Acute Respiratory Distress Syndrome (ARDS) merupakan kondisi paru yang memicu respon inflamasi sistemik dengan etiologi yang berbeda yang memiliki ciri-ciri klinis patologis khas. Penelitian ini menganalisis tentang pemberian posisi prone dan reverse trendelenburg 30 derajat sebagai terapi suportif untuk meningkatkan status oksigenasi pada pasien ARDS serta bagaimana dampaknya terhadap komplikasi edema wajah dan Gastric Residual Volume (GRV). Tujuan penelitian untuk mengetahui pengaruh pemberian posisi prone dan reverse trendelenburg 30 derajat terhadap parameter oksigenasi, edema wajah dan GRV. Desain penelitian dengan menggunakan quasi experiment design melalui pendekatan crossover trial design dengan 11 responden. Analisis data menggunakan uji independent t-test pada data terdistribusi normal dan Mann Whitney pada data terdistribusi tidak normal dan jenis data kategorik. Tidak terdapat perbedaan rata-rata yang signifikan pada parameter oksigenasi; saturasi oksigen (p value 0,685), PaO2/FiO2 rasio (p value 0,358), SpO2/FiO2 rasio (p value 0,850), maupun EtCO2 (p value 0,409) dan edema wajah (p value 0,403) antar kelompok prone dengan kelompok prone dan reverse trendelenburg 30 derajat, namun terdapat perbedaan rata-rata yang signifikan pada variabel GRV antara kelompok prone dengan kelompok prone dan reverse trendelenburg 30 derajat (p value 0,035). Kesimpulan: baik posisi prone maupun posisi prone dengan reverse trendelenburg 30 derajat sama-sama memberikan dampak yang positif pada parameter oksigenasi. Namun kedua posisi ini juga memberikan dampak yang patut diwaspadai terkait kejadian edema wajah dan peningkatan GRV, dengan angka kejadian yang lebih rendah pada pemberian posisi prone dengan reverse trendelenburg 30 derajat.


Acute Respiratory Distress Syndrome (ARDS) is a pulmonary condition that triggers a systemic inflammatory response with different etiologies that has typical pathological clinical characteristics. This study analyzes the provision of 30 degrees prone and reverse Trendelenburg positions as supportive therapy to improve oxygenation status in ARDS patients and its impact on complications of facial edema and Gastric Residual Volume Gastric Residual Volume (GRV). The aim of the research is to determine the effect of 30 degrees prone and reverse trendelenburg positions on oxygenation parameters, facial edema and GRV. The research design used a quasi experimental design using a crossover trial design approach with 11 respondents. Data analysis used the independent t test on normally distributed data and Mann Whitney on non-normally distributed data and categorical data types. There were no significant average differences in oxygenation parameters; oxygen saturation (p value 0.685), PaO2/FiO2 ratio (p value 0.358), SpO2/FiO2 ratio (p value 0.850), as well as EtCO2 (p value 0.409) and facial edema (p value 0.403) between the prone group and the prone group and reverse trendelenburg, there is a significant average difference in the GRV variable between the prone group and the prone and reverse trendelenburg 30 degrees groups (p value 0.035). Conclusion: the prone position and the prone position with reverse trendelenburg 30 degrees both have a positive impact on oxygenation parameters. "Coronavirus Diseases 19 (COVID-19) merupakan penyakit infeksi saluran pernapasan menular yang diakibatkan oleh Severe acute respiratory syndrome coronavirus 2 (SARS-COV-2). Gejala yang ditimbulkan pada pasien COVID-19 dapat berupa gejala ringan hingga berat. Salah satu gejala khas yang sering ditemukan adalah sesak napas yang dapat meningkat menjadi kondisi kritis misalnya Acute Respiratory Distress Syndrome (ARDS). Manajamenen utama yang dilakukan untuk mengatasi gangguan napas pada COVID-19 adalah dengan pemberian terapi oksigen yang disesuaikan dengan kondisi pasien. Selain itu, pemberian posisi prone. Juga dapat dilakukan untuk meningkatkan status oksigenasi pasien. Laporan kasus ini mendeskripsikan kasus ibu S 58 tahun dengan keluhan demam 7 hari sebelum masuk rumah sakit, demam dirasakan meriang, menggigil, batuk, bab cair 3x sejak seminggu lalu. Seminggu sebelum masuk rumah sakit muncul bercak kemerahan, gatal, digaruk-garuk hingga berdarah di kaki kiri, Pasien juga mengatakan sesak, mual dan muntah saat makan. 2 hari sebelum masuk rumah sakit dinyatakan positif COVID-19. Selama perawatan di IGD dan ranap terjadi peningkatan maupun penurunan kondisi hingga akhirnya pasien mengalami perburukan kondisi saat diranap dan di pindahkan ke ruang ICU untuk mendapatkan terapi oksigen dengan ventilator mekanik, selain itu saat hari keempat di ruang ICU pasien juga mendapatkan terapi tambahan berupa diberikan posisi prone untuk meningkatkan status oksigenasi pasien. Setelah dilakukan prone dan mendapatkan terapi ventilator mekanik, kondisi status oksigenasi klien perlahan-lahan membaik. Laporan kasus ini menunjukkan manfaat pemberian posisi prone pada pasien dengan ventilasi mekanik sebagai salah satu treatment untuk membantu mengatasi masalah gangguan ventilasi spontan pada pasien COVID-19.

Coronavirus Diseases (COVID-19) is an infectious respiratory disease caused by Severe acute respiratory syndrome coronavirus 2 (SARS-COV-2). Symptoms caused by COVID-19 patients can range from mild to severe symptoms. One of the typical symptoms that are often found is shortness of breath which can escalate to a critical condition, such as Acute Respiratory Distress Syndrome (ARDS). The main management carried out to overcome respiratory disorders in COVID-19 is to provide oxygen therapy that is tailored to the patient's condition. In addition, presenting a vulnerable position. It can also be done to improve the patient's oxygenation status. A case report of a 58-year-old mother with complaints of fever 7 days before admission to the hospital, felt fever, chills, cough, liquid chapters 3 times since last week. A week before being admitted to the hospital, he appeared to have blotches, itching, scratching and bleeding on his left leg. The patient also reported shortness of breath, nausea and vomiting while eating. 2 days before admission to the hospital tested positive for COVID-19. During treatment in the emergency room and in the hospital, there was an increase or decrease in condition until finally the patient experienced a worsening of his condition when he was hospitalized and was transferred to the ICU to receive oxygen therapy with a mechanical ventilator, besides that on the fourth day in the ICU the patient also received additional therapy in the form of being given a prone position. to improve the patient's oxygenation status. After being prone and receiving mechanical ventilator therapy, the client's oxygenation status slowly improved. This case report benefits from presenting the prone position to the patient by providing assistance as one of the treatments to overcome the problem of spontaneous ventilation disorders in COVID-19 patients. "Latar Belakang: Intensive care unit-acquired weakness (ICU-AW) adalah salah satu masalah yang dihadapi dalam manajemen pasien kritis karena dihubungkan dengan ventilasi mekanik berkepanjangan sehingga meningkatkan risiko komplikasi dan mortalitas selama di ICU. Mobilisasi dini seperti neuromuscular electrical stimulation (NMES) dilaporkan bermanfaat mengurangi durasi penggunaan ventilator di ICU. Telaah sistematik dan meta-analisis ini dibuat untuk menyimpulkan dampak dari NMES terhadap durasi penggunaan ventilator di ICU. Tujuan: Mengetahui dampak spesifik penggunaan NMES terhadap durasi penggunaan ventilator pada pasien ICU. Metode: Studi eligibel hingga Januari 2022 terinklusi dalam studi. Pencarian literatur dilakukan melalui database jurnal berbasis elektronik yaitu Cochrane, EBSCOHost, Scopus, dan Pubmed dengan kata kunci spesifik dan operator boolean. Studi terinklusi dievaluasi untuk risiko bias dengan Cochrane RoB 2 dan estimasi besar efek dilakukan dengan fixed effect model menggunakan perangkat lunak Review Manager 5.4. Hasil: Pencarian literatur menghasilkan 9 studi yang terinklusi dalam meta-analisis. Dari penggabungan data, disimpulkan bahwa penggunaan NMES berhubungan dengan penurunan durasi penggunaan ventilator (MD -1.48; 95% CI: -2,54 – -0,41, p = 0,007, I 2 =30%, fixed-effect modelling). Kesimpulan: Penelitian ini menyimpulkan bahwa pemberian NMES dapat menurunkan durasi penggunaan ventilator di ICU.

Background: Intensive Care Unit-Acquired Weakness (ICU-AW) is one of the problems faced in critical medicine management, associated with prolonged mechanical ventilation (PMV) thereby increasing risk and mortality while in the ICU. Early mobilization such as neuromuscular electrical stimulation (NMES) has been reported to be beneficial in reducing the duration of mechanical ventilation in the ICU. This systematic review and meta-analysis was conducted to conclude the impact of NMES on the duration of mechanical ventilation in the ICU. Objective: To determine the impact of the use of NMES on duration of mechanical ventilation in ICU patients. Methods: Eligible studies up to January 2022 were included in the study. The literature search was carried out through electronic-based journal databases, namely Cochrane, EBSCOHost, Scopus, and Pubmed with specific keywords and boolean operators. The included studies were evaluated for risk of bias with Cochrane RoB 2 and estimation of effect size was performed using a fixed effect modelling using Review Manager 5.4 software. Results: The literature search yielded 9 studies that were included in the meta- analysis. From the pooled data, it was concluded that NMES administration was associated with a decrease in the duration of mechanical ventilation (MD -1.48; 95% CI: -2.54 – -0.41, p = 0.007, I2 = 30%, fixed-effect modeling). Conclusion: This study concluded that the administration of NMES reduces the duration of ventilator use in the ICU."

"ABSTRAK
Pasien yang terpasang ventilator mengalami kesulitan berbicara sehingga mengalami gangguan dalam berkomunikasi dan menggunakan suatu metode dalam berkomunikasi. Penelitian ini menggunakan desain crossectional dengan sampel 45 responden dan tekhnik pengumpulan data purposive sampling. Hasil penelitian menunjukkan metode komunikasi yang digunakan pasien yaitu menunjuk atau membuat isyarat dengan tangan yang digunakan 100% responden, menulis atau menggambar yang digunakan 40% responden, menggerakkan bibir atau mencoba untuk berbisik yang digunakan 91% responden, ekspresi wajah yang digunakan 84% responden, dan menggerakkan kepala, ya atau tidak yang digunakan 93% responden. Metode komunikasi lain yang digunakan adalah mengetuk tempat tidur. Perawat diharapkan mengetahui metode komunikasi yang digunakan pasien yang terpasang ventilator sehingga dapat memberikan penjelasan tentang pilihan metode komunikasi sesuai keadaan pasien serta menyediakan media sesuai metode komunikasi yang dipilih.

ABSTRAK
Patients who have difficulty speaking with mechanical ventilation so that impaired communication and uses a method of communicating. This study used crosssectional design with a sample of 45 respondents and data collection purposive sampling technique. The results showed that method of communication used by patients to refer or make a gesture with the hand were used 100% by the respondents, writing or drawing were used 40% by respondents, moving his lips or trying to whisper were used 91% by respondents, the facial expression were used 84% by respondents, and move his head, yes or not were used 93% by respondents. Another communication method which was used by the patient is knocking on the bed. Nurses are expected to know the method of communication used by patients with mechanical ventilation in order to provide an explanation of the preferred method of communication according to the situation of patients and provide appropriate media communication method selected."

"Permasalahan yang sering muncul dalam merawat klien dengan ventilasi mekanik di PICU adalah masalah keperawatan ketidakefektifan bersihan jalan napas. Salah satu teori keperawatan yang dapat diaplikasikan di ruang perawatan intensif adalah Model Konservasi Myra E. Levine yang mempunyai 3 konsep dasar yaitu konservasi, adaptasi dan keutuhan (wholeness). Tahapan proses keperawatan yang dilakukan adalah melakukan pengkajian, merumuskan trophicognosis dan hypothesis yang tepat, mengimplementasikan rencana dan melakukan evaluasi terhadap setiap respon organismik klien yang bertujuan untuk membantu klien beradaptasi selama dalam perawatan dan mencapai wholeness sebagai seorang individu yang unik. Intervensi yang dapat dilaksanakan untuk mempertahankan bersihan jalan napas yang efektif, antara lain fisioterapi dada, penghisapan lendir, humidifikasi, inhalasi, dan mobilisasi. Peran Ners Spesialis Keperawatan Anak selama merawat anak dengan ventilasi mekanik adalah sebagai praktisi, pendidik, advokat dan peneliti. Ners Spesialis Keperawatan Anak diharapkan terus mengembangkan ilmu pengetahuan dan teknologi dibidang keperawatan untuk meningkatkan kualitas asuhan keperawatan pada klien dan keluarga.

Nursing problem that often arises in caring of pediatric patient with mechanical ventilation at PICU is ineffectiveness of airway clearance. One of the nursing theories that can be applied in intensive care unit is Myra E. Levine Conservation Model which has 3 basic concepts. Those concepts are conservation, adaptation and wholeness. Stages of the nursing process are assesment, formulate appropriate trophicognosis and hypothesis, implementing the intervention and evaluating every client organismic response that is aimed to help clients adapting during the treatment and achieving wholeness as a unique individual. Interventions that can be implemented to maintain an effective airway clearance are chest physiotherapy, mucus suction, humidification, inhalation, and mobilization. The Role of Ners Specialist of Pediatric Nursing for caring of a pediatric patient with mechanical ventilation is as practitioner, educator, advocate and researcher. Ners specialist of pediatric nursing are expected to continue develop knowledge and technology in the field of nursing to improve the quality of nursing care to clients and families."
