"Karya ilmiah akhir ini merupakan kumpulan dari laporan praktik residensi keperawatan medikal bedah yang terdiri dari laporan kasus kelolaan, penerapan edukasi perawatan kolostomi berdasarkan evidence based nursing dan laporan inovasi tentang aplikasi pengkajian ESAS (edmonton symptom assessment system). Praktik ini menerapkan asuhan keperawatan pada pasien kanker dengan menggunakan pendekatan teori peaceful end of life. Fokus dari teori ini adalah menerapkan lima konsep utama yaitu nyeri, nyaman, dihargai dan kedekatan dengan orang yang bermakna. Tujuan dari pendekatan teori ini adalah untuk mencapai kehidupan yang damai dan berarti bagi keluarga dan orang lain diakhir kehidupannya. Salah satu upaya untuk meningkatkan pengetahuan dan ketrampilan pasien adalah edukasi. Edukasi perawatan kolostomi bertujuan untuk memandirikan pasien dalam merawat kolostominya. Metode edukasi perawatan kolostomi dengan audiovisual mampu memberikan motivasi pada pasien kanker. Selama proses perawatan keluhan pasien harus mendapatkan perhatian yang khusus. Pengkajian ESAS akan membantu mencatat keluhan secara sistematis. Kelima konsep dari teori peaceful end of life dapat dijadikan acuan dalam melakukan asuhan keperawatan pada pasien kanker, sedangkan edukasi perawatan kolostomi dengan menggunakan audovisual lebih mudah dipahami oleh pasien, dan format pengkajian ESAS merupakan instrumen pengkajian yang valid, layak dan dapat diandalkan untuk menilai gejala yang dirasakan pasien.

Scientific work is a compilation of the final report medical-surgical nursing practice residency which consists of case reports under management, the application of colostomy care education based on evidence based nursing and innovation reports on applications ESAS assessment (edmonton symptom assessment system). This is implement nursing care to cancer patients using a theoretical approach peaceful end of life. The focus of this theory is the application of the five main concepts of pain, comfortable, valued and meaningful proximity with people. The purpose of this theoretical approach is to achieve a peaceful and meaningful life for families and others at the end of his life. One effort to improve the knowledge and skills of the patient was education. Education aims to colostomy selfcare. Method of education with audiovisual able to motivate cancer patients. During the treatment process of the patient's complaints should get special attention. ESAS assessment will help record complaints systematically. The fifth concept of the peaceful end of life theory can be used as a reference in performing nursing care to cancer patients, whereas colostomy care education by using audiovisual more easily understood by the patient, and assessment formats ESAS is a valid assessment instrument, feasible and reliable.
