"Skripsi ini membahas mengenai penerapan perhitungan biaya kualitas pada perusahaan manufaktur baja yaitu PT KSG dari tahun 2011 sampai dengan 2015 menggunakan P-A-F Preventive-Appraisal-Failure Model. Dalam penelitian ini selain perhitungan biaya kualitas juga dibuat laporan standar yang bisa digunakan oleh manajemen untuk membantu mengambil keputusan terkait dengan pola operasi dan proses bisnis PT KSG. Hasil dari analisa penelitian ini menunjukkan bahwa PT KSG masuk ke dalam kategori daerah proyek peningkatan kualitas zone of improvement project dimana presentase biaya kegagalan > 70 dan presentase biaya pencegahan < 10.

This research discusses the implementation of cost of quality calculation in steel manufacturing company named PT KSG from 2011 to 2015 using the P A F Preventive Appraisal Failure Model. In addition to the cost of quality calculation, this research also provides the standard report which used by management to make decisions regarding operation and business process pattern of PT KSG. The result of this research shows that PT KSG is classified as a zone of improvement project where the percentage of the cost failure 70 and the percentage of the cost prevention 10."

"Pengukuran kinerja yang dilakukan perusahaan konsultan IT selama ini hanya berfokus pada aspek finansial. Dimana pengukuran kinerja ini tidak begitu komprehensif dalam mengukur kinerja suatu perusahaan yang mengakibatkan sulit untuk memperbaiki kondisi internal dan eksternal perusahaan sehingga berdampak pada lambatnya laju pertumbuhan perusahaan. Metode performance PRISM yang dikolaborasikan dengan metode IPA dan AHP digunakan dalam penelitian ini untuk mendapatkan suatu pengukuran kinerja yang komprehensif dengan melibatkan peran stakeholder untuk melengkapi indikator kinerja yang selama ini diberikan dalam mengukur kinerja perusahaan. Dengan identifikasi kebutuhan dan kontribusi stakeholder, diperoleh indikator kinerja sebagai indikator keberhasilan kinerja perusahaan. Dari penelitian ini diketahui bahwa terdapat 30 indikator kinerja atau KPI yang dapat menggambarkan kondisi kinerja perusahaan, terdiri dari 4 KPI kepuasan stakeholder, 10 KPI strategi, 8 KPI proses, 4 KPI kapabilitas, 4 KPI kontribusi stakeholder.

The performance measurement conducted by the IT consultant firms this day focused solely on the financial aspect. Where the performance measurement is not too compherensive in measuring the performance of a company which cause it’s hard to improve the internal and external conditions of a company and has impact on the slow growth rate of a company.

The performance PRISM method colaborated with IPA and AHP methods were used in this study in order to get the a performance measurement comprehensively by involving stakeholder to complete the performance indicators which have been given in measuring the company’s performance. With the identification of the stakeholder needs and contributions, obtained the performance indicators as the success indicators of a company’s performance.

From this research known that there were 30 performance indicators or KPIs which can describe the condition of the company's performance, consisting of 4 KPIs of stakeholder satisfaction, 10 KPIs of strategy, 8 KPIs of process, 4 KPIs of capability, 4 KPIs of stakeholder contributions."
