belong to the wealthier category are significantly advantaged in terms of using antenatal care services compared to the poorest category of mothers. Third, contrary to the common assumption, mothers from the advantaged caste/ethnicity (Bahun/Chhetri and Newar) do not always fare better in all aspects of life; when from the poorest households, they are not significantly different in terms of antenatal care compared the poorest mothers who are from a disadvantaged caste/ethnicity. Tliese findings offer evidence against the misassumption that individuals of advantaged caste/ ethnicities are always privileged, suggesting that health policies should take into account the intertwining effects of both caste/ethnicity and economic status in order to improve women's health and well-being."

"ABSTRAK
Since 2014, there has been an increase in funds for primary health care (PHC) coming from the National Health Insurance program capitation funds and the Health Operational Assistance Fund. This study aimed to explore the effect of this budget increase on the health care services at PHC. The case study used a qualitative approach and interviews from 19 PHC health workers with the highest and lowest budget. Data were analyzed using thematic analysis and supported by quantitative data. The positive impact of the increasing PHC budget was felt by PHC staff due to the addition of operational equipment and incentives provided. Nevertheless, there was also a negative impact such as feeling overwhelmed due to an increase in the quantity of PHC activities and additional administrative affairs. It also triggered a negative interaction between staff due to the number of incentives received. The regulation on the use of budget empowers PHC to better arrange the schedule of activities and manage human resources. However, these regulations are considered restrictive and the administrative flow of funds is too long that hinder the optimal use of the budget."

"The National Health Insurance (NHI) Program was implemented in Indonesia on January 1st 2014. This program definitely brings some changes into managerial
aspect in public health center (PHC). This study aimed to determine an impact of NHI policy in the implementation of health promotion programs at PHC
in South Tangerang City, Indonesia. This study was conducted using qualitative method during February and March 2016. The impact of NHI is seen on policy,
budget, equipment, human resource and implementation of health promotion program. With purposive sampling method, six policy makers, eight service
providers and eight service users were selected for in-depth interview. 17 documents were analyzed. Observation was conducted at four selected PHC’s.
Data analysis used thematic content analysis. There was no difference of PHC’s functions before and after NHI period. Budget expended for health promotion
programs increased after NHI was implemented which could represent an opportunity for PHC to innovate, procure materials and implement better health
promotion programs. Capitation budget which could be used for executing health promotion program and some recently implemented health promotion programs
in the NHI era evidences that NHI policy has a positive impact on the implementation of health promotion programs at PHC.
Jaminan Kesehatan Nasional (JKN) telah diimplementasikan di Indonesia pada 1 Januari 2014. Hal tersebut membawa beberapa perubahan pada aspek
manajerial pada pusat kesehatan masyarakat (puskesmas). Penelitian ini bertujuan untuk mengidentifikasi dampak dari kebijakan JKN terhadap implementasi
program promosi kesehatan di puskesmas di Kota Tangerang Selatan, Indonesia. Penelitian menggunakan metode kualitatif. Data dikumpulkan pada
Februari – Maret 2016. Dampak program dilihat dalam hal kebijakan, pendanaan, fasilitas, sumber daya manusia dan pelaksanaan program promosi kesehatan.
Dengan menggunakan metode purposive sampling, enam pengambil kebijakan, delapan pemberi layanan dan delapan penerima layanan diambil sebagai
informan dalam penelitian ini. Pada analisis dokumen, 17 dokumen telah dianalisis. Observasi dilakukan dengan melihat kegiatan yang dilakukan di
empat puskesmas. Analisis data menggunakan analisis konten tematik. Tidak terdapat perbedaan dari fungsi puskesmas sebelum dan setelah adanya JKN.
Dana yang digunakan untuk kegiatan promosi kesehatan telah mengalami peningkatan setelah implementasi kebijakan JKN dimana dana tersebut dapat digunakan
untuk berinovasi, memberi peralatan dan melakukan promosi kesehatan dengan lebih baik. Dana kapitasi yang dapat digunakan untuk melaksanakan
program promosi kesehatan dan beberapa kegiatan promosi kesehatan yang baru dilaksanakan pada saat era JKN menjadi bukti bahwa kebijakan JKN memiliki
dampak positif terhadap pelaksanaan program promosi kesehatan di puskesmas"

"Jaminan kesehatan nasional merupakan salah satu program pemerintah untuk memfasilitasi pelayanan kesehatan bagi masyrakat. Tujuan penelitian in untuk mengetahui apakah ada dampak Program Jaminan Kesehatan Nasional (JKN) terhadap kualitas profitabilitas dan pelyanan di Rumah sakit Juanda Kuningan, Jawa Barat. Metode menggunakan paired-t test untuk menganalisis perbedaan antara sebelum dan sesudah program Asusransu Kesehatan Nasional. Hasil penelitian menunjukkan bahwa terdapat perbedaan profitabilitas dan kualitas pelayanan antara pra dan pasca penerapan program asuransi kesehatan nasional. Profitabilitas diukur dengan marjin laba kotor, marjin laba bersih, return on total assets, dan return on equity. Hasil ini berarti bahwa nilai profitabilitas perusahaan lebih baik bila program JKN masih berlangsung di rumah sakit Juanda. Sedangkan pada variabel kualitas pelayanan diketahui bahwa nilai rata-rata lebih tinggi bila program JKN telah dilaksanakan di rumah sakit."

"ABSTRAK
Jaminan sosial kesehatan di Indonesia telah dibentuk dengan berlakunya UU No. 40 Tahun 2004 Tentang Sistem Jaminan Sosial Nasional. Undang-Undang tersebut juga mengamanatkan BPJS sebagai penyelenggara sistem jaminan kesehatan nasional. Kehadiran BPJS Kesehatan sebagai alat penyalur pelayanan kesehatan yang bermutu dan terjangkau, tetap menemukan berbagai permasalahan, seperti kekurangan anggaran, pelayanan yang dianggap terlalu rumit atau kurang menyenangkan, serta tenaga ahli yang merasa bahwa imbalan yang didapatkan tidak seimbang dengan usaha yang diberikan. Oleh karena itu, penelitian ini menganalisis dan membandingkan sistem jaminan sosial kesehatan di Indonesia, dengan salah satu sistem jaminan sosial kesehatan yang paling bertahan lama dan berhasil, yakni sistem NHS di Inggris, untuk mencari aspek apa dari sistem NHS yang baik untuk dicontoh oleh BPJS. Bentuk penelitian yang akan digunakan adalah tipe penelitian deskriptif. Metode ini digunakan untuk menganalisis kedua sistem jaminan sosial kesehatan berdasarkan peraturan-peraturan yang terkait. Dalam penlitian ini penulis menarik berbagai kesimpulan, seperti perlunya restrukturisasi bentuk jaminan sosial kesehatan Indonesia yang berbasis asuransi, menjadi sistem yang berbasis pajak. Kesimpulan ini akan dibahas secara mendalam di tulisan ini.

ABSTRACT
The healthcare system in Indonesia has been established with the enactment of UU No. 40 of 2004. The law also mandates BPJS as the organizer of the national healthcare system. The presence of BPJS Kesehatan as a means of channeling quality and affordable health services, still finds various problems, such as budget shortages, services that are considered too complicated or unpleasant, and practitoners who feel that the benefits obtained are not balanced with the effort given. Therefore, this study analyzes and compares the healthcare system in Indonesia, with one of the most durable and successful healthcare systems in the world, namely the NHS system in the UK, to look for aspects of the NHS system that should be emulated by Indonesia. The method of research that will be used in is normative juridical form. This method is used toanalyze both healthcare systems based on relevant regulations. In this study the authors draw various conclusions, among others, the need for restructuring Indonesian healthcare system, from an insurance based system, into a tax-based system. This conclusion will be discussed in depth in this thesis."
