Peraturan Menteri Kesehatan Republik Indonesia Nomor 72 Tahun 2016 Tentang Standar Pelayanan Kefarmasian Di Rumah Sakit menyatakan bahwa salah satu contoh pelayanan farmasi klinik di Rumah Sakit adalah Pemantauan Terapi Obat. Pemantauan Terapi Obat (PTO) merupakan suatu proses yang mencakup kegiatan untuk memastikan terapi Obat yang aman, efektif dan rasional bagi pasien. Tujuan PTO adalah meningkatkan efektivitas terapi dan meminimalkan risiko Reaksi Obat yang Tidak Dikehendaki (ROTD). Pada Praktek Kerja Profesi Apoteker (PKPA) ini, calon Apoteker memperoleh kesempatan untuk melakukan PTO pada pasien TBC. Tugas khusus ini bertujuan untuk meningkatkan pemahaman calon Apoteker mengenai pelaksanaan dan pembuatan laporan PTO pada pasien TBC. Kegiatan PTO dilakukan secara retrospektif pada salah satu pasien di Ruang Lily Gedung A. Kajian ini menggunakan data sekunder berupa rekam medik pasien dan hasil laboratorium. Pemantauan terapi obat pada pasien TBC dapat dilakukan dan dapat diidentifikasi masalah terkait obat, yaitu semua obat yang diberikan kepada pasien telah tepat indikasi dan tepat dosis. Terdapat beberapa obat yang memiliki interaksi, sehingga direkomendasikan agar penggunaan obat-obatan yang memiliki interaksi tersebut dipisah dan dilakukan pemantauan terapi.

Drug Therapy Monitoring (PTO) is a process that includes activities to ensure safe, effective and rational drug therapy for patients. In this Pharmacist Professional Work Practice (PKPA), prospective pharmacists have the opportunity to carry out PTO on TB patients. This special assignment aims to increase the understanding of prospective pharmacists regarding the implementation and preparation of PTO reports for TB patients. PTO activities were carried out retrospectively on one of the patients in the Lily Room, Building A. This study used secondary data in the form of patient medical records and laboratory results. Monitoring drug therapy in TB patients can be carried out and drug-related problems can be identified, namely that all drugs given to patients have the correct indications and the correct dosage. There are several drugs that have interactions, so it is recommended that the use of drugs that have interactions be separated and therapy monitored.
